This thesis examines the present provisions for pre-conception care and the views of the providers of services. Pre-conception care is seen by some clinicians and health educators as a means of making any necessary changes in life style, corrections to imbalances in the nutritional status of the prospective mother (and father) and the assessment of any medical problems, thus maximizing the likelihood of the normal development of the baby. Pre-conception care may be described as a service to bridge the gap between the family planning clinic and the first ante-natal booking appointment. There were three separate foci for the empirical research - the Foresight organisation (a charity which has pioneered pre-conception care in Britain); the pre-conception care clinic at the West London Hospital, Hammersmith; and the West Midlands Regional Health Authority. The six main sources of data were: twenty five clinicians operating Foresight pre-conception clinics, couples attending pre-conception clinics, committee members of the Foresight organisation, staff of the West London Hospital pre-conception clinic, Hammersmith, District Health Education Officers working in the West Midlands Regional Health Authority and the members of the Ante-Natal Care Action Group, a sub-group of the Regional Health Advisory Group on Health Promotion and Preventive Medicine. A range of research methods were adopted. These were as follows: questionnaires and report forms used in co-operation with the Foresight clinicians, interviews, participant observation discussions and informal meetings and, finally, literature and official documentation. The research findings illustrated that pre-conception care services provided at the predominantly private Foresight clinics were of a rather `ad hoc' nature. The type of provision varied considerably and clearly reflected the views held by its providers. The protocol which had been developed to assist in the standardization of results was not followed by the clinicians. The pre-conception service provided at the West London Hospital shared some similarities in its approach with the Foresight provision; a major difference was that it did not advocate the use of routine hair trace metal analysis. Interviews with District Health Education Officers and with members of the Ante Natal Care Action Group revealed a tentative and cautious approach to pre-conception care generally and to the Foresight approach in particular. The thesis concludes with a consideration of the future of pre-conception care and the prospects for the establishment of a comprehensive pre-conception care service.

Con motivo del fallecimiento del poeta sevillano Manuel Machado, se ofrece un repaso a su biografía y obra. Nació en agosto de 1874, en el Palacio de las Dueñas de Sevilla. Pasó parte de su infancia y juventud en París, aunque influyó más en su obra su Sevilla natal. Su padre y hermano también influyeron en su forma de escribir, y con éste último, desarrolló su faceta de dramaturgo escribiendo teatro. Vivió en Madrid hasta el último día, dónde finalmente se le rendiría homenaje en su funeral en el Cementerio de La Almudena.

The most varied ways of manifestation of the violence phenomenon in the contemporary society are each time more found in the media spaces and among society discussions. It makes think about procedures to be taken before the growth and new outbreaks of such phenomenon. The violences manifestations in schools reveal themselves as a reflection of what happens in the scope of social violence. Its dynamics originates in society and reflects itself in scholl, that is, the violences in schools combine internal and external elements to school enviroment of many fields and spheres of which the individuals participate. To reflect about the violences in the schools requires, over all, to make a bridge with the categories: youth and violence. Violences in schools: A new look to the social relations, is a Ms. Sc. Dissertation that has as main objective: to analyze the main existing forms of violence in the school space. For its achievement, it was made a bibliographical survey, questionnaires application (annexed) and observation. Thus, this research articulated the two approaches: qualitative and quantitative. The questionnaires application happened in a state school of the Natal city and amongst yhe criteria for this choice there was the fact of the school had more than 500 pupils, to be located on a strategical place of the city, providing a subjects heterogeneity to be researched, deyond the limitations of available resources financial and material and the available time for the research accomplishment. The scool congregates objective conditions, specifically in what concerns the criteria previously defined: age range, socioenomical level and number of pupils. Amongst the main results obtained, it can be detached that the violence is a phenomenon seen, for the great majority of the research subjects, as a phenomenon connected to the most visible violence forms: the agressions. And a question always present in the public education institutions: infrastructures precariousness, high scool evasion índex and vulnerability among the pupils that makes possible to the pupil to see school as an home extension. Such dissertation concludes that the phenomenon of the violence in scools demands and requires of the most varied subjects involved in the processes na understanding of its determinants so that thus, one can intervene in such phenomenon that does not restrains itself to the physical acts of aggression, but that it is, over all, on a non respecting the different

The present study aims to analyze the spatial organization of the Ribeira district in Natal/ RN from the two circuits of the urban economy. For its realization, was used the bibliographical, documentary and field research. From the information collected in the field research and of secondary data was elaborated the maps, tables, graphics, frames and photographic collections. In the analysis of the results identified the presence of two circuits of the urban economy in the said neighborhood, thus as the predominance of the upper circuit compared to the lower circuit between the surveyed establishments, well as the role that each of the circuits represents to their spatial organization. Some variables such as technology, organization, information, publicity and consumption were important to establish the main differences between the circuits.

Melhoria da Atenção a Usuários com Hipertensão Arterial Sistêmica e/ou Diabetes mellitus na UBSF Planície das Mangueiras, Natal / RN. 2015. 2015. 70f Trabalho de Conclusão de Curso (Curso de Especialização em Saúde da Família) - Departamento de Medicina Social, Faculdade de Medicina, Universidade Federal de Pelotas, Pelotas, 2015. As doenças crônicas não transmissíveis são um problema de Saúde Pública importante e causam impacto na morbimortalidade de indivíduos e populações. Na Unidade Básica de Saúde Planície das Mangueiras, foi realizada uma intervenção com objetivo de melhorar a atenção e assistência direcionadas aos usuários com Hipertensão Arterial Sistêmica (HAS) e/ou Diabetes mellitus (DM). A intervenção ofertou ações nos quatro eixos pedagógicos do curso: organização e gestão do serviço; qualificação da prática clínica; monitoramento e avaliação e por fim, engajamento público. Na área da equipe, temos uma estimativa de 590 usuários com HAS e 169 usuários com DM, de acordo com os dados do Caderno de Ações Programáticas. Antes da intervenção, tínhamos 187 usurários com HAS sendo acompanhados na USF, o que corresponde a 32 % e a cobertura de DM era de 34% o que corresponde a 58 usuários. Dessa forma, tínhamos baixos indicadores de cobertura. Os instrumentos utilizados foram a ficha espelho, a planilha de coleta de dados e os protocolos preconizados pelo Ministério da Saúde. Além da necessidade de aumento das coberturas, em geral, também havia lacunas referentes à atenção, principalmente, ofertas de orientação e realização de exames condizentes com o preconizado pelo Ministério da Saúde. Após os três meses (12 semanas) de intervenção, conseguimos assistir a 113 usuários com HAS que corresponde a 26,1% de cobertura e 58 usuários com DM o que corresponde a um 54,2%. A intervenção conseguiu atingir 100% em todas as metas propostas para qualificação da assistência. A importância da intervenção reflete na melhoria significativa da qualidade de atendimento dos usuários hipertensos e/ou diabéticos assim como um cadastro atualizado dos mesmos, o serviço ficou mais organizado e com maior resolutividade ante as demandas dos usuários. Objetivos: Identificar o perfil sócio-demográfico; características da vida sexual e reprodutiva; característicasdo pré-natal, intercorrências e tipo de parto; tipo de orientações recebidas no pré-natal; freqüência de baixopeso, prematuridade e Apgar. Método: Estudo descritivo, de 84 mães adolescentes com parto entre 01/10/2004 a 01/12/2004. Resultados: Das adolescentes estudadas, 96,4% tinham entre 15 a 19 anos; 73,8% viviamcom o companheiro; 65% recebiam até três salários mínimos; 79,3% nunca tinham trabalhado; 52,4%freqüentavam a escola quando engravidaram. A média de idade da primeira relação sexual foi de 15 anos;64,3% faziam uso de contraceptivo; apenas 9,5% usavam-no quando engravidaram; 100% das adolescentesfizeram pré-natal; 58,5% iniciaram no primeiro trimestre de gravidez; 84,6% fizeram de seis a doze consultas;83,3% eram primíparas e 83,3% não planejaram a gravidez. As complicações maternas foram: 44% anemia;35,7% infecção urinária; 14,3% sangramento vaginal; 14,2% pressão alta; 2,4% diabetes gestacional e 1,2%eclampsia. Parto cesárea foi feito em 61,9%. Receberam orientação para não fazer uso de medicação semordem médica 85,7% das adolescentes; para não usar drogas 73,8%; quanto aos prejuízos do fumo e bebidaalcoólica 72,6%; em relação ao tipo de alimentação na gestação 70,2%; sobre os cuidados com os dentes54,8%; sobre os sinais do início do trabalho de parto 72,6%; quanto aos tipos de parto 60,7%; sobre aimportância do aleitamento materno 76,2%; quanto ao banho do bebê 17,9% e 18,3% sobre o curativo doumbigo. Encontrou-se 6% de recém-nascidos de baixo peso e prematuros; o Apgar foi superior a 8 em 86,9%dos casos no primeiro minuto e 95,1 % no quinto minuto. Neste grupo de adolescentes, a assistência pré-natal adequada (início no primeiro trimestre e número mínimo de seis consultas) permitiu bons resultados,apesar da idade das mães estar associada com gravidezes de risco.

OBJETIVO: Analisar a importância da inclusão da perspectiva das mulheres na avaliação do Programa de Humanização do Pré-Natal e Nascimento. PROCEDIMENTOS METODOLÓGICOS: Estudo qualitativo realizado em base a dados primários coletados para a avaliação do Programa de Humanização do Pré-Natal e Nascimento, do Ministério da Saúde, em 2003, em sete municípios das cinco regiões do Brasil, selecionados a partir de dados extraídos de sistemas de bancos de dados oficiais já existentes. Um dos atores considerado fundamental para a coleta de informações foi a mulher atendida pelo Programa, abordada por meio de dezesseis grupos focais realizados em unidades de saúde. Para o tratamento dos dados empíricos foi utilizado o método do Discurso do Sujeito Coletivo. A análise e discussão foram realizadas com o apoio dos conceitos em saúde pública de acessibilidade e Saúde Paidéia. ANÁLISE DOS RESULTADOS: O Programa estudado normatiza para todos os serviços de saúde do país os procedimentos para a atenção ao pré-natal e o parto e os fluxos a serem observados. A análise do discurso das gestantes, nos grupos focais realizados, trouxe clareza quanto à dissonância existente entre muitas dessas recomendações e os desejos e necessidades da mulher, o que faz com que ela procure traçar para si um outro fluxo de atendimentos. Esta ocorrência traz prejuízos ao vínculo que estabelece com o serviço de saúde, além de dificuldades de controle pelo serviço do seguimento real que está sendo oferecido. CONCLUSÕES: A reflexão realizada do Programa, tomando por base a perspectiva das mulheres atendidas, identificou aspectos cuja consideração no momento da avaliação poderia resultar em maior efetividade e humanização do controle pré-natal oferecido

The mandible has a mixed embryological origin, and its growth is associated with the secondary cartilage of the condyle process (CP). In this area, growth depends on an array of intrinsic and extrinsic factors that influence protein metabolism. In the present study, we used an adolescent rat model to evaluate the growth and development of the CP under conditions of pre- and postnatal protein deficiency, combined with or without the stress of severe burn injury (BI). We found that protein deficiency severely undermined the growth of the CP, by altering the thickness of its constituent layers. BI is also capable of affecting CP growth, although the effect is less severe than protein deficiency. Interestingly, the summed effect of protein deficiency and BI on the CP is less severe than protein deficiency alone. A possible explanation is that the increased carbohydrates in a hypoproteic diet stimulate the production of endogenous insulin and protein synthesis, which partially compensates for the loss of lean body mass caused by BI.

Estimates of greenhouse-gas emissions from deforestation are highly uncertain because of high variability in key parameters and because of the limited number of studies providing field measurements of these parameters. One such parameter is burning efficiency, which determines how much of the original forest`s aboveground carbon stock will be released in the burn, as well as how much will later be released by decay and how much will remain as charcoal. In this paper we examined the fate of biomass from a semideciduous tropical forest in the ""arc of deforestation,"" where clearing activity is concentrated along the southern edge of the Amazon forest. We estimated carbon content, charcoal formation and burning efficiency by direct measurements (cutting and weighing) and by line-intersect sampling (LIS) done along the axis of each plot before and after burning of felled vegetation. The total aboveground dry biomass found here (219.3 Mg ha(-1)) is lower than the values found in studies that have been done in other parts of the Amazon region. Values for burning efficiency (65%) and charcoal formation (6.0%, or 5.98 Mg C ha(-1)) were much higher than those found in past studies in tropical areas. The percentage of trunk biomass lost in burning (49%) was substantially higher than has been found in previous studies. This difference may be explained by the concentration of more stems in the smaller diameter classes and the low humidity of the fuel (the dry season was unusually long in 2007, the year of the burn). This study provides the first measurements of forest burning parameters for a group of forest types that is now undergoing rapid deforestation. The burning parameters estimated here indicate substantially higher burning efficiency than has been found in other Amazonian forest types. Quantification of burning efficiency is critical to estimates of trace-gas emissions from deforestation. (C) 2009 Elsevier B.V. All rights reserved.

The aims of this study were to evaluate whether air pollution during pre-natal and post-natal phases change habituation and short-term discriminative memories and if oxidants are involved in this process. As secondary objectives, it was to evaluate if the change of filtered to nonfiltered environment could protect the cortex of rats against oxidative stress as well as to modify the behavior of these animals. Wistar, male rats were divided into four groups (n = 12/group): pre and post-natal exposure until adulthood to filtered air (FA); pre-natal period to nonfiltered air (NFA-FA); until (21st post-natal day) and post-natal to filtered air until adulthood (PND21); prenatal to filtered air until PND21 and post-natal to nonfiltered air until adulthood (FA-NFA); pre and post-natal to nonfiltered air (NFA). After 150 days of air pollution exposure, animals were tested in the spontaneous object recognition test to evaluate short-term discriminative and habituation memories. Rats were euthanized; blood was collected for metal determination; cortex dissected for oxidative stress evaluation. There was a significant increase in malondialdehyde (MDA) levels in the NFA group when compared to other groups (FA: 1.730 +/- 0.217; NFA-FA: 1.101 +/- 0.217; FA-NFA: 1.014 +/- 0.300; NFA: 5.978 +/- 1.920 nmol MDA/mg total proteins; p = 0.007). NFA group presented a significant decrease in short-term discriminative (FA: 0.603 +/- 0.106; NFA-FA: 0.669 +/- 0.0666; FA-NFA: 0.374 +/- 0.178; NFA: -0.00631 +/- 0.106 sec; p = 0.006) and an improvement in habituation memories when compared to other groups. Therefore, exposure to air pollution during both those periods impairs short-term discriminative memory and cortical oxidative stress may mediate this process.

A local cultural practice that may enhance sexually transmitted infections (STIs) and HIV transmission is vaginal douching and vaginal substance use. These activities also have potential implications for the acceptability of HIV-prevention strategies such as the use of condoms and vaginal microbicides. We aimed to establish the prevalence, determinants and reasons for these practices among sex workers in KwaZulu-Natal, South Africa. A structured questionnaire was administered to 150 sex workers, who were being screened for a vaginal microbicide-effectiveness trial in the province. The questionnaire sought information on the frequency, reasons for and nature of vaginal douching and vaginal substance use and was drawn up on the basis of findings from a pilot study. Seventy per cent (95% CI: 62.0-77.2%) of the sex workers were HIV positive and on average they had five sexual partners per day. Vaginal douching and vaginal substance use were common among the sex workers. Vaginal douching was reported by 97% (n = 146) of the respondents and 94% reported vaginal substance use for 'dry sex'. A combination of traditional remedies, patent medicines, antiseptics and household detergents was used to clean and make the vagina dry and tight. The primary reasons reported for dry sex were to increase men's sexual pleasure (53%) and to attract clients and generate more money (20%). Sixty-five per cent of the women reported the practice of douching mainly for hygienic purposes and 13% for the prevention and treatment of sexually transmitted infections. Douching and dry-sex practices may increase women's risk of HIV and STI infection, and may have implications for the acceptability and development of HIV-prevention barrier methods such as microbicides and the use of condoms. These barrier methods may enhance or reduce sexual pleasure for men and women who engage in the practice of vaginal douching and vaginal substance use for 'dry sex'.

This paper uses a fully operational inter-regional computable general equilibrium (CGE) model implemented for the Brazilian economy, based on previous work by Haddad and Hewings, in order to assess the likely economic effects of road transportation policy changes in Brazil. Among the features embedded in this framework, modelling of external scale economies and transportation costs provides an innovative way of dealing explicitly with theoretical issues related to integrated regional systems. The model is calibrated for 109 regions. The explicit modelling of transportation costs built into the inter-regional CGE model, based on origin-destination flows, which takes into account the spatial structure of the Brazilian economy, creates the capability of integrating the inter-regional CGE model with a geo-coded transportation network model enhancing the potential of the framework in understanding the role of infrastructure on regional development. The transportation model used is the so-called Highway Development and Management, developed by the World Bank, implemented using the software TransCAD. Further extensions of the current model specification for integrating other features of transport planning in a continental industrialising country like Brazil are discussed, with the goal of building a bridge between conventional transport planning practices and the innovative use of CGE models. In order to illustrate the analytical power of the integrated system, the authors present a set of simulations, which evaluate the ex ante economic impacts of physical/qualitative changes in the Brazilian road network (for example, a highway improvement), in accordance with recent policy developments in Brazil. Rather than providing a critical evaluation of this debate, they intend to emphasise the likely structural impacts of such policies. They expect that the results will reinforce the need to better specifying spatial interactions in inter-regional CGE models.
