Uma das práticas mais difundidas para a avaliação de ativos operacionais é o fluxo de caixa descontado, ou FCD. Contudo, uma das versões mais comumente aceitas de FCD, o custo médio ponderado de capital, ou WACC no original inglês, tornou-se obsoleta. Este artigo discute uma metodologia alternativa ao WACC, chamada de VPA (valor presente ajustado), ou APV (adjusted present value), em inglês. O artigo mostra que essa metodologia apresenta vantagens superiores quando comparada ao WACC, especificamente pelo fato de oferecer mais informações de relevância administrativa, a um índice menor de erro, ajudando os executivos a analisar não só quanto vale um ativo, mas também a origem de seu valor.

Valproic acid (VPA) is used widely to treat epilepsy and bipolar disorder. Women undergoing VPA treatment reportedly have an increased incidence of polycystic ovarian syndrome (PCOS)-like symptoms including hyperandrogenism and oligo- or amenorrhoea. To investigate potential direct effects of VPA on ovarian steroidogenesis we used primary bovine theca (TC) and granulosa (GC) cells maintained under conditions that preserve their 'follicular' phenotype. Effects of VPA (7.8-500 µg/ml) on TC were tested with/without LH. Effects of VPA on GC were tested with/without FSH or IGF analogue. VPA reduced (P<0.0001) both basal (70% suppression; IC(50) 67±10 µg/ml) and LH-induced (93% suppression; IC(50) 58±10 µg/ml) androstenedione secretion by TC. VPA reduced CYP17A1 mRNA abundance (>99% decrease; P<0.0001) with lesser effects on LHR, STAR, CYP11A1 and HSD3B1 mRNA (<90% decrease; P<0.05). VPA only reduced TC progesterone secretion induced by the highest (luteinizing) LH dose tested; TC number was unaffected by VPA. At higher concentrations (125-500 µg/ml) VPA inhibited basal, FSH- and IGF-stimulated estradiol secretion (P<0.0001) by GC without affecting progesterone secretion or cell number. VPA reversed FSH-induced upregulation of CYP19A1 and HSD17B1 mRNA abundance (P<0.001). The potent histone deacetylase (HDAC) inhibitors trichostatin A and scriptaid also suppressed TC androstenedione secretion and granulosal cell oestrogen secretion suggesting that the action of VPA reflects its HDAC inhibitory properties. In conclusion, these findings refute the hypothesis that VPA has a direct stimulatory action on TC androgen output. On the contrary, VPA inhibits both LH-dependent androgen production and FSH/IGF-dependent estradiol production in this in vitro bovine model, likely by inhibition of HDAC.

Valproic acid (VPA) and trichostatin A (TSA) are known histone deacetylase inhibitors (HDACIs) with epigenetic activity that affect chromatin supra-organization, nuclear architecture, and cellular proliferation, particularly in tumor cells. In this study, chromatin remodeling with effects extending to heterochromatic areas was investigated by image analysis in non-transformed NIH 3T3 cells treated for different periods with different doses of VPA and TSA under conditions that indicated no loss of cell viability. Image analysis revealed chromatin decondensation that affected not only euchromatin but also heterochromatin, concomitant with a decreased activity of histone deacetylases and a general increase in histone H3 acetylation. Heterochromatin protein 1-α (HP1-α), identified immunocytochemically, was depleted from the pericentromeric heterochromatin following exposure to both HDACIs. Drastic changes affecting cell proliferation and micronucleation but not alteration in CCND2 expression and in ratios of Bcl-2/Bax expression and cell death occurred following a 48-h exposure of the NIH 3T3 cells particularly in response to higher doses of VPA. Our results demonstrated that even low doses of VPA (0.05 mM) and TSA (10 ng/ml) treatments for 1 h can affect chromatin structure, including that of the heterochromatin areas, in non-transformed cells. HP1-α depletion, probably related to histone demethylation at H3K9me3, in addition to the effect of VPA and TSA on histone H3 acetylation, is induced on NIH 3T3 cells. Despite these facts, alterations in cell proliferation and micronucleation, possibly depending on mitotic spindle defects, require a longer exposure to higher doses of VPA and TSA.

Background: Spinal muscular atrophy (SMA) is an autosomal recessive disorder that affects the motoneurons of the spinal anterior horn, resulting in hypotonia and muscle weakness. The disease is caused by deletion or mutation in the telomeric copy of SMN gene (SMN1) and clinical severity is in part determined by the copy number of the centromeric copy of the SMN gene (SMN2). The SMN2 mRNA lacks exon 7, resulting in a production of lower amounts of the full-length SMN protein. Knowledge of the molecular mechanism of diseases has led to the discovery of drugs capable of increasing SMN protein level through activation of SMN2 gene. One of these drugs is the valproic acid (VPA), a histone deacetylase inhibitor. Methods: Twenty-two patients with type II and III SMA, aged between 2 and 18 years, were treated with VPA and were evaluated five times during a one-year period using the Manual Muscle Test (Medical Research Council scale-MRC), the Hammersmith Functional Motor Scale (HFMS), and the Barthel Index. Results: After 12 months of therapy, the patients did not gain muscle strength. The group of children with SMA type II presented a significant gain in HFMS scores during the treatment. This improvement was not observed in the group of type III patients. The analysis of the HFMS scores during the treatment period in the groups of patients younger and older than 6 years of age did not show any significant result. There was an improvement of the daily activities at the end of the VPA treatment period. Conclusion: Treatment of SMA patients with VPA may be a potential alternative to alleviate the progression of the disease.

The reasons for the intra- and interindividual variability in the clearance of valproic acid (VPA) have not been completely characterized. The aim of this study was to examine day-night changes in the clearance of 3-oxo-valproate (3-oxo-VPA), 4-hydroxy-valproate (4-OH-VPA), and valproic acid glucuronides under steady state. Six diurnally active healthy male volunteers ingested 200 mg sodium valproate 12 hourly, at 0800 and 2000, for 28 days. On the last study day, two sequential 12-h urine samples were collected commencing at 2000 the evening before. Plasma samples were obtained at the end of each collection. Following alkaline hydrolysis, urine was analyzed for concentrations of VPA, 3-oxo-VPA, and 4-OH-VPA. A separate aliquot was assayed for creatinine (CR). The plasma concentrations of VPA, 3-oxo-VPA, 2-en-VPA, and CR were determined. The analysis of VPA and its metabolites was performed by CC-MS. There was an increase in plasma 3-oxo-VPA concentration at 0800, sampling as compared to 2000 sampling (p < .05). The urinary excretion of 3-oxo-VPA and VPA glucuronides were decreased between 2000 and 0800, compared to between 0800, and 2000, by 30% and 50% respectively (p < .05). These results indicate a nocturnal decrease in renal clearance of 3-oxo-VPA rather than a decrease in the beta -oxidation of VPA at night. These differences were not explained by differences between the sampling periods in CR excretion. These results indicate the importance of collecting samples of 24-h duration when studying metabolic profiles of VPA.

Acyl glucuronides are reactive metabolites of carboxylate drugs, able to undergo a number of reactions in vitro and in vivo, including isomerization via intramolecular rearrangement and covalent adduct formation with proteins. The intrinsic reactivity of a particular acyl glucuronide depends upon the chemical makeup of the drug moiety. The least reactive acyl glucuronide yet reported is valproic acid acyl glucuronide (VPA-G), which is the major metabolite of the antiepileptic agent valproic acid (VPA). In this study, we showed that both VPA-G and its rearrangement isomers (iso-VPA-G) interacted with bovine brain microtubular protein (MTP, comprised of 85% tubulin and 15% microtubule associated proteins {MAPs}). MTP was incubated with VPA, VPA-G and iso-VPA-G for 2 h at room temperature and pH 7.5 at various concentrations up to 4 mM. VPA-G and iso-VPA-G caused dose-dependent inhibition of assembly of MTP into microtubules, with 50% inhibition (IC50) values of 1.0 and 0.2 mM respectively, suggesting that iso-VPA-G has five times more inhibitory potential than VPA-G. VPA itself did not inhibit microtubule formation except at very high concentrations (greater than or equal to2 mM). Dialysis to remove unbound VPA-G and iso-VPA-G (prior to the assembly assay) diminished inhibition while not removing it. Comparison of covalent binding of VPA-G and iso-VPA-G (using [C-14]-labelled species) showed that adduct formation was much greater for iso-vTA-G. When [C-14]-iso-VPA-G was reacted with MTP in the presence of sodium cyanide (to stabilize glycation adducts), subsequent separation into tubulin and MAPs fractions by ion exchange chromatography revealed that 78 and 22% of the covalent binding occurred with the MAPs and tubulin fractions respectively. These experiments support the notion of both covalent and reversible binding playing parts in the inhibition of microtubule formation from MTP (though the acyl glucuronide of VPA is less important than its rearrangement isomers in this regard), and that both tubulin and (perhaps more importantly) MAPs form adducts with acyl glucuronides. Sports Ererc., Vol. 34, No. 2, pp. 350-355, 2002. Purpose: The purpose of this study was to evaluate age and gender differences in objectively measured physical activity (PA) in a population-based sample of students in grades 1-12. Methods: Participants (185 male, 190 female) wore a CSA 7164 accelerometer for 7 consecutive days. To examine age-related trends. students were grouped as follows: grades 1-3 (N = 90), grades 4-6 (N = 91), grades 7-9 (N = 96). and grades 10-12 (N = 92). Bouts of PA and minutes spent in moderate-to-vigorous PA (MVPA) and vigorous PA (VPA) were examined. Results: Daily MVPA and VPA exhibited a significant inverse relationship with grade level, with the largest differences occurring between grades 1d-3 and 4-6. Boys were more active than girls; however, for overall PA, the magnitudes of the gender differences were modest. Participation in continuous 20-min bouts of PA was low to nonexistent. Conclusion: Our results support the notion that PA declines rapidly during childhood and adolescence and that accelerometers are feasible alternatives to self-report methods in moderately sized population-level surveillance studies.

L'état de mal épileptique est une condition médicale sévère fréquemment rencontrée dont la mortalité est importante. Son traitement représente donc une urgence médicale. Il a déjà été démontré par des études bien conduites (Treiman et al., 1998) que l'administration de benzodiazépines est efficace en première intention. Or, 35-45% des états de mal échappent à ce traitement et malheureusement, les données scientifiques concernant le traitement de deuxième ligne sont nettement moins consistantes. Par ailleurs, si le rôle pronostique des caractéristiques de l'état de mal (type de crise, étiologie, état de conscience, âge du patient) sont connues (Rossetti et al., 2006), (Neligan and Shorvon, 2011), le rôle des comorbidités n'a reçu que peu d'attention à ce Dans la première partie de ce travail publiée dans Epilepsia (Alvarez et al., 2011) nous nous sommes intéressé au traitement de deuxième ligne et principalement aux trois substances les plus prescrites : la phénytoine (PHT), le valproate (VPA) et plus récemment le lévétiracetam (LEV). A ce jour, aucune étude n'avait comparé l'efficacité de ces substances pourtant largement utilisées. Ainsi, afin de savoir lequel ces anti-épileptiques utilisés en 2ème ligne est le plus efficace, nous avons extrait de notre base de données regroupant tout les états de mal épileptiques traités au Centre Hospitalier Universitaire Vaudois, tous les épisodes durant lesquelles le traitement par benzodiazépines à échoué (187 épisodes). Nous avons ensuite comparé les différentes caractéristiques cliniques et les différents outcomes de trois groupes de patients (ceux qui ont reçu de la PHT, du VPA ou du LEV). Nous avons pu mettre ainsi en évidence certaines différences d'efficacité inconnues jusqu'alors entre le VPA et le LEV, impliquant une certaine prudence face à l'emploi grandissant de ce dernier. La seconde partie de notre publiée dans Epilepsia (Alvarez et al., 2012) s'est portée sur les facteurs pronostiques de l'état de mal et plus précisément sur le rôle joué par les comorbidités. En utilisant la même base de données, nous avons pu démontrer que le pronostique d'un état de mal est très majoritairement influencé par l'étiologie et l'âge et que les comorbidités ne jouent qu'un rôle marginal. La présence de comorbidités n'impliquant pas forcément une mauvaise issue, la fragilité de certains patients ne doit pas dissuader les cliniciens à traiter adéquatement ces patients souffrant d'une condition aussi sévère qu'un état de mal épileptique.

Objective: This analysis was performed to assess whether antiepileptic drugs (AEDs) modulate the effectiveness of temozolomide radiochemotherapy in patients with newly diagnosed glioblastoma.Methods: The European Organization for Research and Treatment of Cancer (EORTC) 26981-22981/National Cancer Institute of Canada (NCIC) CE.3 clinical trial database of radiotherapy (RT) with or without temozolomide (TMZ) for newly diagnosed glioblastoma was examined to assess the impact of the interaction between AED use and chemoradiotherapy on survival. Data were adjusted for known prognostic factors.Results: When treatment began, 175 patients (30.5%) were AED-free, 277 (48.3%) were taking any enzyme-inducing AED (EIAED) and 135 (23.4%) were taking any non-EIAED. Patients receiving valproic acid (VPA) only had more grade 3/4 thrombopenia and leukopenia than patients without an AED or patients taking an EIAED only. The overall survival (OS) of patients who were receiving an AED at baseline vs not receiving any AED was similar. Patients receiving VPA alone (97 [16.9%]) appeared to derive more survival benefit from TMZ/RT (hazard ratio [HR] 0.39, 95% confidence interval [CI] 0.24-0.63) than patients receiving an EIAED only (252 [44%]) (HR 0.69, 95% CI 0.53-0.90) or patients not receiving any AED (HR 0.67, 95% CI 0.49-0.93). Conclusions: VPA may be preferred over an EIAED in patients with glioblastoma who require an AED during TMZ-based chemoradiotherapy. Future studies are needed to determine whether VPA increases TMZ bioavailability or acts as an inhibitor of histone deacetylases and thereby sensitizes for radiochemotherapy in vivo.
Hemodynamic consequences of chronic parasympathetic blockade with a peripheral muscarinic antagonist

Résumé Alors que le système nerveux sympathique a un rôle bien établi dans la régulation de la pression artérielle, l'influence de la fonction ou dysfonction du système parasympathique sur la pression artérielle n'est pas évidente, en particulier à long terme. Nous avons testé l'hypothèse que l'inhibition chronique du système parasympathique a un effet persistant sur la pression artérielle (PA), la fréquence cardiaque (FC) et la variabilité de la pression artérielle (VPA). Des rats de type Sprague Dawley ont été instrumentés pour monitoring de la pression artérielle 22h par jour par télémétrie et abrités dans des cages métaboliques. Après guérison de l'intervention et après une période contrôle de référence, de la scopolamine methyl bromide (SMB), un antagoniste muscarinique périphérique, a été perfusé i.v. pendant 12 jours. Le tout fut suivi d'une période de récupération de 10 jours. La SMB a entraîné une augmentation rapide de la PA de 98±2 mmHg à un maximum de 108±2 mmHg le deuxième jour de perfusion de la SMB puis la PA s'est stabilisée à une valeur plateau de +3±1 mmHg au dessus de la valeur contrôle (P < 0.05). Après l'arrêt de la perfusion de SMB, la PA moyenne a diminué de 6±1 mmHg. La FC a augmenté immédiatement et est restée significativement au dessus de la valeur contrôle le dernier jour de 1a perfusion de SMB. La SMB a également induit une diminution de la variabilité à court terme de la FC (au sein d'intervalles de 30 minutes et une augmentation de la variabilité à court terme et à long terme (entre intervalles de 30 minutes) de la PA. Ces données suggèrent que l'inhibition muscarinique périphérique chronique conduit à une augmentation modeste mais persistante de la PA, FC et VPA, qui sont des facteurs de risque cardiovasculaires connus.

Objectives: Phenytoin (PHT), valproic acid (VPA), or levetiracetam (LEV) are commonly used as second-line treatment of status epilepticus (SE), but comparative studies are not available to date.Methods: In our tertiary care hospital, among 279 SE episodes prospectively collected over four years, and occurring in adults, we identified 187 episodes in which PHT, VPA or LEV were prescribed after benzodiazepines. Patients with post-anoxic SE were not included. Demographics, clinical SE features, failure of second-line treatment to control SE, new handicap and mortality at hospital discharge were assessed. Uni- and multivariable statistical analyses were applied to compare the three agents.Results: Each compound was used in about one third of episodes. VPA failed to control SE in 25.4%, PHT in 41.4% and LEV in 48.3% of episodes in which these were prescribed as second-line agents. After adjustment for known SE outcome predictors, LEV failed more often than VPA (OR 2.69; 95% CI 1.19-6.08); in others words, 16.8% (95% CI 6.0-31.4%) of second-line treatment failures could be attributed to prescription for LEV instead of VPA. PHT was statistically not different from the other two compounds. At discharge, second-line treatment did not influence new handicap and mortality, while etiology and severity of the SE episode were robust independent predictors.Conclusions: Even without significant differences on outcome at discharge, LEV seems less efficcacious than VPA to control SE after benzodiazepines. A prospective comparative trial is needed to address this potentially concerning finding. The second interesting finding is that the outcome seems more influenced by the SE characteristics than the treatment.

Les déacetylases d'histones (HDACs) déacétylent non seulement les histones, ce qui a généralement pour effet d'augmenter la transcription et l'expression génique, mais également d'autres protéines comme par exemple des protéines de choc thermique (HSP90), la tubuline alpha, certains récepteurs aux stéroïdes ainsi que de nombreux facteurs de transcription (NF-kB p65, Sp1, etc.). Ainsi les HDACs participent au contrôle de nombreux processus cellulaires. Les inhibiteurs des HDACs (ou HDACi), de part leur capacité à induire la différenciation cellulaire et l'apoptose, sont parmi les anti-cancéreux les plus prometteurs en cours de développement pour dans le traitement des néoplasies solides et hématologiques. Récemment, l'activité anti-inflammatoire et immuno- modulatrice des HDACi a été mise en évidence et exploitée avec succès pour le traitement de pathologies auto-immunes dans des modèles précliniques. L'effet des HDACi sur la réponse immunitaire innée restant largement inconnu, nous avons entrepris la première étude d'envergure dans ce domaine. Dans un premier article, nous démontrons que les HDACi inhibent l'expression de nombreux gènes (récepteurs aux produits microbiens, cytokines, chimiokines, molécules d'adhésion et co-stimulatrices, facteurs de croissance, etc.) impliqués dans les défenses anti¬infectieuses in vitro. En accord avec ces données, les HDACi augmentent la mortalité d'animaux infectés dans des modèles de pneumonie et de candidose bénignes. De manière congruente, les HDACi protègent les animaux de mortalité induite par choc toxique et septique en inhibant la réponse inflammatoire exubérante qui caractérise ces pathologies (Roger T. et al., Blood 2011). Afin de caractériser plus en détails l'influence des HDACi sur la réponse immunitaire innée, nous avons également analysé l'impact de deux HDACi, l'acide valproïque (VPA) et la trichostatin A (TSA), sur les principaux mécanismes de défenses antimicrobiennes des macrophages. Dans un second article (Mombelli et al., Journal of Infectious Diseases 2011), nous rapportons que la VPA et la TSA diminuent la capacité des macrophages à phagocyter et à détruire les bactéries Gram-positives Staphylococcus aureus et Gram-négatives Escherichia coli. En accord avec ces données, les HDACi inhibent l'expression de molécules impliquées dans la phagocytose comme les récepteurs éboueurs (Msr 1 et CD14) et de type lectine (Dectin 1), ainsi que les récepteurs aux opsonines (intégrines). Par ailleurs, les HDACi interfèrent avec l'expression de différentes sous unités de la NADPH oxydase (gp91p"ox, p22 phox, p47 phox, p40 phox, p67 phox et Rac2) et de l'oxyde nitrique (NO) synthétase inductible (iNOS), qui sont responsables de la production de dérivés oxygénés (ROS) et nitrogénés (NO) essentiels à la destruction des microorganismes dans le phagolysosome. En résumé, cette étude décrit des mécanismes par lesquels les HDACi diminuent la capacité d'ingérer et de détruire les bactéries, et ainsi augmentent la susceptibilité aux infections. Globalement, nos données indiquent que les HDACi sont de puissants anti¬inflammatoires qui pourraient favoriser la survenue d'infections chez les patients cancéreux traités avec ces drogues, comme semble par ailleurs le suggérer des études cliniques rapportées dans la littérature. Nous proposons un suivi clinique infectieux strict chez les patients traités avec ces agents.

Os estudos de dinâmica populacional dos principais recursos pesqueiros foram actualizados e foi re-aavaliado o estado dos stocks de garoupa, chicharro e sargo de areia do arquipélago de Cabo Verde. A época de reprodução foi estimada através da análise da evolução mensal do índice gonadosomático. A ogiva de maturação e o L50 (tamanho de primeira maturação) foram estimados segundo o modelo logístico e ajustado segundo o método da Máxima Verosimilhança. Foram estimados, ainda, os parâmetros alpha e beta das relações peso – comprimento e aplicado o Teste F para testar as diferenças da relação entre os sexos. No caso das avaliações, a separação das coortes foi feita através da minimização da soma do quadrado dos resíduos, segundo o método de MacDonald e Pitcher de separação de distribuições mistas. Segundo a natureza dos dados, foram aplicados métodos de avaliação diferentes. Para o stock de garoupa, em que uma Análise de População Virtual e uma Análise de Captura e Biomassa Desovante por recruta forma aplicadas, conclui-se que o Fterm encontra-se aproximadamente ao nível do F0.1 e está aquém do FSSBpR40. A Biomassa desovante por recruta (SSBpR) actual está aproximadamente a 46% da SSBpR virgem. Estes indicadores apontam para um estado de exploração óptimo. No caso do stock de chicharro, onde se aplicou uma Análise da Curva de Captura o estado de exploração é considerado como moderado, uma vez que o Fterm está aquém dos pontos de referência F0.1 e FSSBpR40. A SSBpR actual constitui aproximadamente 50 % da SSBpR virgem. Para o stock de sargo de areia, as análises apontam para uma exploração a níveis bastante intensos, uma vez que o Fterm está ao nível de Fmax, e excede o FSSBpR40 em aproximadamente 40%. A SSBpR actual constitui aproximadamente 32 % da SSBpR virgem.

Genótipos de milho com variabilidade genética contrastante apresentam potenciais produtivos diferentes. Isso pode ser causado, pelo menos em parte, por diferenças morfológicas no sistema radicular e nos parâmetros cinéticos de absorção de nutrientes. Este trabalho objetivou quantificar esses parâmetros em três cultivares de milho. Foram comparados um híbrido simples (HS), um híbrido duplo (HD) e uma variedade de polinização aberta (VPA). Determinaram-se os parâmetros de absorção (influxo máximo, Imax, constante de Michalies-Menten, Km, e concentração na solução onde a absorção cessa, Cmin) para N, P, K, Ca e Mg, além de atributos morfológicos radiculares, em experimentos efetuados em câmara de crescimento, com solução nutritiva. A morfologia das raízes variou pouco entre os genótipos, provavelmente por causa do cultivo das plantas em meio líquido. As diferenças entre genótipos quanto aos parâmetros cinéticos de absorção dependeram do nutriente. O Imax diferiu entre os cultivares para P; o Km, para N e P, e o Cmin, para N e K. A VPA, por apresentar maior variabilidade genética, deveria apresentar menores valores para Km e Cmin do que os híbridos. Contudo, isso só aconteceu para P em relação ao Km. O HS, por apresentar maior potencial produtivo, deveria expressar os maiores valores para Imax, mas isso não ocorreu com nenhum dos macronutrientes avaliados. Portanto, a absorção de nutrientes não parece ser um fator determinante nas diferenças de rendimento de grãos entre genótipos de milho com bases genéticas contrastantes.
