In leptospirosis patients, haematological abnormalities have been reported. The aim of this study was to determine if neutrophil counts were different between patients known to be infected with a range of leptospiral serovars. The study retrospectively compared the neutrophil counts from the first blood samples taken from 210 leptospirosis patients at first presentation to a Queensland Health hospital. Significant differences (p <0.001) were observed in neutrophil counts across the 11 different infecting serovars. These findings suggest that neutrophil counts may be useful in the development of an algorithm determining the infecting serovar in suspected leptospirosis patients. Further studies are required to delineate host cytokine responses which may suggest the underlying aetiology of the observed differences in neutrophil counts. Such studies would also provide valuable therapeutic insights into treating the disease.

Leptospirosis is one of the most common yet under reported zoonoses. Leptospires, the etiological agents of leptospirosis are ubiquitous pathogens, with a world-wide distribution, causing a spectrum of disease ranging from a mild influenza-like illness to Weil’s disease, which manifests itself in multi-organ failure. The following chapter reports on the epidemiology and transmission of the disease in humans and animals. The chapter will also delineate the symptoms observed in humans and animals and in concluding outline unresolved and evolving issues for microbiologists, epidemiologists and public health officials.

Leptospirosis outbreaks have been associated with many common water events including water consumption, water sports, environmental disasters and occupational exposure. The ability of leptospires to survive in moist environments makes them a high risk agent for infection following contact with any contaminated water source. Water treatment processes reduce the likelihood of leptospirosis or other microbial agents causing infection provided they do not malfunction and the distribution networks are maintained. Notably, there are many differences in water treatment systems around the world, particularly between developing and developed countries. Detection of leptospirosis in water samples is uncommonly performed by molecular methods.

La leptospirosis es considerada una de las zoonosis más difundidas en el mundo entero. El perro actúa como un potencial diseminador de esta enfermedad ya que mantiene una estrecha relación con el hombre, y al mismo tiempo con otros animales tanto domésticos como salvajes. Con el objetivo de conocer la presencia de leptospirosis canina, se realizó estudio en ocho barrios del distrito dos de la cuidad de Managua entre 8 de septiembre al 9 de diciembre 2007. Se realizó un muestreo de 76 canes, correspondiente al 5% del total de canes en los ocho barrios. Se extrajo 3ml de sangre de la vena safena externa, luego de procesarlas para separar el suero sanguíneo del plasma mediante centrifugación, fueron trasladadas al laboratorio del Centro Veterinario de Diagnóstico e Investigación CEVEDI, UNAN-León, para su análisis mediante los métodos ELISA y MAT. De acuerdo con los resultados obtenidos mediante la prueba ELISA un 93.4%, 71 canes, son negativos y un 6.6%, o bien 5 especímenes, son seropositivos a Leptospira spp. Según los resultados de la prueba de micro aglutinación MAT, el 96.1% de la población muestreada es negativa y el 3.9% es positiva a leptospirosis encontrándose los siguientes serovares canico/a, icterohemorragiae, pyrogenes.

Con el objetivo de Contribuir a la mejora de la salud pública en la ciudad de Catacamas, Olancho, Honduras, mediante el diagnóstico de la prevalencia de Leptospirosis canina. Utilizando la técnica de Micro-aglutinación (MAT), para el registro y control de casos En el periodo del 11 de marzo al 24 de agosto del 2009. Para conocer la población canina con posibilidades de infestación con Leptospirosis según, sexo, edad, factores de riesgo. Y determinar el grado de infestación y una estrategia técnica y económica para el control y seguimiento contra la Leptospirosis. Se realizo un muestreo de 380 canes, correspondiente al 10% de la población canina en dicha ciudad. Extrayéndose 3 ml de sangre de la vena radial y safena externa, luego de procesarla para separar el suero mediante precipitación, fueron trasladados al laboratorio del INSTITUTO HONDUREÑO DE INVESTIGACIONES MEDICO VETERINARIO. Para su análisis mediante la técnica MAT. Luego se tomo una segunda muestra de orina a los animales reactivos, para realizar un cultivo haciendo uso de un medio de cultivo con antibiótico flurouracilo (5-fu) De acuerdo con los resultados obtenidos, de 380 canes, 5.1% son positivos y 94.9 % negativos a Leptospirosis encontrándose los siguientes serovares: canicola, icterohemorragica, hardjo, grippotyphosa.

El presente estudio se realizo entre el 10 de Octubre de 1962 y el 1° de Septiembre de 1963, en los Departamentos de Esteli, Chontales y Leon. Se estudiaron los siguientes aspectos: Pre valencia de la Leptospirosis en vacas en producción, los serotipos existentes, las edades mas comunmente atacadas y la prevalencia observada en las razas. Los resultados en conjunto fueron: Prevalencia de la Leptospirosis en las vacas de los 3 Departamentos estudiados. EL Departamento mas atacado fue el de Chontales, luego el de Leon y por ultimo el de Esteli. Los serotipos encontrados en orden de importancia numérica son: L. hardjo, L. pomona, L. grippotyphosa, L. canicola, L. ballum, L. autumnalis, L. bataviae, L. icterehaomorrhagiae, L. alexi, L. australis. Los títulos mayormente encontrados fueron los de 100 y 400. Se encontró diferencia significativa para grupos y Departamentos. La prevalencia observada respecto a las edades fue que la Leptospirosis ademas de producir anticuerpos en animales de todas las edades estudiadas (excepto 2 años), fue mayor en los animales entre 3 y 7 años.

We enrolled consecutive febrile admissions to two hospitals in Moshi, Tanzania. Confirmed leptospirosis was defined as a ≥ 4-fold increase in microscopic agglutination test (MAT) titer; probable leptospirosis as reciprocal MAT titer ≥ 800; and exposure to pathogenic leptospires as titer ≥ 100. Among 870 patients enrolled in the study, 453 (52.1%) had paired sera available, and 40 (8.8%) of these met the definition for confirmed leptospirosis. Of 832 patients with ≥ 1 serum sample available, 30 (3.6%) had probable leptospirosis and an additional 277 (33.3%) had evidence of exposure to pathogenic leptospires. Among those with leptospirosis the most common clinical diagnoses were malaria in 31 (44.3%) and pneumonia in 18 (25.7%). Leptospirosis was associated with living in a rural area (odds ratio [OR] 3.4, P < 0.001). Among those with confirmed leptospirosis, the predominant reactive serogroups were Mini and Australis. Leptospirosis is a major yet underdiagnosed cause of febrile illness in northern Tanzania, where it appears to be endemic.

Leptospirosis is a globally important zoonotic infection caused by spirochaetes of the genus Leptospira. It is transmitted to humans by direct contact with infected animals or indirectly via contaminated water. It is mainly a problem of the resource-poor developing countries of the tropical and sub-tropical regions of the world but outbreaks due to an increase in travel and recreational activities have been reported in developed and more industrialized areas of the world. Current methods of diagnosis are costly, time-consuming and require the use of specialized laboratory equipment and personnel. The purpose of this paper is to report the validation of the 'Leptorapide®' test (Linnodee Ltd, Northern Ireland) for the diagnosis of human leptospirosis. It is a simple one-step latex agglutination assay performed using equal volumes of serum sample and antigen-bound latex beads. Evidence of leptospiral antibodies is determined within minutes. Agglutination is scored on a scale of 1-5 and the results interpreted using a score card provided with the kit. Validation has been performed with a large sample size obtained from individuals originating from various parts of the world including Brazil and India. The test has shown sensitivity and specificity values of 97·1% and 94·0%, respectively, relative to the microscopic agglutination test. The results demonstrate that Leptorapide offers a cost-effective and accurate alternative to the more historical methods of antibody detection.

Leptospirosis is a widespread but under-reported cause of morbidity and mortality. Global re-emergence of leptospirosis has been associated with the growth of informal urban settlements in which rodents are thought to be important reservoir hosts. Understanding the multi-host epidemiology of leptospirosis is essential to control and prevent disease. A cross-sectional survey of rodents in the Kibera settlement in Nairobi, Kenya was conducted in September–October 2008 to demonstrate the presence of pathogenic leptospires. A real-time quantitative polymerase chain reaction showed that 41 (18.3%) of 224 rodents carried pathogenic leptospires in their kidneys, and sequence data identified Leptospira interrogans and L. kirschneri in this population. Rodents of the genus Mus (37 of 185) were significantly more likely to be positive than those of the genus Rattus (4 of 39; odds ratio = 15.03). Questionnaire data showed frequent contact between humans and rodents in Kibera. This study emphasizes the need to quantify the public health impacts of this neglected disease at this and other urban sites in Africa.

No presente estudo foi avaliada, através da reação de soroaglutinação microscópica, a eficiência de quatro estirpes apatogênicas de Leptospira biflexa (Buenos Aires, Patoc 1, Rufino e São Paulo) como antígeno único para o diagnóstico sorológico em cobaias experimentalmente infectadas com sete diferentes sorotipos de Leptospira interrogans (canicola, grippotyphosa, hardjo, icterohaemorrhagiae, pomona, tarassovi e wolffi). As estirpes Buenos Aires, Patoc 1, Rufino e São Paulo não foram eficientes em revelar títulos de anticorpos nos soros das cobaias infectadas com Leptospira interrogans. Durante o experimento, foi observada a ocorrên cia de reações sorológicas cruzadas entre as estirpes Patoc 1 e São Paulo e também entre os sorotipos wolffi e hardjo.

Avaliou-se a reação de contraimunoeletroforese (CIE) como teste gênero-específico para diagnóstico da leptospirose suína, usando-se três extratos solúveis de Leptospira sp, sorovares pomona, icterohaemorrhagiae e patoc, obtidos pelo tratamento com Triton X-100 a quente e aplicados a amostras de soro de suínos subdivididos em três grupos: Grupo 1, 10 suínos experimentalmente infectados com estirpe Pomona; Grupo 2, 50 suínos naturalmente infectados e Grupo 3, controle. As amostras de soros foram submetidas à reação de CIE e os resultados comparados aos da Soroaglutinação Microscópica (SAM), técnica de referência pela WHO. Os Grupos 1 e 3 foram monitorados por 93 dias após a inoculação (p.i.). Pela SAM a soroconversão do Grupo 1 ocorreu por volta do 10º dia p.i., enquanto pela CIE, empregando-se qualquer extrato antigênico, foi anterior à SAM. Quando a CIE foi realizada frente a antigeno homólogo à infecção, seus resultados foram equivalentes aos da SAM, não se verificando o mesmo frente aos antígenos heterólogos. Neste aspecto, os Grupos 1 e 3 mostraram comportamento diferente pois não houve diferença significativa entre os resultados da CIE frente aos três antígenos, o que poderia significar serem independentes do sorovar responsável pelo surto ou infectante. Embora a CIE seja segura, rápida, de fácil execução, de baixo custo e ideal para análise em grande escala de amostras, revelou-se de limitada capacidade gênero-específica, o que não é desejavel para testes de triagem de campo; mas poderia ser útil na detecção precoce de resposta sorológica em relação à SAM.
