OBJETIVO: Avaliar a percepção da fala de crianças deficientes auditivas com o aparelho de amplificação sonora individual (AASI) e sistema de frequência modulada (FM) em situações de ruído em campo livre e em sala de aula. MÉTODOS: Participaram 13 crianças deficientes auditivas entre 7 e 17 anos. Foi aplicado o Hearing in Noise Test (HINT) com AASI e com o FM. Também foi aplicado o questionário Avaliação do Sistema FM, respondido pelos professores das crianças, com o intuito de avaliar, individualmente, o desempenho da criança em diferentes situações auditivas somente com AASI e com o AASI e o sistema FM. RESULTADOS: Houve diferença para todas as situações com e sem FM no teste HINT. O mesmo aconteceu com os resultados do questionário, sendo que sem FM a pontuação foi sempre menor do que com FM, independentemente da condição. CONCLUSÃO: O uso de medidas subjetivas, como o questionário, é fundamental para determinar a eficácia da indicação dos dispositivos auxiliares para o deficiente auditivo. A efetividade do sistema FM pode ser observada pela "vantagem FM", que é a diferença média mínima de 10 dB encontrada nas avaliações de percepção da fala com e sem FM nas diferentes situações de ruído. Os benefícios encontrados na presente pesquisa com o uso do sistema FM na melhora da percepção da fala podem ser extrapolados não só para a sala de aula e para a legislação da educação inclusiva, mas também para atividades sociais e de lazer.

O implante coclear (IC) tem sido indicado para crianças deficientes auditivas de grau severo e/ou profundo que não tem benefício com o aparelho de amplificação sonora individual (AASI), e que apresentem família adequada e motivada para o uso do dispositivo, bem como condições adequadas de reabilitação na cidade de origem. Atualmente, a procura pelo IC também ocorre por pais surdos, fluentes na Língua Brasileira de Sinais (LIBRAS), que recorrem a este tratamento para oferecer outra realidade para seus filhos. O ambiente destas crianças é bilíngue, dado pela LIBRAS dos pais e pela linguagem oral dos familiares próximos, do fonoaudiólogo e da escola. Neste sentido, o presente estudo visou acompanhar quatro crianças deficientes auditivas implantadas, sendo duas crianças filhas de pais deficientes auditivos fluentes na LIBRAS (expostas a ambiente bilíngue) e duas crianças filhas de pais sem alterações auditivas (expostas a ambiente oral). Para tanto, as habilidades de audição e de aquisição da linguagem oral foram comparadas nas quatro crianças implantadas. Foi possível observar que as quatro crianças apresentaram habilidades auditivas e de linguagem semelhantes ao longo do primeiro ano de uso do IC. Contudo, a partir disto, as crianças inseridas em ambiente bilíngue apresentaram melhor desempenho auditivo e linguístico, comparado ao desenvolvimento das outras crianças. As crianças inseridas em ambiente bilíngue podem se beneficiar do IC, desenvolvendo habilidades auditivas e de linguagem similares às das crianças inseridas em ambiente oral. Ressalta-se que os benefícios do dispositivo são obtidos a partir de aspectos multifatoriais, e estudos mais aprofundados são necessários.

Devido aos avanços na tecnologia dos implantes cocleares e das estratégias de processamento, indivíduos com surdez grave a profunda puderam ouvir sons e reconhecer fala em diferentes graus. A variabilidade nos resultados audiológicos em portadores de surdez pós-lingual tem sido grande e a indicação para o implante coclear tem se estendido e inclui uma população cada vez maior. OBJETIVO: Avaliar em quais pacientes portadores de surdez pós-lingual o implante coclear traz benefício auditivo superior ao da prótese auditiva convencional. MATERIAIS E MÉTODOS: consulta a artigos científicos por busca no banco de dados SciELO, Cochrane, MEDLINE e LILACS-BIREME. Foram selecionadas publicações com força de evidência A ou B até a data da investigação, que comparassem aparelhos de amplificação sonora e implante coclear na população com surdez pós-lingual. Desenho do Estudo: revisão sistemática. RESULTADOS: Entre os 2169 artigos consultados, 11 trabalhos se mostram pertinentes ao tema e apresentaram força de evidência B. Seis estudos são do tipo coorte prospectivo, quatro são estudos transversais e um ensaio clinico. CONCLUSÃO: A avaliação dos benefícios obtidos por portadores de surdez pós-lingual reabilitados com o uso de implante coclear mostra que este dispositivo é efetivo e apresenta melhores resultados quando comparado aos aparelhos de amplificação sonora individual.

Apesar dos avanços tecnológicos empregados nas estratégias de processamento do sinal, um dos obstáculos ainda existentes são as lacunas de detalhes espectrais na informação elétrica transmitida. Considerando a sua importância na percepção da fala, pesquisadores investigaram mecanismos para otimizar o detalhamento espectral, por meio dos canais espectrais virtuais. A aplicação clínica desta técnica resultou em uma nova estratégia de processamento de sinal - a HiRes 120. OBJETIVO: Avaliar o desempenho auditivo de usuários de Implante Coclear com a HiRes 120. METODOLOGIA: Levantamento bibliográfico conduzido em base eletrônica de dados, com busca padronizada até o ano de 2011, utilizando-se palavras-chave específicas. Para a seleção e avaliação dos estudos científicos levantados na busca, foram estabelecidos critérios, contemplando os aspectos: tipo de estudo, participantes, intervenção adotada e avaliação dos resultados. CONCLUSÃO: As evidências científicas apontam uma melhora do desempenho auditivo nas situações de ruído com a estratégia HiRes 120, mas tal fato não ocorre nas situações de silêncio. A otimização da percepção da fala com o uso da estratégia está intimamente relacionada com a idade do usuário de implante coclear, com o tempo de privação sensorial e o tempo de aclimatização necessário para o aproveitamento das informações espectrais da estratégia.

INTRODUÇÃO: A maioria dos pacientes com perda auditiva, incluindo casos de perdas severas, é beneficiada com o uso de aparelho de amplificação sonora individual. Acredita-se que o implante coclear estabeleça melhores resultados na reabilitação de uma criança com perda auditiva nos casos em que a gravidade da deficiência torna os aparelhos acústicos convencionais incapazes de fornecer informação sonora adequada, já que estes necessitam de reserva coclear suficiente para que ocorra detecção acústica. OBJETIVO: Avaliar se em pacientes portadores de surdez pré-lingual o implante coclear traz benefício auditivo superior ao da prótese auditiva convencional. SÍNTESE DOS DADOS: Revisão sistemática realizada a partir de consulta a artigos científicos selecionados por busca no banco de dados SciELO, Cochrane, MEDLINE e LILACS-BIREME. Entre os 2169 artigos consultados, 12 trabalhos se mostram pertinentes ao tema e apresentaram força de evidência B. Entre os desenhos de estudos analisados na revisão, 7 são do tipo coorte prospectivo e 5 são estudos transversais. CONCLUSÃO: O implante coclear demonstrou, através de diversos estudos, ser atualmente, a melhor alternativa para os casos de perdas auditivas bilaterais severas ou profundas, atingindo resultados superiores em percepção e desenvolvimento de fala em crianças pré-linguais quando comparados aos aparelhos de amplificação sonora convencionais.

INTRODUÇÃO: Quanto mais precoce o diagnóstico e intervenção da deficiência auditiva, menor será o impacto para o desenvolvimento das habilidades cognitivas, auditivas e de linguagem da criança. OBJETIVO: Caracterizar a idade no diagnóstico e no início da intervenção da perda auditiva e o acompanhamento de crianças atendidas em um serviço público de saúde auditiva brasileiro - Espaço Reouvir do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. MÉTODO: Estudo retrospectivo com informações de 166 prontuários de crianças no que se refere a: gênero; etiologia, tipo, grau e lateralidade da deficiência auditiva; idade do diagnóstico e da adaptação do Aparelho de Amplificação Sonora Individual (AASI) e acompanhamento no serviço. RESULTADOS: A amostra foi composta por 56% homens e 44% mulheres. A etiologia predominante foi a de origem multifatorial. A perda auditiva do tipo neurossensorial ocorreu em 88,6% dos casos. O grau de perda auditiva moderado foi o de maior ocorrência (30,7%), simetria entre as orelhas foi encontrada em 69,9% dos casos e perda auditiva unilateral em 2,4%. A idade média no diagnóstico foi de 5,46 anos e na intervenção de 6,86 anos. Um total de 96,98% das crianças já havia completado o processo de adaptação e 78,32% permaneciam em acompanhamento. CONCLUSÃO: O Programa Reouvir - HCFMUSP ainda recebe crianças, seja para o diagnóstico e/ou para a intervenção, de maneira tardia. Entretanto, ainda assim faz-se possível à realização do acompanhamento de um número significativo de crianças usuárias de AASI, possibilitando um processo de adaptação mais efetivo.

This study had the aim to investigate the auditory and communicative abilities of children diagnosed with Auditory Neuropathy Spectrum Disorder due to mutation in the Otoferlin gene. It is a descriptive and qualitative study in which two siblings with this diagnosis were assessed. The procedures conducted were: speech perception tests for children with profound hearing loss, and assessment of communication abilities using the Behavioral Observation Protocol. Because they were siblings, the subjects in the study shared family and communicative context. However, they developed different communication abilities, especially regarding the use of oral language. The study showed that the Auditory Neuropathy Spectrum Disorder is a heterogeneous condition in all its aspects, and it is not possible to make generalizations or assume that cases with similar clinical features will develop similar auditory and communicative abilities, even when they are siblings. It is concluded that the acquisition of communicative abilities involves subjective factors, which should be investigated based on the uniqueness of each case.

PURPOSE: To verify the interference of visual stimuli in written production of deaf signers with no complaints regarding reading and writing. METHODS: The research group consisted of 12 students with education between the 4th and 5th grade of elementary school, with severe or profound sensorineural hearing loss, users of LIBRAS and with alphabetical writing level. The evaluation was performed with pictures in a logical sequence and an action picture. The analysis used the communicative competence criteria. RESULTS: There were no differences in the writing production of the subjects for both stimuli. In all texts there was no title and punctuation, verbs were in the infinitive mode, there was lack of cohesive links and inclusion of created words. CONCLUSION: The different visual stimuli did not affect the production of texts.

Hepatic insulin resistance is the major contributor to fasting hyperglycemia in type 2 diabetes. The protein kinase Akt plays a central role in the suppression of gluconeogenesis involving forkhead box O1 (Foxo1) and peroxisome proliferator-activated receptor gamma co-activator 1 alpha (PGC-1a), and in the control of glycogen synthesis involving the glycogen synthase kinase beta (GSK3 beta) in the liver. It has been demonstrated that endosomal adaptor protein APPL1 interacts with Akt and blocks the association of Akt with its endogenous inhibitor, tribbles-related protein 3 (TRB3), improving the action of insulin in the liver. Here, we demonstrated that chronic exercise increased the basal levels and insulin-induced Akt serine phosphorylation in the liver of diet-induced obese mice. Endurance training was able to increase APPL1 expression and the interaction between APPL1 and Akt. Conversely, training reduced both TRB3 expression and TRB3 and Akt association. The positive effects of exercise on insulin action are reinforced by our findings that showed that trained mice presented an increase in Foxo1 phosphorylation and Foxo1/PGC-1a association, which was accompanied by a reduction in gluconeogenic gene expressions (PEPCK and G6Pase). Finally, exercised animals demonstrated increased at basal and insulin-induced GSK3 beta phosphorylation levels and glycogen content at 24?h after the last session of exercise. Our findings demonstrate that exercise increases insulin action, at least in part, through the enhancement of APPL1 and the reduction of TRB3 expression in the liver of obese mice, independently of weight loss. BACKGROUND & AIMS: Homozygous loss of function mutations in interleukin-10 (IL10) and interleukin-10 receptors (IL10R) cause severe infantile (very early onset) inflammatory bowel disease (IBD). Allogeneic hematopoietic stem cell transplantation (HSCT) was reported to induce sustained remission in 1 patient with IL-10R deficiency. We investigated heterogeneity among patients with very early onset IBD, its mechanisms, and the use of allogeneic HSCT to treat this disorder. METHODS: We analyzed 66 patients with early onset IBD (younger than 5 years of age) for mutations in the genes encoding IL-10, IL-10R1, and IL-10R2. IL-10R deficiency was confirmed by functional assays on patients' peripheral blood mononuclear cells (immunoblot and enzyme-linked immunosorbent assay analyses). We assessed the therapeutic effects of standardized allogeneic HSCT. RESULTS: Using a candidate gene sequencing approach, we identified 16 patients with IL-10 or IL-10R deficiency: 3 patients had mutations in IL-10, 5 had mutations in IL-10R1, and 8 had mutations in IL-10R2. Refractory colitis became manifest in all patients within the first 3 months of life and was associated with perianal disease (16 of 16 patients). Extraintestinal symptoms included folliculitis (11 of 16) and arthritis (4 of 16). Allogeneic HSCT was performed in 5 patients and induced sustained clinical remission with a median follow-up time of 2 years. In vitro experiments confirmed reconstitution of IL-10R-mediated signaling in all patients who received the transplant. CONCLUSIONS: We identified loss of function mutations in IL-10 and IL-10R in patients with very early onset IBD. These findings indicate that infantile IBD patients with perianal disease should be screened for IL-10 and IL-10R deficiency and that allogeneic HSCT can induce remission in those with IL-10R deficiency.

Aim This study aimed to investigate whether chronic antigen-induced arthritis (AIA) influences infection-induced periodontitis (PD) in mice and whether PD modifies the clinical course of AIA. The contribution of anti-TNF-a therapy was also evaluated. Materials and methods The PD was induced in C57BL/6 mice by oral infection with Aggregatibacter actinomycetemcomitans. AIA was induced after infection. Anti-TNF-a and chlorhexidine therapies were used to investigate the role of TNF-a and oral infection on PD and AIA interaction. Maxillae, knee joints, lymph nodes and serum samples were used for histomorphometric, immunoenzymatic and/or real time-PCR analyses. Results Antigen-induced arthritis exacerbated alveolar bone loss triggered by PD infection. In contrast, PD did not influence AIA in the evaluated time-points. PD exacerbation was associated with enhanced production of IFN-? in maxillae and expression of the Th1 transcription factor tBET in submandibular lymph nodes. Increased serum levels of IL-6 and C-reactive protein were also detected. Anti-TNF-a and antiseptic therapies prevented the development and exacerbation of infectious-PD. Anti-TNF-a therapy also resulted in reduced expression of IFN-?, TNF-a and IL-17 in maxillae. Conclusions Altogether, the current results indicate that the exacerbation of infection-induced PD by arthritis is associated with an alteration in lymphocyte polarization pattern and increased systemic immunoreactivity. This process was ameliorated by anti-TNF-a and antiseptic therapies.

Background: RNA interference (RNAi) is a post-transcriptional gene silencing process in which double-stranded RNA (dsRNA) directs the degradation of a specific corresponding target mRNA. The mediators of this process are small dsRNAs of approximately 21 to 23 bp in length, called small interfering RNAs (siRNAs), which can be prepared in vitro and used to direct the degradation of specific mRNAs inside cells. Hence, siRNAs represent a powerful tool to study and control gene and cell function. Rapid progress has been made in the use of siRNA as a means to attenuate the expression of any protein for which the cDNA sequence is known. Individual siRNAs can be chemically synthesized, in vitro-transcribed, or expressed in cells from siRNA expression vectors. However, screening for the most efficient siRNAs for post-transcriptional gene silencing in cells in culture is a laborious and expensive process. In this study, the effectiveness of two siRNA production strategies for the attenuation of abundant proteins for DNA repair were compared in human cells: (a) the in vitro production of siRNA mixtures by the Dicer enzyme (Diced siRNAs); and (b) the chemical synthesis of very specific and unique siRNA sequences (Stealth RNai (TM)). Materials, Methods & Results: For in vitro-produced siRNAs, two segments of the human Ku70 (167 bp in exon 5; and 249 bp in exon 13; NM001469) and Xrcc4 (172 bp in exon 2; and 108 bp in exon 6; NM003401) genes were chosen to generate dsRNA for subsequent "Dicing" to create mixtures of siRNAs. The Diced fragments of siRNA for each gene sequence were pooled and stored at -80 degrees C. Alternatively, chemically synthesized Stealth siRNAs were designed and generated to match two very specific gene sequence regions for each target gene of interest (Ku70 and Xrcc4). HCT116 cells were plated at 30% confluence in 24- or 6-well culture plates. The next day, cells were transfected by lipofection with either Diced or Stealth siRNAs for Ku70 or Xrcc4, in duplicate, at various doses, with blank and sham transfections used as controls. Cells were harvested at 0, 24, 48, 72 and 96 h post-transfection for protein determination. The knockdown of specific targeted gene products was quantified by Western blot using GAPDH as control. Transfection of gene-specific siRNA to either Ku70 or Xrcc4 with both Diced and Stealth siRNAs resulted in a down regulation of the targeted proteins to approximately 10 to 20% of control levels 48 h after transfection, with recovery to pre-treatment levels by 96 h. Discussion: By transfecting cells with Diced or chemically synthesized Stealth siRNAs, Ku70 and Xrcc4, two highly expressed proteins in cells, were effectively attenuated, demonstrating the great potential for the use of both siRNA production strategies as tools to perform loss of function experiments in mammalian cells. In fact, down-regulation of Ku70 and Xrcc4 has been shown to reduce the activity of the non-homologous end joining DNA pathway, a very desirable approach for the use of homologous recombination technology for gene targeting or knockout studies. Stealth RNAi (TM) was developed to achieve high specificity and greater stability when compared with mixtures of enzymatically-produced (Diced) siRNA fragments. In this study, both siRNA approaches inhibited the expression of Ku70 and Xrcc4 gene products, with no detectable toxic effects to the cells in culture. However, similar knockdown effects using Diced siRNAs were only attained at concentrations 10-fold higher than with Stealth siRNAs. The application of RNAi technology will expand and continue to provide new insights into gene regulation and as potential applications for new therapies, transgenic animal production and basic research.

It is well known that endocannabinoids play an important role in the regulation of food intake and body weight. Endocannabinoids and cannabinoid receptors are found in the hypothalamus and brainstem, which are central areas involved in the control of food intake and energy expenditure. Activation of these areas is related to hypophagia observed during inflammatory stimulus. This study investigated the effects of cannabinoid (CB1) receptor blockade on lipopolysaccharide (LPS)-induced hypophagia. Male Wistar rats were pretreated with rimonabant (10 mg/kg, by gavage) or vehicle; 30 min later they received an injection of either LPS (100 mu g/kg, intraperitoneal) or saline. Food intake, body weight, corticosterone response, CRF and CART mRNA expression, Fos-CRF and Fos-alpha-MSH immunoreactivity in the hypothalamus and Fos-tyrosine hydroxylase (TH) immunoreactivity in the brainstem were evaluated. LPS administration decreased food intake and body weight gain and increased plasma corticosterone levels and CRF mRNA expression in the PVN. We also observed an increase in Fos-CRF and Fos-TH double-labeled neurons after LPS injection in vehicle-pretreated rats, with no changes in CART mRNA or Fos-alpha-MSH immunoreactive neurons in the ARC. In saline-treated animals, rimonabant pretreatment decreased food intake and body weight gain but did not modify hormone response or Fos expression in the hypothalamus and brainstem compared with vehicle-pretreated rats. Rimonabant pretreatment potentiated LPS-induced hypophagia, body weight loss and Fos-CRF and Fos-TH expressing neurons. Rimonabant did not modify corticosterone, CRF mRNA or Fos-alpha-MSH responses in rats treated with LPS. These data suggest that the endocannabinoid system, mediated by CB1 receptors, modulates hypothalamic and brainstem circuitry underlying the hypophagic effect during endotoxemia to prevent an exaggerated food intake decrease. The Pierre Auger Observatory is exploring the potential of the radio detection technique to study extensive air showers induced by ultra-high energy cosmic rays. The Auger Engineering Radio Array (AERA) addresses both technological and scientific aspects of the radio technique. A first phase of AERA has been operating since September 2010 with detector stations observing radio signals at frequencies between 30 and 80 MHz. In this paper we present comparative studies to identify and optimize the antenna design for the final configuration of AERA consisting of 160 individual radio detector stations. The transient nature of the air shower signal requires a detailed description of the antenna sensor. As the ultra-wideband reception of pulses is not widely discussed in antenna literature, we review the relevant antenna characteristics and enhance theoretical considerations towards the impulse response of antennas including polarization effects and multiple signal reflections. On the basis of the vector effective length we study the transient response characteristics of three candidate antennas in the time domain. Observing the variation of the continuous galactic background intensity we rank the antennas with respect to the noise level added to the galactic signal.

This study evaluated the impact of a participatory program to reduce noise in a neonatal intermediate care unit of a university hospital. A time-series quasi-experimental design was used, in which sound pressure levels were measured before and after the intervention was implemented using the Quest-400 dosimeter. Non-parametric statistical tests were used to compare noise with the level of significance fixed at 5%. Results showed significant reduction of sound pressure levels in the neonatal unit after the intervention program was implemented (p<0.0001). The average Leq before the intervention was 62.5dBA and was reduced to 58.8dBA after the intervention. A reduction of 7.1dBA in the average Lmax(from 104.8 to 87.7dBA) and of 30.6dBA in the average Lpeak(from 138.1 to 107.5dBA) was observed. The program was proven to be effective in significantly reducing noise levels in the neonatal unit, although levels were still more intense than recommended.
