PURPOSE To determine the variability of apparent diffusion coefficient (ADC) values in various anatomic regions in the upper abdomen measured with magnetic resonance (MR) systems from different vendors and with different field strengths. MATERIALS AND METHODS Ten healthy men (mean age, 36.6 years ± 7.7 [standard deviation]) gave written informed consent to participate in this prospective ethics committee-approved study. Diffusion-weighted (DW) MR imaging was performed in each subject with 1.5- and 3.0-T MR systems from each of three vendors at two institutions. Two readers independently measured ADC values in seven upper abdominal regions (left and right liver lobe, gallbladder, pancreas, spleen, and renal cortex and medulla). ADC values were tested for interobserver differences, as well as for differences related to field strength and vendor, with repeated-measures analysis of variance; coefficients of variation (CVs) and variance components were calculated. RESULTS Interreader agreement was excellent (intraclass coefficient, 0.876). ADC values were (77.5-88.8) ×10(-5) mm(2)/sec in the spleen and (250.6-278.5) ×10(-5) mm(2)/sec in the gallbladder. There were no significant differences between ADC values measured at 1.5 T and those measured at 3.0 T in any anatomic region (P >.10 for all). In two of seven regions at 1.5 T (left and right liver lobes, P < .023) and in four of seven regions at 3.0 T (left liver lobe, pancreas, and renal cortex and medulla, P < .008), intervendor differences were significant. CVs ranged from 7.0% to 27.1% depending on the anatomic location. CONCLUSION Despite significant intervendor differences in ADC values of various anatomic regions of the upper abdomen, ADC values of the gallbladder, pancreas, spleen, and kidney may be comparable between MR systems from different vendors and between different field strengths.

PURPOSE To systematically evaluate the dependence of intravoxel-incoherent-motion (IVIM) parameters on the b-value threshold separating the perfusion and diffusion compartment, and to implement and test an algorithm for the standardized computation of this threshold. METHODS Diffusion weighted images of the upper abdomen were acquired at 3 Tesla in eleven healthy male volunteers with 10 different b-values and in two healthy male volunteers with 16 different b-values. Region-of-interest IVIM analysis was applied to the abdominal organs and skeletal muscle with a systematic increase of the b-value threshold for computing pseudodiffusion D*, perfusion fraction Fp , diffusion coefficient D, and the sum of squared residuals to the bi-exponential IVIM-fit. RESULTS IVIM parameters strongly depended on the choice of the b-value threshold. The proposed algorithm successfully provided optimal b-value thresholds with the smallest residuals for all evaluated organs [s/mm2]: e.g., right liver lobe 20, spleen 20, right renal cortex 150, skeletal muscle 150. Mean D* [10(-3) mm(2) /s], Fp [%], and D [10(-3) mm(2) /s] values (±standard deviation) were: right liver lobe, 88.7 ± 42.5, 22.6 ± 7.4, 0.73 ± 0.12; right renal cortex: 11.5 ± 1.8, 18.3 ± 2.9, 1.68 ± 0.05; spleen: 41.9 ± 57.9, 8.2 ± 3.4, 0.69 ± 0.07; skeletal muscle: 21.7 ± 19.0; 7.4 ± 3.0; 1.36 ± 0.04. CONCLUSION IVIM parameters strongly depend upon the choice of the b-value threshold used for computation. The proposed algorithm may be used as a robust approach for IVIM analysis without organ-specific adaptation. Magn Reson Med, 2014. © 2014 Wiley Periodicals, Inc.

Echinococcus multilocularis is an important pathogenic zoonotic parasite of health concern, though absent in the United Kingdom. Eurasian beavers (Castor fiber) may act as a rare intermediate host, and so unscreened wild caught individuals may pose a potential risk of introducing this parasite to disease-free countries through translocation programs. There is currently no single definitive ante-mortem diagnostic test in intermediate hosts. An effective non-lethal diagnostic, feasible under field condition would be helpful to minimise parasite establishment risk, where indiscriminate culling is to be avoided. This study screened live beavers (captive, n = 18 or wild-trapped in Scotland, n = 12) and beaver cadavers (wild Scotland, n = 4 or Bavaria, n = 11), for the presence of E. multilocularis. Ultrasonography in combination with minimally invasive surgical examination of the abdomen by laparoscopy was viable under field conditions for real-time evaluation in beavers. Laparoscopy alone does not allow the operator to visualize the parenchyma of organs such as the liver, or inside the lumen of the gastrointestinal tract, hence the advantage of its combination with abdominal ultrasonography. All live beavers and Scottish cadavers were largely unremarkable in their haematology and serum biochemistry with no values suspicious for liver pathology or potentially indicative of E. multilocularis infection. This correlated well with ultrasound, laparoscopy, and immunoblotting, which were unremarkable in these individuals. Two wild Bavarian individuals were suspected E. multilocularis positive at post-mortem, through the presence of hepatic cysts. Sensitivity and specificity of a combination of laparoscopy and abdominal ultrasonography in the detection of parasitic liver cyst lesions was 100% in the subset of cadavers (95%Confidence Intervals 34.24-100%, and 86.7-100% respectively). For abdominal ultrasonography alone sensitivity was only 50% (95%CI 9.5-90.6%), with specificity being 100% (95%CI 79.2-100%). For laparoscopy alone sensitivity was 100% (95% CI 34.2-100%), with specificity also being 100% (95% CI 77.2-100%). Further immunoblotting, PCR and histopathological examination revealed one individual positive for E. multilocularis, whilst the other individual was positive for Taenia martis.

The incidence of uterine torsion in cattle is 0.5–1 per cent of all calvings and up to 30 per cent of all dystocia cases (Berchtold and Rüsch 1993). The unstable suspension of the bovine uterus is a predisposition cited by different authors (Pearson 1971, Schulz and others 1975, Berchtold and Rüsch 1993). Age of the cow, season and weight and sex of the calf have been inconsistently reported to be associated with uterine torsion (Distl 1991, Frazer and others 1996, Tamm 1997). Small amount of fetal fluids and a large abdomen may contribute to uterine torsion (Berchtold and Rüsch 1993). Furthermore, some authors describe a predisposition in the Brown Swiss breed (Distl 1991, Schmid 1993, Frazer and others 1996) and in cows kept in alpine regions (Schmid 1993). Uterine torsion is predominantly seen under parturition, and the degree of torsion is most often between 180° and 360°. The direction is counter-clockwise in 60–90 per cent of the cases (Pearson 1971, Berchtold and Rüsch 1993, Erteld and others 2012). Vaginal delivery is possible after manual detorsion or after rolling of the cow, whereas caesarean section has to be performed after unsuccessful detorsion or if the cervix is not dilating adequately following successful correction of the torsion (Berchtold and Rüsch 1993, Frazer and others 1996). Out of all veterinary-assisted dystocia cases, 20 per cent (Aubry and others 2008) to 30 per cent (Berchtold and Rüsch 1993) are due to uterine torsion. Many publications describe fertility variables after dystocia, but only Schönfelder and coworkers described that 40 per cent of the cows got pregnant after uterine torsion followed by caesarean section (Schönfelder and Sobiraj 2005).

Purpose: The purpose of this study was to evaluate the diagnostic accuracy of full-body linear X-ray scanning (LS) in multiple trauma patients in comparison to 128-multislice computed tomography (MSCT). Materials and Methods: 106 multiple trauma patients (female: 33; male: 73) were retrospectively included in this study. All patients underwent LS of the whole body, including extremities, and MSCT covering the neck, thorax, abdomen, and pelvis. The diagnostic accuracy of LS for the detection of fractures of the truncal skeleton and pneumothoraces was evaluated in comparison to MSCT by two observers in consensus. Extremity fractures detected by LS were documented. Results: The overall sensitivity of LS was 49.2 %, the specificity was 93.3 %, the positive predictive value was 91 %, and the negative predictive value was 57.5 %. The overall sensitivity for vertebral fractures was 16.7 %, and the specificity was 100 %. The sensitivity was 48.7 % and the specificity 98.2 % for all other fractures. Pneumothoraces were detected in 12 patients by CT, but not by LS. 40 extremity fractures were detected by LS, of which 4 fractures were dislocated, and 2 were fully covered by MSCT. Conclusion: The diagnostic accuracy of LS is limited in the evaluation of acute trauma of the truncal skeleton. LS allows fast whole-body X-ray imaging, and may be valuable for detecting extremity fractures in trauma patients in addition to MSCT. Purpose. To evaluate trends in the utilization of head, abdominal, thoracic and other body regions CTs in the management of victims of MVC at a level I trauma center from 1996 to 2006.^ Method. From the trauma registry, I identified patients involved in MVC's in a level I trauma center and categorized them into three age groups of 13-18, 19-55 and ≥56. I used International Classification of Disease (ICD-9-CM) codes to find the type and number of CTs examinations performed for each patient. I plotted the mean number of CTs per patient against year of admission to find the crude estimate of change in utilization pattern for each type of CT. I used logistic regression to assess whether repetitive CTs (≥ 2) for head, abdomen, thorax and other body regions were associated with age group and year of admission for MVC patients. I adjusted the estimates for gender, ethnicity, insurance status, mechanism and severity of injury, intensive care unit admission status, patient disposition (dead or alive) and year of admission.^ Results. Utilization of head, abdominal, thoracic and other body regions CTs significantly increased over 11-year period. Utilization of head CT was greatest in the 13-18 age group, and increased from 0.58 CT/patient in 1996 to 1.37 CT/patient in 2006. Abdominal CTs were more common in the ≥56+ age group, and increased from 0.33 CT/patient in 1996 to 0.72 CT/patient in 2006. Utilization of thoracic CTs was higher in the 56+ age group, and increased from 0.01 CT/patient in 1996 to 0.42 CT/patient in 2006. Utilization of other CTs did not change materially during the study period for adolescents, adults or older adults. In the multivariable analysis, after adjustment for potential confounders, repetitive head CTs significantly increased in the 13-18 age group (95% CI: 1.29-1.87, p=<0.001) relative to the 19-55 age group. Repetitive thoracic CT use was lower in adolescents (95% CI: 0.22-0.70, p=<0.001) relative to the 19-55 age group.^ Conclusion. There has been a substantial increase in the utilization of head, abdominal, thoracic and other CTs in the management of MVC patients. Future studies need to identify if increased utilization of CTs have resulted in better health outcome for these patients. ^

Objetivo: Analizar la mortalidad en la Unidad de Cuidados Intensivos (UCI) del Hospital Central de Mendoza y evaluar el valor predictivo de la escala APACHE II (Evaluación Fisiológica Aguda y de Salud Crónica). Material y Método: Se realizó un estudio retrospectivo y observacional de los pacientes ingresados a la Unidad de Cuidados Intensivos del Hospital Central de Mendoza, desde el 01/11/06 hasta el 31/03/08. Se calculó la distribución de sexos y de edades de la muestra, la estadía promedio, principales motivos de ingreso a la UCI y la puntuación APACHE II en las primeras 24 horas de internación. Se calculó la mortalidad esperada y la mortalidad obtenida global y se analizó el coeficiente entre ambas mortalidades. Resultados: Se incluyeron 904 pacientes, 61,82% masculinos y 38,18% femeninos, con una edad media 46 años (±19,36). Estadía promedio en la UCI 8,5 días promedio. El principal motivo de internación fueron los Traumatismos Encéfalocraneanos (TEC) con un 27,7% del total (86% asociados a politraumatismo grave). La mortalidad global obtenida fue del 41,48% vs. 24,08% esperable, con un coeficiente de mortalidad de 1,72 (p<0,0001). Conclusiones: La UCI estudiada presenta por las características de la población asistida un elevado índice de mortalidad global. La mortalidad obtenida fue 72% mayor a la mortalidad esperable según la puntuación APACHE II, demostrando esta Escala un bajo valor predictivo en nuestra UCI. La diferencia entre mortalidades podría parcialmente explicarse por la alta prevalencia de entidades con mortalidades subvaloradas por este modelo pronóstico, como pacientes politraumatizados y neurocríticos. En nuestro estudio, la Escala APACHE II presentó una franca subestimación de la mortalidad en ambas patologías. Sugerimos la realización de un estudio de regresión logística local para determinar un factor de corrección y/o adicionar puntos al valor APACHE II según el diagnóstico de ingreso del paciente. Asimismo, proponemos evaluar el empleo de medidas alternativas para predecir mortalidad, como sistemas de tercera generación (por ejemplo: APACHE III, MPM II y SAPS II).

Pese a los indicios que podrían encontrarse en la filosofía griega, la noción de persona es de origen netamente cristiano y no pudo haber sido formulada sino dentro de ese horizonte de pensamiento. El hombre ha sido creado a imagen de Dios y es persona porque, en primer término, Dios lo es. Aquí se enlazan, durante el medioevo, las cuestiones antropológicas y teológicas (trinitarias y cristológicas). Un ejemplo paradigmático se encuentra en las Sentencias de Pedro Lombardo y sus comentadores, entre los que hemos reparado especialmente en Tomás de Aquino. En este contexto, “naturaleza" (divina o humana) y “persona" son nociones íntimamente vinculadas, pues es propio de tales naturalezas el existir y manifestarse como seres personales. Esa relación, sin embargo, se pierde durante la modernidad, época en que persona y naturaleza se vuelven términos antagónicos. Martin Heidegger, agudo crítico de esa transformación en la historia del pensar, propone una concepción de lo humano que, no obstante su “ateísmo metodológico", finalmente parece aproximarse a la noción cristiana de persona.

Enmarcada en el ámbito amplio del pensamiento posmoderno y, en lo literario, dentro del fenómeno que la crítica ha denominado posboom, Luis Britto García ha construido una de las obras más fascinantes de la literatura venezolana actual, donde predominan un humor ácido y corrosivo, una imaginación desbordada que no sabe de fronteras, una acentuada erudición y un agudo sentido crítico. Este artículo se centra en el análisis de las posibilidades genéricas de su obra Rajatabla, publicada a comienzos de la década del 70. Su particular modo de construcción a partir de la suma de breves enunciados, su polifacético diseño estructural y la amplitud semántica que logra un universo asfixiante y apocalíptico plantean una serie de problemas en torno a su posible clasificación genérica para el que resulta esclarecedor el concepto de minificción.

La fascitis eosinofílica, se caracteriza por cambios cutáneos símil- esclerodermia y eosinofília periférica. Describimos un paciente de 54 años, de sexo masculino con diagnóstico de fascitis eosinofílica y seguimiento de 44 meses. Presentaba tumefacción, engrosamiento de la piel de antebrazos, piernas y flancos de abdomen, signo del surco y pérdida del pelo de antebrazos y piernas. El paciente fue tratado con glucocorticoides e hidroxicloroquina con evolución favorable. Concomitante a la remisión de los signos cutáneos observamos una hipertricosis de las zonas previamente afectadas. Comunicamos esta observación inusual en fascitis eosinofílica, secundaria a la mejoría del proceso inflamatorio cutáneo.

La EPA (Environmental Protection Agency) ha establecido estándares de seguridad para más de 80 contaminantes que pueden encontrarse en el agua potable y presentan un riesgo a la salud humana. Los efectos agudos ocurren dentro de unas horas o días posteriores al momento en que la persona consume un contaminante. Casi todos los contaminantes pueden tener un efecto agudo si se consume en niveles extraordinariamente altos en el agua potable, en esos casos los contaminantes más probables que causen efectos agudos son las bacterias y virus. Los contaminantes en el agua potable que pueden causar efectos crónicos son los químicos (tales como solventes y plaguicidas), radionucléidos (tal como el radio) y minerales (tal como el arsénico y algunas sales). Entre los ejemplos de efectos crónicos de los contaminantes del agua potable, están el cáncer, problemas del hígado o riñones o dificultades en la reproducción. Este estudio persigue los siguientes objetivos: -Determinar si las características del agua de consumo de El Manzano son determinante de la alta prevalencia de HTA entre sus habitantes -Conocer las características físicas de la composición del agua de consumo del Paraje El Cortaderal. -Comparar la calidad del agua de consumo de las localidades de El Cortaderal y El Manzano. -Comparar si los hábitos alimenticios de los parajes estudiados presentan diferencias significativas que pudieran incidir en la aparición de las Patologías como la HTA y la Fluorosis
