Uma revisão dirigida foi realizada nas bases de dados IBECS, LILACS e MEDLINE, até fevereiro/2011, para identificar intervenções farmacêuticas (IF) na atenção farmacêutica em saúde mental e os seus resultados. Para a busca utilizaram-se os descritores em saúde: Pharmaceutical Care, Pharmaceutical Services, Medication Adherence, Pharmacists, Mental Health, Mental Health Services, Mental Health Assistance, Community Mental Health Services, Mentally Ill Persons andMental Disorders. Identificaram-se 1686 publicações, das quais 21 contemplaram os critérios de inclusão. Após exploração do material, apenas cinco estudos tratavam-se de IF. Todos foram conduzidos no nível secundário de atenção, com abordagem individual, por meio do acompanhamento da terapia (3), intervenção educativa por cartas a médicos e pacientes (1), aconselhamento farmacêutico presencial e remoto e inserção de terapia com sistema transdérmico de nicotina (1). Os resultados, tais como promoção da adesão e resolução de problemas relacionados a medicamentos foram positivos para a terapêutica. No entanto, é necessário que as IF monitorem os parâmetros clínicos, as mudanças de hábitos, a melhora na qualidade de vida e os aspectos farmacoeconômicos a fim de avaliar os seus impactos. ABSTRACT Pharmaceutical interventions in mental health services: a review A directed review was performed in IBECS, LILACS and MEDLINE databases, until February/2011, in order to identify the studies which developed pharmaceutical interventions (PI) in pharmaceutical care in mental health services and estimated their results. The search was carried out using the follow health science descriptors: Pharmaceutical Care, Pharmaceutical Services, Medication Adherence, Pharmacists, Mental Health, Mental Health Services, Mental Health Assistance, Community Mental Health Services, Mentally Ill Persons andMental Disorders. It was identified 1686 manuscripts, of whose 21 contemplated the inclusion criteria. After the content analysis of the eligible manuscripts, only five developed PI. All of them were conducted in the second level of health care, with individual approach, through: therapy follow-up (3), educational interventions by letters to physicians and patients (1), presence or remote pharmaceutical counseling and inclusion of therapy with nicotine transdermal patch (1). The data, such as adherence promotion and solving drug related problems, were positive for the therapeutic. However, it is necessary that the PI monitor the clinical parameters, the habit changes, the improvement in the quality of life and the pharmacoeconomic aspects, in order to assess their impacts. O presente estudo partiu de reflexões acerca dos movimentos da reforma assistencial psiquiátrica e o processo de desinstitucionalização. Apresenta-se a evolução dos espaços de loucura e da Saúde Mental até aos conceitos de comunidade terapêutica, reabilitação psicossocial e suporte social. A Saúde mental não pode ser considerada de forma restrita ao indivíduo pois reflete uma experiência grupal. Objetivo: Identificar variáveis capazes de influenciar o decurso de um processo terapêutico, em regime de comunidade terapêutica na área da Saúde Mental. Método: estudo de natureza mista, de carácter exploratório, contou com a informação relativa a 112 pacientes que estão ou estiveram incluídos no programa terapêutico de uma instituição vocacionada para intervenção em sujeitos com patologia mental severa. Foi utilizada uma grelha por nós construída de forma a obter dados relativos às variáveis idade, diagnóstico, nível de retração social, especificidades da estrutura familiar, suporte disponibilizado, grau de motivação e tipo de alta. Resultados: Os dados revelam a importância do tipo de diagnóstico, especificidades da estrutura familiar e grau de motivação para o processo terapêutico. Por meio destas variáveis podemos indiciar o sucesso ou insucesso terapêutico de um doente mental com uma probabilidade de 71,4%. Os dados sublinham ainda a importância da interação social e comportamentos da família para a motivação do doente para o tratamento. / This study was based on reflections about the movements of the psychiatric care reform and the process of deinstitutionalization. It presents the evolution of the spaces of madness and mental health to the concepts of therapeutic community, psychosocial rehabilitation and social support. The mental health can not be considered narrowly as the individual reflects a group experience. Objective: To identify variables that influence the course of a therapeutic process, on a therapeutic community in the area of Mental Health. Method: study of a mixed nature, exploratory, had information on 112 patients who are or have been included in the therapeutic program of an institution devoted to intervention in subjects with severe mental illness. We used a grid constructed by us to obtain data on age, diagnosis, level of social withdrawal, specific family structure, support available, motivation level and type of discharge. Results: The data reveal the importance of the type of diagnosis, the specific family structure and degree of motivation for the therapeutic process. By means of these variables can indicate the success or treatment failure of a mental patient with a probability of 71.4%. The data underline the importance of social interaction and behavior of the family to the patient's motivation for treatment.

Este estudo pretende explorar a natureza da vergonha, enquanto vivência emocional, e a sua relação com a solidão nos doentes mentais crónicos. Foi avaliada a vergonha interna e externa, a solidão, e estados emocionais negativos, nomeadamente a depressão, ansiedade e stress. Nesta investigação participaram 41 utentes com diagnóstico de perturbação mental. Para avaliar as variáveis psicológicas pretendidas foi utilizada a Escala de Vergonha Interna (ISS), a Escala de Vergonha Externa (OAS), A Escala de Solidão UCLA e a Escala de Depressão, Ansiedade e Stress (DASS-21). Os resultados indicaram que os doentes mentais da nossa amostra apresentam maiores níveis de vergonha comparativamente à população geral. São as mulheres que apresentam valores mais elevados de vergonha interna, enquanto os divorciados apresentam maiores índices de vergonha externa e de solidão. Os doentes que têm uma actividade laboral manifestam menores níveis de solidão e de depressão. Quanto mais elevadas as habilitações literárias dos participantes, menor é o nível de ansiedade. No estudo de comparação entre doentes institucionalizados e não-institucionalizados, verificou-se que os primeiros apresentam níveis significativamente mais elevados de ansiedade, não se distinguindo relativamente à percepção de solidão e de vergonha (interna e externa). A análise da relação entre as variáveis evidenciou que os valores de vergonha interna e de solidão estavam associados de forma positiva e elevada à depressão e de forma moderada à ansiedade e stress. Não obstante as limitações reconhecidas, o presente estudo contribuiu para um melhor conhecimento dos estados emocionais negativos nos doentes mentais. / This study aims to explore the nature of shame, while emotional experience and its relationship with loneliness in the chronic mentally ill. We evaluated the internal and external shame, loneliness, and negative emotional states, including depression, anxiety and stress. 41 users participated in this investigation with a diagnosis of mental disorder. To assess the psychological variables was intended to use Internal Shame Scale (ISS), the Foreign Shame Scale (OAS), the UCLA Loneliness Scale and the Scale for Depression, Anxiety and Stress (DASS-21). The results indicated that the mentally ill in our sample have higher levels of shame compared to the general population. They are women who have higher levels of internal shame, while the divorced have higher rates of external shame and loneliness. Patients who have a work activity demonstrate lower levels of loneliness and depression. The higher the educational level of participants, the lower the level of anxiety. In the comparative study of institutionalized patients and non-institutionalized, it was found that the former have significantly higher levels of anxiety, not distinguishing relation to the perception of loneliness and shame (internal and external). The analysis of the relationship between the variables showed that the values of internal shame and loneliness were positively associated with depression and high and moderately to anxiety and stress. Despite the recognized limitations, this study contributes to a better understanding of negative emotional states in the mentally ill.

One of the most important events which characterizes the process of transitioning to the European Union is the ratification of the Convention for the Protection of Human Rights and Fundamental Freedoms by the European Council in 1950. Since then, the topic of human rights has become the inspiring principle in the construction of the European Community and afterwards the institutional apparatus which constitutes the Union. The primary objective of the European Union States currently is to promote a harmonization of the national legislations on mental health, favoring a central health policy which reduces inequalities amongst the member States. For this reason Europe is a region of the world in which is more abundant the normative one about mental health, especially in form of Recommendations directed to the States by the Council of Europe, although norms of direct application also exist. Special interest has the sentences dictated by the European Court of Human Rights and the conclusions of the European Committee for the Prevention of Torture and Inhuman or Degrading Treatment or Punishment. It should be mentioned the work of European Union equally and of the Office for Europe of the World Organization of the Health. This group of juridical instruments configures the most complete regulation on the mental patient's rights.

Los conceptos para la comprensión del estigma, inician con Goffman en los años sesenta, hasta los estudios de los noventa liderados la World Psychiatri Asociation (WPA). ¿Cómo se crea el estigma?, ¿cómo se difunde? y ¿que efectos tiene sobre los enfermos mentales, sus familias, la sociedad y los sistemas de salud?; es necesario caracterizarlas. Diferencias entre los países desarrollados y en vía de desarrollo, en las características del estigma, sus efectos y consecuencias se han observado; podrían explicarse por las diferencias socioculturales. Este artículo busca determinar cuáles y como son las experiencias que los pacientes mentales crónicos tienen con los trabajadores de la salud en siete regiones colombianas y que factores se encuentran asociados a éstas. Es uno de los objetivos específicos de un protocolo que evalúa otros aspectos del estigma; para la caracterización de las experiencias de estigma en Colombia. Se aplicó entrevista tipo encuesta (Inventario de Experiencias de Estigma), auto diligenciad, a quinientos pacientes de siete regiones del país; la cual se usa en investigaciones de la WPA. Se hallaron niveles bajos pero significativos, de experiencias de estigma con los trabajadores de la salud, se evidenció que la mayoría de experiencias están relacionadas con el llamado “estigma estructural”, de igual manera aparecen algunos factores asociados como el tipo de institución donde ha sido hospitalizado, el numero de hospitalizaciones, el tipo de persona con la que convive, y haber recurrido a la tutela, entre otros.

The overall aim with this thesis is to describe and analyze women’s and men’s recovery processes. More specifically, the aim is to determine what women and men with experience of mental illness describe as contributing to the personal recovery process. The point of departure for the studies was 30 in-depth interviews conducted with 15 men and 15 women. The selection of interview subjects was limited to individuals who had been treated in 24-hour psychiatric care and diagnosed as having schizophrenia, psychosis, a personality disorder, or a bipolar disorder.   Four studies have been carried.  Study 1 was a baseline article that examined what people in recovery from mental illness outline as facilitating factors to their recovery. The results that emerged from that study indicated areas for further analysis to condense the understanding of the recovery process. In study 2 the similarities and the differences in recovery described by women and men were examined. In Study 3 women’s and men’s meaning-making with reference to severe mental illness facilitate the recovery process were studied. The forth study explored how peer-support contribute to women’s and men’s recovery from mental illness.   The results emphasize recovery from mental illness as a social process in which relationships play a key role in creating new identities beside the mental illness. For a majority of the participants meeting peers facilitated the recovery process. The participants described how peer support meant an end to isolation and became an arena for identification, connection, and being important to others. Throughout these recovery processes the impact of gender has been emphasized. The results from this thesis provide new insight into gender as an important factor in understanding the recovery processes. The results from the four studies emphasize the mental patient, the psychiatric interventions and the individual recovery strategies as being influenced by gender constructions.

Este artigo é uma reflexão teórica acerca de como os familiares estiveram incluídos na assistência ao portador de sofrimento psíquico. Iniciamos a partir da constituição da psiquiatria enquanto ciência médica e buscamos chegar até os nossos dias. Percebemos que a família foi excluída do cuidado ao doente mental e que só veio receber a atenção e ser investigada a partir da Segunda Guerra Mundial quando começou o processo de desospitalização. No Brasil as estratégias visando auxiliar a família no enfrentamento do sofrimento psíquico ainda se encontram incipientes.

Purpose: This study investigates the influence of age at onset of OCS on psychiatric comorbidities, and tries to establish a cut-off point for age at onset. Methods: Three hundred and thirty OCD patients were consecutively recruited and interviewed using the following structured interviews: Yale-Brown Obsessive Compulsive Scale; Yale Global Tic Severity Scale and the Structured Clinical Interview for DSM-IV. Data were analyzed with regression and cluster analysis. Results: Lower age at onset was associated with a higher probability of having comorbidity with tic, anxiety, somatoform, eating and impulse-control disorders. Longer illness duration was associated with lower chance of having tics. Female gender was associated with anxiety, eating and impulse-control disorders. Tic disorders were associated with anxiety disorders and attention-deficit/hyperactivity disorder. No cut-off age at onset was found to clearly divide the sample in homogeneous subgroups. However, cluster analyses revealed that differences started to emerge at the age of 10 and were more pronounced at the age of 17, suggesting that these were the best cut-off points on this sample. Conclusions: Age at onset is associated with specific comorbidity patterns in OCD patients. More prominent differences are obtained when analyzing age at onset as an absolute value. © 2008 Elsevier Masson SAS. All rights reserved.

This study aimed to establish a profile of users of the mental health service for homeless in Cork, comparing this group with those attending a General Adult Service. The homeless group were significantly more likely to be male (89% v 46%), unemployed (96% v 68%), unmarried (98% v 75%) and under 65 (94% v 83%). Diagnostically, there was a significantly higher prevalence of schizophrenia (50% v 34%); personality disorder (37% v 11%) and substance dependence (74% v 19%) in the homeless service users. They were more likely to have a history of deliberate self harm (54% v 21%) and violence (48% v 10%). Severe mental illness has a high prevalence in the homeless population, with particularly high levels of factors associated with suicide and homicide. Poor compliance and complexity of illness lead to a requirement for significant input from multidisciplinary mental health teams members.This resource was contributed by The National Documentation Centre on Drug Use.

This study explores areas which need to be improved to develop the quality of patient education to support self-management of patients with mental illness in psychiatric hospitals. The study was conducted in five phases during the period 2000 – 2007. First, patients‘ (n = 313) satisfaction with patient education were investigated. Second, patients' (n = 51) experiences of patient education were explored. Third, a national survey was conducted to investigate realisation of patient education from the staff (n = 55) viewpoint. Fourth, outcomes of patient education were investigated by evaluating the impacts of different patient education methods on patients‘ (n = 311) attitudes towards medication, knowledge level and importance of information. Fifth, patients‘ (n = 16) perceptions of different patient education methods were explored. Patients reported poor satisfaction with patient education (Phase I), and they have considerable need to receive information during their hospital stay (Phase II). Described by staff, the content of patient education covered almost all informational areas investigated. However, discrepancies related to the realisation of patient education were found. (Phase III.) Evaluation of different patient education methods indicate that patients derived benefits from structured patient education with supportive methods (Phase IV) and patients also perceived that these methods supported their information receiving (Phase V). In order to improve the quality of patient education to support self-management of patients with mental illness patient education should be systematically and individually provided to all patients by using different educational methods. Realisation of this should be ensured by providing written instructions, improving nurses‘ knowledge and skills as well ensuring operating conditions.

The case is discussed of a 74-year-old patient hospitalised with acute colitis, who newly developed headache, fever, and mental status changes on the 14th day after admission. The course of the disease was characterised by rapid progression with loss of consciousness and the development of extensive brain oedema, despite broad-spectrum antibiotic therapy. The patient died on the 17th of hospitalisation.
