The guardians of children brought to the Port Moresby General Hospital's Children's Outpatient Department with a chief complaint of diarrhoeal disease were questioned regarding their preference of glucose-based vs rice-based oral rehydration solution (ORS) in order to determine the acceptability of a rice-based ORS. Of the 93 guardians interviewed, greater than 60% preferred the glucose-based solution in its mixability, appearance and taste, and 65% initially reported that their children preferred the taste of the glucose solution. However, after a 30-minute trial, only 58% of children still preferred the glucose solution. In a country where diarrhoeal disease is a leading cause of child death and guardians are the primary health care providers, the acceptability of an ORS is critical to the morbidity and mortality of Papua New Guinea's children. Killing an estimated 2.9 million children annually, diarrheal disease is the second leading cause of child mortality worldwide. Diarrheal disease is also the second leading cause of child mortality in Papua New Guinea (PNG), killing an average 193 inpatient children per year over the period 1984-90. However, despite the high level of diarrhea-related mortality and the proven efficacy of oral rehydration therapy (ORT) in managing diarrhea-related dehydration, standardized ORT has been underutilized in PNG. The current glucose-based oral rehydration solution (ORS) does not reduce the frequency or volume of a child's diarrhea, the most immediate concern of caregivers during episodes of illness. Cereal-based ORS, made from cereals which are commonly available as food staples in most countries, better address the short-term concerns of caregivers while offering a superior nutritional profile. A sample of guardians of children brought to the Port Moresby General Hospital's Children's Outpatient Department complaining of child diarrhea were asked about their preferences on glucose-based versus rice-based ORS in order to determine the acceptability of a rice-based ORS. More than 60% of the 93 guardians interviewed preferred the glucose-based solution for its mixability, appearance, and taste. 65% initially reported that their children preferred the taste of the glucose solution. PRINCIPLES: Coeliac disease (gluten sensitive enteropathy) is a genetically determined disorder with an incidence in the general population that is comparable to type 2 diabetes mellitus. Awareness of this fact and of the often atypical and oligosymptomatic manifestations is only now gaining ground in the medical profession. A high index of suspicion is important in order to minimise diagnostic and therapeutic delay. METHODS: Testing patterns and follow-up for coeliac disease in our institution have been analysed retrospectively for the past five years. The current literature was reviewed with respect to recommendations for clinical practice. RESULTS: A total of 271 patients were tested for coeliac disease over a period of five years. Only in 24 patients were positive results found; after further work-up, the final number of cases with certain or presumed coeliac disease was four. Followup was often difficult, many patients being lost after a single visit. CONCLUSIONS: This study showed that the number of tests ordered in our institution, more often for abdominal than atypical symptoms, has started to increase in the past two years. It also showed that screening tests have found their place in general clinical practice, while the final choice of tests needs to be determined in accordance with available guidelines and local resources. Upper endoscopy with small bowel biopsy remains the gold standard for diagnosis, but its place in follow-up is less certain. Coeliac disease is a disorder for which there is a definite treatment (gluten free diet); if it is left untreated diminished quality of life and potentially serious complications may ensue. Further education of the medical profession regarding coeliac disease, its incidence, presentation and treatment, is clearly indicated..

We compared the costs incurred by families attending outpatient appointments at the Royal Children's Hospital (RCH) in Brisbane with those incurred by families who had a consultation via videoconference in their regional area. In each category 200 families were interviewed. The median time spent travelling for videoconferences was 30 min compared with 80 min for face-to-face appointments. Families interviewed in the outpatient department had travelled a median distance of 70 km, while those who had a videoconference at the local hospital had travelled only 20 km. It cost these families much more to attend an appointment at the RCH than to attend a videoconference. Ninety-six per cent of families (193) reported at least one of the following types of expense: 150 families had expenses related to parking (median A$10), 156 had fuel expenses (median A$10) and 122 reported costs related to meals purchased at the RCH (median A$10). Only 21 families who had their appointment via local videoconference reported any additional costs. Specialist appointments via videoconference were a more convenient and cheaper option for families living in regional areas of Queensland than the conventional method of attending outpatient appointments at the specialist hospital in Brisbane.

Background: There is a paucity of research assessing health-related quality of life (HRQoL) and self-efficacy in caregivers of relatives with dementia in mainland China. Aims: To compare the level of HRQoL between caregivers and the general population in mainland China and to assess the role of caregiver self-efficacy in the relationship between caregiver social support and HRQoL. Methods: A cross-sectional study was conducted in Shanghai, China. The caregivers were recruited from the outpatient department of a teaching hospital. A total of 195 participants were interviewed, using a survey package including the Chinese version of the 36-Item Short-Form Health Survey (SF-36), demographic data, the variables associated with the impairments of care recipients, perceived social support and caregiver self-efficacy. The caregivers' SF-36 scores were compared with those of the general population in China. Results: The results indicated that the HRQoL of the caregivers was poorer compared with that of the general population when matched for age and gender. Multiple regression analyses revealed that caregiver self-efficacy is a partial mediator between social support and HRQoL, and a partial mediator between behavioral and psychological symptoms of dementia (BPSD) and caregiver mental health. Conclusion: Assisting with managing BPSD and enhancing caregiver self-efficacy can be considered integral parts of interventions to improve caregiver HRQoL.

Measurements were made of the intake of a WHO/UNICEF glucose-based and a rice cereal-based oral rehydration solution (ORS) by children with diarrhoea. Twenty children who presented to the Children's Outpatient Department at Port Moresby General Hospital with acute diarrhoea and mild dehydration were randomly assigned to an ORS and measurements were taken over the following 3 hours. For data analysis, the patients were paired by weight. Testing the means of the paired samples by t test showed that there was no significant difference between the amount of rice ORS and the amount of glucose ORS taken over 3 hours. The discovery of oral rehydration solution (ORS) for the treatment of diarrheal disease has been heralded as the most important medical discovery of the century. Cereal-based ORS is able to decrease stool output and the duration of diarrheal illness more than the standard glucose-based ORS, through the increased absorption provided by oligosaccharides without the imposition of a greater osmotic penalty. Moreover, the peptides in cereals enhance amino acid and water absorption, while providing nutritional benefits. UNICEF's glucose-based ORS is becoming more widely used in Papua New Guinea (PNG). 20 children aged 6-37 months (mean age, 15 months) who presented to the Children's Outpatient Department at Port Moresby General Hospital during September-October 1993 with acute diarrhea and mild dehydration were randomly assigned to receive either a rice-based ORS or standard glucose ORS, and measurements were taken over the following 3 hours. The patients were paired by weight for analysis. No statistically significant difference was found between the amount of rice ORS and the amount of glucose ORS taken over 3 hours.

Background: Acute febrile respiratory illnesses, including influenza, account for a large proportion of ambulatory care visits worldwide. In the developed world, these encounters commonly result in unwarranted antibiotic prescriptions; data from more resource-limited settings are lacking. The purpose of this study was to describe the epidemiology of influenza among outpatients in southern Sri Lanka and to determine if access to rapid influenza test results was associated with decreased antibiotic prescriptions.

Methods: In this pretest- posttest study, consecutive patients presenting from March 2013- April 2014 to the Outpatient Department of the largest tertiary care hospital in southern Sri Lanka were surveyed for influenza-like illness (ILI). Patients meeting World Health Organization criteria for ILI-- acute onset of fever ≥38.0°C and cough in the prior 7 days--were enrolled. Consenting patients were administered a structured questionnaire, physical examination, and nasal/nasopharyngeal sampling. Rapid influenza A/B testing (Veritor System, Becton Dickinson) was performed on all patients, but test results were only released to patients and clinicians during the second phase of the study (December 2013- April 2014).

Results: We enrolled 397 patients with ILI, with 217 (54.7%) adults ≥12 years and 188 (47.4%) females. A total of 179 (45.8%) tested positive for influenza by rapid testing, with April- July 2013 and September- November 2013 being the periods with the highest proportion of ILI due to influenza. A total of 310 (78.1%) patients with ILI received a prescription for an antibiotic from their outpatient provider. The proportion of patients prescribed antibiotics decreased from 81.4% in the first phase to 66.3% in the second phase (p=.005); among rapid influenza-positive patients, antibiotic prescriptions decreased from 83.7% in the first phase to 56.3% in the second phase (p=.001). On multivariable analysis, having a positive rapid influenza test available to clinicians was associated with decreased antibiotic use (OR 0.20, 95% CI 0.05- 0.82).

Conclusions: Influenza virus accounted for almost 50% of acute febrile respiratory illness in this study, but most patients were prescribed antibiotics. Providing rapid influenza test results to clinicians was associated with fewer antibiotic prescriptions, but overall prescription of antibiotics remained high. In this developing country setting, a multi-faceted approach that includes improved access to rapid diagnostic tests may help decrease antibiotic use and combat antimicrobial resistance.

A survey was made of patients receiving H2-receptor antagonists in a group practice serving 8600 patients. Two hundred and twelve patients (2%) who had received at least one prescription for H2 antagonists in a 12 month period were identified. When compared with the practice population, men and patients over 50 years old were more likely to be taking these drugs (P less than 0.01 and P less than 0.001, respectively). One hundred and fifty-seven patients (74%) were investigated before commencing therapy; 114 (73%) of these patients were investigated via the hospital outpatient department, despite the general practitioners having full open access to barium meals. Only 23 (15%) of the patients investigated were found to have no active pathology. Twenty-nine (14%) of the 212 study patients had received one or more gastrointestinal investigations in the 18 months subsequent to starting H2-antagonist therapy. Twenty-five of these patients had also received an investigation before starting therapy. One hundred and eleven patients (52%) had had their H2 antagonist therapy initiated by their general practitioner.

Objetivo: Describir los factores relacionados con la toma de decisión de manejo quirúrgico en pacientes con hidronefrosis secundaria a estrechez de la unión pieloureteral en el servicio de Urología Pediátrica de una institución de IV nivel. Materiales y Métodos: Se realizó un estudio descriptivo retrospectivo. Se seleccionaron por conveniencia a 100 pacientes con diagnóstico antenatal de hidronefrosis, 37 fueron llevados a manejo quirúrgico por estrechez de la unión pieloureteral (EUPU) entre los años 2009 y 2012. Se evaluaron los factores que llevaron a la toma| de esta decisión. Resultados: Los pacientes con diagnóstico postnatal de EUPU representaron el 37% de la población, la indicación de manejo quirúrgico en 13 pacientes (35,1%) fue dilatación caliceal (SFU 3), en 21 pacientes (56,8%) de deterioro de la función renal y en los restantes (8,1%) infección urinaria recurrente. Se encontró una progresión de 30% en la severidad de la dilatación en el periodo postnatal, habían 9 pacientes (24% de la muestra) SFU de 3 y 4 en el periodo prenatal y 20 (54%) en el periodo postnatal que fueron llevados a manejo quirúrgico. De los pacientes que disponíamos de datos precisos de valores de variación porcentual de gammagrafía 16% de la muestra, se encontró que había una variación del 50% en deterioro de la función renal. Conclusión: En el grupo de pacientes colombianos de la consulta externa del servicio de urología pediátrica estudiado se encontró que la decisión de manejo quirúrgico en pacientes con EUPU, está en concordancia con lo encontrado en la literatura mundial, siendo estos la presencia de dilatación caliceal deterioro de la función renal en gammagrafía DMSA.

En este estudio se realizó una encuesta con el fin de evidenciar el conocimiento y la práctica de la Educación Sexual en adolescentes que acudieron al servicio de consulta externa de Medicina General en dos Instituciones Prestadoras de Servicios de Salud (IPS) de la ciudad de Bogotá, una de régimen contributivo y otra de régimen subsidiado. Se realizó un estudio observacional, descriptivo, de corte transversal, a 100 adolescentes, mujeres y hombres, a través de la aplicación de un instrumento prediseñado con un contenido de 17 preguntas donde se indagó acerca de: planificación familiar, conducta sexual, educación sexual, embarazo adolescente y conductas de riesgo en adolescentes. En relación al régimen de aseguramiento en salud que tenían los adolescentes, las diferencias encontradas en sus respuestas fueron a nivel de: educación sexual recibida, acceso a los servicios de planificación familiar de las Empresas Promotoras de Salud (EPS), presencia de embarazo adolescente y consumo de alcohol asociado a la práctica de relaciones sexuales. Tras el análisis de la información obtenida con la aplicación de la encuesta, se concluye que es indispensable crear en las instituciones educativas públicas y privadas de la ciudad de Bogotá, una nueva herramienta metodológica a través de la cual se realice promoción de la salud y refuerzo de prácticas protectoras en educación sexual para adolescentes, pues persiste desconocimiento de la misma.

A incontinência fecal é a incapacidade de manter o controle da eliminação do conteúdo intestinal em local e tempo socialmente adequados, resultando em escape de gases e fezes. Esta condição acarreta grande prejuízo na vida social dos acometidos. A causa conhecida mais comum é o trauma perineal, porém em uma grande proporcão a incontinência é idiopática. A avaliação da função esfincteriana anal é fundamental para o diagnóstico e para a conduta terapêutica na incontinência fecal. Para o entendimento da fisiopatologia desta condição desenvolveram-se vários exames de investigação. A manometria ano-retal é considerado imprescindível na avaliação. A correlação dos dados da manometria com a gravidade da doença e com estudos eletrofisiológicos ainda não estão bem estabelecidos. O objetivo deste estudo é correlacionar os dados da manometria ano-retal e o estudo do tempo de latência motora terminal do nervo pudendo com a incontinência fecal e comorbidades. Foram estudados prospectivamente todos os pacientes com queixa clínica de incontinência fecal atendidos no ambulatório de Serviço de Coloproctologia do Hospital Nossa Senhora da Conceição, de Porto Alegre (RS), entre março de 1997 e junho de 2000. De todos os pacientes foram coletados dados da anamnese que os classificaram segundo escore de incontinência proposto por JORGE & WEXNER (1993). Todos foram submetidos a manometria ano-retal, estudo do tempo de latência motora terminal do nervo pudendo bilateralmente e exame proctológico. Foram excluídos os pacientes que não concluíram toda a investigação, com cirurgias colo-retais baixas prévias ou neoplasia de reto e canal anal. Para análise estatística, os pacientes foram separados em grupos segundo a manometria normal ou alterada (hipotonia), presença ou não de neuropatia de nervo pudendo, por idade e por sexo. Foram estudados 39 pacientes, 85,6% do sexo feminino com idade média de 60,1 anos (±12,89). A média do escore de incontinência fecal foi de 9,30±4,93. À manometria ano-retal, vinte e três pacientes (59%) apresentaram pressões reduzidas. As pressões foram significativamente mais elevadas nos pacientes do sexo masculino. O tempo de latência motora terminal do nervo pudendo (neuropatia) foi prolongado em 14 doentes (35,9%). A idade e o tempo de latência motora terminal do nervo pudendo correlacionaram-se significativamente, r=0,422, (P=0,007). A demais correlações entre idade, pressões da manometria, tempo de latência motora terminal do nervo pudendo e escore de incontinência não foram estatisticamente significativas.

Introduction: The ability to walk is impaired in obese by anthropometric factors (BMI and height), musculoskeletal pain and level of inactivity. Little is known about the influence of body adiposity and the acute response of the cardiovascular system during whole the 6-minute walk test (6mWT). Objective: To evaluate the effect of anthropometric measures (BMI and WHR waist-to-hip ratio), the effort heart and inactivity in ability to walk the morbidly obese. Materials and Methods: a total 36 morbidly obese (36.23 + 11.82 years old, BMI 49.16 kg/m2) were recruited from outpatient department of treatment of obesity and bariatric surgery in University Hospital Onofre Lopes and anthropometric measurements of obesity (BMI and WHR), pulmonary function, pattern habitual physical activity (Baecke Questionnaire) and walking capacity (6mWT). The patient was checking to measure: heart rate (HR), breathing frequency (BF), peripheral oxygen saturation, level of perceived exertion, systemic arterial pressure and duplo-produto (DP), moreover the average speed development and total distance walking. The data were analysed between gender and pattern of body adiposity, measuring the behavior minute by minute of walking. The Pearson and Spearmam correlation coefficients were calculated, and stepwise multiple Regression examined the predictors of walking capacity. All analyses were performed en software Statistic 6.0. Results: 20 obese patients had abdominal adiposity (WHR = 1.01), waist circumference was 135.8 cm in women (25) and 139.8 cm in men (10). Walked to the end of 6mWT 412.43 m, with no differences between gender and adiposity. The total distance walked by obesity alone was explained by BMI (45%), HR in the sixth minute (43%), the Baecke (24%) and fatigue (-23%). 88.6% of obese (31) performed the test above 60% of maximal HR, while the peak HR achieved at 5-minute of 6mWT. Systemic arterial pressure and DP rised after walking, but with no differences between gender and adiposity. Conclusion: The walk of obese didn´t suffers influence of gender or the pattern of body adiposity. The final distance walked is attributed to excess body weight, stress heart, the feeling of effort required by physical activity and level of sedentary to obese. Descreve-se a inovação pedagógica de dois casos na graduação médica: uma matéria denominada Saúde Coletiva III, com (Administração, Ciências Sociais, Epidemiologia, Ética e Nutrição em Saúde Publica) e a disciplina de Semiologia Pediátrica. Para descrição e avaliação dos casos, utilizaram-se métodos qualitativos. A Saúde Coletiva III foi organizada por núcleos temáticos: Problemas em Saúde Publica; Nutrição em Saúde Publica; Planejamento em Saúde. O modelo de ensino centrou-se na problematização de situações vivenciadas na prática da Saúde Publica, trabalhando-se em centros, serviços e organizações de saúde. A Semiologia Pediátrica privilegiou a atenção integral à saúde da criança. O modelo de ensino centrou-se na aprendizagem baseada em problemas e no aprendizado prático da semiologia pediátrica em diferentes cenários, enfatizando-se o ensino ambulatorial. Privilegiou-se o trabalho em pequenos grupos, com a orientação docente. A principal missão voltou-se à utilização de estratégias que valorizassem o ensino centrado no estudante e sua capacidade de construir conhecimento com autonomia. No caso da Pediatria, avançou-se rumo a Medicina Integral, com enfoque amplo do modelo de atenção à criança. A Saúde Coletiva aproximou-se da Medicina Comunitária problematizando situações concretas no SUS e na atenção primária. O estudo mostra a possibilidade de inovação no ensino, e podendo contribuir para a mudança institucional.

CONTEXT: Epidemiological studies have demonstrated higher frequencies of the O blood group and the non-secretor phenotype of ABH antigens among patients suffering from peptic ulcers. Since Helicobacter pylori has been established as the main etiological factor in this disease, controversies about the associations of the ABO and Lewis blood group phenotypes and secretor and non-secretor phenotypes in relation to susceptibility towards infection by this bacillus have been presented. OBJECTIVE: To verify the frequencies of ABO, Lewis blood group phenotypes, secretor and non-secretor phenotypes in patients infected or uninfected by H. pylori. DESIGN: Cross-sectional study. SETTING: Outpatient clinic. PARTICIPANTS: One hundred and twenty patients with dyspeptic symptoms who underwent endoscopy. MAIN MEASUREMENTS: ABO and Lewis blood group phenotypes were determined by a standard hemagglutination test and the secretor and non-secretor phenotypes were evaluated by saliva samples using the inhibitor hemagglutination test. RESULTS: The diagnosis of infection, made via breath and urea tests and confirmed using polymerase chain reaction (PCR) in gastric biopsy fragments, showed the presence of H. pylori in 61.7% of the patients and absence in 38.3%. The differences between the frequencies of the ABO blood group phenotypes among infected (A 27.0%; B 12.2%; AB 4.0% and O 56.8%) and uninfected patients (A 58.7%; B 13.0%; AB 4.3% and O 24.0%) were significant. The Lewis blood type, secretor and non-secretor phenotypes showed homogeneous distribution between the groups of patients analyzed. CONCLUSIONS: Our results suggest that the infection of H. pylori can be related to ABO blood groups but not to the Lewis blood group nor to secretor and non-secretor phenotypes.
