The paper explores the attitudes of medical physicians towards adverse incident reporting in health care, with particular focus on the inhibiting factors or barriers to participation. It is recognised that there are major barriers to medical reporting, such as the ‘culture of blame’. There are, however, few detailed qualitative accounts of medical culture as it relates to incident reporting. Drawing on a 2-year qualitative case study in the UK, this paper presents data gathered from 28 semi-structured interviews with specialist physicians. The findings suggest that blame certainly inhibits medical reporting, but other cultural issues were also significant. It was commonly accepted by doctors that errors are an ‘inevitable’ and potentially unmanageable feature of medical work and incident reporting was therefore ‘pointless’. It was also found that reporting was discouraged by an anti-bureaucratic sentiment and rejection of excessive administrative duties. Doctors were also apprehensive about the increased potential for managers and non-physicians to engage in the regulation of medical quality through the use of incident data. The paper argues that the promotion of incident reporting must engage with more than the ubiquitous ‘culture of blame’ and instead address the ‘culture of medicine’, especially as it relates to the collegial and professional control of quality.

BACKGROUND: The purpose of this communication is to report a severe occlusive vasculitis as a complication of cat scratch. HISTORY AND SIGNS: A 34-year-old Hispanic woman presented with a sudden visual loss of the right eye associated with shivers, high fever and arthritis which developed 2 months after a cat's bite. Fundus examination showed papillitis and a palor of the paramacular zone of the retina. Fluorescein angiography revealed multiple arterial and venous vasculitic occlusions. THERAPY AND OUTCOME: Auto-immune disease and endocarditis were ruled out by an extensive medical work-up.The diagnosis of Bartonella henselae was confirmed by a positive serology. A systemic antibiotherapy with azithromycin, doxycyclin, rifampicin and steroid therapy resulted in a good clinical response, including a rapid visual recovery with a visual acuity of 20/20 and no relapse of the disease at 6 months follow-up. CONCLUSIONS: Ocular complications associated with cat scratch disease may include vasculitis with both arterial and venous occlusions causing severe visual loss.

OBJECTIVES: Women with a history of preeclampsia (PE) are at increased risk of long term cardiovascular and end-stage renal diseases. However, follow up of preeclamptic women is often omitted, mainly due to a weakness of knowledge of maternal caregivers and lack of comprehensive guidelines. The aim of this study was to define the prevalence of albuminuria, high blood pressure, and renal dysfunction 6 weeks after a preeclampsia. METHODS: This is a prospective case-control study comparing women presenting with preeclampsia to an unmatched control group of women with no hypertensive disorders of pregnancy. A complete medical assessment was performed at 6 weeks post-partum. Recruitment started in June 2010. RESULTS: 324 women were included in the PE group and 50 in the control one. Characteristics of both groups and results of the medical work-up at 6 weeks post-partum are presented in Table 1. Women with preeclampsia presented with a higher BMI, higher prevalence of office high blood pressure, pathological albuminuria and renal hyper-filtration than women in the control group. CONCLUSIONS: Prevalence of post-partum hypertension, and renal dysfunction is higher in women with PE than in uncomplicated pregnancies. Systematic assessment of renal risk factors 6 weeks after preeclampsia allows identification of high-risk women and early implementation of preventive and therapeutic strategies. DISCLOSURES: A. O desvelamento das racionalidades imersas nas ações docentes de uma escola de qualificação profissional de auxiliares de enfermagem, compõem a área temática deste estudo. No intuito de atingirmos esta dimensão de análise, abordamos as seguintes questões de pesquisa: - Qual o entendimento das docentes sobre o seu trabalho enquanto enfermeiras e professoras?; - Qual o entendimento das docentes sobre o trabalho e a qualificação profissional de auxiliares de enfermagem?; - Quais as ações que orientam a prática docente na qualificação profissional, como indicativos da(s) racionalidade(s)?; - Que dimensões estariam presentes na ação docente que possibilitariam uma ação comunicativa?. A teoria de Jürgen Habermas é utilizado como aporte para as idéias trabalhadas neste estudo. A Teoria da Ação Comunicativa, em sua proposta básica, centra-se nas dimensões de desvelamento ideológico de interesses, uso do conhecimento e da racionalidade na busca da emancipação e da autonomia. Esta teoria diferencia três tipos de racionalidades ou ações: instrumental, estratégica e comunicativa. Na coleta de dados é empregada entrevista semi-estruturada, cujas perguntas contemplam as questões acima delineadas, aplicadas à oito professores de uma escola escolhida intencionalmente. A análise de dados utiliza o método de análise de discurso conforme a Pragmática Formal apresentada por Habermas (1987), a qual se preocupa com as dimensões internas e externas dos atos de fala, nem sempre vinculados a questões gramaticais, valendo mais o significado e a consideração de falibilidade da verdade expressa (Habermas, 1987). A dimensão do trabalho das Professoras como categoria teórica, envolve a atividade docente e de enfermeira. No significado que possuem de trabalho, o mesmo é concebido como um agir-racional-com-respeito-a-afins, uma vez que é entendido como uma relação meio-fim. A divisão social e técnica do trabalho, bem como a fragmentação entre a concepção intelectual e manual, são apresentadas em situações distintas ao se referirem ao seu trabalho e ao das auxiliares de enfermagem. Nos seus atos de fala expressam o descontentamento em seguirem normas pré-estabelecidas, com a rotinização dos cuidados e a burocratização dos serviços. No entanto, reproduzem este sistema hierarquizado na relação com as auxiliares de enfermagem, articulando o seu poder como forma de manterem o controle sobre o produto do trabalho. No relacionamento com profissionais de 3º grau surge a hegemonia médica, assumindo as Professoras um posicionamento de submissão, justificado por não considerarem o cuidado de enfermagem como detentor de saber próprio, além do que o trabalho médico teria um conhecimento reconhecido; outras Professoras, em menor expressão, reconhecem a existência de hegemonia, posicionam-se frente a esses profissionais em uma relação de iguais, questionando-os. A qualificação profissional das auxiliares de enfermagem é trazida nos atos de fala em uma racionalidade instrumental, visando a formação técnico-científica. As ações docentes não trazem as bases de uma racionalidade comunicativa. No entanto, é realizado pela pesquisadora, a proposta de desenvolvimento desta realidade na qualificação profissional de auxiliares de enfermagem.

Objectives: to evaluate the structure and process of the prenatal and puerperal care given by Direção Regional de Saúde (DIR) XI, at the city of Botucatu, State of São Paulo, Brazil. Methods: an evaluation of resources and activities developed during prenatal and puerperal care in twenty municipalities comprising DIR XI which had adhered the Prenatal and Birth Humanization Program until 2003. Interviews with city managers and analysis of 385 sampled patient charts taking into account recommendations by the Health Department. Results: structure analysis showed that caregiving was centered on medical work; basic equipment and instruments were available; 85.2% of patients began prenatal care with up to 120 days of pregnancy, and 75.9% had at least six prenatal consultations. The active search for absentees and strategies for early prenatal care initiation were observed in 30% and 50% of the municipalities, respectively. Process indicators showed that 3.6% of women had six prenatal consultations, one puerperal consultation, all basic exams and tetanus immunization. Recording of gestational age, arterial blood pressure and weight was of approximately 90%; 58.7% of the women underwent childbirth review and 31.5% were vaccinated. Conclusions: the performance of the set of activities is a challenge to prenatal care at DIR XI.

Background. No consensus between guidelines exists regarding neuroimaging in firstepisode psychosis. The purpose of this study is to assess anomalies found in structural neuroimaging exams (brain computed tomography (CT) and magnetic resonance imaging (MRI)) in the initial medical work-up of patients presenting first-episode psychosis. Methods. The study subjects were 32 patients aged 18–48 years (mean age: 29.6 years), consecutively admitted with first-episode psychosis diagnosis. Socio-demographic and clinical data and neuroimaging exams (CT and MRI) were retrospectively studied. Diagnostic assessments were made using the Operational Criteria Checklist +. Neuroimaging images (CT and MRI) and respective reports were analysed by an experienced consultant psychiatrist. Results. None of the patients had abnormalities in neuroimaging exams responsible for psychotic symptoms. Thirty-seven percent of patients had incidental brain findings not causally related to the psychosis (brain atrophy, arachnoid cyst, asymmetric lateral ventricles, dilated lateral ventricles, plagiocephaly and falx cerebri calcification). No further medical referral was needed for any of these patients. No significant differences regarding gender, age, diagnosis, duration of untreated psychosis, in-stay and cannabis use were found between patients who had neuroimaging abnormalities versus those without. Discussion. This study suggests that structural neuroimaging exams reveal scarce abnormalities in young patients with first-episode psychosis. Structural neuroimaging is especially useful in first-episode psychosis patients with neurological symptoms, atypical clinical picture and old age.

Nos hemos introducido en la obra médica de Ramón Llull en el contexto de la filosofía natural, estudiando la relación entre la obra médica y el ars luliano -en especial el Liber Principiorum Medicinae- así como el sentido metafórico que Llull intenta aportarle. Hemos analizado algunas de sus aportaciones a la medicina y hemos puesto de manifiesto el interés por seguir las huellas de sus fuentes textuales, en el ámbito de las transferencias del conocimiento entre algunas de las escuelas médicas en el s. XIII.

Introduction: Amyloidosis is used to describe a range of disorders deined by extracellular deposition of abnormal protein ibrils. The larynx is the most common site of localized amyloidosis in the head and neck region and constitutes less than 1% of benign laryngeal lesions. Hoarseness is the most common symptom. Objective: Prospective clinical evaluation of patients with localized laryngeal amyloidosis. Clinical cases: Presented are 4 cases of patients with localized laryngeal amyloidosis who were treated at the Otolaryngology and Head and Neck Surgery Department at the “Dr. José Eleuterio González” University Hospital in Monterrey, Mexico. Three patients underwent phonomicrosurgery by direct microlaryngoscopy with the removal of the amyloid implantation using a cold knife excision with great results. In each patient the major site of involvement was the supraglottis with a small focus on the false vocal cord. A medical work-up, including a complete blood count (CBC), a basic metabolic panel, urinalysis, liver function test, chest X-ray and physical examination were performed to rule out the presence of systemic disease; no amyloidosis or signs of systemic disease were found. Congo red staining conirms the diagnosis of amyloidosis in all surgical specimens. Conclusions: In laryngeal amyloidosis, the treatment should be directed toward the improvement of the voice and the maintenance of the airway.

We present a qualitative analysis of organizational improvisation and provide a preliminary insight into the following question: how is improvisation present in tightly controlled work environments? We conducted in situ observations of, and interviews with, several emergency medical teams and complemented this information with statistical and media data. Using grounded theory, we developed four propositions that were arranged into a model that allowed the identification of two use levels of established routines: (1) the visible side that accommodates contextual requirements, and (2) the improvisational side that provides a response to activity characteristics. This dual process is related to the existence of pressures that operate at the institutional level with practical needs emerging from the operational domain. In contrast with most of the literature, this study reveals that the presence of a broad procedural organizational memory does not restrict improvisation but enables a bureaucratic system to produce flexible improvised performance.
