Background: Gingival fibromatosis is a rare condition characterized by a generalized enlargement of the buccal and lingual aspects of the attached and marginal gingiva.Methods: This case report describes the periodontal management of a 13-year-old female patient with gingival fibromatosis associated with Zimmermann-Laband syndrome. The patient presented with gingival enlargement involving the maxillary and the mandibular arches, anterior open bite, and non-erupted teeth. Periodontal treatment included gingivectomy in all four quadrants.Results: Histopathologic evaluation of the excised tissue supported the diagnosis of gingival flbromatosis. A significant improvement in esthetic appearance and eruption of the non-erupted teeth were obtained. The patient was referred for appropriate orthodontic treatment and has been closely followed for the earliest signs of recurrence of gingival enlargement.Conclusions: the successful therapy for gingival fibromatosis depends on correctly identifying the etiological factors and improving the impaired function and esthetic appearance through surgical intervention and adjunctive orthodontics. Maintaining treatment results depends on preservation of periodontal health.

Odontomas are developmental disturbances which manifest in the form of denticles or amorphous informes masses comprising all dental tissues, especially enamel and dentin, with variable amounts of pulp and cement. We describe here two clinical cases of odontomas in children, focusing on diagnostic means and the importance of early treatment of these lesions. The standard treatment for the two present cases was surgical removal.

Objective: Comparison of the clinical efficacy of 4% articaine in relation to 2% mepivacaine, both with 1:100,000 epinephrine, in the prevention of postoperative pain after lower third molar removal. Study design: Twenty patients underwent removal of bilateral lower third molars under local anesthesia (articaine or mepivacaine) in 2 separate appointments, in a double-blind, randomized, and crossed manner. Objective and subjective parameters were recorded for paired comparison of postoperative courses. Results: Duration of analgesia provided by articaine and mepivacaine was 198.00 ± 25.86, and 125.40 ± 13.96 min, respectively (P = .02), whereas the duration of anesthesia was 273.80 ± 15.94 and 216.85 ± 20.15 min, respectively (P = .06). Both solutions exerted no important effects upon arterial pressure, heart rate, or oxygen saturation (P > .05). Conclusions: Articaine provides a longer period of analgesic effect and a tendency for a longer period of anesthesia as compared to mepivacaine. The presence of a vasoconstrictor agent in local anesthetic solutions does not seem to influence hemodynamic parameters during lower third molar removal in healthy subjects. © 2006 Mosby, Inc. Cleidocranial dysplasia (CCD) is a rare syndrome usually caused by an autosomal dominant gene, although 40% of cases of CCD appear spontaneously with no apparent genetic cause. This condition is characterized by several cranial malformations and underdevelopment, absence of the clavicles, and multiple supernumerary and impacted permanent teeth. The diagnosis of this condition is usually based on the presence of the main features (supernumerary teeth, partial or total absence of one or both the clavicles, and bony malformations) and on clinical and familial evidence. The bony and dental features of CCD may be visualized on radiographic images of the face and skull. Here, we present a familial case of CCD and discuss the importance of dental radiographs in diagnosis of the condition.

The frequency of simultaneously impacted second and third molars in teenagers is increasing and becoming a common occurrence in adolescent oral surgery practice. The traditional treatment is the removal of the third molar by conventional access but repositioning of the surgical flap to the distal face of the first molar can predispose to complications such as pericoronitis and delayed healing of the attached gingiva. We present a case in which we use the germectomy approach to remove the impacted third molar for the eruption of the second molar through a vestibular incision. This incision offers excellent bone exposure and exit route for the third molar without disturbing the gingiva attached architecture on the distal face of the first molar providing good healing environment.

Osteopetrosis (OP) is a rare hereditary disorder characterized by a dysfunction of the osteoclasts that impairs bone resorption, which together with the normal osteoblastic activity forms intense bone sclerosis with reduction of marrow. A common complication that arises, most frequently, as a result of tooth extraction is mandibular osteomyelitis. There is no consensus on the literature about the treatment of this infection in an osteopetrotic patient, therefore, the purpose of this paper is to report a case of marginal resection for treatment of mandibular osteomyelitis in an osteopetrotic patient and discuss relevant features of this procedure. © 2010 European Association for Cranio-Maxillo-Facial Surgery.

Although in vitro studies have shown encouraging results for root surface conditioning with demineralizing agents, in vivo studies have failed to show its benefits in periodontal healing. This can be attributed to several factors, among which, the hypermineralization of dental surface. Therefore, this in vitro study compared, using scanning electron microscopy (SEM), the effect of root surface conditioning with different conditioners (1% and 25% citric acid, 24% EDTA and 50 mg/mL tetracycline hydrochloride) in impacted teeth and in teeth that had their roots exposed to the oral environment. One trained examiner assessed the SEM micrographs using a root surface modification index. There was a tendency of more root surface modification in the group of impacted teeth, suggesting that the degree of root mineralization influences its chemical demineralization.

This pathology is most common in the maxillary anterior area, called mesiodens, and their prevalence is generally reported to occur more frequently in male than female, in a ratio of 2:1. The aim of this article was to report the clinical procedures performed in the Clinic of Pediatric Dentistry at the Araraquara School of Dentistry when the presence of two mesiodens, radiographically shown, caused retention of permanent anterior teeth, resulting in esthetic and funcional disturb. All the surgical procedures, etiology and the importancy of the early diagnostic for the appropriate treatment was detailed described.

A displasia cementária periapical (DCP) consiste em uma lesão óssea não neoplásica geralmente assintomática, sendo detectada em exame radiográfico de rotina. Em seu primeiro estágio apresenta-se radigraficamente semelhante a uma lesão periapical inflamatória, todavia na DCP os dentes encontram-se vitais. Neste relato de caso foi realizado o acompanhamento por sete meses de uma paciente negra com 37 anos de idade afetada por DCP no periápice de incisivos, caninos e pré-molares inferiores bilateralmente. Ao exame radiográfico foi possível notar que a lesão apresentou-se em dois estágios: osteolítico e de maturação. Os dentes reagiram positivamente aos testes de vitalidade/sensibilidade pulpar descartando, com auxílio do exame de imagem, diversas hipóteses diagnósticas do grupo das periapicopatias crônicas, contribuindo assim para uma adequada escolha do tratamento, evitando iatrogenias.

Os caninos permanentes exercem um papel importante na dentição tanto por razões estéticas quanto funcionais. Os dentes seguem uma sequência de erupção favorável no desenvolvimento da oclusão normal, mas algum distúrbio desse mecanismo, nesse período de transição da dentição decídua para a permanente, pode levar a alterações na sequência ou mesmo no trajeto de erupção, levando a impactação de dentes. São os caninos superiores permanentes, depois dos terceiros mo lares que apresentam maior ocorrência de impactação. Na impossibilidade do diagnóstico precoce, esforços serão empregues para reposicionar o dente no arco dentário evitando sua extração, dada a sua importância no equilíbrio, harmonia e função. Nos casos não diagnosticados ou tratados inadequadamente podem ocorrer perturbações mecânicas, infecciosas ou neoplásicas. O prognóstico depende da posição do canino em relação às estruturas adjacentes e à possibilidade de movimentação ortodôntica. O presente trabalho aborda os principais fatores a serem considerados nos casos de caninos superiores permanentes impactados, tais como processo de erupção, etiologia, diagnóstico, tratamento e principalmente patologias associadas a caninos inclusos.

Impaction of maxillary canines can be prevented by early intervention in the mixed dentition phase after the correct diagnosis of malocclusion, reducing the complexity of the treatment. This article reports the case of a 10-year-old patient who possessed impacted maxillary canines and, after early extraction of primary canines, had reestablished favorable permanent successors' eruption axis. This 5-year radiographic follow-up study with panoramic radiography shows that this can be used in practice and that an effective control strategy ensures the accuracy in the inclination of the impacted canines. Treatment success is related to early diagnosis and strategic interceptive treatment choice.

This study had the objective to evaluate the homeopathic action of Arnica montana 6 cH and placebo on edema, mouth opening (trismus) and pain in patients submitted to extraction of bilaterally impacted lower third molars. The experiment was carried out as a crossover and double-blind study. The data showed that edema was significantly reduced by the treatment with Arnica montana 6 cH (p<0.05) and did not demonstrate significant effect on trismus and pain as compared to control group.
