Tämän pro gradu –tutkielman aihe on sosiaalityön asiantuntijuus moniammatillisessa psykiatrisessa avohoitotyössä. Tutkielman tehtävä on analysoida psykiatrisessa avoterveydenhuollossa työskentelevien eri ammattialojen asiantuntijoiden kokemuksia ja käsityksiä sosiaalityön roolista ja asemasta moniammatillisessa toimintaympäristössä. Tutkimustyötä ohjaavat seuraavat kysymyksenasettelut: Mikä on sosiaalityön asiantuntijuuden rooli psykiatrisessa avohoitotyössä? Miten eri työntekijäryhmät ja työntekijät määrittelevät sosiaalityön asemaa ja tehtäviä psykiatrisessa avohoidossa? Onko havaittavissa eroja sosiaalityöntekijöiden ja muiden psykiatrisen avohoidon ammattiryhmien tavassa määritellä sosiaalityön asemaa ja tehtäviä psykiatrisessa hoitotyössä? Miten psykiatrinen sosiaalityö mahdollisesti poikkeaa muusta terveydenhuollon sosiaalityöstä, mitkä ovat sen erityispiirteitä? Tutkimusaineisto koostuu kymmenestä teemahaastattelusta. Haastattelut toteutettiin osassa Helsingin kaupungin psykiatriayksikön avopalveluiden toimipisteitä kesän 2011 aikana. Haastateltaviksi valikoitui viisi sosiaalityöntekijää, kolme sairaanhoitajaa ja kaksi psykologia. Tutkimuksen lähestymistapa on laadullinen. Litteroidut haastattelut analysoitiin sisällönanalyysin avulla. Analyysin aikana aineistosta muodostettiin teemoja sosiaalityön työnkuvaan, erityisosaamiseen sekä moniammatilliseen yhteistyöhön liittyen. Sosiaalityöntekijöiden työnkuva näyttäytyi tutkimustuloksissa kaksijakoisena. Sosiaalityöntekijät toimivat usein sekä hoitavina työntekijöinä että erityistyöntekijöinä. Hoitavina työntekijöinä toimiessaan sosiaalityöntekijät osallistuivat potilaiden hoitoon supportiivisten ja terapeuttishenkisten keskustelukäyntien kautta. Erityistyöntekijän rooli puolestaan näyttäytyi konsultatiivisena, jolloin sosiaalityöntekijät toimivat yhteiskunnallisten ja sosiaalisten kysymysten erityisasiantuntijoina. Sosiaalityön erityisasiantuntijuuden osa-alueiksi nousivat sosiaaliturva- ja sosiaalipalvelut, yhteiskuntatieteellinen näkökulma, tuki ja kuunteleminen sekä verkostot, tiedottaminen ja työn kehittäminen. Muiden työntekijöiden haastatteluissa korostui erityisesti sosiaalityön asema sosiaaliturva- ja sosiaalipalveluiden asiantuntijana-ammattina. Sosiaalityöntekijät puolestaan nostivat esiin yhteiskuntatieteellisen näkökulman sekä ihmissuhdetyön osaamisen merkitystä psykiatrisessa sosiaalityössä. Etenkin sosiaalityöntekijät kokivat kaksijakoisen työnkuvansa ajoittain hankalaksi sekä toivat esiin toivetta työnkuvan selkeyttämisestä erityisasiantuntijan roolin suuntaan. Sosiaalityön osaaminen ja rooli psykiatriassa eivät aina näyttäytyneet selkeinä myöskään muiden ammattiryhmien edustajille, vaan sosiaalityöstä ja sen sisällöistä kaivattiin lisää tietoa. Moniammatillisessa yhteistyössä korostui vuorovaikutuksen ja avoimen työilmapiirin sekä mahdollisimman selkeän työnjaon merkitys. Yhteistyötä vaikeutti etenkin resurssien puute ja kiire. Sekä sosiaalityöntekijät että muut työntekijät korostivat aktiivisen ja vuorovaikutteisen työotteen merkitystä juuri terveydenhuollon sosiaalityössä, kun sosiaalityötä tehdään ensisijaisesti lääke- ja hoitotieteen toimintaympäristössä. Soft tissue sarcomas are malignant tumours of mesenchymal origin. Because of infiltrative growth pattern, simple enucleation of the tumour causes a high rate of local recurrence. Instead, these tumours should be resected with a rim of normal tissue around the tumour. Data on the adequate margin width are scarce. At Helsinki University Central Hospital (HUCH) a multidisciplinary treatment group started in 1987. Surgical resection with a wide margin (2.5 cm) is the primary aim. In case of narrower margin radiation therapy is necessary. The role of adjuvant chemotherapy remains unclear. Our aims were to study local control by the surgical margin and to develop a new prognostic tool to aid decision-making on which patients should receive adjuvant chemotherapy. Patients with soft tissue sarcoma of the extremity or the trunk wall referred to HUCH during 1987-2002 form material in Studies I and II. External validation material comes from the Lund university sarcoma registry. The smallest surgical margin of at least 2.5 centimetres yielded local control of 89 per cent at five years. Amputation rate was 9 per cent. The proposed prognostic model with necrosis, vascular invasion, size on a continuous scale, depth, location and grade worked well both in Helsinki material and in the validation material, and it also showed good calibration. Based on the present study, we recommend the smallest surgical margin of 2-3 centimetres in soft tissue sarcoma irrespective of grade. Improvement in local control was present but modest in margins wider than 1 centimetre. In cases where gaining a wider margin would lead to a considerable loss of function, smaller margin is to be considered combined to radiation therapy. Patients treated with inadequate margins should be offered radiation therapy irrespective of tumour grade. Our new prognostic model to estimate 10-year survival probability in patients with soft tissue sarcoma of the extremities or trunk wall showed good dicscrimination and calibration. For time being the prognostic model is available for scientific use and further validations. In the future, the model may aid in clinical decision-making. For operable osteosarcoma, neoadjuvant multidrug chemotherapy followed by delayed surgery and multidrug adjuvant chemotherapy is the treatment of choice. Overall survival rates at five years are approximately 75 per cent in modern trials with classical osteosarcoma. All patients diagnosed and reported to the Finnish Cancer Registry with osteosarcoma in Finland during 1971-2005 form the material in Studies III and IV. Limb-salvage rate increased from 23 per cent to 78 per cent during 1971-2005. The 10-year sarcoma-specific survival for the whole study population improved from 32 per cent to 62 per cent. It was 75 per cent for patients with a local high-grade osteosarcoma of the extremity diagnosed during 1991-2005. This study outlines the improved prognosis of osteosarcoma patients in Finland with modern chemotherapy. The 10-year survival rates are good also in an international scale. Nonetheless, their limb-salvage rate remains inferior to those seen for highly selected patient series. Overall, the centralisation of osteosarcoma treatment would most likely improve both survival and limb-salvage rates even further.

Puudutemyrkytysten hoitoon suositellaan joissain maissa laskimonsisäisesti annettavaa rasvaemulsiota. Suomessa kansallista suositusta ei kuitenkaan ole, eikä kyseisen hoidon yleisyyttä ole aiemmin kotimaassa tutkittu puudute- tai muissakaan lääkemyrkytyksissä. Tutkimuksen tavoitteena on selvittää millaiseen näyttöön hoito perustuu käymällä läpi aiheesta julkaistu kirjallisuus sekä selvittää sähköpostikyselyn avulla kuinka yleisesti anestesiaosastoilla on laadittu omia ohjeistuksia rasvaemulsion käytöstä puudutemyrkytystapauksissa. Lisäksi selvitetään onko kyseistä hoitoa jo käytetty, hoitosuositusten perustaa, käytettyjä rasvaemulsiota ja missä niitä säilytetään. Kysely lähetettiin myös kahteen ensihoitoyksikköön muiden lääkemyrkytysten hoitoon laadittujen ohjeistusten osalta. Hieman yli puolella Suomen anestesiaosastoista on laadittuna oma ohjeistus laskimonsisäisen rasvaemulsion käytöstä puudutemyrkytyksissä, vaikka hoidon tehosta on ristiriitaisaa näyttöä eikä kontrolloituja ihmistutkimuksia ole julkaistu. Ensihoitoyksiköillä taas ei ole omia suosituksia hoidosta. Hoitoa on myös käytetty viidellä anestesiaosastolla. Hoitosuositukset eri osastoilla on kuitenkin johdettu useista eri lähteistä. Käytetyin rasvaemulsio on Intralipid® ja myrkytysten hoitoon tarkoitettuja emulsioita säilytetään pääasiassa leikkaussalissa tai sen yhteydessä.

To protect and restore lake ecosystems under threats posed by the increasing human population, information on their ecological quality is needed. Lake sediments provide a data rich archive that allows identification of various biological components present prior to anthropogenic alterations as well as a constant record of changes. By providing a longer dimension of time than any ongoing monitoring programme, palaeolimnological methods can help in understanding natural variability and long-term ecological changes in lakes. As zooplankton have a central role in the lake food web, their remains can potentially provide versatile information on past trophic structure. However, various taphonomic processes operating in the lakes still raise questions concerning how subfossil assemblages reflect living communities. This thesis work aimed at improving the use of sedimentary zooplankton remains in the reconstruction of past zooplankton communities and the trophic structure in lakes. To quantify interspecific differences in the accumulation of remains, the subfossils of nine pelagic zooplankton taxa in annually laminated sediments were compared with monitoring results for live zooplankton in Lake Vesijärvi. This lake has a known history of eutrophication and recovery, which resulted from reduced external loading and effective fishing of plankti-benthivorous fish. The response of zooplankton assemblages to these known changes was resolved using annually laminated sediments. The generality of the responses observed in Lake Vesijärvi were further tested with a set of 31 lakes in Southern Finland, relating subfossils in surface sediments to contemporary water quality and fish density, as well as to lake morphometry. The results demonstrated differential preservation and retention of cladoceran species in the sediment. Daphnia, Diaphanosoma and Ceriodaphnia were clearly underrepresented in the sediment samples in comparison to well-preserved Bosmina species, Chydorus, Limnosida and Leptodora. For well-preserved species, the annual net accumulation rate was similar to or above the expected values, reflecting effective sediment focusing and accumulation in the deepest part of the lake. The decreased fish density and improved water quality led to subtle changes in zooplankton community composition. The abundance of Diaphanosoma and Limnosida increased after the reduction in fish density, while Ceriodaphnia and rotifers decreased. The most sensitive indicator of fish density was the mean size of Daphnia ephippia and Bosmina (E.) crassicornis ephippia and carapaces. The concentration of plant-associated species increased, reflecting expanding littoral vegetation along with increasing transparency. Several of the patterns observed in Lake Vesijärvi could also be found within the set of 31 lakes. According to this thesis work, the most useful cladoceran-based indices for nutrient status and planktivorous fish density in Finnish lakes were the relative abundances of certain pelagic taxa, and the mean size of Bosmina spp. carapaces, especially those of Bosmina (E.) cf. coregoni. The abundance of plant-associated species reflected the potential area for aquatic plants. Lake morphometry and sediment organic content, however, explained a relatively high proportion of the variance in the species data, and more studies are needed to quantify lake-specific differences in the accumulation and preservation of remains. Commonly occurring multicollinearity between environmental variables obstructs the cladoceran-based reconstruction of single environmental variables. As taphonomic factors and several direct and indirect structuring forces in lake ecosystems simultaneously affect zooplankton, the subfossil assemblages should be studied in a holistic way before making final conclusions about the trophic structure and the change in lake ecological quality.

Chronic myeloid leukemia (CML) is one of the most studied human malignancies. It is caused by an autonomously active tyrosine kinase BCR-ABL, which is a result from a translocation between chromosomes 9 and 22 in the hematopoietic stem cell. As an outcome, a Philadelphia (Ph) chromosome is formed. BCR-ABL causes disturbed cell proliferation among other things. Although targeted tyrosine kinase inhibitor therapy has been developed in the beginning of the millenium and the survival rate has increased significantly, it is still not known why some patients benefit more from the treatment than others. Furthermore, the therapy is not considered to be curative. Before the era of tyrosine kinase inhibitors, the first-line treatment for CML was interferon-? (IFN-?). However, only a small proportion of patients benefitted from the treatment. Of these patients, a few were able to discontinue the treatment without renewal of the disease. The mechanism of IFN-? is not completely understood, but it is believed that differences in the immune system can be one of the reasons why some patients have better therapy response. Kreutzman, Rohon et al. have recently discovered that patients who have been able to stop IFN-? treatment have an increased number of NK- and T-cells. They also have a unique clonal T-cell population and more cytotoxic CD8+ T-cells and less CD4+ T-cells. The aim of this master’s thesis was to study the function of T- and NK-cells in IFN-? treated patients. Although it was shown earlier that IFN-? treated patients have increased NK-cell count, the function of these cells was unknown. Therefore, we have now investigated the killing potential of patients’ NK-cells, their activation status and cell surface antigen expression. In addition, we have also studied the activation status of patients’ T-cells and their cytotoxic properties. We observed that NK-cells from patients treated with IFN-? are unable to kill leukemic cells (K562) than NK-cells from healthy controls. In addition, patients on IFN-? treatment have more active T-cells and their NK-cells have an undifferentiated immunoregulatory phenotype. Patients that have been able to stop the treatment have anergic T-and NK-cells. As a conclusion our results suggest that IFN-? therapy induces increased NK-cell count, NK-cell immunoregulatory functions and more active T-cells. After stopping IFN-? therapy, NK- and T-cells from CML patients restore anergy typical for CML.

In recent years urban hydrology and individual urban streams have been in focus and subjects to research also in Helsinki. However, until now there has been lack of research covering simultaneously the whole area of the city of Helsinki. The aim of this study was to find out the general state of water quality in small urban streams in the city of Helsinki. 21 streams were studied: Mätäjoki, Korppaanoja, Mätäpuro, Näsinoja-Tuomarinkylänoja, Tuomarinkartanonpuro, Kumpulanpuro, Tapaninkylänpuro, Tapaninvainionpuro, Puistolanpuro, Longinoja, Säynäslahdenpuro, Viikinoja, Porolahdenpuro, Mustapuro, Marjaniemenpuro, Mellunkylänpuro, Vuosaarenpuro, Rastilanpuro, Ramsinkannaksenpuro, Skatanpuro and Yliskylänpuro. Water samples were collected from 48 sampling points, each stream having at least one point. Four water samples were collected from each point, sampling periods being 9.-11.2., 26.-28.4., 29.6.-1.7. and 25.-27.10.2004. Field measurements associated with water sampling included water temperature, oxygen concentration, pH and electrical conductivity. Water samples were analysed in the Laboratory of Physical Geography in the University of Helsinki and in the Environmental Laboratory of the City of Helsinki Environment Centre for following properties: suspended solids, dissolved substances, alkalinity, principal anions and cations (Na+, K+, Mg2+, Ca2+, F-, Cl-, NO3-, PO43- and SO42-), colour, turbidity, biological and chemical oxygen demand (BOD7 and CODMn-values), nutrient concentrations and bacterial indicators of hygienic quality. The main water quality issues found in this study were low oxygen levels in many streams and poor hygienic quality at least occasionally. E.g. in summer oxygen levels were under 60 % in every stream. Amount of total dissolved substances and nutrients were high in some of the streams studied. Compared to other Finnish streams the values of alkalinity and pH were higher. Although these problems were common, the variation between different streams and sampling points was significant. This was probably due to local conditions. Best overall water quality was found in Mätäpuro and Tuomarinkartanonpuro streams. Seasonal variation was evident in almost all water quality properties. For example the total amount of dissolved substances was largest in winter and decreased during the year. Colour and turbidity were smallest in winter and increased towards the end of the year. The same was true for suspended solids, which had smallest concentration in winter and greatest in autumn. It must be kept in mind that the spring samples were collected after the spring flood otherwise the largest suspended solid concentrations would have been expected in spring. Finnish general water quality classification was used to assess the quality of urban stream waters. Its suitability for small urban streams is not, however, completely trouble-free. This classification does not take into account the quick changes in such small streams but evaluates only the yearly mean values. This can oversimplify the picture of the water quality situation in the streams. Also in order to better reflect the urban environment the analysed water quality properties should also include total dissolved substances and e.g. concentrations of chloride and sodium.
