Työn tavoitteena oli selvittää Suomenlahden rannalta merkittävän suuruisen alusöljyvahingon jälkeen kerättävän öljyisen jätteen käsittelymahdollisuudet ja -kapasiteetit sekä loppusijoitusmahdollisuudet ja -kapasiteetit Kymenlaakson alueen näkökulmasta. Tarkoituksena oli selvittää, missä jätteiden käsittely voidaan toteuttaa sekä, miten öljyisiä jätteitä voidaan esikäsitellä välivarastoinnin aikana puhdistuksen ja loppusijoituksen tehostamiseksi. Tutkimuksen kohteena oli sekä rannalta kerättävät kiinteät öljyiset ainekset että öljyinen merivesi. Työn alussa on perehdytty jätehuoltovastuuseen, eli kenen vastuulla öljyalusonnettomuuksissa syntyvät öljyiset jätteet ovat. Työssä on esitelty lyhyesti öljyvahinkojätteille teknisesti soveltuvien käsittelymenetelmien periaatteet ja menetelmien rajoituksia käsitellä öljyvahinkojätetteitä. Työssä on myös mainittu aiemmin Suomea koskettaneiden tai maailmalla tapahtuneiden alusöljyvahinkojen jätemääriä ja tapauksissa käytettyjä jätteiden käsittelymenetelmiä. Työ painottuu esittelemään Kymenlaakson alueen laitosten, Riihimäen Ekokem Oy Ab:n ja siirrettävien laitteistojen mahdollisuuksia käsitellä öljyisiä jätteitä. Lisäksi on esitelty öljyisen meriveden käsittelyyn soveltuvia laitoksia Kymenlaakson alueen näkökulmasta. Tietoja on kerätty puhelimitse ja sähköpostitse yritysten edustajilta vuoden 2007 aikana. Kymenlaakson alueella voidaan polttaa voimalaitosten leijupedeissä puhtaaseen polttoaineeseen sekoitettuja öljyisiä orgaanisia aineksia ja murskautuvia puhdistustyössä käytettyjä varusteita noin 10 000 t/a, homogenoitua öljyistä orgaanista ainesta voidaan polttaa Leca-soratehtaan rumpu-uunissa noin 1 200 t/a. Alueen polttokapasiteetti kasvaa, kun työn aikana rakenteilla oleva jätteenpolttolaitos valmistuu ja jätettä voidaan polttaa laitoksen arinalla. Haihtuvilla öljy-yhdisteillä pilaantuneita maa-aineksia voidaan alipainekäsitellä, jos yhdisteet eivät ole haihtuneet jo merellä. Erityisesti öljyiset maaainekset voidaan käsitellä alhaisilla öljypitoisuuksilla (öljypitoisuus noin alle 1-2 %) bitumistabiloimalla, aumakompostoimalla tai pesemällä siirrettävällä pesulaitteistolla. Kymenlaakson alueelle voidaan tuoda myös alueen ulkopuolelta siirrettäviä laitteistoja. Siirrettävät termodesorptiolaitteistot on tehty pilaantuneen maa-aineksen ensisijaiseen käsittelyyn, mutta samalla voidaan käsitellä myös muita jätejakeita, joilla on pieni partikkelikoko (alle 5-10 cm). Savaterra Oy:n siirrettävän termodesorptiolaitteiston kapasiteettiarvio on 100 000 t/a. Myös Niska & Nyyssönen Oy:llä on siirrettävä termodesorptiolaitteisto. Doranova Oy:n siirrettävän pesulaitteiston kapasiteettiarvio on 30 000- 50 000 t/a öljyistä maa-ainesta. Tutkimuksessa on ollut mukana myös Riihimäen Ekokem Oy Ab:n jätevoimala, jonka kapasiteettiarvio on 40 000-45 000 t/a erityisesti öljyisille orgaanisille aineksille, varusteille ja kuolleille eläimille. Riihimäen Ekokem Oy Ab:n ongelmajätelaitoksen rumpuuuneissa voidaan käsitellä arviolta 80 000-100 000 t/a öljyisiä maa-aineksia eli kiinteitä jätteitä, joiden partikkelikoko on suunnilleen alle 10 cm, ja 20 000 t/a nestemäisiä öljyisiä jätteitä. Työn loppupuolella on esitelty myös öljyisen meriveden käsittelyyn soveltuvia laitoksia ja niiden rajoituksia käsitellä kyseistä jätettä. Kyseisten laitosten kapasiteetit selviävät usein vasta onnettomuuden sattuessa. Kaikkiin annettuihin kapasiteettiarvioihin vaikuttaa merkittävästi jätteen koostumus. Raportin lopussa on esitelty alustava toimintasuunnitelma öljyvahinkojätteen käsittelemiseksi. Suunnitelmaan sisältyvät eri jätejakeille laaditut kaaviot, joista voi nähdä muun muassa eri jätekoostumuksille teknisesti soveltuvat käsittelymenetelmät ja käsittelymenetelmiä suorittavat yritykset. Öljyalusonnettomuuden sattuessa soveltuviin yrityksiin tulee ottaa yhteyttä ja selvittää kyseisellä hetkellä vapaana oleva käsittelykapasiteetti. Raportissa on myös esitelty käsittelykustannuksiin vaikuttavia tekijöitä ja arvioitu aiheutuvia kuljetuskustannuksia. Saadut tutkimustulokset ovat hyödynnettävissä erityisesti Kymenlaakson alueella. Tiedot käsittelymenetelmistä ja niiden rajoitteista ovat hyödynnettävissä valtakunnallisesti.

Steamer accidents, through contact with the bucket wheel, are very seldom today. No publication of such a kind of fatal accident could be found in literature. We present the case of a fatal steamer accident, in which the findings of a blunt traumatization of a person by the ship was completely documented by post-mortem combined multi-slice computed tomography (MSCT) and magnetic resonance imaging (MRI) examinations. A rupture of the aorta was detected using both radiological methods without use of radiopaque material. Radiological examination revealed a comminuted fracture of the thorax vertebrae at the same level as the aortic rupture. Injuries of the soft tissues of the back, caused by the bucket wheel of the steamer, were also diagnosed. In addition to the signs of blunt force trauma the findings of drowning such as an over inflation of the lungs, fluid in the stomach and duodenum were revealed. Furthermore, algological analysis detected diatoms in the lung tissue and blood from the left heart. Therefore, the cause of death was considered being a combination of fatal hemorrhage, caused by the aortic rupture, and drowning. We conclude that virtual autopsy using combined post-mortem MSCT and MRI is a useful tool for documentation, visualisation and analysis of the findings of blunt force trauma and drowning with a large potential in forensic medicine.

The purpose of this article is to present a quantitative analysis of the human failure contribution in the collision and/or grounding of oil tankers, considering the recommendation of the ""Guidelines for Formal Safety Assessment"" of the International Maritime Organization. Initially, the employed methodology is presented, emphasizing the use of the technique for human error prediction to reach the desired objective. Later, this methodology is applied to a ship operating on the Brazilian coast and, thereafter, the procedure to isolate the human actions with the greatest potential to reduce the risk of an accident is described. Finally, the management and organizational factors presented in the ""International Safety Management Code"" are associated with these selected actions. Therefore, an operator will be able to decide where to work in order to obtain an effective reduction in the probability of accidents. Even though this study does not present a new methodology, it can be considered as a reference in the human reliability analysis for the maritime industry, which, in spite of having some guides for risk analysis, has few studies related to human reliability effectively applied to the sector.

During the course of transmission electron microscopic studies of adult Ancylostoma caninum removed from a dog, several Giardia trophozoites were found in sections of the buccal cavity, oesophagus and intestine of several hookworms. Although the protozoa appeared viable, this unusual finding probably represents accidental uptake by, rather than an established infection of, the hookworm. It is feasible, however, that the trophozoites might have survived and even multiplied in this aberrant site. (C) 2001 Elsevier Science B.V. All rights reserved.

Risk equations have been developed to assist in determining fitness for work of people with diseases that may cause rapid loss of control. The four equations calculate the frequency of fatal injury to the person with the disease, the frequency of fatal injury to colleagues in the workplace, and the cost of fatal injury and property damage to the employer, it is suggested that the additional risk of fatal injury to the person with the disease should not exceed the fatal injury rate in high-risk industries such as forestry, fishing and mining. it is also suggested that the additional risk of fatal injury to each colleague should be no more than one-tenth of the fatal injury rate due to motor vehicle accidents in the community. Two hypothetical case examples are given, demonstrating the use of the equations. The equations highlight the need to examine the risks associated with individuals, their specific jobs and their workplaces. They also highlight significant uncertainties in the determination of fitness, which perhaps have been underestimated in the past. Wherever possible, redundant defences should be utilized to prevent accidents in the event of sudden incapacity.

Complex industrial plants exhibit multiple interactions among smaller parts and with human operators. Failure in one part can propagate across subsystem boundaries causing a serious disaster. This paper analyzes the industrial accident data series in the perspective of dynamical systems. First, we process real world data and show that the statistics of the number of fatalities reveal features that are well described by power law (PL) distributions. For early years, the data reveal double PL behavior, while, for more recent time periods, a single PL fits better into the experimental data. Second, we analyze the entropy of the data series statistics over time. Third, we use the Kullback–Leibler divergence to compare the empirical data and multidimensional scaling (MDS) techniques for data analysis and visualization. Entropy-based analysis is adopted to assess complexity, having the advantage of yielding a single parameter to express relationships between the data. The classical and the generalized (fractional) entropy and Kullback–Leibler divergence are used. The generalized measures allow a clear identification of patterns embedded in the data.

In February 2012, an outbreak of respiratory illness occurred on the cruise ship MSC Armonia in Brazil. A 31-year-old female crew member was hospitalized with respiratory failure and subsequently died. To study the etiology of the respiratory illness, tissue taken at necropsy from the deceased woman and respiratory specimens from thirteen passengers and crew members with respiratory symptoms were analyzed. Influenza real-time RT-PCR assays were performed, and the full-length hemagglutinin (HA) gene of influenza-positive samples was sequenced. Influenza B virus was detected in samples from seven of the individuals, suggesting that it was the cause of this respiratory illness outbreak. The sequence analysis of the HA gene indicated that the virus was closely related to the B/Brisbane/60/2008-like virus, Victoria lineage, a virus contained in the 2011-12 influenza vaccine for the Southern Hemisphere. Since the recommended composition of the influenza vaccine for use during the 2013 season changed, an intensive surveillance of viruses circulating worldwide is crucial. Molecular analysis is an important tool to characterize the pathogen responsible for an outbreak such as this. In addition, laboratory disease surveillance contributes to the control measures for vaccine-preventable influenza.

The first reported case of an accident with Bothriopsis taeniata in Brazil is described. The victim, a 43-year-old man, was bitten just above his right heel and presented a clinical condition compatible with mild Bothrops poisoning: local edema with hemorrhage at the bite site and pain, although without coagulopathy.

Conus regius is a venomous mollusc in the Conidae family, which includes species responsible for severe or even fatal accidents affecting human beings. This is the first report on a clinical case involving this species. It consisted a puncture in the right hand of a diver who presented paresthesia and movement difficulty in the whole limb. The manifestations disappeared after around twelve hours, without sequelae.

Describes the case of a 6-year-old girl who was stung by a Centruroides testaceus, a scorpion native to the Lesser Antilles, in the Guarulhos International Airport, São Paulo, Brazil, as she disembarked from a flight coming from the Caribbean. The patient presented only local symptoms (a small area of erythema and pain at the sting site), which were resolved after a few hours with analgesics, without the need for antivenom. Physicians who treat patients stung by scorpions should be alert to the possibility of such accidents being caused by non native species, especially those cases that occur near airports or ports.

A particular event concerning a Swan-Ganz catheter complication is reported. A 41-year-old woman was admitted at the emergency room of our hospital with massive gastrointestinal bleeding. A total gastrectomy was performed. During the postoperative period in the intensive care unit , the patient maintained hemodynamic instability. Invasive hemodynamic monitoring with a pulmonary artery catheter was then indicated. During the maneuvers to insert the catheter, a true knot formation was identified at the level of the superior vena cava. Several maneuvers by radiological endovascular invasive techniques allowed removal of the catheter. The authors describe the details of this procedure and provide comments regarding the various techniques that were employed in overcoming this event. A comprehensive review of evidence regarding the benefits and risks of pulmonary artery catheterization was performed. The consensus statement regarding the indications, utilization, and management of the pulmonary artery catheterization that were issued by a consensus conference held in 1996 are also discussed in detail.

Ship tracking systems allow Maritime Organizations that are concerned with the Safety at Sea to obtain information on the current location and route of merchant vessels. Thanks to Space technology in recent years the geographical coverage of the ship tracking platforms has increased significantly, from radar based near-shore traffic monitoring towards a worldwide picture of the maritime traffic situation. The long-range tracking systems currently in operations allow the storage of ship position data over many years: a valuable source of knowledge about the shipping routes between different ocean regions. The outcome of this Master project is a software prototype for the estimation of the most operated shipping route between any two geographical locations. The analysis is based on the historical ship positions acquired with long-range tracking systems. The proposed approach makes use of a Genetic Algorithm applied on a training set of relevant ship positions extracted from the long-term storage tracking database of the European Maritime Safety Agency (EMSA). The analysis of some representative shipping routes is presented and the quality of the results and their operational applications are assessed by a Maritime Safety expert.
