CONTEXT: Erythema induratum of Bazin (EIB) is considered to be a tuberculid reaction and consists of recurrent painful nodules. The differential diagnosis includes diseases like nodular vasculitis, perniosis, polyarteritis nodosa and erythema nodosum. CASE REPORT: We report the case of a woman with EIB who developed Addison's disease during treatment with anti-tuberculosis drugs with good response to glucocorticoid replacement. The diagnosis was obtained through the clinical picture, positive tuberculin test and positive BCG (bacillus Calmette-Guérin) test on the histological sample. Anti-tuberculosis drugs and glucocorticoid replacement led to disappearance of the signs and symptoms. CONCLUSIONS: This is the first description of an association between EIB and Addison's disease. It should be borne in mind that tuberculosis is an important etiological factor for Addison's disease.

OBJECTIVE: To compare low and high MELD scores and investigate whether existing renal dysfunction has an effect on transplant outcome. METHODS: Data was prospectively collected among 237 liver transplants (216 patients) between March 2003 and March 2009. Patients with cirrhotic disease submitted to transplantation were divided into three groups: MELD > 30, MELD < 30, and hepatocellular carcinoma. Renal failure was defined as a ± 25% decline in estimated glomerular filtration rate as observed 1 week after the transplant. Median MELD scores were 35, 21, and 13 for groups MELD > 30, MELD < 30, and hepatocellular carcinoma, respectively. RESULTS: Recipients with MELD > 30 had more days in Intensive Care Unit, longer hospital stay, and received more blood product transfusions. Moreover, their renal function improved after liver transplant. All other groups presented with impairment of renal function. Mortality was similar in all groups, but renal function was the most important variable associated with morbidity and length of hospital stay. CONCLUSION: High MELD score recipients had an improvement in the glomerular filtration rate after 1 week of liver transplantation.

PURPOSE: Chronic renal insufficiency (CRI) is the last stage of a chronic renal condition in which the kidney loses its filtration and endocrine functions. Chronic endocrine hypofunction causes generalized damage to the body known as Uremic Syndrome, which affects the central nervous system as well as the cardiovascular, hematologic, dermatologic, ophthalmic, endocrine, respiratory, gastrointestinal and skeletal systems. The present study reports the case of a female patient with CRI who presented facial osteodystrophy of the osteitis fibrosa type, and highlights the main features of this condition. CASE DESCRIPTION: A 24-year old, female, Caucasian patient presented chronic glomerulonephritis recurrence and lost the transplanted kidney five years before, undergoing arteriovenous fistula hemodialysis three times a week. She presented swelling of the left masseter area with a hard consistency on palpation, covered by intact skin, swelling at the bottom of the left atrium, with a hard consistency on palpation, a mucosa-like color and absence of inflammation signs, suggesting expansive bone lesions on the face. These features were compatible with hyperparathyroidism brown tumor and/or osteodystrophy. The CT scan showed expansive bone lesions of heterogeneous appearance on the left jaw, maxilla/nasal floor, and right frontotemporal suture areas. The clinical and histopathological characteristics of the lesion, in association with PHT hormone high serum levels led to renal osteodystrophy diagnosis. The patient was referred to the nephrology services. CONCLUSION: Osteodystrophic bone alterations have a high prevalence in renal disease patients, and the dentist must take these alterations into consideration in bone lesion diagnosis for this specific group of patients.

Analisar práticas alimentares de crianças desnutridas menores de dois anos. Estudo exploratório e descritivo, com análise qualitativa dos dados, realizado a partir de observação participante e entrevistas. Participaram 42 sujeitos. Os dados foram submetidos à análise temática. Ao explorar as práticas alimentares dessas crianças, os temas que emergiram foram: quem prepara a alimentação e o que é preparado; como são preparados os alimentos; a rotina alimentar das crianças; onde e como as crianças são servidas. A alimentação era pouco variada; láctea, no café da manhã e lanches, e no almoço com alimentos como arroz, batata, feijão e, às vezes, carnes. Frutas e hortaliças eram escassas, e alimentos industrializados estavam presentes em todos os domicílios. Foi possível apreender o cotidiano das práticas alimentares, suas inadequações, insuficiência de alimentos, higiene precária dos alimentos complementares, influência das avós, ambiente inadequado às refeições e situação de vida das famílias.

INTRODUÇÃO: Diversas alterações anatômicas e funcionais são observadas nos indivíduos portadores de fissuras labiopalatinas, dentre elas anormalidades estruturais da musculatura orofaríngea, que podem causar prejuízo da função velofaríngea e, consequentemente, de funções como sucção, deglutição e fala. O presente estudo teve por objetivo avaliar a presença de fraca pressão aérea intraoral na fala de indivíduos com fissura palatina submetidos a palatoplastia primária em tempo adequado e tardiamente, e comparar se o momento do procedimento cirúrgico pode acarretar maior ocorrência do distúrbio. A hipótese é de que os indivíduos submetidos a palatoplastia primária tardiamente apresentam maior ocorrência de fraca pressão aérea intraoral em comparação àqueles que realizaram a palatoplastia primária no momento adequado. MÉTODO: Participaram do estudo 37 indivíduos de ambos os sexos, com diagnóstico de fissura de palato associada ou não à fissura de lábio, divididos em dois grupos: 1) grupo precoce (GP), composto por 22 pacientes submetidos a palatoplastia primária até o 2º ano de vida; e 2) grupo tardio (GT), composto por 15 pacientes submetidos a palatoplastia primária tardiamente, após o período de aquisição dos sons da fala. Todos os participantes foram submetidos a rotina de avaliação fonoaudiológica com intervalo de, no mínimo, 3 meses de pós-operatório. Dentre os parâmetros avaliados encontra-se a análise da fraca pressão aérea intraoral, que foi a variável considerada para este estudo. RESULTADOS: A análise dos dados possibilitou observar maior frequência de fraca pressão aérea intraoral no GT (33%) em comparação ao GP (18%). Entretanto, tal diferença não foi estatisticamente significante (P = 0,44). CONCLUSÕES: A hipótese do estudo foi rejeitada. A presença de fraca pressão aérea intraoral foi observada na fala dos dois grupos estudados, não sendo a idade na ocasião da palatoplastia primária um fator determinante.

Objetivo: Verificar os efeitos da remediação neuropsicológica em um sujeito com Síndrome Velocardiofacial. Método: Participou do estudo um sujeito do sexo masculino, 13 anos de idade, queixa de baixo rendimento escolar, diagnosticado com Síndrome Velocardiofacial (Del22q11.2) e fissura de palato submucosa. Na avaliação pré e pós programa remediativo utilizou-se: BANI-TS; Matrizes Progressivas, Teste Gestáltico Visomotor; Escala de Inteligência Wechsler para Crianças- WISC-III e o Teste Wisconsin de Classificação de Cartas. O programa de remediação neuropsicológica, com ênfase nas habilidades de execução, atenção, organização perceptual utilizou do delineamento comportamental, treino sistemático de ensino-aprendizagem viso-construtivas, e estratégias diferenciadas na tarefa. Resultado: No pré-teste, as habilidades intelectuais mostraram classificação rebaixado à idade. Na prova cognitiva verbal (QIV=91) apresentou classificação média e execução (QI = 71), limítrofe. Os índices atencionais e de processamento da informação indicaram perfis limítrofes e os de organização perceptual, deficientes. Na avaliação gráfico-percepto-motora obteve resultados inferiores a 6 anos. Na pós-testagem, as habilidades intelectuais mantiveram classificação rebaixada, porém, com aumento do número de respostas corretas. Na escala de execução (QIE) os resultados foram na média, com escore elevado em 77%; a organização perceptual elevou-se para nível médio-inferior. Os resultados percepto-motores demonstraram aumento da pontuação, de 06 para 07 anos. Conclusão: Os resultados obtidos no presente estudo de caso demonstraram que programas remediativos, focados em habilidades específicas, pode ser uma opção a mais na reabilitação de sujeitos com fissura de palato, com queixas na aprendizagem.

Primary lung lymphoma is a rare entity accounting for approximately 0.3% of all primary neoplasia of the lung and includes diffuse large B-cell lymphoma (DLBL) and lymphomatoid granulomatosis (LYG). Considering that clinical features may be similar, whereas epidemiology, morphology, and radiological features are different, the authors report a case of a middle-aged man who presented multiple pulmonary nodules in the lower lobes and groundglass opacities scattered bilaterally on computed tomography. Clinically, he presented a consumptive syndrome with respiratory failure and pleurisy, which progressed until death. The autopsy findings were consistent with lymphomatoid granulomatosis (LYG) grade 3/ diffuse large B-cell lymphoma (DLBL). The authors call attention to the difficulty of establishing an accurate diagnosis, mainly when the demonstration of EBV-infected atypical B-cells fails.

Renovascular hypertension induced by 2 Kidney-1 Clip (2K-1C) is a renin-angiotensin-system (RAS)-dependent model, leading to renal vascular rarefaction and renal failure. RAS inhibitors are not able to reduce arterial pressure (AP) and/or preserve the renal function, and thus, alternative therapies are needed. Three weeks after left renal artery occlusion, fluorescently tagged mesenchymal stem cells (MSC) (2×10(5) cells/animal) were injected weekly into the tail vein in 2K-1C hypertensive rats. Flow cytometry showed labeled MSC in the cortex and medulla of the clipped kidney. MSC prevented a further increase in the AP, significantly reduced proteinuria and decreased sympathetic hyperactivity in 2K-1C rats. Renal function parameters were unchanged, except for an increase in urinary volume observed in 2K-1C rats, which was not corrected by MSC. The treatment improved the morphology and decreased the fibrotic areas in the clipped kidney and also significantly reduced renal vascular rarefaction typical of 2K-1C model. Expression levels of IL-1β, TNF-α angiotensinogen, ACE, and Ang II receptor AT1 were elevated, whereas AT2 levels were decreased in the medulla of the clipped kidney. MSC normalized these expression levels. In conclusion, MSC therapy in the 2K-1C model (i) prevented the progressive increase of AP, (ii) improved renal morphology and microvascular rarefaction, (iii) reduced fibrosis, proteinuria and inflammatory cytokines, (iv) suppressed the intrarenal RAS, iv) decreased sympathetic hyperactivity in anesthetized animals and v) MSC were detected at the CNS suggesting that the cells crossed the blood-brain barrier. This therapy may be a promising strategy to treat renovascular hypertension and its renal consequences in the near future.

The paraventricular nucleus (PVN) of the hypothalamus plays an important role in the regulation of sympathetic nerve activity, which is significantly elevated in chronic heart failure (CHF). Fractalkine (FKN) and its cognate receptor, CX3CR1, are constitutively expressed in the central nervous system, but their role and physiological significance are not well known. The aims of the present study were to determine whether FKN plays a cardiovascular role within the PVN and to investigate how the actions of FKN might be altered in CHF. We show that both FKN and CX3CR1 are expressed on neurons in the PVN of rats, suggesting that they may have a physiological function in this brain nucleus. Unilateral microinjection of FKN directly into the PVN of anaesthetized rats elicited a significant dose-related decrease in blood pressure (1.0 nmol, -5 ± 3 mmHg; 2.5 nmol, -13 ± 2 mmHg; 5.0 nmol, -22 ± 3 mmHg; and 7.5 nmol, -32 ± 3 mmHg) and a concomitant increase in heart rate (1.0 nmol, 6 ± 3 beats min(-1); 2.5 nmol, 11 ± 3 beats min(-1); 5 nmol, 18 ± 4 beats min(-1); and 7.5 nmol, 27 ± 5 beats min(-1)) compared with control saline microinjections. In order to determine whether FKN signalling is altered in rats with CHF, we first performed quantitative RT-PCR and Western blot analysis and followed these experiments with functional studies in rats with CHF and sham-operated control rats. We found a significant increase in CX3CR1 mRNA and protein expression, as determined by quantitative RT-PCR and Western blot analysis, respectively, in the PVN of rats with CHF compared with sham-operated control rats. We also found that the blood pressure effects of FKN (2.5 nmol in 50 nl) were significantly attenuated in rats with CHF (change in mean arterial pressure, -6 ± 3 mmHg) compared with sham-operated control rats (change in mean arterial pressure, -16 ± 6 mmHg). These data suggest that FKN and its receptor, CX3CR1, modulate cardiovascular function at the level of the PVN and that the actions of FKN within this nucleus are altered in heart failure
