Background: Congenital heart defects include a wide range of inborn malformations. Depending on the defect, the life expectancy of a newborn with cardiac anomaly varies from a few days to a normal life span. In most instances surgery, is the only treatment available. The late results of surgery have not been comprehensively investigated. Aims: Mortality, morbidity and the life situation of all Finnish patients who had been operated on for congenital heart defect during childhood were investigated. Methods: Patient and surgical data were gathered from all hospitals that had performed heart surgeries on children. Late mortality and survival data were obtained from the population registry, and the causes of deaths from Statistics Finland. Morbidity of patients operated on during 1953-1989 was assessed by the usage of medicines. The pharmacotherapy data of patients and controls were obtained from the Social Insurance Institute. The life situation of patients was surveyed by mailed questionnaire. Survival, causes of deaths and life situation of patients were compared with those of the general population. Results: A total of 7240 cardiac operations were performed on 6461 children during the first 37 years of cardiac surgery (1953-1989). The number of procedures constantly rose during this period, and the increase continued in later years. The patient material varied over time, as more defects became surgically treatable. During 1953-1989 the operative mortality (death within 30 days of surgery) was 6.9%. In the 1990s a slight rise occurred in early mortality, as increasingly complicated patients were surgically treated. During 2000-2003 practically no defects were beyond the operative range. Thus, the operative mortality of 4.4% was excellent, decreasing even further to 2.0% in 2004-2007. The overall 45-year survival of patients operated on in 1953-1989 was 78%, and the corresponding figure for the general population was 93%. Survival depended on the defect, being worst among patients with univentricular heart. Late survival was also better during the 1990s and at the beginning of the 21st century. Of the 6028 early survivors, 592 died late (>30 days) after surgery. A total of 397 deaths (67%) were related and 185 (31%) unrelated to congenital heart defect. The cause of death was unknown in 10 cases. Of those 5774 patients who survived their first operation and had complete follow-up, 16% were operated on several times. Seventeen percent of patients used medicines for cardiac symptoms (heart failure, arrhythmia, hypertension and coronary disease). Patients risk of using cardiac medicines was 2.16 (Cl 1.97-2.37) times higher than that of controls. Patients also had more genetic syndromes and mental retardation and more often used medicines for asthma and epilepsy. Adult patients who had been operated on as children had coped surprisingly well with their defects. Their level of education was similar and their employment level even higher than expected, and they were living in a steady relationship as often as the general population. Conclusions: Cardiac surgery developed rapidly, and nowadays practically all defects can be treated. The overall survival of all operated patients was 78%, 16% less than that of the general population. However, it was significantly better than the anticipated natural survival. However, many patients had health problems; 16% needed reoperations and 17% cardiac medicines to maintain their condition. Most of the patients assessed their general health as good and lived a normal life.

Having to do with residential areas, geographical image research in Finland has concentrated mainly on those areas with a relatively negative image, such as eastern parts of Helsinki. However, Kumpula and Toukola are former working class residential areas whose image nowadays is mainly positive. This research aims at understanding the process through which their image has gradually come to be that way. Theoretical background of the research relies on human geography and it s viewpoints on places, spaces and areas. Areas, in this research, are understood to be founded on discursive processes that form meanings in societies. This approach is useful because it provides a way to research newspapers and to see how they affect the society. In addition I lean on Sirpa Tani s research on place images to study image and it s formation process. Her point of view covers especially well the effect of media on images and their formation. Articles published in Helsingin Sanomat and Ilta-Sanomat between the years 1963 and 1999 form the data of the research. Methodologically I proceeded by using content analysis to see what kind of topics have been dominating the news feed from Kumpula and Toukola. Content analysis was followed by discourse analysis, which allowed me to focus on the ways of speaking about and representing Kumpula and Toukola. Discourse analysis also reveals whose viewpoint is being represented in media when it comes to publishing news from these parts of the city. It is clearly visible from the results of this research that the image of Kumpula and Toukola has gone through a significant change between 1963 and 1999. In the 1960s discussion in newspapers was dominated by the need for more effective city planning. This meant that Kumpula and Toukola were under a demolition threat in order for the city to built more effectively on those areas. At the same time there was discussion about wooden houses that were built in Kumpula and Toukola right after the second World War. Those houses were in a poor condition, it was even said in the newspapers that people were living in slum-like conditions in them. By the 1980s the image of Kumpula and Toukola gradually started to change. At this time gentrification process was affecting the areas and well-educated working force moved to Kumpula and Toukola. Already in the beginning of 1990s the image of the areas was highly positive. Throughout this decade newspapers published news on Kumpula and Toukola that commented favorably on the atmosphere and the feeling of togetherness among the residents. In addition Kumpula village carnivals, that were first held in 1991, brought a lot of positive publicity to the areas. This research has revelead that especially the active participationg of the residents to promote joint causes has positively affected the image of Kumpula and Toukola. Since the 1960s fighting for the preservation of the areas has provided a reason for a stronger feeling of communality and identifying in the community. This feeling of togetherness in a community has carried all the way to the 1990s, when the areas, having been affected by gentrification, could make good use of the positive image in order to promote joint causes. Työ on laajaan dokumenttiaineistoon, haastatteluihin ja havaintoihin perustuva living lab - tyyppisen tuotekehitysprosessin yksityiskohtainen kuvaus. Hankehistoriallisessa tapaustutkimuksessa on käytetty historiantutkimuksen menetelmiä. Vanhustenkeskuksessa toteutetun living lab -hankkeen lopputuloksena syntyi vanhusten liikkeitä monitoroiva hoitotyön apuväline: turvalattia. Living lab -tyyppisessä tuotekehittämisessä on keskeistä teknologian kehittäminen tuotteen todellisessa käyttöympäristössä siten, että käyttäjät osallistuvat aktiivisesti kehitystyön kaikkiin vaiheisiin. Tutkimuksessa hankkeen etenemistä ja turvalattiateknologian rakentumista tarkastellaan toimijaverkostoteorian ja yhteisöllisen oppimisen käsitteen tarjoamista näkökulmista. Tutkimuksessa kiinnitetään erityisesti huomiota siihen, minkälaisia intressejä teknologian kehittämiseen osallistuvilla ryhmillä on ja minkälaisia konflikteja näiden intressien pohjalta syntyy. Lisäksi tarkastelun keskiössä on käyttäjien ja tuotekehittäjien välinen oppiminen, jonka mahdollistaminen ja edistäminen on living lab -kehittämisen päämäärä. Tutkimuksen keskeinen johtopäätös on, että käyttäjiltä saatu palaute on välttämätöntä käyttökelpoisen sosiaali- ja terveydenhuollon alan teknologian aikaansaamiseksi, ja turvalattian tapauksessa käyttäjäpalaute myös uudelleensuuntasi teknologisen järjestelmän innovaatioprosessia. Käyttäjien ja tuotekehittäjien yhteistyön myötä turvalattiaan kehitettiin joukko uusia toiminnallisuuksia ja käsitys tuotteen ydinominaisuuksista sekä hyötylupauksesta muuttuivat. Turvalattian tapaus osoittaa lisäksi yhteisöllisen oppimisen merkityksen innovaatioprosessissa sekä living lab -tyyppisen tuotekehittämisen potentiaalin näiden oppimisprosessien mahdollistajana ja kiihdyttäjänä. Tapaus myös valottaa sitä, kuinka monimutkainen toimintakenttä vanhustenhuolto on teknologian kehittämisen ja sen käyttöönoton näkökulmasta. Näyttäisi kuitenkin siltä, että living lab -metodologian avulla voidaan ylittää joitakin geronteknologian kehittämiseen ja myös käyttöönottoon liittyviä haasteita.

Many economic events involve initial observations that substantially deviate from long-run steady state. Initial conditions of this type have been found to impact diversely on the power of univariate unit root tests, whereas the impact on multivariate tests is largely unknown. This paper investigates the impact of the initial condition on tests for cointegration rank. We compare the local power of the widely used likelihood ratio (LR) test with the local power of a test based on the eigenvalues of the companion matrix. We find that the power of the LR test is increasing in the magnitude of the initial condition, whereas the power of the other test is decreasing. The behaviour of the tests is investigated in an application to price convergence.

Työ on laajaan dokumenttiaineistoon, haastatteluihin ja havaintoihin perustuva living lab - tyyppisen tuotekehitysprosessin yksityiskohtainen kuvaus. Hankehistoriallisessa tapaustutkimuksessa on käytetty historiantutkimuksen menetelmiä.
