OBJECTIVE: To determine if some stimulated whole saliva parameters are influenced by an increase of Body Mass Index. METHODS: Controlled cross-sectional study involving 90 school children of both genders between 7 and 10 years of age, from Bragança Paulista - SP. Three groups were formed: overweight, obese and control. Body Mass Index and diet intake by the Food Register method were evaluated. The salivary pH, flow rate, buffer capacity, protein, phosphate, calcium, fluoride, total and free sialic acid, and peroxidase activity were determined. RESULTS: The overweight and obese groups showed greater energy and lipid intake (P< 0.001) than the control group. There was no difference in the saliva flow rate between groups, however only the control group showed a mean value considered normal. In the overweight and obese groups a decrease in both the concentration of phosphate (P< 0.001) and peroxidase activity (P<0.001) was observed. In the obese group an increase in the concentrations of free sialic acid (P= 0.004) and protein (P= 0.003) occurred. CONCLUSION: Overweight and obese children show alterations in the concentrations of phosphate, free sialic acid and proteins, and in the peroxidase activity that are favorable conditions for dental caries.

De etiologia discutível, as ulcerações aftosas recorrentes (UAR) são, na maioria das vezes, consideradas em um quadro de deficiência imunológica. OBJETIVOS: Este trabalho baseia-se na revisão de literatura e em investigações clínico-laboratoriais dos títulos salivares de imunoglobulina A secretora (IgA-s), proteínas totais e fluxo de saliva (em repouso) de pacientes nos períodos de atividade e quiescência das Ulcerações aftosas recorrentes. Forma de Estudo: Estudo clínico e experimental. MATERIAL E MÉTODOS: Empregou-se a Nefelometria na quantificação das IgA-s salivares, o vermelho de Pirogalol para as proteínas totais salivares e a análise gravimétrica para a determinação do fluxo de saliva. RESULTADOS: Os resultados demonstraram aumento estatisticamente significativo das IgA-s salivares nos períodos de lesão ativa em relação aos de quiescência. Não houve diferença estatística nas concentrações de proteínas totais e no fluxo de saliva. CONCLUSÃO: Sugere-se que a IgA-s pode ser usada como parâmetro do status imune da mucosa oral.

The purpose of this study was to investigate the activities of the total acid phosphatase (TAP), tartrate-resistant acid phosphatase (TRAP), low molecular weight protein tyrosine phosphatase (LMW-PTP) and alkaline phosphatase (ALP) enzymes, as well as the possible correlation in the serum and in unstimulated whole saliva of children. Enzymatic activities were measured in pairs of concurrently obtained serum and salivary samples from 32 children in good oral and systemic health (16 of each sex) with a median age of 6.4 ± 3.3 years (range 1.08 – 12.92 years). All collections were made between the hours of 08:00 – 10:00 a.m. We used p-nitrophenyl phosphate as the substrate in the enzymatic assay for TAP, TRAP and LMW-PTP, and thymolphthalein monophosphate as the substrate for ALP. The enzymatic activities of all the studied enzymes were higher in serum than in saliva. The mean of enzymatic activities of serum TAP, TRAP, LMW-PTP and ALP were 36.51 ± 8.21, 23.99 ± 5.73, 11.16 ± 5.65 and 76.50 ± 17.32 U/L, respectively, while the mean salivary TAP, TRAP, LMW-PTP and ALP enzymatic activities were 9.60 ± 5.04, 1.36 ± 0.87, 5.65 ± 3.07 and 4.08 ± 1.83 U/L in this order. The TRAP revealed a positive linear correlation between its activity in the serum and saliva (Spearman r = 0,4685, p < 0,05). We concluded that the salivary TRAP has a potential to be use as biomarkers of pathologies and states that modify its activity in the serum.

OBJETIVO: Comparar os níveis de cortisol sérico e salivar, alfa-amilase salivar (sAA) e fluxo de saliva não estimulada (UWS) em gestantes e não gestantes. MÉTODOS: Trata-se de um estudo longitudinal realizado no centro de promoção da saúde de um hospital universitário. Nove gestantes e 12 não gestantes participaram do estudo. Foram coletados e analisados soro e UWS nos três trimestres gestacionais e duas vezes por mês durante o ciclo menstrual. A análise do cortisol salivar e sérico foi realizada com o uso de quimiluminescência e a atividade da sAA foi determinada por meio de analisador automático para bioquímica. RESULTADOS: Foi verificado que a mediana (intervalo interquartil) dos níveis de cortisol sérico no grupo de gestantes foi maior que 23,8 µL/dL (19,4-29,4) quando comparado ao grupo de não gestantes, que teve média de 12,3 (9,6-16,8; p<0,001). Os níveis de sAA seguiram o mesmo padrão, com médias de 56,7 U/L (30,9-82,2) e 31,8 (18,1-53,2; p<0,001), respectivamente. Foram observadas diferenças dos níveis de cortisol sérico e salivar (µL/dL) e de sAA entre a fase folicular versus a fase lútea (p<0,001). As medianas dos fluxos salivares (UWS) foram semelhantes em gestantes (0,26 [0,15-0,30] mL/min) e não gestantes (0,23 [0,20-0,32] mL/min). Foram encontradas correlações significativas entre o cortisol salivar e o sérico (p=0,02) e entre o cortisol salivar e a sAA (p=0,01). CONCLUSÕES: Os níveis de cortisol sérico de sAA durante a gestação elevam-se. Na fase lútea do ciclo ovariano, os níveis de cortisol salivar aumentam ao passo que os níveis de cortisol sérico e sAA diminuem. _______________________________________________________________________________________ ABSTRACT

O objeto apresenta logo de início uma explicação sobre o que vem a ser a saliva, além de sua composição, chegando a elencar o que é adicionado a ela até ser conceituada como saliva total. Ressalta ainda que um fluxo salivar adequado é pré-requisito essencial para uma boa saúde bucal e que a identificação e o conhecimento dos seus componentes evidenciaram a complexidade do fluído salivar. Segue apresentando suas funções e o controle da produção e excreção salivar, além das características dos parassimpaticomiméticos na elevação da secreção e os parassimpaticolíticos, que propiciam a inibição da secreção salivar. Esclarece a aparente confusão no uso dos termos xerostomia e hipossalivação, que muitas vezes são empregados como sinônimos e lista 32 fármacos que apresentavam a xerostomia como possível efeito colateral. Termina por indicar o tratamento da boca seca através de controle de sinais e sintomas, paliativo, quando não for possível a substituição dos medicamentos e fornece uma lista com sete formas alternativas de tratamento paliativo. Unidade 5 do módulo 8 que compõe o Curso de Especialização em Saúde da Família.

INTRODUÇÃO: A Doença do Refluxo Gastroesofágico (DRGE) é a doença digestiva mais prevalente da atualidade e, recentemente, tem sido implicada em uma gama de alterações do seguimento laringofaríngeo (RLF). No entanto, pouco se sabe dos mecanismos fisiopatológicos destas manifestações supraesofágicas da DRGE. Os achados clínicos contraditórios e recentes pesquisas sugerem haver deficiências na capacidade de defesa deste seguimento. Uma das principais responsáveis pela homeostase da mucosa oral e do trato digestivo é a saliva com seu conteúdo orgânico e inorgânico. Tanto alterações do pH quanto do volume salivar já foram correlacionados com os sintomas e sinais sugestivos da DRGE e RLF. Estudo recente de nossa autoria demonstra diminuição estatisticamente significante do pH salivar de indivíduos com RLF quando comparado a controles sem a doença. Outro estudo constatou correlação entre a redução do volume X pH da saliva em indivíduos com DRGE, estando esta redução diretamente relacionada aos níveis de pH esofágico constatados durante pH-metria esofágica de 24 horas. OBJETIVOS: Avaliar como se comportam o pH e volume da saliva em um mesmo indivíduo com DRGE e RLF antes e após o tratamento clínico. MATERIAL E MÉTODO: Vinte e três pacientes com RLF tiveram o pH e volume da saliva total testados antes e após receberem tratamento com droga bloqueadora de bomba de prótons durante 12 semanas. RESULTADOS: Houve uma diferença estatisticamente significante (p<0,001) entre o pH da saliva antes e após o tratamento, estando este maior após o controle clínico da doença. O volume de saliva no paciente tratado foi significativamente maior do que no paciente pré-tratamento (p=0.009). DISCUSSÃO: Os achados sugerem que o pH salivar é influenciado pela presença de refluxo gastroduodenal à região laringofaríngea. Caso estudos futuros com populações maiores realmente comprovem esta correlação, poderemos cogitar a possibilidade de usar a mensuração do pH salivar, que é feita de forma rápida e não invasiva, como um meio de diagnosticar e avaliar o comportamento e controle do Refluxo Laringofaríngeo.

Objetivos: Determinar la concentración de glucosa salival de individuos sanos y compararla con la glicemia capilar. Diseño del estudio: Se realizó una recolección de saliva total en reposo, en 63 individuos no diabéticos. Se midió la concentración de glucosa salival y sanguinea capilar en todos los individuos. La glucosa salival fue determinada por método enzimático y espectrofotometría. Los datos fueron analizados usando el test de correlación de Spearman, considerando significativos valores de p<0,05. Resultados: Del total de la muestra, 47,6% eran varones y 52,4% mujeres, con una media de edad de 37,5±15,7 años. Las medias del flujo salival en reposo fueron de 0,41±0,2l ml/min en el género masculino y de 0,31±0,15 ml/min en el género femenino. No hubo diferencia significativa (p=0,07S). La media de glucosa sanguínea entre los varones fue de l00,05± 13,51 mg/dL y de 99 ,5± 13,9 mg/dL en las mujeres. La media de glucosa salival en el total de la muestra fue de 5,97±1 ,87 mg/dL, siendo 5,91±2, 19m9/dL en los varones y 5,97±1 ,56mg/dL en las mujeres, sin presentarse diferencias significativas (p=0,908). La concentración de la glucosa salival no presentó correlación estadísticamente significativa con la glicemia capilar (p=0,732). Conclusiones: De los resultados se desprende que: la concentración de la glucosa salival no depende de la glicemia capilar; la concentración de la glucosa salival no presenta diferencias entre géneros

The aim of this study was to examine the role of nifedipine and Nitric Oxide (NO) on salivary flow and compounds (salivary amylase, saliva total proteins, saliva calcium, sodium and potassium). Male Holtzman rats weighting 200-250 g were anesthetized with zoletil 50 mg kg -1 (tiletamine chloridrate 125.0 mg and zolazepan chloridrate 125.0 mg) into quadriceps muscle and stainless steel cannulas were implanted into their lateral ventricle of the brain (LV). Animals in divided group were injected with nifedipine (50 μg μL -1) alone and in combination with 7-nitroindazol (7-NIT) (40 μg μL -1), neuronal NO Sinthase Inhibitor (nNOSI) and Sodium Nitroprussate (SNP) (30 μg μL -1) NO donor agent. As a secretory stimuli, pilocarpine dissolved in isotonic was administered intraperitoneally (ip) at a dosage of 10 mg kg -1 body weight. Saliva was collected for 7 min with four cotton balls weighing approximately 20 mg each, two of which were placed on either side of the oral cavity, with the other two placed under the tongue. Nifedipine treatment induced a reduction in saliva secretion rate and concentration of amylase, total protein and calcium without changes in sodium and potassium concentration in comparison with controls. Co-treatment of animals with nifedipine and SNP retained flow rate and concentration of amylase, total protein and calcium in normal levels. Co-treatment of animals with nifedipine and 7-NIT potentiated the effect of nifedipine on the reduction of saliva secretion and concentrations of amylase, total protein and calcium. Nifedipine (dihydroperidine) calcium-channel blocker widely in use is associated with salivary dysfunction acting in the central nervous system structures. NO might be the mechanism for protective effect against the nifedipine-induce salivary dysfunction, acting in the CNS. © 2006 Asian Network for Scientific Information.

Objetivos: El Síndrome de Sjögren (SS) afecta las glándulas exócrinas que provocan boca seca y ojos secos. El diagnóstico de SS requiere una evaluación por distintos especialistas médicos. El propósito de este estudio era el de correlacionar distintos métodos que se utilizan para el diagnóstico de SS. Materiales y Métodos: El estudio se realizó en la clínica del Programa de Periodoncia UANL. Los parámetros que se compararon incluyen: 1. duración de la enfermedad (reportada: <1 Año, 1-5 Años, >5 Años); 2. Síntomas de boca seca, ojos secos y dificultad para tragar (reportada: positiva o negativa); 3. Rango del flujo de saliva total (RFST) (reportada: positiva <1.5 ml en 15 minutos o negativa ≥1.5 ml in 15 minutes); 4. Biopsia de glándula salivar menor (BGSM) (reportada: positiva o negativa); 5. Atrofia (reportada: presente o ausente). Se utilizó una prueba exacta de Fisher y de chi-square de independencia se utilizaron para el análisis. Resultados y Estadística: 116 pacientes se incluyeron en el estudio, todos femeninos. 76 pacientes tenían SS primario y 40 SS secundario con un promedio de edad de 52.9 ± 12.0 años. 44% presentaron una duración de la enfermedad de 1 a 5 años; 86% positivos a boca seca; 81% tuvieron un RFST reducido; 91.4% positivo para ojos secos; 64.7% positivo en dificultad para tragar; 83.5% positivo para la biopsia y 31.9% tenían presencia de atrofia. El análisis estadístico reveló una correlación significativa entre la duración de la enfermedad y ojos secos (p=0.034) y entre la duración de la enfermedad y RFST (p=0.021). Asociaciones significativas fueron encontradas entre RFST y boca seca (p=0.008), entre RFST y ojos secos (p=0.014), entre RFST y dificultad para tragar (p=0.001). La atrofia no fue asociada significativamente con boca seca o RFST. Conclusiones: En general, los resultados de este estudio sugieren que la medida del RFST es un método no invasivo útil para la evaluación de rutina y el monitoreo de los pacientes con Síndrome de Sjögren.

This study evaluated in vitro the shear bond strength (SBS) of a resin-based pit-and-fissure sealant [Fluroshield (F), Dentsply/Caulk] associated with either an etch-and-rinse [Adper Single Bond 2 (SB), 3M/ESPE] or a self-etching adhesive system [Clearfil S3 Bond (S3), Kuraray Co., Ltd.] to saliva-contaminated enamel, comparing two curing protocols: individual light curing of the adhesive system and the sealant or simultaneous curing of both materials. Mesial and distal enamel surfaces from 45 sound third molars were randomly assigned to 6 groups (n=15), according to the bonding technique: I - F was applied to 37% phosphoric acid etched enamel. The other groups were contaminated with fresh human saliva (0.01 mL; 10 s) after acid etching: II - SB and F were light cured separately; III - SB and F were light cured together; IV - S3 and F were light cured separately; V - S3 and F were light cured simultaneously; VI - F was applied to saliva-contaminated, acid-etched enamel without an intermediate bonding agent layer. SBS was tested to failure in a universal testing machine at 0.5 mm/min. Data were analyzed by one-way ANOVA and Fisher's test (α=0.05).The debonded specimens were examined with a stereomicroscope to assess the failure modes. Three representative specimens from each group were observed under scanning electron microscopy for a qualitative analysis. Mean SBS in MPa were: I-12.28 (±4.29); II-8.57 (±3.19); III-7.97 (±2.16); IV-12.56 (±3.11); V-11.45 (±3.77); and VI-7.47 (±1.99). In conclusion, individual or simultaneous curing of the intermediate bonding agent layer and the resin sealant did not seem to affect bond strength to saliva-contaminated enamel. S3/F presented significantly higher SBS than the that of the groups treated with SB etch-and-rinse adhesive system and similar SBS to that of the control group, in which the sealant was applied under ideal dry, noncontaminated conditions.

Introduction: Whole blood is used for diagnosis of lead exposure. A non-invasive method to obtain samples for the biomonitoring of lead contamination has become a necessity. This study 1) compares the lead content in whole saliva samples (Pb-saliva) of children from a city with no reported lead contamination (Ribeirao Preto, Sao Paulo State, Brazil) and children of a region notoriously contaminated with lead (Bauru, Sao Paulo State, Brazil), and 2) correlates Pb-saliva with the lead content in the enamel microbiopsy samples (Pb-enamel) in the case of these two populations. Methods: From a population of our previous study that had included 247 children (4- to 6-year-old) from Ribeirao Preto, and 26 children from Bauru, Pb-saliva was analyzed in 125 children from Ribeirdo Preto and 19 children from Bauru by inductively coupled plasma mass spectrometry (ICPMS). To correlate Pb-saliva with Pb-enamel, we used Pb-enamel data obtained in our previous study. The Mann-Whitney test was employed to compare the Pb-saliva data of the two cities. Pb-saliva and Pb-enamel values were then Log(10) transformed to normalize data, and Pb-saliva and Pb-enamel were correlated using Pearson`s correlation coefficient. Results: Median Pb-saliva from the Ribeirao Preto population (1.64 mu g/L) and the Bauru population (5.85 mu g/L) were statistically different (p<0.0001). Pearson`s correlation coefficient for Log(10) Pb-saliva versus Log(10) Pb-enamel was 0.15 (p=0.08) for Ribeirao Preto and 0.38 (p=0.11) for Bauru. Conclusions: A clear relationship between Pb-saliva and environmental contamination by lead is shown. Further studies on Pb-saliva should be undertaken to elucidate the usefulness of saliva as a biomarker of lead exposure, particularly in children. (C) 2008 Elsevier B.V. All rights reserved.

There is an increasing interest about the use of stable isotopes for body composition analysis in pediatrics. To ensure the success of total body water analysis by the deuterium dilution method, it is fundamental to determine the equilibrium tune (plateau) of deuterium in the body fluid studied. Objectives: We report here the equilibration time of deuterium oxide in the saliva of newborns after oral intake of the isotope. Methods: Twenty healthy term newborn infants, 10 males and 10 females, were analyzed. Saliva was collected from each newborn before the oral administration of a 100 mg/kg dose of deuterium oxide (baseline sample) and then at 1-hour intervals for 5 hours after administration. Deuterium enrichment of saliva was determined by isotope ratio mass spectrometry according to the recommendations of the International Atomic Energy Agency. Results: The plateau time of deuterium in saliva occurred 3 hours after oral administration of the stable isotope. Conclusion: These data are essential for further studies on the body composition of newborn infants. To the best of our knowledge, this is the first study regarding the equilibration time of deuterium in the saliva of term newborns. JPGN 48:471-474, 2009.

The aim of this longitudinal study was to determine salivary levels of total IgA, IgG and IgM in 84 preterm and 214 full-term infants, from birth to 18 months of age. Unstimulated whole saliva was collected from each infant at birth, and subsequently at 3-monthly intervals. Immunoglobulin levels were estimated using an ELISA technique. The possibility of detecting acute infection and immunity using body fluids that are easier to collect than blood, mainly in children, would facilitate the investigation and follow-up of outbreaks of hepatitis A (HAV). Our study was carried out to evaluate the detection of anti-HAV IgM, IgA and total antibodies in saliva using serum samples as reference. Forty three paired serum and saliva samples were analyzed. From this total, 24 samples were obtained from children and 1 from one adult during the course of acute hepatitis A; an additional 18 samples were obtained from health professionals from Adolfo Lutz Institute. The sensitivity to detect anti-HAV IgM was 100% (95%CI: 79.1 to 100.0%), employing saliva as clinical samples. In detecting anti-HAV IgA, the sensitivity was 80.8% (95%CI: 60.0 to 92.7%) and for the total antibodies was 82.1% (95%CI: 62.4 to 93.2%). The specificity was 100% for each. The rate of agreement was high comparing the results of serum and saliva samples for detecting HAV antibodies. We conclude that saliva is an acceptable alternative specimen for diagnosing acute hepatitis A infection, and for screening individuals to receive hepatitis A vaccine or immunoglobulin.
