Maintenance of breeding efficiency and high semen quality is essential for reproductive success in farm animals. Early recognition of possible inheritable factors causing infertility requires constant attention. This thesis focuses on describing different manifestations of impaired spermatogenesis, their impact on fertility and partly also their incidence in populations. The reasons for spermatogenic failure are various. An interruption of germ cell differentiation, spermatogenic arrest, can lead to infertility. The incidence of azoospermia was investigated in the 1996 2005 survey of Finnish AI and farm breeding boars. We focused on the diagnosis, testicular morphometry and the possible reasons for the condition. The incidence of azoospermia was significantly higher in Yorkshire boars than in the Landrace breed. The most common diagnosis in Yorkshire boars was germ cell arrest at the primary spermatocyte level. The second most frequent diagnosis in Yorkshire boars was segmental aplasia of the Wolffian ducts with idiopathic epididymal obstruction. Other reasons for azoospermia were infrequent. In the second study we investigated the incidence of two relatively well-defined specific sperm defects in Finnish Yorkshire and Landrace boars during the same survey, the immotile short-tail sperm (ISTS) defect and the knobbed acrosome (KA) defect. In the Finnish Yorkshire boars the inherited ISTS defect, and the probably inherited KA defect, were important causes of infertility during 1996 2005. The ISTS defect was found in 7.6% and the KA defect in 0.8% of the Yorkshire boars. No Landrace boars were diagnosed with either of these two defects. In the third study we described a new sterilizing sperm defect in an oligoasthenoterazoospermic bull. Because of its morphological characteristics this defect was termed the multinuclear-multiflagellar sperm (MNMFS) defect. The number of Sertoli cells in the seminiferous tubuli was highly increased in the MNMFS bull compared with the number in normal bulls. In the following two studies we used a combined approach of fluorescence in situ hybridization (FISH), flow cytometry and morphometric studies to provide information on the cytogenetic background of macrocephalic bull spermatozoa. We described cellular features of diploid spermatozoa and compared the failures in the first and second meiotic divisions. In the last study we describe how the transplantation of testicular cells was used to determine whether spermatogonia derived from donor animals are able to colonize and produce motile spermatozoa in immune-competent unrelated boars suffering the ISTS defect. Transplantation resulted in complete focal spermatogenesis, indicated by the appearance of motile spermatozoa and confirmed by genotyping.

Objective: To study the efficacy of long-term buserelin acetate infusion to desensitize pituitary and block testicular function in adult male monkeys (Macaca radiata). Animals: Proven fertile male monkeys exhibiting normal testicular function. Protocol: Each of the control (n = 5) and experimental monkeys (n = 10) received a fresh miniosmotic pump every 21 days, whereas pumps in controls delivered vehicle of experimentals released 50-mu-g buserelin acetate every 24 hours. On day 170 (renewed every 60 days) a silastic capsule containing crystalline testosterone (T) was implanted in the experimental monkeys. At the end of 3 years, treatment was stopped, and recovery of testicular function and fertility monitored. Results: (1) Treatment resulted in marked reduction of nocturnal but not basal serum T; (2) the pituitary remained desensitized to buserelin acetate throughout the 3-year period; (3) animals were largely azoospermic with occasional oligospermia exhibited by two monkeys; and (4) withdrawal of treatment restored testicular function, with 70% of animals regaining fertility. Conclusion: Long-term infertility (but restorable) can be induced in male monkeys by constant infusion of buserelin acetate and T.

It is well known that hyperprolactinaemia in the human leads to infertility. The therapy of choice in India has been the administration of bromocriptine (BCR) as tablets, This mode of administration is generally accompanied by undesirable side-effects such as giddiness, nausea, vomiting and postural hypotension, We demonstrate here the efficacy of microdoses of BCR administered intranasally (IN) to hyperprolactinaemic patients (n = 6) in reducing significantly the elevated serum prolactin levels and maintain them within the normal range, The IN mode of BCR administration, in addition to reducing the effective dose of the drug by 4-20-fold, results in little or no side-effects otherwise associated with oral therapy.

A study of 140 days duration was performed to examine if human male volunteers (n = 5) respond to ovine follicle stimulating hormone (oFSH) immunization (administered adsorbed on Alugel on days 1, 20, 40 and 70) by producing antibodies capable of both binding and neutralizing bioactivity of human FSH. The kinetics of antibody production for both the immunogen (oFSH) and the cross-reactive antigen (hFSH) were essentially similar, The volunteers responded only to the first two immunizations, The boosters given on days 40 and 70 were ineffective, probably because of the presence of substantial amounts of circulating antibody to oFSH. Of the antibodies generated to oFSH, 25-45% bound hFSH with a mean binding affinity of 0.65 x 10(9) +/- 0.53 M(-1). The binding capacities at the time of high (30-80 days of immunization) and low (>110 days) titres were 346 +/- 185 and 10.5 +/- 5.8 ng hFSH/ml respectively, During the period of high titre, free serum FSH (value in normal males 1-5 ng/ml) was not monitorable, A 50 mu l aliquot of the antiserum obtained from different volunteers between days 30 and 80 and on day 140 blocked binding of I-125-labelled hFSH to its receptor by 82 +/- 9.7 and 53 +/- 12.2% respectively, The antibody produced was specific for FSH, and no significant change in the values of related glycoprotein hormones (luteinizing hormone/testosterone and thyroid stimulating hormone/thyroxine) were recorded, Seminal plasma transferrin, a marker of Sertoli cell as well as of seminiferous tubular function, showed marked reduction (30-90%) following immunization with oFSH. Considering that endogenous FSH remained neutralized for approximately one sperm cycle only (65 days), the reduction in sperm counts (30-74%) exhibited by some volunteers is encouraging, Immunization with oFSH did not result in any significant changes in haematology, serum biochemistry or hormonal profiles, There was no production of antibodies capable of interacting with non-specific tissues, It is concluded that it should be possible to obtain a sustained long-term blockade of endogenous FSH action in men by using oFSH as an immunogen, This is a prerequisite for obtaining significant reduction in the quality and quantity of spermatozoa produced, thus leading to infertility.

Luteal insufficiency affects fertility and hence study of mechanisms that regulate corpus luteum (CL) function is of prime importance to overcome infertility problems. Exploration of human genome sequence has helped to study the frequency of single nucleotide polymorphisms (SNPs). Clinical benefits of screening SNPs in infertility are being recognized well in recent times. Examining SNPs in genes associated with maintenance and regression of CL may help to understand unexplained luteal insufficiency and related infertility. Publicly available microarray gene expression databases reveal the global gene expression patterns in primate CL during the different functional state. We intend to explore computationally the deleterious SNPs of human genes reported to be common targets of luteolysin and luteotropin in primate CL Different computational algorithms were used to dissect out the functional significance of SNPs in the luteinizing hormone sensitive genes. The results raise the possibility that screening for SNPs might be integrated to evaluate luteal insufficiency associated with human female infertility for future studies. (C) 2012 Elsevier B.V. Blastocyst hatching is critical for successful implantation leading to pregnancy. Its failure causes infertility. The phenomenon of blastocyst hatching in humans is poorly understood and the available information on this stems from studies of rodents such as mice and hamsters. We and others showed that hamster blastocyst hatching is characterized by firstly blastocyst deflation followed by a dissolution of the zona pellucida (zona) and accompanied by trophectodermal projections (TEPs). We also showed that embryo-derived cathepsins (Cat) proteases, specifically Cat-L, -B and -P act as zonalysins and are responsible for hatching. In this study, we show the expression and function of one of the potential regulators of embryogenesis, cyclooxygenase (COX)-2 during blastocyst development and hatching. The expression of COX-2 mRNA and protein was observed in 8-cell through hatched blastocyst stages and it was also localized to blastocysts TEPs. Specific COX-2 inhibitors, NS-398 and CAY-10404, inhibited blastocyst hatching; percentages achieved were only 28.4 5.3 and 32.3 5.4, respectively, compared with 90 with untreated embryos. Interestingly, inhibitor-treated blastocysts failed to deflate, normally observed during hatching. Supplementation of prostaglandins (PGs)-E-2 or -I-2 to cultured embryos reversed the inhibitors effect on hatching and also the deflation behavior. Importantly, the levels of mRNA and protein of Cat-L, -B and -P showed a significant reduction in the inhibitor-treated embryos compared with untreated embryos, although its mechanism remains to be examined. These data provide the first evidence that COX-2 is critical for blastocyst hatching in the golden hamster.

Aims: Administration of estradiol or compounds with estrogenic activity to newborn female rats results in irreversible masculinization as well as defeminization in the brain and the animals exhibit altered reproductive behavior as adults. The cellular and molecular mechanism involved in inducing the irreversible changes is largely unknown. In the present study, we have monitored the changes in the expression of selected synaptogenesis related genes in the sexually dimorphic brain regions such as POA, hypothalamus and pituitary following 17 beta-estradiol administration to neonatal female rats. Main methods: Female Wistar rats which were administered 17 beta-estradiol on day 2 and 3 after birth were sacrificed 120 days later and the expression levels of genes implicated in synaptogenesis were monitored by semi-quantitative reverse transcription PCR. Since estradiol induced up-regulation of COX-2 in POA is a marker for estradiol induced masculinization as well as defeminization, in the present study only animals in which the increase in expression of COX-2 gene was observed in POA were included in the study. Key findings: Down-regulation of genes such as NMDA-2B, NETRIN-1, BDNF, MT-5 MMP and TNF-alpha was observed in the pre-optic area of neonatally E2 treated female rat brain but not in hypothalamus and pituitary compared to the vehicle- treated controls as assessed by RT-PCR and Western blot analysis. Significance: Our results suggest a possibility that down-regulation of genes associated with synaptogenesis in POA, may be resulting in disruption of the cyclical regulation of hormone secretion by pituitary the consequence of which could be infertility and altered reproductive behavior. Blastocyst implantation into the uterine endometrium establishes early pregnancy. This event is regulated by blastocyst- and/or endometrium-derived molecular factors which include hormones, growth factors, cell adhesion molecules, cytokines and proteases. Their coordinated expression and function are critical for a viable pregnancy. A rate-limiting event that immediately precedes implantation is the hatching of blastocyst. Ironically, blastocyst hatching is tacitly linked to peri-implantation events, although it is a distinct developmental phenomenon. The exact molecular network regulating hatching is still unclear. A number of implantation-associated molecular factors are expressed in the pre-implanting blastocyst. Among others, cytokines, expressed by peri-implantation blastocysts, are thought to be important for hatching, making blastocysts implantation competent. Pro-inflammatory (IL-6, LIF, GM-CSF) and anti-inflammatory (IL-11, CSF-1) cytokines improve hatching rates; they modulate proteases (MMPs, tPAs, cathepsins and ISP1). However, functional involvement of cytokines and their specific mediation of hatching-associated proteases are unclear. There is a need to understand mechanistic roles of cytokines and proteases in blastocyst hatching. This review will assess the available knowledge on blastocyst-derived pro-inflammatory and anti-inflammatory cytokines and their role in potentially regulating blastocyst hatching. They have implications in our understanding of early embryonic loss and infertility in mammals, including humans.

El Informe Warnock aborda parte de los problemas que surgen a partir del diagnóstico de esterilidad, tanto para los individuos como para toda la sociedad. Justifica el recurso de técnicas para solucionar la imposibilidad de concebir. Se describen los distintos procedimientos dando las recomendaciones para cada una de las técnicas empleadas hasta el momento: inseminación artificial (homóloga y heteróloga), fecundación in vitro, donación de óvulos y embriones, útero subrogado. Aborda el uso extendido de las técnicas de tratamiento de la esterilidad para otros fines: evitar la transmisión de enfermedades hereditarias y selección de sexo. Asimismo, realiza sugerencias para una práctica correcta en todos los procedimientos y propone seguimientos y revisiones. En paralelo al análisis exhaustivo de la Comisión Warnock, se actualizan los datos procedimentales, evaluando los aspectos éticos y la eficiencia de algunas técnicas.

Al cumplirse 25 años de la publicación del Informe Warnock (25 de junio de 1984), evaluamos la necesidad de realizar una revisión de los argumentos presentados en ese momento, haciendo un análisis científico comparativo, con los conocimientos actuales, para determinar el valor del mismo y, finalmente, concluir con una reflexión bioética. Esta revisión, sugerida en el mismo Informe y nunca concretada, fue realizada por cinco comisiones de investigación en las cuales se dividió el informe para que sea didáctico: filosofía; aspectos jurídicos y legislación; el embrión humano; técnicas de tratamiento de la esterilidad y técnicas de investigaciones con embriones humanos o sus células. En este apartado se incluyen las consideraciones generales, las expresiones de disentimiento de algunos miembros y la reflexión bioética general.

Apesar de a reprodução assistida possibilitar transformações importantes na parentalidade e nas relações familiares, suas tecnologias têm sido mais frequentemente usadas para reiterar o modelo tradicional de reprodução biológica e social. Este estudo qualitativo, de cunho exploratório, analisou quais normas estariam presentes nas práticas de saúde relativas à dificuldade de engravidar e o que poderia ser revelado a partir destas práticas. Foram observadas interações entre profissionais e pacientes atendidos em um serviço público de reprodução humana no Rio de Janeiro. A discussão dos diagnósticos de infertilidade e de risco, duas importantes estratégias biopolíticas usadas como critério de elegibilidade para o acesso às novas tecnologias reprodutivas, revelou como algumas práticas de saúde reiteram normas de gênero e de reprodução social. Atrelados à condição socioeconômica de seus usuários, estes diagnósticos tendem a agravar exclusões e desigualdades no exercício dos direitos reprodutivos no país. A análise da atenção médica possibilitou conhecer em parte o difícil cotidiano não apenas de homens e mulheres, que por anos persistem em seus desejos por filhos, mas também de profissionais que enfrentam antigas barreiras políticas, econômicas e burocráticas do serviço público de saúde. Este estudo corrobora a visão de que o serviço representa um avanço em termos de direitos sexuais e reprodutivos, apesar de ainda ser longo o caminho para o acesso igualitário e equânime às tecnologias reprodutivas pelo sistema único de saúde brasileiro (SUS).

The presence of endogenous opioid peptides in different testicular cell types has been extensively characterized and provides evidence for the participation of the opioid system in the regulation of testicular function. However, the exact role of the opioid system during the spermatogenesis has remained controversial since the presence of the mu-, delta-and kappa-opioid receptors in spermatogenic cells was yet to be demonstrated. Through a combination of quantitative real-time PCR, immunofluorescence, immunohistochemistry and flow cytometry approaches, we report for the first time the presence of active mu-, deltaand kappa-opioid receptors in mouse male germ cells. They show an exposition time-dependent response to opioid agonist, hence suggesting their active involvement in spermatogenesis. Our results contribute to understanding the role of the opioid receptors in the spermatogenesis and could help to develop new strategies to employ the opioid system as a biochemical tool for the diagnosis and treatment of male infertility.

Atualmente, no Brasil e no mundo, há um aumento do número de mulheres que postergam a gravidez para após os 35 anos em função da estabilidade profissional, espera de um relacionamento, infertilidade, incerteza sobre o desejo de ser mãe. Independente do motivo, na proporção em que acontece, o aumento do número de mulheres que vivenciam a gravidez tardia configura-se um fenômeno irreversível, que só tende a aumentar. Esta pesquisa teve como objeto a vivência da gravidez tardia e seus objetivos foram: descrever a vivência da mulher que engravida após os 35 anos e analisar a vivência sob a perspectiva de gênero e vulnerabilidade. Estudo descritivo, com abordagem qualitativa, tendo como técnica de coleta de dados a entrevista aberta, realizada com 16 mulheres no ciclo gravídico puerperal, com idade entre 36 e 48 anos, internadas em duas maternidades públicas, cenários da pesquisa, situadas no município do Rio de Janeiro. Foram observados os princípios da Resolução n 466 de 12 de dezembro de 2012 do Conselho Nacional de Saúde. Realizou-se análise temática dos dados categorizados. O estudo mostrou que fazem parte da vivência da mulher que engravida após os 35 anos, o desejo de ser mãe; aspectos positivos relativos à idade, maturidade, discernimento, profissão mais definida, aspectos que interferem favoravelmente na relação com a criança; e como aspectos negativos, cansaço, discriminação geracional e problemas de saúde. A espiritualidade, religiosidade e rede de apoio fortalecem a mulher para o enfrentamento de possíveis dificuldades. O planejamento familiar mostrou-se inexistente para as participantes e conclui-se que ao postergar a gravidez a mulher reduz a sua prole. Os resultados evidenciaram que para o planejamento da assistência o enfermeiro ou profissional de saúde deve trabalhar em equipe e conhecer o risco e a vulnerabilidade (individual, social e programática) da sua cliente. Deve considerar a situação socioeconômica e aspectos da vida familiar e afetiva, profissional, jornada de trabalho, trabalho doméstico, rede de apoio e acessibilidade aos serviços. A fim de detectar aspectos individuais, coletivos e contextuais que aumentam a suscetibilidade da cliente. A escuta qualificada é a primeira providência para identificar as necessidades e singularidades das clientes e definir a estratégia de cuidados.

Os desreguladores endócrinos são compostos micropoluentes de ação deletéria que causam efeitos aos animais e aos seres humanos. Estes vêm trazendo a várias décadas, problemas relacionados à má formação ou ainda infertilidade de várias espécies. A presença destes compostos na ordem de ng L-1 já é capaz de gerar efeitos nocivos ao sistema endócrino. Este trabalho visou à determinação da presença e quantidade de alguns destes compostos como 17α-etinilestradiol, bisfenol A e estrona entre outros, nas águas do rio Arroio Fundo, localizado em Jacarepaguá no RJ que possui um índice de qualidade de água ruim. A primeira etapa deste trabalho foi a implementação e validação de metodologia para determinação do 17α-etinilestradiol por cromatografia líquida acoplada a detector de fluorescência onde foram estabelecido limite de detecção, de quantificação e linearidade conforme orientação do Inmetro (2010). Além da avaliação das amostras ambientais através da metodologia estabelecida. Na etapa subsequente foram realizados os experimentos relativos à determinação dos desreguladores endócrinos através da espectrometria de massas por detector tipo tandem quadrupolo, onde também foram realizados os procedimentos de validação e determinação dos compostos bisfenol A e estrona que não foram suscetíveis a efeito de matriz durante a ionização, que acabou impedindo a determinação de outros desreguladores endócrinos. Os resultados indicam que não há redução da presença dos desreguladores endócrinos detectados, e de outros compostos orgânicos também detectados, mas não identificados, entre os pontos a montante e a jusante da Unidade de Tratamento do Rio Arroio Fundo. Todavia em função do grande efeito de matriz oriundo do fato da amostra ser de extrema complexidade, não foi possível realizar uma identificação e quantificação de todos os compostos alvos.

A endometriose é uma doença benigna que causa dor e infertilidade. As disfunções sexuais são frequentes, especialmente a dispareunia de profundidade, interferindo na qualidade de vida e particularmente na vida conjugal dessas pacientes. O objetivo deste trabalho foi avaliar a satisfação sexual de pacientes com endometriose infiltrativa profunda. Foram analisadas cinquenta e sete pacientes com diagnóstico de endometriose infiltrativa profunda acompanhadas no Hospital Universitário Pedro Ernesto (HUPE) de julho de 2010 a dezembro de 2011. O grupo controle incluiu 38 pacientes saudáveis do ambulatório de planejamento familiar do HUPE. Foi aplicado o Female Sexual Function Index (FSFI), questionário validado para avaliação funcional da atividade sexual. Em relação ao resultados não houve diferença estatisticamente significativa no escore total do FSFI entre os dois grupos. No domínio dor, as pacientes com endometriose apresentaram escores significativamente menores, ou seja, maior intensidade de dor, do que o grupo controle. O resultado deste estudo sugere que as pacientes com endometriose apresentam um comprometimento do domínio dor, sem prejuízo potencial da função sexual global.
