Severe spinal deformity in young children is a formidable challenge for optimal treatment. Standard interventions for adolescents, such as spinal deformity correction and fusion, may not be appropriate for young patients with considerable growth remaining. Alternative surgical options that provide deformity correction and protect the growth remaining in the spine are needed to treat this group of patients 1, 2. One such method is the use of shape memory alloy staples. We report our experience to date using video-assisted thoracoscopic insertion of shape memory alloy staples. A retrospective review was conducted of 13 patients with scoliosis, aged 7 to 13 years, who underwent video-assisted thoracoscopic insertion of shape memory staples. In our experience, video-assisted thoracoscopic insertion of shape memory alloy staples is a safe procedure with no complications noted. It is a reliable method of providing curve stability, however the follow up results to date indicate that the effectiveness of the procedure is greater in younger patients.

Background: Fusionless scoliosis surgery is an early-stage treatment for idiopathic scoliosis which claims potential advantages over current fusion-based surgical procedures. Anterior vertebral stapling using a shape memory alloy staple is one such approach. Despite increasing interest in this technique, little is known about the effects on the spine following insertion, or the mechanism of action of the staple. The purpose of this study was to investigate the biomechanical consequences of staple insertion in the anterior thoracic spine, using in vitro experiments on an immature bovine model. Methods: Individual calf spine thoracic motion segments were tested in flexion, extension, lateral bending and axial rotation. Changes in motion segment rotational stiffness following staple insertion were measured on a series of 14 specimens. Strain gauges were attached to three of the staples in the series to measure forces transmitted through the staple during loading. A micro-CT scan of a single specimen was performed after loading to qualitatively examine damage to the vertebral bone caused by the staple. Findings: Small but statistically significant decreases in bending stiffness occurred in flexion,extension, lateral bending away from the staple, and axial rotation away from the staple. Each strain-gauged staple showed a baseline compressive loading following insertion which was seen to gradually decrease during testing. Post-test micro-CT showed substantial bone and growth plate damage near the staple. Interpretation: Based on our findings it is possible that growth modulation following staple insertion is due to tissue damage rather than sustained mechanical compression of the motion segment.

A total of 549 samples of rice, maize, wheat, sorghum and millet were obtained from markets in Ghana, the EU, US and Asia. Analysis of the samples, originating from 21 countries in 5 continents, helped to establish global mean trace element concentrations in grains: thus placing the Ghanaian data within a global context. Ghanaian rice was generally low in potentially toxic elements, but high in essential nutrient elements. Arsenic concentrations in rice from US (0.22 mg/kg) and Thailand (0.15 mg/kg) were higher than in Ghanaian rice (0.11 mg/kg). Percentage inorganic arsenic content of the latter (83%) was, however, higher than for US (42%) and Thai rice (67%). Total arsenic concentration in Ghanaian maize, sorghum and millet samples (0.01 mg/kg) was an order of magnitude lower than in Ghanaian rice, indicating that a shift from rice-centric to multigrain diets could help reduce health risks posed by dietary exposure to inorganic As. La recherche de nouvelles voies de correction de la scoliose idiopathique a une longue histoire. Le traitement conventionnel de la scoliose idiopathique est présenté par le port du corset ou par la correction opératoire de la déformation. Depuis leur introduction, les deux méthodes ont prouvé leur efficacité. Cependant, malgré des caractéristiques positives évidentes, ces méthodes peuvent causer un nombre important d'effets indésirables sur la santé du patient. Les techniques sans fusion pour le traitement de la scoliose semblent être une alternative perspective de traitement traditionnel, car ils apportent moins de risques et des complications chirurgicales que les méthodes conventionnelles avec la conservation de la mobilité du disque intravertébral. Cependant, l'utilisation de techniques mentionnées exige une connaissance profonde de la modulation de croissance vertébrale. L'objectif principal de la présente étude est d'estimer le potentiel d'agrafes à l’AMF de moduler la croissance des vertèbres porcines en mesurant la croissance osseuse sur la plaque de croissance de vertèbres instrumentées en comparaison avec le groupe contrôle. La méthode est basée sur la loi de Hueter-Volkmann. Nous avons choisi NiTi agrafes à l’AMF pour notre étude et les porcs de race Landrace comme un animal expérimental. Les agrafes ont été insérés sur 5 niveaux thoracique de T6 à T11. En outre, les radiographies ont été prises toutes les 2 semaines. La présence d'agrafes en alliage à mémoire de forme a produit la création de courbes scoliotiques significatives dans 4 de 6 animaux chargés et le ralentissement considérable de la croissance osseuse (jusqu'à 35,4%) comparativement aux groupes contrôle et sham. L'étude a démontré in vivo le potentiel d'agrafes en alliage à mémoire de formes de moduler la croissance des vertèbres en créant des courbes scoliotiques sur les radiographies et en ralentissant le taux de croissance sur les plaques de croissance instrumenté. La position précise de l'agrafe est essentielle pour la modulation de croissance osseuse et le développement de la scoliose expérimentale.

This article offers a reading of Suzanne Fisher Staples's novels Shabanu, Haveli,and Under the Persimmon Tree, drawing on postcolonial theory (in particular, Chandra Talpade Mohanty's essay "Under Western Eyes: Feminist Scholarship and Colonial Discourses" and Edward Said's Orientalism) to inform its analysis of the construction of girls and women in these texts. It argues that the novels' representations of Muslim girls are built on a naturalized contrast between liberal humanist paradigms of individualism and personal freedom, and homogenizing depictions of oppressed Muslim females, thus producing Orientalist distinctions between the West and the Orient.

Redox-active disulfides are capable of being oxidized and reduced under physiological conditions. The enzymatic role of redox-active disulfides in thiol-disulfide reductases is well-known, but redox-active disulfides are also present in non-enzymatic protein structures where they may act as switches of protein function. Here, we examine disulfides linking adjacent β-strands (cross-strand disulfides), which have been reported to be redox-active. Our previous work has established that these cross-strand disulfides have high torsional energies, a quantity likely to be related to the ease with which the disulfide is reduced. We examine the relationship between conformations of disulfides and their location in protein secondary structures. By identifying the overlap between cross-strand disulfides and various conformations, we wish to address whether the high torsional energy of a cross-strand disulfide is sufficient to confer redox activity or whether other factors, such as the presence of the cross-strand disulfide in a strained β-sheet, are required.

Staple entanglement in mohair fleeces occurs when adhesions form between longer and faster growing fibres and shorter and slower growing fibres. This results in accentuated crimp of the longer fibres and an "apparently" reduced staple length. The appearance in the fleece of Angora goats of staple entanglements can lead to the downgrading of the mohair to poorer style and shorter length grades, resulting in up to 60% price reductions. This study examined how staple entanglement score (SES) is related to lifetime factors of Angora goats, and how this relationship can be explained by variations in animal size and fleece attributes. SES was scored using a five-point scale: 5, long free fibres easily separated as no adhesions; 4, some adhesions between fibres; 3, some effort to separate fibres as many adhesions; 2, many adhesions, staple fibres entangled, shortening of staple; 1, very entangled and shortened staple. Measurements were made over 9 shearing periods on a population of Angora castrated males (wethers) goats representing the current range and diversity of genetic origins in Australia, including South African, Texan and interbred admixtures of these and Australian sources. Data on genetic origin, sire, dam, date of birth, dam age, birth weight, birth parity, weaning weight, live weight, fleece growth and fleece attributes were recorded. Two restricted maximum likelihood (REML) models were developed to relate SES with age, animal lifetime factors, fleece quality attributes and live weight. One model allowed fleece quality and live weight traits in the model and the other excluded these traits. Staple entanglement was almost eliminated in mohair harvested from goats shorn every 3. months but was common in mohair from goats shorn twice or once per year. SES was less in goats of Texan genetic background, and was generally less in winter grown mohair. SES was higher for mohair with low fibre curvature (FC, 10°/mm) and a high clean washing yield (CWY, 90%) compared with mohair with low FC and lower CWY (80%), and compared with all mohair with high FC (18°/mm). The response of SES to shearing regime, genetic background, shearing season, age of goat and a response to dam age were almost identical whether or not an adjustment was made for CWY and FC. There was a moderate amount of variability due to sires and individuals. We can conclude that a large part of these effects observed, namely breed, dam age, sire, and a component of the FC and CWY effects, are genetic. Mohair producers can manage the genetic effects by careful selection of sires, especially avoiding those with low CWY or high FC, and avoiding sires with higher levels of staple entanglement or that have produced progeny with higher levels of staple entanglement. Also, unidentified environmental effects are affecting staple entanglement, although a lack of a live weight change effect on entanglement indicates that this effect might not be due to nutrition. © 2013 Elsevier B.V.
