Introdução: A violência por parceiro íntimo (VPI) na gravidez conduz a fatores de risco com impacto na saúde materno-fetal. Existe tendência para gravidezes indesejadas, não planeadas e baixa adesão aos serviços de saúde, aumentando o risco de complicações na gestação (OMS,2003). Objetivos: Descrever as prevalências da VPI durante a gravidez na Região Centro de Portugal e analisar a sua associação ao planeamento da gravidez e frequência de consultas pré-natais. Método: Estudo Epidemiológico que abrangeu uma amostra de 1219 mulheres puérperas, internadas em Hospitais de Apoio Perinatal e Apoio Perinatal Diferenciado da Região Centro de Portugal. A recolha de dados foi feita por questionário completado com dados no processo clínico, entre Setembro de 2012 e Setembro de 2013. Resultados: Prevalências de agressão psicológica 41,6%, coerção sexual 13,7%, agressão física 8,4%; injúrias 2,5%. Associação estatisticamente significativa entre planeamento da gravidez e ocorrência de agressão psicológica e coerção sexual. Existe associação estatisticamente significativa entre o início das consultas e a frequência de agressão psicológica, agressão física e injúrias cuja probabilidade é superior quando o início das consultas ocorreu após o 2º mês de gravidez. Conclusões: Verificaram-se prevalências de VPI na gravidez com associação significativa ao planeamento da gravidez. Apesar da maioria das mulheres ter frequentado consultas de vigilância na última gravidez, o início tardio associa-se à probabilidade de frequência de algum tipo de VPI.

Objectives: To investigate the long-term effects of pharyngeal flap surgery (PFS) on nasal and nasopharyngeal dimensions of patients with velopharyngeal insufficiency (VPI) and to correlate the findings with the onset of respiratory complaints after surgery. Design/Participants: Prospective study in 58 nonsyndromic patients with repaired cleft palate and VPI, evaluated 2 days before and 5 months (POST1) and 1 year (POST2) after PFS, on average. Patients were divided into two groups: one consisting of patients with postoperative respiratory complaints (RC group) and the other without complaints (NRC group). Interventions: Superiorly based PFS. Main Outcome Measures: Respiratory complaints (self reports of mouth breathing, snoring, and other sleep obstructive events) assessed at POST1 and POST2, and minimum nasal (NCSA) and nasopharyngeal (NPA) cross-sectional areas assessed by rhinomanometry at POST2. Results: Respiratory complaints were reported by 55% and 36% of the patients evaluated at POST1 and POST2, respectively. Posterior rhinomanometry showed a significant postoperative reduction of mean NCSA in the RC and NRC groups (p < .05), to subnormal levels in some of them. The decrease was more pronounced in the RC group. No significant changes in NCSA were observed by anterior rhinomanometry. Similar results were obtained when NPA was assessed by modified anterior rhinomanometry. Conclusion: In the long-term, PFS yielded a significant reduction in upper airways dimensions beyond what should be expected and associated with persistent respiratory complaints in some cases.

The architectonic features of the thalamic ventrobasal complex (Vb) of two species of Megachiropteran (Grey-headed flying fox, Pteropus poliocephalus, and the Eastern tube-nosed bat, Nyctimene robinsoni) are compared with those of a Microchiropteran (Australian ghost bat, Macroderma gigas). The somatosensory system was chosen for comparison as it represents a sensory system that has undergone analogous modifications in both Chiropteran lineages (the evolution of the wing). The components of Vb were examined as there are taxon-specific features in this region of the brain. Within the Megachiropteran Vb, four subnuclei were recognized: the ventral posterior medial (VPM), the ventral posterior lateral (VPL), the ventral posterior inferior (VPI), and the basal ventral medial (VMb). In the ghost bat only VPM and VPL were identified with certainty. No VPI was evident in the ghost bat, however a putative VMb was observed. Vb of the ghost bat also lacked the arcuate lamina, which distinguishes VPM from VPL in the Megachiropterans and many other mammals. These taxon-specific differences lend support to the proposal that the order Chiroptera has a diphyletic origin.

OBJETIVO: Analisar a associação entre a exposição à violência por parceiro íntimo (VPI) contra a mulher com desajustes comportamentais e problemas escolares entre os filhos. MÉTODOS: Inquérito populacional participante do WHO Multicountry Study on Violence Against Women, com 790 mulheres que coabitam com filhos de cinco a 12 anos, residentes no Município de São Paulo, SP, e na Zona da Mata de Pernambuco. Foram realizados três modelos múltiplos para estimar a força da associação entre variáveis explanatórias de apoio social e comunitário, eventos de vida estressantes, fatores sociodemográficos e gravidade da VPI, entre outras. Os modelos incluíram três respectivos desfechos: número de problemas de comportamento; agressividade; e interrupção abandono ou repetência escolar. RESULTADOS: A exposição à VPI física e/ou sexual grave esteve associada à ocorrência de problemas escolares, de problemas de comportamento em geral e de comportamentos agressivos na análise de regressão logística univariada. A exposição à VPI grave manteve-se associada à ocorrência de três ou mais problemas de comportamento entre seus filhos, independentemente do transtorno mental comum, da baixa escolaridade, de a mãe (avó) ter sido vítima de VPI física e do apoio social e comunitário nos modelos de regressão logística múltiplos. A VPI grave esteve associada ao comportamento agressivo e aos problemas escolares, depois do ajuste por outras variáveis sociodemográficas, entre outras. O estado de saúde mental materna constituiu-se em fator mediador da relação entre a exposição à VPI e os problemas de comportamento, sobretudo agressividade. CONCLUSÕES: A VPI grave afeta o comportamento dos filhos e deve ser incluída na assistência à saúde das crianças em idade escolar, por meio de intervenções conjuntas entre crianças e mães.

OBJETIVO: Descrever as formas de enfrentamento à violência física adotadas por mulheres agredidas por parceiro íntimo. MÉTODOS: Estudo transversal realizado na linha de base de estudo de coorte, com gestantes cadastradas no Programa Saúde da Família, entre julho de 2005 e março de 2006, em Recife, PE. Foram selecionadas 283 gestantes de 18 a 49 anos com histórico de violência física pelo parceiro de então ou mais recente antes e/ou durante a gestação. As entrevistas foram realizadas face a face, com questionário estruturado e pré-codificado, e realizou-se análise descritiva. Foi coletada informação sobre características sociodemográficas das mulheres, tipos e gravidade da violência física cometida pelo parceiro, formas de enfrentamento da violência, pessoas e serviços de apoio procurados pelas mulheres, motivos para a mulher ter alguma vez abandonado e retornado a casa em razão da violência. RESULTADOS: Das mulheres que sofreram violência física pelo parceiro íntimo, 57,6% conversaram com alguém, 3,5% procuraram ajuda institucionalizada, 17,3% conversaram e procuraram ajuda institucionalizada e 21,6% não procuraram nenhuma forma de ajuda. As pessoas mais procuradas foram os pais (42,0%), amigo/amiga (31,6%) e irmão/irmã (21,2%). Os serviços mais procurados pelas mulheres foram: polícia/delegacia (57,6%), serviços de saúde (27,1%) e instituições religiosas (25,4%). Relataram não ter obtido qualquer tipo de ajuda 44,8% das mulheres; 32,1% disseram ter saído de casa alguma vez na vida, pelo menos por uma noite, das quais 5,9% não retornaram a casa. Foram motivos para deixar a casa: a exacerbação da violência e o medo de ser morta; para o retorno: a esperança de mudança do parceiro e o desejo de preservar a família. CONCLUSÕES: Grande parte das mulheres que sofriam violência por parceiro íntimo buscou alguma forma de ajuda. A rede social primária (familiares e amigos) foi a mais procurada pelas mulheres para romper o ciclo violento. Os resultados apontam a necessidade de maior divulgação dos serviços de apoio e a importância da ampliação e qualificação da rede de serviços (polícia, justiça, saúde, assistência psicossocial) para que estes possam acolher e apoiar as mulheres, dando-lhes suporte efetivo para romper com a situação de VPI.

One hundred seventy nine Vibrio cholerae non-O1/non-O139 strains from clinical and different environmental sources isolated in Brazil from 1991 to 2000 were serogrouped and screened for the presence of four different virulence factors. The Random Amplification of Polymorphic DNA (RAPD) technique was used to evaluate the genetic relatedness among strains. Fifty-four different serogroups were identified and V. cholerae O26 was the most common (7.8%). PCR analysis for three genes (ctxA, zot, ace) located of the CTX genetic element and one gene (tcpA) located on the VPI pathogenicity island showed that 27 strains harbored one or more of these genes. Eight (4.5%) strains possessed the complete set of CTX element genes and all but one of these belonged to the O26 serogroup suggesting that V. cholerae O26 has the potential to be an epidemic strain. The RAPD profiles revealed a wide variability among strains and no genetic correlation was observed.

The neuraminidase gene, nanH, is present in the O1, non-toxigenic Vibrio cholerae Amazonia strain. Its location has been assigned to a 150 kb NotI DNA fragment, with the use of pulsed-field gel electrophoresis and DNA hybridization. This NotI fragment is positioned inside 630 kb SfiI and 1900 kb I-CeuI fragments of chromosome 1. Association of the pathogenicity island VPI-2, carrying nanH and other genes, with toxigenic strains has been described by other authors. The presence of nanH in a non-toxigenic strain is an exception to this rule. The Amazonia strain nanH was sequenced (Genbank accession No. AY825932) and compared to available V. cholerae sequences. The sequence is different from those of pandemic strains, with 72 nucleotide substitutions. This is the first description of an O1 strain with a different nanH allele. The most variable domain of the Amazonia NanH is the second lectin wing, comprising 13 out of 17 amino acid substitutions. Based on the presence of nanH in the same region of the genome, and similarity of the adjacent sequences to VPI-2 sequences, it is proposed that the pathogenicity island VPI-2 is present in this strain.

Velopharyngeal insufficiency (VPI) is a structural or functional trouble, which causes hypernasal speech. Velopharyngeal flaps, speech therapy and augmentation pharyngoplasty, using different implants, have all been used to address this trouble. We hereby present our results following rhinopharyngeal autologous fat injection in 18 patients with mild velopharyngeal insufficiency (12 soft palate clefts, 4 functional VPI, 2 myopathy). 28 injections were carried out between 2004 and 2007. The degree of hypernasal speech was evaluated pre- and postoperatively by a speech therapist and an ENT specialist and quantified by an acoustic nasometry (Kay Elemetrics). All patients were exhaustively treated with preoperative speech therapy (average, 8 years). The mean value of the nasalance score was 37% preoperatively and 23% postoperatively (p = 0.015). The hypernasality was reduced postoperatively in all patients (1-3 degrees of the Borel-Maisonny score). There were no major complications, two minor complications (one hematoma, one cervical pain). The autologous fat injection is a simple, safe, minimally invasive procedure. It proves to be efficient in cases of mild velopharyngeal insufficiency or after a suboptimal velopharyngoplasty.

A violência entre parceiros íntimos é um problema de saúde pública, mas, em geral, a integração da atenção a esta complexa problemática social é insatisfatória na formação e na atenção em saúde. Essa situação é examinada neste artigo, com base em evidências e questões levantadas na literatura. Nas experiências internacionais, constatou-se que a busca ativa dos casos nos serviços é priorizada, porém encontra dificuldades na institucionalização das rotinas e sistematização da capacitação desde a educação inicial. No contexto brasileiro, o pouco conhecimento sobre o assunto entre estudantes e profissionais de saúde foi sinalizado, sendo que a incorporação da normatização da assistência à violência sexual dá visibilidade ao despreparo das equipes. As conclusões ratificam limites da formação em saúde baseada no modelo biomédico e do processo de trabalho centrado na figura do médico. Recomenda-se a "problematização" da VPI via processos de ensino-aprendizagem que valorizem saberes e experiências dos educandos e garantam espaços coletivos de discussão, com apoio da incorporação dos referenciais das Ciências Humanas e Sociais nos currículos médicos e de mudanças na educação médica orientadas pela integralidade e intersetorialidade das ações.

OBJETIVOS: Avaliar a prevalência e fatores associados à violência praticada por parceiro íntimo (VPI), entre mulheres usuárias das Unidades Básicas de Saúde (UBS) do Estado de São Paulo. MÉTODOS: Foi realizado um estudo descritivo de corte transversal, a partir da análise secundária de dados de entrevista a mulheres usuárias de 75 UBS de 15 Departamentos Regionais do Estado de São Paulo, no período de Agosto/2008 a Maio/2009. Foi utilizado questionário baseado no Abuse Assessment Screen e o Conflict Tactics Scales modificado pelo Violence Against Women Study (VAW), estruturado e pré-testado. As variáveis estudadas foram os tipos de VPI (psicológica, física e sexual) e variáveis sociodemográficas (idade, escolaridade, cor da pele, trabalho remunerado, religião, estado marital e classe econômica). Foram entrevistadas 2.379 mulheres de 18 a 60 anos. RESULTADOS: A prevalência de VPI durante a vida foi de 55,7%, sendo a psicológica, física e sexual de 53,8, 32,2 e 12,4%, respectivamente. As mulheres sem companheiro, mas com casamento anterior, com escolaridade <8 anos e da classe econômica mais baixa tiveram maior risco para todos os tipos de VPI, outros fatores ainda foram associados à VIP psicológica e sexual. CONCLUSÕES: A prevalência de VPI nas UBS do Estado de São Paulo é alta. Os profissionais de saúde da atenção primária devem atentar para a detecção da VPI.

Permanent magnet generators (PMG) represent the cutting edge technology in modern wind mills. The efficiency remains high (over 90%) at partial loads. To improve the machine efficiency even further, every aspect of machine losses has to be analyzed. Additional losses are often given as a certain percentage without providing any detailed information about the actual calculation process; meanwhile, there are many design-dependent losses that have an effect on the total amount of additional losses and that have to be taken into consideration. Additional losses are most often eddy current losses in different parts of the machine. These losses are usually difficult to calculate in the design process. In this doctoral thesis, some additional losses are identified and modeled. Further, suggestions on how to minimize the losses are given. Iron losses can differ significantly between the measured no-load values and the loss values under load. In addition, with embedded magnet rotors, the quadrature-axis armature reaction adds losses to the stator iron by manipulating the harmonic content of the flux. It was, therefore, re-evaluated that in salient pole machines, to minimize the losses and the loss difference between the no-load and load operation, the flux density has to be kept below 1.5 T in the stator yoke, which is the traditional guideline for machine designers. Eddy current losses may occur in the end-winding area and in the support structure of the machine, that is, in the finger plate and the clamping ring. With construction steel, these losses account for 0.08% of the input power of the machine. These losses can be reduced almost to zero by using nonmagnetic stainless steel. In addition, the machine housing may be subjected to eddy current losses if the flux density exceeds 1.5 T in the stator yoke. Winding losses can rise rapidly when high frequencies and 10–15 mm high conductors are used. In general, minimizing the winding losses is simple. For example, it can be done by dividing the conductor into transposed subconductors. However, this comes with the expense of an increase in the DC resistance. In the doctoral thesis, a new method is presented to minimize the winding losses by applying a litz wire with noninsulated strands. The construction is the same as in a normal litz wire but the insulation between the subconductors has been left out. The idea is that the connection is kept weak to prevent harmful eddy currents from flowing. Moreover, the analytical solution for calculating the AC resistance factor of the litz-wire is supplemented by including an end-winding resistance in the analytical solution. A simple measurement device is developed to measure the AC resistance in the windings. In the case of a litz-wire with originally noninsulated strands, vacuum pressure impregnation (VPI) is used to insulate the subconductors. In one of the two cases studied, the VPI affected the AC resistance factor, but in the other case, it did not have any effect. However, more research is needed to determine the effect of the VPI on litz-wire with noninsulated strands. An empirical model is developed to calculate the AC resistance factor of a single-layer formwound winding. The model includes the end-winding length and the number of strands and turns. The end winding includes the circulating current (eddy currents that are traveling through the whole winding between parallel strands) and the main current. The end-winding length also affects the total AC resistance factor.
Characterization and Pathogenicity of Vibrio cholerae and Vibrio vulnificus from Marine environments

The genus Vibrioof the family Vibrionaceae are Gram negative, oxidasepositive, rod- or curved- rodshaped facultative anaerobes, widespread in marine and estuarine environments. Vibrio species are opportunistic human pathogens responsible for diarrhoeal disease, gastroenteritis, septicaemia and wound infections and are also pathogens of aquatic organisms, causing infections to crustaceans, bivalves and fishes. In the present study, marine environmental samples like seafood and water and sediment samples from aquafarms and mangroves were screened for the presence of Vibrio species. Of the134 isolates obtained from the various samples, 45 were segregated to the genus Vibrio on the basis of phenotypic characterization.like Gram staining, oxidase test, MoF test and salinity tolerance. Partial 16S rDNA sequence analysis was utilized for species level identification of the isolates and the strains were identified as V. cholerae(N=21), V. vulnificus(N=18), V. parahaemolyticus(N=3), V. alginolyticus (N=2) and V. azureus (N=1). The genetic relatedness and variations among the 45 Vibrio isolates were elucidated based on 16S rDNA sequences. Phenotypic characterization of the isolates was based on their response to 12 biochemical tests namely Voges-Proskauers’s (VP test), arginine dihydrolase , tolerance to 3% NaCl test, ONPG test that detects β-galactosidase activity, and tests for utilization of citrate, ornithine, mannitol, arabinose, sucrose, glucose, salicin and cellobiose. The isolates exhibited diverse biochemical patterns, some specific for the species and others indicative of their environmental source.Antibiogram for the isolates was determined subsequent to testing their susceptibility to 12 antibiotics by the disc diffusion method. Varying degrees of resistance to gentamycin (2.22%), ampicillin(62.22%), nalidixic acid (4.44%), vancomycin (86.66), cefixime (17.77%), rifampicin (20%), tetracycline (42.22%) and chloramphenicol (2.22%) was exhibited. All the isolates were susceptible to streptomycin, co-trimoxazole, trimethoprim and azithromycin. Isolates from all the three marine environments exhibited multiple antibiotic resistance, with high MAR index value. The molecular typing methods such as ERIC PCR and BOX PCR revealed intraspecies relatedness and genetic heterogeneity within the environmental isolatesof V. cholerae and V. vulnificus. The 21 strains of V. choleraewere serogroupedas non O1/ non O139 by screening for the presence O1rfb and O139 rfb marker genes by PCR. The virulence/virulence associated genes namely ctxA, ctxB, ace, VPI, hlyA, ompU, rtxA, toxR, zot, nagst, tcpA, nin and nanwere screened in V. cholerae and V. vulnificusstrains.The V. vulnificusstrains were also screened for three species specific genes viz., cps, vvhand viu. In V. cholerae strains, the virulence associated genes like VPI, hlyA, rtxA, ompU and toxR were confirmed by PCR. All the isolates, except for strain BTOS6, harbored at least one or a combination of the tested genes and V. choleraestrain BTPR5 isolated from prawn hosted the highest number of virulence associated genes. Among the V. vulnificusstrains, only 3 virulence genes, VPI, toxR and cps, were confirmed out of the 16 tested and only 7 of the isolates had these genes in one or more combinations. Strain BTPS6 from aquafarm and strain BTVE4 from mangrove samples yielded positive amplification for the three genes. The toxRgene from 9 strains of V. choleraeand 3 strains of V. vulnificus were cloned and sequenced for phylogenetic analysis based on nucleotide and the amino acid sequences. Multiple sequence alignment of the nucleotide sequences and amino acid sequences of the environmental strains of V. choleraerevealed that the toxRgene in the environmental strains are 100% homologous to themselves and to the V. choleraetoxR gene sequence available in the Genbank database. The 3 strains of V. vulnificus displayed high nucleotide and amino acid sequence similarity among themselves and to the sequences of V. cholerae and V. harveyi obtained from the GenBank database, but exhibited only 72% homology to the sequences of its close relative V. vulnificus. Structure prediction of the ToxR protein of Vibrio cholerae strain BTMA5 was by PHYRE2 software. The deduced amino acid sequence showed maximum resemblance with the structure of DNA-binding domain of response regulator2 from Escherichia coli k-12 Template based homology modelling in PHYRE2 successfully modelled the predicted protein and its secondary structure based on protein data bank (PDB) template c3zq7A. The pathogenicity studies were performed using the nematode Caenorhabditiselegansas a model system. The assessment of pathogenicity of environmental strain of V. choleraewas conducted with E. coli strain OP50 as the food source in control plates, environmental V. cholerae strain BTOS6, negative for all tested virulence genes, to check for the suitability of Vibrio sp. as a food source for the nematode;V. cholerae Co 366 ElTor, a clinical pathogenic strain and V. cholerae strain BTPR5 from seafood (Prawn) and positive for the tested virulence genes like VPI, hlyA, ompU,rtxA and toxR. It was found that V. cholerae strain BTOS6 could serve as a food source in place of E. coli strain OP50 but behavioral aberrations like sluggish movement and lawn avoidance and morphological abnormalities like pharyngeal and intestinal distensions and bagging were exhibited by the worms fed on V. cholerae Co 366 ElTor strain and environmental BTPR5 indicating their pathogenicity to the nematode. Assessment of pathogenicity of the environmental strains of V. vulnificus was performed with V. vulnificus strain BTPS6 which tested positive for 3 virulence genes, namely, cps, toxRand VPI, and V. vulnificus strain BTMM7 that did not possess any of the tested virulence genes. A reduction was observed in the life span of worms fed on environmental strain of V. vulnificusBTMM7 rather than on the ordinary laboratory food source, E. coli OP50. Behavioral abnormalities like sluggish movement, lawn avoidance and bagging were also observed in the worms fed with strain BTPS6, but the pharynx and the intestine were intact. The presence of multi drug resistant environmental Vibrio strainsthat constitute a major reservoir of diverse virulence genes are to be dealt with caution as they play a decisive role in pathogenicity and horizontal gene transfer in the marine environments.
