Aim: Visual acuity outcome of amblyopia treatment depends on the compliance. This study aimed to determine parental predictors of poor visual outcome with occlusion treatment in unilateral amblyopia and identify the relationship between occlusion recommendations and the patient's actual dose of occlusion reported by the parents. Methods: This study comprised three phases: refractive adaptation for a period of 18 weeks after spectacle correction; occlusion of 3 to 6 hours per day during a period of 6 months; questionnaire administration and completion by parents. Visual acuity as assessed using the Sheridan-Gardiner singles or Snellen acuity chart was used as a measure of visual outcome. Correlation analysis was used to describe the strength and direction of two variables: prescribed occlusion reported by the doctor and actual dose reported by parents. A logistic binary model was adjusted using the following variables: severity, vulnerability, self-efficacy, behaviour intentions, perceived efficacy and treatment barriers, parents' and childrens' age, and parents' level of education. Results: The study included 100 parents (mean age 38.9 years, SD approx 9.2) of 100 children (mean age 6.3 years, SD approx 2.4) with amblyopia. Twenty-eight percent of children had no improvement in visual acuity. The results showed a positive mild correlation (kappa = 0.54) between the prescribed occlusion and actual dose reported by parents. Three predictors for poor visual outcome with occlusion were identified: parents' level of education (OR = 9.28; 95%CI 1.32-65.41); treatment barriers (OR = 2.75; 95%CI 1.22-6.20); interaction between severity and vulnerability (OR = 3.64; 95%CI 1.21-10.93). Severity (OR = 0.07; 95%CI 0.00-0.72) and vulnerability (OR = 0.06; 95%CI 0.05-0.74) when considered in isolation were identified as protective factors. Conclusions: Parents frequently do not use the correct dosage of occlusion as recommended. Parents' educational level and awareness of treatment barriers were predictors of poor visual outcome. Lower levels of education represented a 9-times higher risk of having a poor visual outcome with occlusion treatment.

Introdução – A Polineuropatia Amiloidotica Familiar (PAF) é uma amiloidose sistémica heredo-degenerativa e autossómica dominante, e cuja manifestação predominante é a polineuropatia mista sensitiva motora e autonómica. Aos sintomas motores e sensitivos associam-se também alterações cardíacas, renais, urinárias, genitais e digestivas que se traduzirão em incapacidade funcional progressiva. Esta doença endémica de Portugal apresenta o seu início em média depois dos 20 anos com progressão para a morte em cerca de 10 anos. O tratamento etiopatogénico desta doença, com reconhecida eficácia, passa ainda pelo transplante hepático que apesar de tudo é efectuado já numa fase sintomática da doença e ao qual se encontra associada medicação com efeitos negativos para o metabolismo muscular e consequentemente para a capacidade de produção de força. Tanto a progressão da PAF como o próprio transplante resultam em limitações funcionais que podem ser melhoradas através do exercício físico. Do nosso conhecimento não existem estudos sobre a aptidão física destes pacientes submetidos a transplante hepático nem sobre os efeitos de um programa de exercício físico. Objectivo: Verificar o efeito de um programa de exercício físico combinado na aptidão física de indivíduos portadores de PAF submetidos a transplante hepático há menos de 12 meses. Adicionalmente, analisar de que modo algumas componentes da aptidão física (composição corporal, força muscular e capacidade funcional) seriam ou não e em que medida, diferentes da população saudável. Material e métodos: Trinta e nove pacientes PAF transplantados (4.3±3.3 meses pós transplante foram avaliados quanto à sua composição corporal (DEXA), força de preensão (dinamometro de preensão E-Link), força isométrica do quadricipete (dinamometria isocinética-Biodex) e capacidade funcional (teste 6 minutos de marcha - 6MWT e capacidade para a marcha – 6MWT×peso). Os níveis de actividade física e de qualidade de vida percebida foram avaliados respectivamente através dos questionários Seven Days Physical Activity Recall (7-PAR) e Medical Outcome Study item shortform Health Survey (SF-36Ò). Os pacientes foram alocados em três grupos: grupo de controlo(GC) com 16 pacientes (13H e 3M; 33±9 anos, IMC 22.6±3.3 kg.m-2) sem programa de exercício; grupo de exercício supervisionado (GES) com 8 pacientes (5H e 3M; 34±±7 years; IMC 20.4±4.5 kg.m-2) submetidos a um programa de exercício físico combinado trissemanal com a duração de 24 semanas e um grupo de exercício físico domiciliário ou home-based (GHB) com 15 pacientes (4H e 11M; 35±5 anos; IMC 22.3±4.3 kg.m-2) que fizeram exercício físico em casa durante 24 semanas. Para comparar variáveis da aptidão física entre pacientes PAF transplantados e indivíduos aparentemente saudáveis, submetemos 48 indivíduos PAF transplantados há menos de 12 meses e 24 indivíduos saudáveis não activos, ao mesmo protocolo de avaliação (composição corporal, força muscular e capacidade funcional). Resultados: O programa de exercício influenciou positivamente a composição corporal (peso, IMC, massa magra total e regional, massa muscular esquelética total e T-score do fémur proximal), o risco de incapacidade física, a capacidade para a marcha e a força de preensão da mão direita, mas não influenciou a percepção de qualidade de vida dos pacientes. A distância percorrida aumentou de forma clinicamente significativa apenas no grupo GES. Os pacientes PAF transplantados apresentaram valores inferiores aos indivíduos saudáveis na composição corporal (peso, IMC, massa óssea, massa gorda, massa magra e massa muscular) força muscular e capacidade funcional. Conclusões: O programa de exercício físico melhorou a composição corporal, capacidade funcional e força muscular e aumentou o peso corporal e o IMC nos doentes PAF. Esta melhoria foi maior no grupo GES do que no grupo GHB em todas as variáveis analisadas.

Purpose – Quantitative instruments to assess patient safety culture have been developed recently and a few review articles have been published. Measuring safety culture enables healthcare managers and staff to improve safety behaviours and outcomes for patients and staff. The study aims to determine the AHRQ Hospital Survey on Patient Safety Culture (HSPSC) Portuguese version's validity and reliability. Design/methodology/approach – A missing-value analysis and item analysis was performed to identify problematic items. Reliability analysis, inter-item correlations and inter-scale correlations were done to check internal consistency, composite scores. Inter-correlations were examined to assess construct validity. A confirmatory factor analysis was performed to investigate the observed data's fit to the dimensional structure proposed in the AHRQ HSPSC Portuguese version. To analyse differences between hospitals concerning composites scores, an ANOVA analysis and multiple comparisons were done. Findings – Eight of 12 dimensions had Cronbach's alphas higher than 0.7. The instrument as a whole achieved a high Cronbach's alpha (0.91). Inter-correlations showed that there is no dimension with redundant items, however dimension 10 increased its internal consistency when one item is removed. Originality/value – This study is the first to evaluate an American patient safety culture survey using Portuguese data. The survey has satisfactory reliability and construct validity.

X-ray fluoroscopy is essential in both diagnosis and medical intervention, although it may contribute to significant radiation doses to patients that have to be optimised and justified. Therefore, it is crucial to the patient to be exposed to the lowest achievable dose without compromising the image quality. The purpose of this study was to perform an analysis of the quality control measurements, particularly dose rates, contrast and spatial resolution of Portuguese fluoroscopy equipment and also to provide a contribution to the establishment of reference levels for the equipment performance parameters. Measurements carried out between 2007 and 2013 on 143 fluoroscopy equipment distributed by 34 nationwide health units were analysed. The measurements suggest that image quality and dose rates of Portuguese equipment are congruent with other studies, and in general, they are as per the Portuguese law. However, there is still a possibility of improvements intending optimisation at a national level.

Objectives - To identify occupational stressors and coping resources in a group of physiotherapists, and to analyse interactions between subjective levels of stress, efficacy in stress resolution and coping resources used by these professionals. Design - A sample of 55 physiotherapists working in three general hospitals in Portugal completed the Coping Resources Inventory for Stress, the Occupational Stressors Inventory and two subjective scales for stress and stress resolution. Main results - Most physiotherapists perceived that they were moderately stressed (19/55, 35%) or stressed (20/55, 36%) due to work, and reported that their efficacy in stress resolution was moderate (25/54, 46%) or efficient (23/54, 42%). Issues related to lack of professional autonomy, lack of organisation in the hierarchical command chain, lack of professional and social recognition, disorganisation in task distribution and interpersonal conflicts with superiors were identified as the main sources of stress. The most frequently used coping resources were social support, stress monitoring, physical health and structuring. Perceived efficacy in stress resolution was inversely related to perceived level of occupational stress (r = −0.61, P < 0.01). Significant correlations were found between several coping resources and the perceived level of stress and efficacy in stress resolution. Associations between problem solving, cognitive restructuring and stress monitoring and both low levels of perceived stress and high levels of perceived efficacy were particularly strong. Implications for practice - The importance of identifying stressors and coping resources related to physiotherapists’ occupational stress, and the need for the development of specific training programmes to cope with stress are supported.
