This review is addressed two pathophysiologic mechanisms implicated in the pathogenesis of nasal polyposis: the unique remodeling process found in nasal polyp tissue and the immune response of patients with nasal polyposis to Staphylococcus aureus. These two theories converge to the same direction in different aspects, including decreased extracellular matrix production, impaired T regulation and favoring of a Th2 immune response. In patients with nasal polyposis, an exaggerated immune response to Staphylococcus aureus may aggravate the airway remodeling process.

Asthma is a multifactorial disease for which a variety of mouse models have been developed. A major drawback of these models is represented by the transient nature of the airway pathology peaking 24 to 72 hours after challenge and resolving in 1 to 2 weeks. The objective of this study is to characterize the temporal evolution of pulmonary inflammation and remodeling in a recently described mouse model of chronic asthma (8 week treatment with 3 allergens relevant for the human pathology: Dust mite, Ragweed, and Aspergillus; DRA). We studied the DRA model taking advantage of fluorescence molecular tomography (FMT) imaging using near-infrared probes to non-invasively evaluate lung inflammation and airway remodeling. At 4, 6, 8 or 11 weeks, cathepsin- and metalloproteinase-dependent fluorescence was evaluated in vivo. A subgroup of animals, after 4 weeks of DRA, was treated with Budesonide (100 µg/kg intranasally) daily for 4 weeks. Cathepsin-dependent fluorescence in DRA-sensitized mice resulted significantly increased at 6 and 8 weeks, and was markedly inhibited by budesonide. This fluorescent signal well correlated with ex vivo analysis such as bronchoalveolar lavage eosinophils and alveolar cell infiltration. Metalloproteinase-dependent fluorescence was significantly increased at 8 and 11 weeks, nicely correlated with collagen deposition, as evaluated histologically by Masson’s Trichrome staining, and airway epithelium hypertrophy, and was also partly inhibited by budesonide. In conclusion, FMT proved suitable for longitudinal study to evaluate asthma progression, both in terms of inflammatory cell infiltration and airway remodeling, allowing the determination of treatment efficacy in a chronic asthma model in mice.

The small airways of the human lung undergo pathological changes in pulmonary disorders, such as chronic obstructive pulmonary disease (COPD), asthma, bronchiolitis obliterans and cystic fibrosis. These clinical problems impose huge personal and societal healthcare burdens. The changes, termed 'pathological airway remodeling', affect the epithelium, the underlying mesenchyme and the reciprocal trophic interactions that occur between these tissues. Most of the normal human airway is lined by a pseudostratified epithelium of ciliated cells, secretory cells and 6-30% basal cells, the proportion of which varies along the proximal-distal axis. Epithelial abnormalities range from hypoplasia (failure to differentiate) to basal- and goblet-cell hyperplasia, squamous- and goblet-cell metaplasia, dysplasia and malignant transformation. Mesenchymal alterations include thickening of the basal lamina, smooth muscle hyperplasia, fibrosis and inflammatory cell accumulation. Paradoxically, given the prevalence and importance of airway remodeling in lung disease, its etiology is poorly understood. This is due, in part, to a lack of basic knowledge of the mechanisms that regulate the differentiation, maintenance and repair of the airway epithelium. Specifically, little is known about the proliferation and differentiation of basal cells, a multipotent stem cell population of the pseudostratified airway epithelium. This Perspective summarizes what we know, and what we need to know, about airway basal cells to evaluate their contributions to normal and abnormal airway remodeling. We contend that exploiting well-described model systems using both human airway epithelial cells and the pseudostratified epithelium of the genetically tractable mouse trachea will enable crucial discoveries regarding the pathogenesis of airway disease.

We evaluated the effects of aerobic exercise (AE) on airway inflammation, exhaled nitric oxide levels (ENO), airway remodeling, and the expression of Thl, Th2 and regulatory cytokines in a guinea pig asthma model. Animals were divided into 4 groups: non-trained and non-sensitized (C), non-sensitized and AE (AE), ovalbumin-sensitized and non-trained (OVA), and OVA-sensitized and AE (OVA + AE). OVA inhalation was performed for 8 weeks, and AE was conducted for 6 weeks beginning in the 3rd week of OVA sensitization. Compared to the other groups, the OVA + AE group had a reduced density of eosinophils and lymphocytes, reduced expression of interleukin (IL)-4 and IL-13 and an increase in epithelium thickness (p < 0.05). AE did not modify airway remodeling or ENO in the sensitized groups (p > 0.05). Neither OVA nor AE resulted in differences in the expression of IL-2, IFN-gamma, IL-10 or IL1-ra. Our results show that AE reduces the expression of Th2 cytokines and allergic airway inflammation and induces epithelium remodeling in sensitized guinea pigs. (c) 2012 Elsevier B.V. The objective of these studies was to evaluate possible airway inflammation and remodeling at the bronchial level in cross-country skiers without a prior diagnosis of asthma, and relate the findings to patients with mild chronic asthma and patients with newly diagnosed asthma. We also studied the association of airway inflammatory changes and bronchial hyperresponsivess (BHR), and treatment effects in cross-country skiers and in patients with newly diagnosed asthma. Bronchial biopsies were obtained from the subjects by flexible bronchoscopy, and the inflammatory cells (eosinophils, mast cells, T-lymphocytes, macrophages, and neutrophils) were identified by immunohistochemistry. Tenascin (Tn) immunoreactivity in the bronchial basement membrane (BM) was identified by immunofluorescence staining. Lung function was measured with spirometry, and BHR was assessed by methacholine (skiers) or histamine (asthmatics) challenges. Skiers with BHR and asthma-like symptoms were recruited to a drug-intervention study. Skiers were given treatment (22 weeks) with placebo or budesonide (400 µg bid). Patients with newly diagnosed asthma were given treatment for 16 weeks with placebo, salmeterol (SLM) (50 µg bid), fluticasone propionate (FP) (250 µg bid), or disodium cromoglicate (DSCG) (5 mg qid). Bronchial biopsies were obtained at baseline and at the end of the treatment period. In the skiers a distinct airway inflammation was evident. In their bronchial biopsy specimens, T-lymphocyte, macrophage, and eosinophil counts were, respectively greater by 43-fold (P<0.001), 26-fold (P<0.001, and 2-fold (P<0.001) in skiers, and by 70-fold (p>0.001), 63-fold (P<0.001), and 8-fold (P<0.001) in asthmatic subjects than in controls. In skiers, neutrophil counts were more than 2-fold greater than in asthmatic subjects (P<0.05). Tn expression was higher in skiers than in controls and lower in skiers than in mild asthmatics. No significant changes were seen between skiers with or without BHR in the inflammatory cell counts or Tn expression. Treatment with inhaled budesonide did not attenuate asthma-like symptoms, the inflammatory cell infiltration, or BM Tn expression in the skiers. In newly diagnosed asthmatic patients, SLM, FP, and DSCG reduced asthma symptoms, and need for rescue medication (P<0.04). BHR was reduced by doubling doses 2.78, 5.22, and 1.35 respectively (all P<0.05). SLM and placebo had no effect on cell counts or Tn expression. FP and DSCG reduced eosinophil counts in the bronchial biopsy specimens (P<0.02 and <0.048, respectively). No significant change in tenascin expression appeared in any treatment group. Regarding to atopy, no significant differences existed in the inflammatory cell counts in the bronchial mucosa of subjects with newly diagnosed asthma or in elite cross country skiers. Tn expression in the BM was significantly higher in atopic asthma than in those with nonatopic asthma. Airway inflammation occurred in elite cross-country skiers with and without respiratory symptoms or BHR. Their inflammatory cell pattern differed from that in asthma. Infiltration with eosinophils, macrophages, and mast cells was milder, but lymphocyte counts did not differ from counts in asthmatic airways. Neutrophilic infiltration was more extensive in skiers than in asthmatics. Remodeling took place in the skiers’ airways, as reflected by increased expression of BM tenascin These inflammatory changes and Tn expression may be caused by prolonged exposure of the lower airways to inadequately humidified cold air. In skiers inflammatory changes and remodeling were not reversed with anti-inflammatory treatment. In contrast, in patients with newly diagnosed asthma, anti-inflammatory treatment did attenuate eosinophilic inflammation in the bronchial mucosa. In skiers, anti-inflammatory treatment did not attenuate BHR as it did in asthmatic patients. The BHR in skiers was attenuated spontaneously during placebo treatment, with no difference from budesonide treatment. Lower training intensity during the treatment period may explain this spontaneous decrease in BHR. The origin of BHR probably differs in skiers and in asthmatics. No significant association between BHR and inflammatory cell counts or between BHR and Tn expression was evident in cross-country skiers or asthmatic subjects. Airway remodeling differed between atopic and nonatopic asthma. As opposed to nonatopic asthma, Tn expression was higher in atopic asthma and is related to inflammatory cell densities.

This study focused on three-dimensional (3D) airway space changes and stability following simultaneous maxillomandibular counterclockwise rotation, mandibular advancement, and temporomandibular joint (TMJ) reconstruction with custom-made total joint prostheses (TMJ Concepts®). Cone beam computed tomography (CBCT) scans of 30 consecutive female patients with irreversibly compromised TMJs were obtained at the following intervals: T1, presurgery; T2, immediately after surgery; and T3, at least 6 months after surgery. The CBCT volumetric datasets were analysed with Dolphin Imaging ® software to evaluate surgical and postsurgical changes to oropharyngeal airway parameters. The average changes in airway surface area (SA), volume (VOL), and minimum axial area (MAA) were, 179.50 mm2, 6302.60 mm3, and 92.23 mm2, respectively, at the longest follow-up (T3 - T1) (P ≤ 0.001). Significant correlations between the amount of mandibular advancement and counterclockwise rotation of the occlusal plane and 3D airway changes were also found (P ≤ 0.01). The results of this investigation showed a significant immediate 3D airway space increase after maxillomandibular counterclockwise rotation and mandibular advancement with TMJ Concepts total joint prostheses, which remained stable over the follow-up period. © 2013 International Association of Oral and Maxillofacial Surgeons.

Strongyloidiasis is an intestinal parasitosis with an obligatory pulmonary cycle. A Th2-type immune response is induced and amplifies the cellular response through the secretion of inflammatory mediators. Although this response has been described as being similar to asthma, airway remodeling during pulmonary migration of larvae has not yet been established. The aim of this study was to identify the occurrence of airway remodeling during Strongyloides venezuelensis (S. v.) infection and to determine the ability of dexamethasone treatment to interfere with the mechanisms involved in this process. Rats were inoculated with 9,000 S. v. larvae, treated with dexamethasone (2 mg/kg) and killed at 1, 3, 5, 7, 14 and 21 days. Morphological and morphometric analyzes with routine stains and immunohistochemistry were conducted, and some inflammatory mediators were evaluated using ELISA. Goblet cell hyperplasia and increased bronchiolar thickness, characterized by edema, neovascularization, inflammatory infiltrate, collagen deposition and enlargement of the smooth muscle cell layer were observed. VEGF, IL1-beta and IL-4 levels were elevated throughout the course of the infection. The morphological findings and the immunomodulatory response to the infection were drastically reduced in dexamethasone-treated rats. The pulmonary migration of S. venezuelensis larvae produced a transitory, but significant amount of airway remodeling with a slight residual bronchiolar fibrosis. Tissue remodeling often reflects alterations in local mechanical conditions and manifests as an integrated response among the different cell types that share, and thus cooperatively manage, an extracellular matrix. Here we examine how two different cell types, one that undergoes the stress and the other that primarily remodels the matrix, might communicate a mechanical stress by using airway cells as a representative in vitro system. Normal stress is imposed on bronchial epithelial cells in the presence of unstimulated lung fibroblasts. We show that (i) mechanical stress can be communicated from stressed to unstressed cells to elicit a remodeling response, and (ii) the integrated response of two cell types to mechanical stress mimics key features of airway remodeling seen in asthma: namely, an increase in production of fibronectin, collagen types III and V, and matrix metalloproteinase type 9 (MMP-9) (relative to tissue inhibitor of metalloproteinase-1, TIMP-1). These observations provide a paradigm to use in understanding the management of mechanical forces on the tissue level.

A asma é um distúrbio crônico pulmonar caracterizado por inflamação, obstrução e remodelamento brônquico, levando a sintomas como sibilo, tosse e falta de ar. A terapia antiasmática consiste em corticosteroides inalados e agonistas β2 de curta ou longa duração. O tratamento é limitado por efeitos colaterais e refratariedade de alguns pacientes, justificando a necessidade de novas terapias. Estudos demonstram que a 15-deoxy-delta- 12,14-prostaglandina J2 (15d-PGJ2), um ligante endógeno de receptores ativados por proliferadores de peroxissomos do tipo gama (PPAR-γ), é capaz de reduzir a expressão de citocinas pró-inflamatórias, o que pode resultar em benefícios no tratamento de doenças com esse perfil. O objetivo deste estudo foi avaliar o potencial anti-inflamatório e antiasmático da 15d-PGJ2 em modelos experimentais de asma. Camundongos A/J machos foram sensibilizados nos dias 0 e 7 através de injeção subcutânea (s.c.), contendo ovoalbumina (OVA) e Al(OH)3, e desafiados com 4 instilações intranasais (i.n.) de OVA em intervalos semanais. O tratamento com 15d-PGJ2 (30 e 100 g/Kg, s.c.) foi realizado 30 min antes dos desafios a partir da terceira provocação antigênica. Em outro modelo, camundongos A/J foram desafiados intranasalmente com extrato de ácaro 3 vezes por semana durante 3 semanas. As administrações de 15d-PGJ2 (30, 70 e 100 g/Kg, s.c. e 0,65; 1,5 e 2,3 g/animal, i.n.) foram realizadas a partir da 3 semana, 30 min antes dos desafios. As análises ocorreram 24 h após o último desafio. Nossos resultados mostraram que, em camundongos previamente sensibilizados e desafiados com OVA, a administração de 15d-PGJ2 limitou significativamente o influxo peribrônquico de eosinófilos e neutrófilos, bem como a produção de muco por células caliciformes e fibrose sub-epitelial, além da hiperreatividade das vias aéreas e produção de IL-5. A redução do epitélio brônquico e das citocinas IL-13 e TNF-α foram observadas somente na maior dose administrada. No modelo HDM a inflamação e o remodelamento foram atenuados em todas as doses administradas do composto, enquanto que a hiperresponssividade brônquica foi inibida apenas nas doses de 70 e 100 μg/Kg (via sistêmica) e na dose intermediária dada topicamente (1,5 μg/animal, i.n.). Os níveis de citocinas foram atenuados pelo tratamento subcutâneo, porém somente os níveis de IL-17, eotaxina-1 e TNF-α foram inibidos com a dose intranasal de 0,65 g/animal. O aumento da expressão de NF-κB, induzido por provocação com HDM também foi reduzido significativamente pela administração de 15d-PGJ2. Em conjunto, nossos dados indicam que o tratamento com 15d-PGJ2 inibe alterações cruciais associadas à patogênese da asma, em modelos experimentais distintos da doença, demonstrando possuir grande potencial para controlar e reverter inflamação, hiperreatividade e remodelamento pulmonar desencadeados por provocação alérgica.

La transplantation pulmonaire pour les patients avec une maladie pulmonaire en phase terminale est leur seul espoir de survie. Malheureusement, certains greffés du poumon rencontrent des difficultés après la transplantation du poumon, dont l'un est le rejet chronique du greffon pulmonaire également connu histologiquement comme la bronchiolite oblitérante et cliniquement comme syndrome de bronchiolite oblitérante. L'étiologie exacte de la BO reste mal comprise. Certaines hypothèses suggèrent l'implication des cellules épithéliales dans le processus de remodelage des voies respiratoires, conduisant à l'obstruction des voies aériennes. Un des mécanismes proposés est un processus de transition, connue sous le nom de transition épithéliale-mésenchymateuse (TEM). Lors de ce processus, les cellules perdent leurs propriétés épithéliales, acquièrent un phénotype mésenchymateux et deviennent plus mobiles et envahissantes. Cette transformation leur permet de participer activement au processus de remodelage bronchique dans la bronchiolite oblitérante. L’induction de la TEM peut être due à certains facteurs tels que l'inflammation et l'apoptose. Le principal objectif de ce travail de maîtrise est de détecter in vivo la présence de la TEM dans des biopsies transbronchiques obtenues chez des greffés et de l’associer à leurs conditions cliniques. Le deuxième objectif est d'induire la TEM in vitro dans les cellules épithéliales des petites voies aériennes à l'aide de milieux conditionnés apoptotiques et non apoptotiques produits par les cellules endothéliales microvasculaires humaines du poumon. D’autre part, nous avons évalué si des médiateurs connus pour participer au processus de TEM tels que le facteur de croissance du tissu conjonctif (CTGF)et le facteur de croissance transformant bêta (TGF-beta) ainsi que le perlecan sont présents dans les milieux conditionnés utilisés.

Il a été démontré que les chevaux atteints du souffle présentent une augmentation de la masse de muscle lisse entourant les voies respiratoires comparativement à des chevaux sains (Herszberg, Ramos-Barbon et al. 2006, Leclere, Lavoie-Lamoureux et al. 2011). L’augmentation de la masse de muscle lisse ainsi observée résulte d’une hyperplasie, et possiblement, d’une hypertrophie des myocytes. Les traitements usuels du souffle ne sont que partiellement efficaces à diminuer cette augmentation. L’objectif de cette étude était d’explorer les mécanismes moléculaires impliqués dans ces changements affectant la cellule musculaire lisse dans la pathologie du souffle chez le cheval. Pour ce faire, nous avons examiné les effets d’une exposition antigénique sur l’expression du «Serum Response Factor» (SRF) dans le muscle lisse bronchique. Le SRF est un facteur de transcription localisé dans le noyau de la cellule musculaire lisse et régulant l’expression génique de celle-ci en favorisant un phénotype prolifératif ou contractile. Les résultats démontrent qu’avant exposition antigénique, les pourcentages de cellules exprimant le SRF sont faibles. Une augmentation significative du pourcentage de myocytes exprimant le SRF survient suite à une stimulation antigénique chez les chevaux atteints de souffle alors qu’aucune augmentation n’est observée chez les chevaux contrôles. Ces résultats suggèrent que le SRF pourrait contribuer au remodelage du muscle lisse péribronchique dans la pathologie du souffle.

La présente étude visait à développer un protocole de fixation et d'échantillonnage pour le poumon équin suivant les directives publiées sur l’utilisation d’une approche stéréologique de type « design-based ». Les poumons gauches de chevaux contrôles et atteints du souffle ont été fixés avec du formaldéhyde 10% pendant 48h à une pression constante de 25-30 cm d’H2O. Les poumons ont été sectionnés en 20-21 tranches d’une épaisseur d'environ 2,5 cm chacune; de 10-11 tranches ont été sélectionnées de façon aléatoire et systématique pour la mesure du volume de référence avec la méthode de Cavalieri. Un protocole d’échantillonnage systématique, aléatoire et uniforme utilisant le principe du « smooth fractionator » et un poinçon à biopsie de 17 mm ont été utilisés pour échantillonner une fraction représentative de chaque poumon. Les méthodes d’échantillonnage de sections verticales, uniformes et aléatoires (VUR) et d’échantillonnage isotropique, uniforme et aléatoire (IUR) ont toutes deux été effectuées pour comparer le nombre de voies respiratoires en coupe perpendiculaire obtenues à partir de chaque méthode. L'architecture globale et la qualité des tissus fixés ont également été évaluées. Des spécimens pulmonaires équins ont été échantillonnés avec succès selon un protocole visant à produire des données morphométriques valides. Les tissus ont été fixés avec un minimum d'artéfacts et contenaient une quantité suffisante de voies respiratoires en coupe perpendiculaire dans les deux types d’échantillons. En conclusion, un protocole de fixation et d'échantillonnage adapté au poumon équin permettant l'utilisation d'une approche stéréologique de type « design-based » a été élaboré pour l’étude du remodelage des voies respiratoires.

Aerobic conditioning (AC) performed either during or after sensitization reduces allergic inflammation in mice; however, the effects of AC performed before and during allergic sensitization on airway inflammation are unknown. Mice were divided into Control, AC, OVA, and AC + OVA groups. Mice were trained in a treadmill followed by either ovalbumin (OVA) sensitization or saline administration. Peribronchial inflammation, OVA-specific IgE and IgG1 titers, the expression of Th1 and Th2 cytokines, and airway remodeling were evaluated, as well as the expression of Eotaxin, RANTES, ICAM-1, VCAM-1, TGF-beta and VEGF. Aerobic conditioning performed before and during allergic sensitization displayed an inhibitory effect on the OVA-induced migration of eosinophils and lymphocytes to the airways, a reduction of IgE and IgG1 titers and an inhibition of the expression of Th2 cytokines. The AC + OVA group also demonstrated reduced expression of ICAM-1, VCAM-1, RANTES, TGF-beta and VEGF, as well as decreased airway remodeling (p < 0.05). The effects of AC before and during the sensitization process inhibit allergic airway inflammation and reduce the production of Th2 cytokines and allergen-specific IgE and IgG1.

Nuclear factor of activated T cells (NFAT) ist eine Familie der Transkriptionsfaktoren, welche eine wichtige Rolle bei der Regulation der T-Zellvermittelten Signalkaskade in der Lymphozytenpopulation spielt. In dieser Arbeit konnte gezeigt werden, dass Nuclear Factor of Activated T cells-2 (NFATc2) defiziente Mäuse einen erhöhten Atemwegswiderstand, einen pathologische Veränderung der Lunge und einen erhöhten IgE Spiegel im Vergleich zu den Wildtypen vorweisen. Die NFATc2 Defizienz konnte ebenfalls sowohl mit einer erhöhten Anzahl an Th2 und Th17 Zellen, die eine erhöhte Proliferation vorweisen, als auch einer erniedrigten Anzahl an CD8+ CD122- T-Zellen, die geringere Mengen an IFN-g produzieren, in Verbindung gebracht werden. Die aus den NFATc2(-/-) Mäusen isolierten CD4+ T-Zellen zeigen im Vergleich zu denen der Wildtypen neben der erhöhten Proliferation einen vermehrte Aktivierung (CD4higCD44highCD69high). Weiterhin konnte in dieser Arbeit gezeigt werden, dass in Anwesenheit eines Allergens, die NFATc2(-/-) Mäuse eine erhöhte Anzahl an regulatorischen T-Zellen (CD4+CD25+Foxp3+GITR++) in der Lunge vorweisen, die wiederum die Effektorzellen in diesen hemmen. Ein Grund für die geringere Freisetzung an IFN-g durch die CD8+ T-Zellen in den NFATc2 defizienten Mäusen ist eine erhöhte Subpopulation von CD8+CD122+ (IL-2Rb Kette) CD127hi (IL-7Ra Kette) „long-lived memory Zellen“ in den NFATc2(-/-) Mäusen. Diese besitzen einen regulatorischen Effekt, so dass immundefiziente SCID Mäuse, die in einem adoptiven Transfer mit OVA-spezifischen CD8+ und CD4+ T-Zellen, welchen aus NFATc2(-/-) Mäuse isoliert werden, behandelt wurden, eine erhöhten Atemwegswiderstand, eine erhöhte IL-17 und eine erniedrigte IFN-g Produktion vorweisen. Eine Depletion der memory CD8+CD122+IL-7Rhigh T-Zellen hebt dagegen die verringerte IFN-g Produktion der CD8+CD122- T-Zellen auf und führt zu einer Erniedrigung des Atemwegswiderstandes in einem SCID Model Zusammenfassend zeigen unsere Untersuchungen, dass sowohl die IFN-g Produktion der CD8+ Effektor T-Zellen als auch die Anzahl an CD4+CD25+Foxp3+GITR++ regulatorischen T-Zellen die Entwicklung der Th2 und Th17 als auch die Höhe des Atemwegswiderstandes unterdrückt.

Das allergische Asthma ist eine weit verbreitete, immunologische Erkrankung, deren Prävalenz in den vergangenen 20 Jahren vor allem in industrialisierten Regionen drastisch zugenommen hat. Trotz intensiver Forschung und Entwicklung medikamentöser Therapien steigt die Zahl der Patienten stetig an. Charakteristisch für diese Erkrankung sind entzündliche Veränderungen in der Lunge, erhöhte Atemwegsüberempfindlichkeit (AHR), Mukusproduktion und in chronischen Fällen auch Atemwegsobstruktion. Bei der Entstehung des allergischen Asthmas wird ein anfälliges Individuum durch die Inhalation eines normalerweise unschädlichen, in der Umwelt vorkommenden Antigens (Allergen) sensibilisiert, wodurch im Körper eine eigentlich unangebrachte Immunreaktion in Gang gesetzt wird. CD4+ T-Lymphozyten und ganz besonders die Subpopulationen der T-Helfer 1 (Th1) und Th2 Zellen spielen in dem Prozess eine zentrale Rolle. Obwohl ein Großteil der Asthmatiker mit einer Atemwegseosinophilie und erhöhter Expression der Th2-typischen Zytokine IL-4 und IL-13 ein Th2-typisches Krankheitsbild aufweisen, wurden weitere Asthmaphänotypen identifiziert. Vornehmlich in Patienten, die an schwerem Asthma leiden, sind dominierende Neutrophilie und erhöhte Mengen IFN-γ in den Atemwegen nachweisbar, was auf eine Th1-gesteuerte Immunreaktion hindeutet. Eine effektive, heilende Therapie des Asthmas wurde bislang nicht entwickelt. Die Inhibition der T-Zellantwort etwa durch Applikation allergenspezifischer, regulatorischer T-Zellen (Tregs) gilt als ein vielversprechender, aber nicht vollständig erforschter Ansatz zur Kontrolle der Krankheitssymptome. In diesem Zusammenhang wurden in der vorliegenden Arbeit die Mechanismen und Effekte natürlich vorkommender CD4+CD25+Foxp3+ regulatorischer T-Zellen (nTregs) auf eine Th1 bzw. Th2-induzierte allergische Atemwegserkrankung untersucht. Anhand eines adoptiven Zelltransfermodells unter Einsatz lymphozytendefizienter Rag2-/- Mäuse konnte gezeigt werden, dass sowohl Th1 als auch Th2 Zellen, kombiniert mit mehrfacher, inhalativer Allergenprovokation, eine erhöhte AHR induzieren. Während der Transfer allergenspezifischer Th2 Zellen eine Eosinophilie in der bronchoalveolären Lavage (BAL) und vermehrte Mukusproduktion in den Atemwegen hervorrief, war in Th1-transferierten Tieren zwar eine massive Infiltration neutrophiler Granulozyten zu beobachten, eine Becherzellmetaplasie mit vermehrten, mukusproduzierenden Atemwegsepithelzellen blieb allerdings aus. In vitro und in vivo waren voraktivierte nTregs (preTregs) nur eingeschränkt in der Lage, die Th2-gesteuerte Atemwegserkrankung zu inhibieren. Im Gegensatz dazu konnten die Th1-Effektorfunktionen in vitro und die Th1-induzierte AHR und Atemwegsentzündung in vivo durch preTregs effektiv gehemmt werden, was auf eine unterschiedliche Empfindlichkeit der Th-Subpopulationen weist. Innerhalb der nTreg-vermittelten Suppression wird der sekundäre Botenstoff cAMP auf die zu supprimierende Zelle übertragen und führt zur Hemmung von Proliferation und Zytokinproduktion. Dass dieser Mechanismus nicht nur in vitro, sondern auch in der Suppression der Th2-gesteuerten allergischen Atemwegserkrankung eine Rolle spielt, konnte durch die Störung des intrazellulären cAMP-Abbaus mittels PDE4-Inhibitoren verdeutlicht werden. Sowohl die prophylaktische, als auch die therapeutische Applikation der PDE4-Inhibitoren verstärkte den regulativen Effekt der nTregs auf AHR und Entzündung, korrelierend mit erhöhten, zytosolischen cAMP-Konzentrationen in den Th2 Zellen der Lunge. Trotz des Fortschritts in der Isolation und In vitro-Expansion humaner nTregs ist die Ausbeute an Zellen äußerst limitiert und die Übertragbarkeit größerer Zellmengen nicht zuletzt aufgrund von hohem Kontaminationsrisiko während mehrtägiger In vitro-Expansion fragwürdig. Die Ergebnisse der vorliegenden Arbeit zeigen, dass eine Behandlung mit dem PDE4-Inhibitor die suppressive Kapazität der allergenspezifischen nTregs deutlich erhöhte. Den nTreg-vermittelten Suppressionsmechanismus durch den Einsatz von Pharmazeutika zu unterstützen bietet einen viel versprechenden und realistischen Ansatz zur Therapie des allergischen Asthmas.
