Background: EUS is being increasingly utilized for the diagnosis of choledocholithiasis and microlithiasis, especially in patients with biliary colic. Simultaneously, there is also a rising interest in the use of EUS for therapeutic interventions. Objectives: Our goal was to assess the effectiveness of EUS-directed common bile duct (CBD) stone removal to compare its safety and effectiveness with ERCP-directed intervention. Design: interim results of a prospective, randomized, single-center blinded clinical trial. Setting: A single tertiary care referral center. Patients: Fifty-two patients with uncomplicated CBD stones were prospectively randomized to CBD cannulation and stone removal under EUS or ERCP guidance. Main Outcome Measurements and Interventions: Primary outcome measure was the rate of successful cannulation of the CBD. Secondary Outcome measures included Successful removal of stones and overall complication rates. Results: CBD cannulation followed by stone extraction was successful in 23 of 26 patients (88.5%) in the EUS group (1) versus 25 of 26 patients (96.2%) in the ERCP group (11) (95% CI, -27.65%, 9.88%). Overall, there were 3 complications in the EUS group and 4 complications in the ERCP group. Limitation: The current study is an interim report from a single center report and performed by a single operator. Conclusions: Our preliminary analysis indicates that Outcomes following EUS-guided CBD stone retrieval are equivalent to those following ERCP EUS-related adverse events are similar to those following ERCP. ERCP and EUS-guided stone retrieval appears to be equally effective for therapeutic interventions of the bile duct. Additional studies are required to validate these preliminary results and to determine predictors of success of EUS-guided stone removal. (Gastrointest Endosc 2009;69:238-43.)

The Commission on Liquor Licensing was established by the Minister for Justice, Equality and Law Reform in November 2000 to review the Liquor Licensing system in Ireland. The Commission published an Interim Report with a set of recommendations1, one of which was the establishment of a task force. A High Level Inter-DepartmentalWorking Group considered the report and certain actions were agreed by different Government Departments.The Department of Health and Children agreed to establish a Strategic Task Force on Alcohol. This is their interim report. Â Â

This project arises from the need for an enhanced focus on workforce planning having regard, in particular, to the availability of sufficient nurses now and in the future. For a country which historically has had an abundance of nurses, the recruitment difficulties now being experienced by the health services are unprecedented. Up to the mid 1990s, there was a surplus of registered nurses here and other countries sent recruitment teams to Ireland in order to address their own shortages. As will be seen in this report, the concern of the professional interest groups at the time was the high levels of unemployment among nurses. The conventional wisdom was that the health services were training too many nurses for the jobs available. However, there has been a sea change in the situation over the past five years and the stage has now been reached where Irish health care agencies, particularly the major acute Dublin hospitals, are actively seeking to recruit nurses from abroad. Interim Report of the Steering Group on the Review of the Mental Health Act 2001 There is a commitment in the Programme for Government to review the Mental Health Act 2001. In line with this, Minister Lynch appointed a Steering Group to review the Mental Health Act 2001. Their report is now with an Expert Group. The review takes accounts of the significant changes in thinking about the delivery of mental health services that have taken place in the last ten years (including the introduction of the UN Convention on the Rights of People with Disabilities (CRPD), the shift to community based services, the adoption of a recovery approach in every aspect of service delivery and the involvement of service users as partners in their own care and in the development of the service). This report sets out a revised costing methodology and an estimate of the gap which currently exists between private and semi-private bed charges and the average economic cost. While the Steering Group considers the costing methodology proposed as an improvement on the approach taken in previous years and a good overall approximation of the difference on average between economic costs and current charges, it recognises that the current charging regime does not take sufficient account of the variation between different categories of patient. This report describes the work accomplished to date on research project HR-173, A Computer Based Information System for County Equipment Cost Records, and presents the initial design for this system. The specific topics discussed here are findings from the analysis of information needs, the system specifications developed from these findings, and the proposed system design based upon the system specifications. The initial system design will include tentative input designs for capturing input data, output designs to show the output formats and the items to be output for use in decision making, file design showing the organization of information to be kept on each piece of equipment in the computer data file, and general system design explaining how the entire system will operate. The Steering Committee appointed by Iowa Highway Research Board is asked to study this report, make appropriate suggestions, and give approval to the proposed design subject to any suggestions made. This approval will permit the designer to proceed promptly with the development of the computer program implementation phase of the design.

Over the past several years we conducted a comprehensive study on the pore systems of limestones used as coarse aggregate in portland cement concrete (pee) and their relationship to freeze-thaw aggregate failure. A simple test called the Iowa Pore Index Test was developed and used to identify those coarse aggregates that had freeze-thaw susceptible pore systems. Basically, it identified those aggregates that could take on a considerable amount of water but only at a slow rate. The assumption was that if an aggregate would take on a considerable amount of water at a slow rate, its pore system would impede the outward movement of water through a critically saturated particle during freezing, causing particle fracture. The test was quite successful when used to identify aggregates containing susceptible pore systems if the aggregates were clean carbonates containing less than 2% or 3% insolubles. The correlation between service record, ASTM C666B and the pore index test was good, but not good enough. It became apparent over the past year that there were factors other than the pore system that could cause an aggregate to fail when used in pee. The role that silica and clay play in aggregate durability was studied.

Approximately ten million tons of waste bituminous roofing are torn off annually in the United States. This volume is a major factor in the rapid filling of landfills. In 1995, Benton County, Iowa initiated a program to cost effectively recycle torn off waste shingles. Nine hundred tons of waste shingles were ground using a Maxigrind. A magnetic roller on the discharge conveyor removed most of the nails. Five hundred tons of the ground waste shingles were blade mixed into 0.6 km (0.4 mi) of a crushed stone granular surfaced Benton County rural secondary roadway. A magnet attached to the motor grader removed another 1/3 kg (3/4 lb) of nails during the spreading and mixing operation on the 0.6 km (0.4 mi) section of roadway. The bitumen of the waste shingles was very effective in providing a dust free granular surfaced roadway. It remains relatively dust free one year after treatment.

The Iowa Department of Transportation (Iowa DOT) UTW Project (HR-559) initiated Ultra-Thin Whitetopping in Iowa. The project is located on Iowa Highway 21 between Iowa Highway 212 and U.S. Highway 6 in Iowa County, near Belle Plaine, Iowa. The above listed research project lasted for five years, and then was extended for another five year period. The new phase of the project (TR 432) was initiated by removing cracked panels existing in the 2-inch thick PCC sections and replacing them with three inches of PCC. The project extension provides an increased understanding of slab bonding conditions over a longer period, as well as knowledge regarding the behavior of the newly rehabilitated areas. This report documents the rehabilitation of the PCC patching of all fractured panels and several cracked panels, taking place in September of 2001.

This report provides updates on the WPAC recommendations to legislature, including actions taken on those recommendations and any follow-up recommendations from WPAC. Recommendations include documentation of activities, and the needs and challenges toward making progress in protecting Iowa’s water resources, identified by WPAC in coordination with all agencies and stakeholders in the management of the state’s water resources in a sustainable, fiscally responsible, and environmentally conscientious manner.
