Este Ponto de Vista resume as conclusões de um Workshop conjunto, organizado pelos três Programas da FAPESP na Área Ambiental - BIOTA (O Instituto Virtual da Biodiversidade) - BIOEN (Pesquisa em Bioenergia) - Mudanças Climáticas, para discutir a contribuição da comunidade científica para a RIO+20, a Conferência das Nações Unidas para o Desenvolvimento Sustentável. O grupo de pesquisadores brasileiros reunidos pela FAPESP no início de março de 2012 levantou as seguintes preocupações: a) o número reduzido de oportunidades para a comunidade científica interagir com Conferências como a RIO+20; b) as graves deficiências do ZeroDraft, documento produzido pela Divisão das Nações Unidas para o Desenvolvimento Sustentável para a RIO +20; c) o fato do foco de pesquisa dos três Programas de Pesquisa Ambiental da FAPESP - biodiversidade, bioenergia e mudanças climáticas - não estarem na pauta das discussões da RIO+20; d) que pouca ênfase é dada aos oceanos na Agenda da Conferência; e) em relação aos mecanismos de mercado associados com a transição para uma economia mais verde, a necessidade de enfatizar a redução de subsídios perversos e a promoção de incentivos econômicos para atividades ou processos de mitigação e/ou seqüestro de carbono; f) a necessidade de estimular o desenvolvimento e a consolidação da pesquisa na área de avaliação e valoração de serviços ambientais, no Brasil. Os participantes do Workshop reconheceram a necessidade de aprofundar o conhecimento sobre as convenções, tratados e acordos internacionais assinados e ratificados pelo Brasil, bem como as instituições internacionais, programas e iniciativas que promovem a participação da comunidade científica no debate de políticas ambientais globais. Finalmente, do ponto de vista dos três programas da FAPESP dois pontos foram destacados: a) que é imperativo aprofundar o conhecimento científico em cada uma das três áreas focais - biodiversidade, bioenergia e mudanças climáticas - porque é necessário aumentar a massa crítica de pesquisadores e do conhecimento para participar das discussões internacionais nessas áreas estratégicas; b) também é imperativo apoiar e promover projetos de pesquisa que integrem as áreas focais dos três programas, estimulando a constituição de equipes inter e transdisciplinares. Esta é uma tendência mundial na área das mudanças ambientais globais, e os participantes dos três programas sentem que podem dar uma contribuição significativa para o avanço do conhecimento, para o debate internacional e para a efetiva solução dos problemas.

Tornou-se lugar comum, sobretudo em correntes ligadas aos movimentos sociais mais amplos e às questões ambientais, a crítica ao reducionismo da ciência clássica, à dinâmica de trabalho individualizado, à desconexão e à falta de integração com os problemas reais. Como resultado, cresce o número de formadores de opinião em favor de uma ciência mais integrada aos problemas reais e de um conhecimento como processo coletivo. O Conselho Nacional de Desenvolvimento Científico e Tecnológico (CNPq) desenvolveu um Diretório dos Grupos de Pesquisa existentes no Brasil, alguns destes grupos possuem modelo de rede. Com o cenário apresentado, a reflexão sobre a importância de avaliar o potencial destes grupos foi incentivada para influenciar a formulação e a implantação de políticas públicas ambientais. Foi possível identificar fatores chave para o fortalecimento da influência nas políticas públicas, destacando-se dentre estes, avanços em abordagens interdisciplinares. Porém, os grupos possuem dificuldades na utilização de ferramentas de comunicação mais eficientes para o trabalho em rede e para atingir os tomadores de decisões; no acesso a fundos e nos critérios para integrar os grupos estudados.

O presente texto configura-se como um ensaio teórico acerca do tema desenvolvimento humano na obra de Lev Vigotski, bem como das implicações desse tema para a Neuropsicologia na obra de Alexander Luria. O trabalho foi feito com base nas obras completas em espanhol de Vigotski e nas obras em português de Vigotski e de Luria. Foram eleitos como focos de investigação considerações acerca do funcionamento cerebral humano, do conceito de compensação, dos estudos sobre deficiência e do conceito de Sistema Funcional Complexo. Apesar de os inúmeros estudos e diferentes traduções da obra de Vigotski terem ampla divulgação e repercussão, principalmente nos campos da Psicologia e da Educação, questões concernentes à Neuropsicologia não têm tido igual relevância. Em vista disto este texto procura realçar as contribuições e a fecundidade da obra no que se refere à originalidade das ideias do autor acerca dos estudos da Neuropsicologia e do desenvolvimento humano.

O trabalho apresenta os resultados da aplicação do modelo de sistematização do processo de análise das relações de influências entre os agentes públicos e privados na busca do desenvolvimento sustentável da Região do Alto Tietê, em particular, na indústria de celulose e papel da região. Para tanto, desenvolveu-se um referencial teórico ajustado com a dinâmica conflituosa entre os interesses econômicos, sociais e ambientais. A sustentação teórica foi alcançada com a composição das seguintes abordagens conceituais: Teoria dos Sistemas Abertos, Nova Economia Institucional e Organizações em Rede. A pesquisa-ação foi o método adotado para aprofundar o estudo das relações dos agentes envolvidos na indústria de celulose e papel do município de Suzano. A ilustração da aplicação do modelo, com os dados obtidos, serve para mostrar uma perspectiva mais analítica para o estudo das relações entre os agentes envolvidos em atividades econômicas, respeitando os preceitos do desenvolvimento sustentável.

Objective: To determine plasma homocysteine levels during fasting and after methionine overload, and to correlate homocysteinemia according to methylenetetrahydrofolate reductase (MTHFR) polymorphism in type 2 diabetic adults. Subjects and methods: The study included 50 type 2 diabetic adults (DM group) and 52 healthy subjects (Control group). Anthropometric data, and information on food intake, serum levels of vitamin B 12, folic acid and plasma homocysteine were obtained. The identification of C677T and A1298C polymorphisms was carried out in the MTHFR gene. Results: There was no significant difference in homocysteinemia between the two groups, and hyperhomocysteinemia during fasting occurred in 40% of the diabetic patients and in 23% of the controls. For the same polymorphism, there was not any significant difference in homocysteine between the groups. In the Control group, homocysteinemia was greater in those subjects with C677T and A1298C polymorphisms. Among diabetic subjects, those with the A1298C polymorphism had lower levels of homocysteine compared with individuals with C677T polymorphism. Conclusion: The MTHFR polymorphism (C677T and A1298C) resulted in different outcomes regarding homocysteinemia among individuals of each group (diabetic and control). These data suggest that metabolic factors inherent to diabetes influence homocysteine metabolism. Arq Bras Endocrinol Metab. 2012;56(7):429-34

Objective: To screen for mutations in AMH and AMHR2 genes in patients with persistent Mullerian duct syndrome (PMDS). Patients and method: Genomic DNA of eight patients with PMDS was obtained from peripheral blood leukocytes. Directed sequencing of the coding regions and the exon-intron boundaries of AMH and AMHR2 were performed. Results: The AMH mutations p.Arg95*, p.Arg123Trp, c.556-2A>G, and p. Arg502Leu were identified in five patients; and p.Gly323Ser and p.Arg407* in AMHR2 of two individuals. In silico analyses of the novel c.556-2A>G, p.Arg502Leu and p.Arg407* mutations predicted that they were harmful and were possible causes of the disease. Conclusion: A likely molecular etiology was found in the eight evaluated patients with PMDS. Four mutations in AMH and two in AMHR2 were identified. Three of them are novel mutations, c.556-2A>G, and p. Arg502Leu in AMH; and p.Gly323Ser in AMHR2. Arq Bras Endocrinol Metab. 2012;56(8):473-8

Objective: To estimate the frequency and describe the clinical characteristics and respective treatments of previous history of wheezing. Methods: Infants aged 6-23 months with upper respiratory tract complaints and reporting previous wheezing were followed-up retrospectively. Data were registered on a validated standardized form. Results: Out of 451 infants, 164 (36.4%; 95%CI: 31.9-41.0) had a report of prior history of wheezing, 148 (32.8%; 95%CI: 28.5-37.4) during the first year of life. The mean age at the first episode of wheezing was 5.3 +/- 3.9 months. Among those who had had their first episode before 12 months of age, 38.5% reported 3 to 6 episodes and 14.2% > 6 episodes. Mean age at first episode was lower for those with a >= 3 episodes in comparison with those with <= 2 episodes (3.2 +/- 2.7 vs. 5.7 +/- 2.5 months, p < 0.001). Conclusion: One third of the infants reported wheezing during the first year of life. The earlier the first episode occurs, the more frequently wheezing recurs.

We present here the clinical and molecular data of two patients with acromegaly treated with octreotide LAR after non-curative surgery, and who presented different responses to therapy. Somatostatin receptor type 2 and 5 (SSTR2 and SSTR5), and aryl hydrocarbon receptor-interacting protein (AIP) expression levels were analyzed by qPCR. In both cases, high SSTR2 and low SSTR5 expression levels were detected; however, only one of the patients achieved disease control after octreotide LAR therapy. When we analyzed AIP expression levels of both cases, the patient whose disease was controlled after therapy exhibited AIP expression levels that were two times higher than the patient whose disease was still active. These two cases illustrate that, although the currently available somatostatin analogs bind preferentially to SSTR2, some patients are not responsive to therapy despite high expression of this receptor. This difference could be explained by differences in post-receptor signaling pathways, including the recently described involvement of AIP. Arq Bras Endocrinol Metab. 2012;56(8):501-6

Maturity-onset diabetes of the young (MODY) is characterized by an autosomal dominant mode of inheritance, early onset of hyperglycemia, and defects of insulin secretion. MODY subtypes described present genetic, metabolic, and clinical differences. MODY 2 is characterized by mild asymptomatic fasting hyperglycemia, and rarely requires pharmacological treatment. Hence, precise diagnosis of MODY is important for determining management and prognosis. We report two heterozygous GCK mutations identified during the investigation of short stature. Case 1: a prepubertal 14-year-old boy was evaluated for constitutional delay of growth and puberty. During follow-up, he showed abnormal fasting glucose (113 mg/dL), increased level of HbA1c (6.6%), and negative beta-cell antibodies. His father and two siblings also had slightly elevated blood glucose levels. The mother had normal glycemia. A GCK heterozygous missense mutation, p.Arg191Trp, was identified in the proband. Eighteen family members were screened for this mutation, and 11 had the mutation in heterozygous state. Case 2: a 4-year-old boy investigated for short stature revealed no other laboratorial alterations than elevated glycemia (118 mg/dL); beta-cell antibodies were negative. His father, a paternal aunt, and the paternal grandmother also had slightly elevated glycemia, whereas his mother had normal glycemia. A GCK heterozygous missense mutation, p.Glu221Lys, was identified in the index patient and in four family members. All affected patients had mild elevated glycemia. Individuals with normal glycemia did not harbor mutations. GCK mutation screening should be considered in patients with chronic mild early-onset hyperglycemia, family history of impaired glycemia, and negative beta-cell antibodies. Monitoring the extent of and trends in multidrug-resistant tuberculosis (MDR-TB) is a priority of the Brazilian National Tuberculosis Control Programme. The current study aimed to estimate the incidence of MDR-TB, describe the profile of TB drug resistance in risk groups and examine whether screening for MDR-TB adhered to the recommended guidelines. A descriptive study that examined diagnosed cases of pulmonary TB was conducted in the city of Santos, Brazil, between 2000-2004. Of the 2,176 pulmonary TB cases studied, 671 (30.8%) met the criteria for drug sensitivity testing and, of these cases, 31.7% (213/671) were tested. Among the tested cases, 9.4% were resistant to one anti-TB drug and 15% were MDR. MDR was observed in 11.6% of 86 new TB cases and 17.3% of 127 previously treated cases. The average annual incidence of MDR-TB was 1.9 per 100,000 inhabitants-years. The extent of known MDR-TB in the city of Santos is high, though likely to be underestimated. Our study therefore indicates an inadequate adherence to the guidelines for MDR-TB screening and suggests the necessity of alternative strategies of MDR-TB surveillance.

Objectives: To assess QoL of obese patients in the Brazilian public healthcare system, before and after bariatric surgery, and to determine the appropriateness of the Moorehead-Ardelt Questionnaire II (M-A-QoLQII) compared with the Short-Form Health Survey (SF-36). Subjects and methods: Forty-one severe obese patients in a waiting-list, and 84 patients who underwent bariatric surgery were included. Correlations were tested and reliability determined by the Cronbach's coefficient. Results: BMI differed between the pre- and post-surgery groups (52.3 +/- 8.3 kg/m(2) vs. 32.5 +/- 6.4 kg/m(2), p < 0.001). The latter showed better scores in the SF-36 domains than in the pre-surgery. SF-36 and M-A-QoLQII categories were correlated (r = 0.53, 0.49 and 0.47, for vitality, mental health, and general health domains, p < 0.001). In the logistic regression, age, previous BMI, and loss of excess weight were associated with functional capacity. Conclusions:The outcomes of bariatric surgery obtained in a Brazilian public healthcare center were successful. M-A-QoLII represents a useful tool to assess surgery outcomes, including QoL. Objective: To assess the availability of food in relation to their degree of industrial processing and the types of food stores in the perimeters of elementary schools. Method: This is a cross-sectional study. 82 food stores located within a 500 m radius buffer of three public schools located in three distinct regions with different socioeconomic levels in the municipality of Santos, state of Sao Paulo, Brazil, were assessed. All streets within a 500-meter radius of the schools were covered, geographic coordinates were recorded and information about the stores and food items available were collected by direct observation and interview with store managers. Available food items were classified in relation to their degree of industrial processing as ultra-processed foods and minimally processed foods. Kernel's density maps were used to assess the degree of agglomeration of stores near the schools. Results: The stores that offered mostly ultra-processed foods were significantly closer to schools than those who offered mostly minimally processed foods. There was a significant difference between the availability of processed food in different types of stores and between the three regions assessed. Conclusions: The data found by this work evidences that children who attend the three public schools assessed are exposed to an environment that encourages the consumption of ultra-processed foods through easier access of these products in the studied stores.

Leprosy is a spectral disease exhibiting two polar sides, namely, lepromatous leprosy (LL) characterised by impaired T-cell responses and tuberculoid leprosy in which T-cell responses are strong. Proper T-cell activation requires signalling through costimulatory molecules expressed by antigen presenting cells and their ligands on T-cells. We studied the influence of costimulatory molecules on the immune responses of subjects along the leprosy spectrum. The expression of the costimulatory molecules was evaluated in in vitro-stimulated peripheral blood mononuclear cells of lepromatous and tuberculoid patients and healthy exposed individuals (contacts). We show that LL patients have defective monocyte CD86 expression, which likely contributes to the impairment of the antigen presentation process and to patients anergy. Accordingly, CD86 but not CD80 blockade inhibited the lymphoproliferative response to Mycobacterium leprae. Consistent with the LL anergy, there was reduced expression of the positive signalling costimulatory molecules CD28 and CD86 on the T-cells in these patients. In contrast, tuberculoid leprosy patients displayed increased expression of the negative signalling molecules CD152 and programmed death-1 (PD-1), which represents a probable means of modulating an exacerbated immune response and avoiding immunopathology. Notably, the contacts exhibited proper CD86 and CD28 expression but not exacerbated CD152 or PD-1 expression, suggesting that they tend to develop a balanced immunity without requiring immunosuppressive costimulatory signalling.

Objective: To describe the epidemiological aspects of childhood tuberculosis (TB) in a Brazilian reference hospital. Methods: This was a retrospective study (1999-2008) of 473 subjects (0-14 year olds) with confirmed TB, or with clinical improvement by the fourth month of treatment under the unit's care, including the review of medical records, monitoring reports and notifications by the TB unit. Results: Among 473 TB cases included in the study, positive tuberculin skin test was observed in 52%, history of contact with a patient with pulmonary tuberculosis in 66%, mostly intra-household, and with the father/stepfather most commonly involved; and disseminated TB in 22%. The result of HIV testing was obtained in 265 (56%) cases, being positive in 45 (17%). The diagnosis of TB was confirmed in 31% of cases, most frequently in children older than 5 years, with negative tuberculin skin test, and in disseminated forms. Of the 65 cultures positive for TB performed in the study, drug sensitivity testing to anti-TB drugs was done in 30 (46%) clinical samples, among which 10 (33%) were resistant to one or more anti-TB drugs, and 2 (0.8%) were multi-drug-resistant. Among patients with confirmed pulmonary TB, 31% did not meet the criteria for starting anti-TB treatment according to the scores of the Ministry of Health (<= 25 points). Conclusion: The high proportion of drug-resistant TB and co-infection with HIV identified in this study highlight the necessity to carry out additional studies in order to evaluate the impact of TB control activities on childhood TB.

The purpose of this article is to present a method which consists in the development of unit cell numerical models for smart composite materials with piezoelectric fibers made of PZT embedded in a non-piezoelectric matrix (epoxy resin). This method evaluates a globally homogeneous medium equivalent to the original composite, using a representative volume element (RVE). The suitable boundary conditions allow the simulation of all modes of the overall deformation arising from any arbitrary combination of mechanical and electrical loading. In the first instance, the unit cell is applied to predict the effective material coefficients of the transversely isotropic piezoelectric composite with circular cross section fibers. The numerical results are compared to other methods reported in the literature and also to results previously published, in order to evaluate the method proposal. In the second step, the method is applied to calculate the equivalent properties for smart composite materials with square cross section fibers. Results of comparison between different combinations of circular and square fiber geometries, observing the influence of the boundary conditions and arrangements are presented.
