The objective of this study was to analyze the effect of two different high-intensity interval training (HIT) programs on selected aerobic physiological indices and 1500 and 5000 m running performance in well-trained runners. The following tests were completed (n = 17): (i) incremental treadmill test to determine maximal oxygen uptake (VO2max), running velocity associated with VO2 max (VVO2max), and the velocity corresponding to 3.5 mmol/L of blood lactate concentration (vOBLA); (ii) submaximal constant-intensity test to determine running economy (RE); and (iii) 1500 and 5000 m time trials on a 400 m track. Runners were then randomized into 95% vVO(2max) or 100% vVO(2max) groups, and undertook a 4 week training program consisting of 2 HIT sessions (performed at 95% or 100% vVO(2max), respectively) and 4 submaximal run sessions per week. Runners were retested on all parameters at the completion of the training program. The VO2 max values were not different after training for both groups. There was a significant increase in post-training vVO(2 max), RE, and 1500 in running performance in the 100% vVO(2 max) group. The vOBLA and 5000 m running performance were significantly higher after the training period for both groups. We conclude that vOBLA and 5000 m running performance can be significantly improved in well-trained runners using a 4 week training program consisting of 2 HIT sessions (performed at 95% or 100% vVO(2max)) and 4 submaximal run sessions per week. However, the improvement in vVO(2 max), RE, and 1500 in running performance seems to be dependent on the HIT program at 100% vVO(2 max).

[EN] With altitude acclimatization, blood hemoglobin concentration increases while plasma volume (PV) and maximal cardiac output (Qmax) decrease. This investigation aimed to determine whether reduction of Qmax at altitude is due to low circulating blood volume (BV). Eight Danish lowlanders (3 females, 5 males: age 24.0 +/- 0.6 yr; mean +/- SE) performed submaximal and maximal exercise on a cycle ergometer after 9 wk at 5,260 m altitude (Mt. Chacaltaya, Bolivia). This was done first with BV resulting from acclimatization (BV = 5.40 +/- 0.39 liters) and again 2-4 days later, 1 h after PV expansion with 1 liter of 6% dextran 70 (BV = 6.32 +/- 0.34 liters). PV expansion had no effect on Qmax, maximal O2 consumption (VO2), and exercise capacity. Despite maximal systemic O2 transport being reduced 19% due to hemodilution after PV expansion, whole body VO2 was maintained by greater systemic O2 extraction (P < 0.05). Leg blood flow was elevated (P < 0.05) in hypervolemic conditions, which compensated for hemodilution resulting in similar leg O2 delivery and leg VO2 during exercise regardless of PV. Pulmonary ventilation, gas exchange, and acid-base balance were essentially unaffected by PV expansion. Sea level Qmax and exercise capacity were restored with hyperoxia at altitude independently of BV. Low BV is not a primary cause for reduction of Qmax at altitude when acclimatized. Furthermore, hemodilution caused by PV expansion at altitude is compensated for by increased systemic O2 extraction with similar peak muscular O2 delivery, such that maximal exercise capacity is unaffected.

[EN] Acute hypoxia (AH) reduces maximal O2 consumption (VO2 max), but after acclimatization, and despite increases in both hemoglobin concentration and arterial O2 saturation that can normalize arterial O2 concentration ([O2]), VO2 max remains low. To determine why, seven lowlanders were studied at VO2 max (cycle ergometry) at sea level (SL), after 9-10 wk at 5,260 m [chronic hypoxia (CH)], and 6 mo later at SL in AH (FiO2 = 0.105) equivalent to 5,260 m. Pulmonary and leg indexes of O2 transport were measured in each condition. Both cardiac output and leg blood flow were reduced by approximately 15% in both AH and CH (P < 0.05). At maximal exercise, arterial [O2] in AH was 31% lower than at SL (P < 0.05), whereas in CH it was the same as at SL due to both polycythemia and hyperventilation. O2 extraction by the legs, however, remained at SL values in both AH and CH. Although at both SL and in AH, 76% of the cardiac output perfused the legs, in CH the legs received only 67%. Pulmonary VO2 max (4.1 +/- 0.3 l/min at SL) fell to 2.2 +/- 0.1 l/min in AH (P < 0.05) and was only 2.4 +/- 0.2 l/min in CH (P < 0.05). These data suggest that the failure to recover VO2 max after acclimatization despite normalization of arterial [O2] is explained by two circulatory effects of altitude: 1) failure of cardiac output to normalize and 2) preferential redistribution of cardiac output to nonexercising tissues. Oxygen transport from blood to muscle mitochondria, on the other hand, appears unaffected by CH.

Bioenergetics and neuromuscular determinants of the time to exhaustion at velocity corresponding to (V) over dotO(2)max in recreational long-distance runners. J Strength Cond Res 26(8): 2096-2102, 2012-The purpose of this study was to investigate the main bioenergetics and neuromuscular determinants of the time to exhaustion (T-lim) at the velocity corresponding to maximal oxygen uptake in recreational long-distance runners. Twenty runners performed the following tests on 5 different days: (a) maximal incremental treadmill test, (b) 2 submaximal tests to determine running economy and vertical stiffness, (c) exhaustive test to measured the T-lim, (d) maximum dynamic strength test, and (e) muscle power production test. Aerobic and anaerobic energy contributions during the T-lim test were also estimated. The stepwise multiple regression method selected 3 independent variables to explain T-lim variance. Total energy production explained 84.1% of the shared variance (p = 0.001), whereas peak oxygen uptake ((V) over dotO(2)peak) measured during T-lim and lower limb muscle power ability accounted for the additional 10% of the shared variance (p = 0.014). These data suggest that the total energy production, (V) over dotO(2)peak, and lower limb muscle power ability are the main physiological and neuromuscular determinants of T-lim in recreational long-distance runners.

Aims To test the effects of early exercise training (ET) on left ventricular (LV) and autonomic functions, haemodynamics, tissues blood flows (BFs), maximal oxygen consumption (VO(2) max), and mortality after myocardial infarction (MI) in rats. Methods and results Male Wistar rats were divided into: control (C), sedentary-infarcted (SI), and trained-infarcted (TI). One week after MI, TI group underwent an ET protocol (90 days, 50-70% VO2 max). Left ventricular function was evaluated noninvasively and invasively. Baroreflex sensitivity, heart rate variability, and pulse interval were measured. Cardiac output (CO) and regional BFs were determined using coloured microspheres. Infarcted area was reduced in TI (19 +/- 6%) compared with SI (34 +/- 5%) after ET. Exercise training improved the LV and autonomic functions, the CO and regional BF changes induced by MI, as well as increased SERCA2 expression and mRNA vascular endothelial growth factor levels. These changes brought about by ET resulted in mortality rate reduction in the TI (13%) group compared with the SI (54%) group. Conclusion Early aerobic ET reduced cardiac and peripheral dysfunctions and preserved cardiovascular autonomic control after MI in trained rats. Consequently, these ET-induced changes resulted in improved functional capacity and survival after MI.

Recentemente, surgiram alguns trabalhos que ressaltaram a importância do cálculo do volume da aurícula esquerda (VAE) como um marcador de eventos cardíacos adversos. Foi objectivo deste estudo avaliar a importância prognóstica deste parâmetro em doentes (dts) com deficiente função ventricular esquerda e correlacioná-lo com outros parâmetros clássicos de prognóstico – consumo de O2 (VO2 max) e pro-BNP (pBNP). Métodos: Analisou-se o volume da aurícula esquerda (VAE) por método de Simpson, numa população de 35 dts com cardiopatia dilatada (idiopática e isquémica) com fracção de ejecção (FE) 31±9,6% doentes (dts) eram de sexo masculino e a média de idades foi de 50,5±10,5 anos. Toda a população efectuou estudos de ecocardiografia convencional (incluindo avaliação por M-mode, bidimensional e Doppler), prova cardiorespiratória (VO2max) e doseamento de pro-BNP. O tempo médio de seguimento foi de 24 ± 4 meses, tendo-se considerado como eventos cardíacos (EC): internamento por insuficiência cardíaca, transplante e morte. Resultados: Dos parâmetros da ecocardiografia - o diâmetro da AE foi de 46,6±5,7mm, as dimensões do VE em diástole – 73,5±10mm e em sístole -58,9±11mm, a média da fracção de ejecção foi de 31±9,6%, o VAE foi de 78,6±33 ml, os volumes do VE foram de 214±82ml em diástole e de 153±75ml em sístole, 15 dts tinham padrão restritivo de enchimento ventricular (E/A>2), a média da área (Doppler cor) da insuficiência mitral foi de 4±3,3cm2, 14 dts tinham E/E’>15. O VO2 max médio foi de 20±5,8ml/kg/min e o pro-BNP de 3146±4629pg/mL. Para além da correlação de outros parâmetros clássicos ecocardiográficos com o prognóstico (volumes VE, FE e E/E’), o VAE e o volume indexado da AE (VAE/SC) mostraram uma correlação com o prognóstico (EC) com r=0,4 (p=0,02) que não se verificou para o diâmetro da AE (p=ns). Em relação à tolerância ao esforço, houve uma correlação inversa entre o diâmetro, o volume e o volume indexado da AE e o VO2max, com maior significado estatístico para o VAE e VAE/SC com r=-0,48, p=0,008. Quanto ao pro-BNP, quer o diâmetro, quer o VAE (ou volume indexado) tiverem o mesmo nível de significado estatístico (r=0,43; p=0,02). O valor predictivo de eventos (curvas ROC) para o VAE foi de 70ml e de 37ml/m2 para o VAE/m2. Conclusão: O volume da aurícula esquerda/volume indexado é um parâmetro ecocardiográfico com significado prognóstico em dts com deficiente função ventricular esquerda, correlacionando-se com a tolerância ao esforço e pro-BNP.

Previous studies have shown that a ratio of early transmitral flow velocity to early diastolic velocity of the mitral annulus (E/E') of > 15, obtained by tissue Doppler imaging (TDI), correlates with left ventricular filling pressure. OBJECTIVE: The aim of our study was to assess whether E/E' provides prognostic information in patients with dilated cardiomyopathy. METHODS: We studied 33 patients with dilated cardiomyopathy and mean ejection fraction of 31%. All the patients underwent routine two-dimensional and Doppler echocardiographic examination and TDI to determine early peak velocity of the mitral annulus. Pro-B-type natriuretic peptide (pro-BNP) and peak oxygen consumption (VO2max) were also measured. Patients were divided into two groups according to the value of E/E': Group I (n = 15 patients) with E/E' > or = 15 and Group II (n = 18 patients) with E/E' < 15. Patients were followed for 12+/-4 months; new hospital admission due to heart failure, heart transplantation and death were considered as cardiac events. RESULTS: There were significant differences between the two groups in conventional two-dimensional echocardiographic measurements (dimensions and ejection fraction) and Doppler parameters (mitral inflow). With regard to mitral annular velocities obtained by TDI at two different points (septum and lateral wall), the E', A' and S' velocities differed significantly between the two groups, with lower velocities in Group I. Systolic velocity measured in the lateral portion of the mitral annulus showed the most significant difference: Group I - 4.46 cm/sec versus Group II - 7.19 cm/sec, p < 0.00001. Pro-BNP was 5622 pg/ml in Group I, and 1254 pg/ml in Group II, p = 0.004. VO2 max was significantly different between the two groups: Group I - 17.6 ml/kg/min versus Group II - 22.8 ml/kg/min, p = 0.004. During follow-up, events were more common in Group I, with 9 patients (60%) having events, while in Group II, the event rate was 11.1% (2 patients), p = 0.004. CONCLUSION: The ratio of early transmitral flow velocity to early diastolic velocity of the mitral annulus is a powerful predictor of clinical outcome. Lower velocities of mitral annulus on TDI are expected in patients with E/E' > or = 15. Systolic velocities of under 5 cm/sec measured in the lateral portion of the mitral annulus appeared to be strongly related to prognosis.

A insuficiência cronotrópica constitui achado comum entre os pacientes chagásicos. Novas metodologias estão sendo empregadas na avaliação da resposta cronotrópica em vários grupos de pacientes. O índice cronotrópico-metabólico, um desses novos métodos, quantifica a relação entre o aumento da freqüência cardíaca e o consumo máximo de oxigênio (VO2 max) durante o teste ergométrico. A resposta normal é linear, com índice em torno de 1,0. Objetivamos avaliar a resposta cronotrópica e em indivíduos saudáveis e pacientes chagásicos com e sem disfunção ventricular esquerda, utilizando-se do índice cronotrópico-metabólico. Foram avaliados 171 pacientes com doença de Chagas sem doenças associadas e 24 controles submetidos a protocolo clínico e ao teste ergométrico máximo. Os chagásicos foram divididos em dois grupos: Ch1= pacientes com fração de ejeção (FE) > 39% e Ch 2= FE<40%. A análise e o cálculo do índice cronotrópico-metabólico foram feitos pelo método de Wilkoff. Os pacientes chagásicos apresentaram maior idade e maior prevalência de bloqueio completo de ramo direito, assim como menor VO2 max ao teste ergométrico. Ambos os grupos de chagásicos apresentaram menor inclinação do índice cronotrópico-metabólico (Ch1: 0,91±0,10, Ch2: 0,89±0,08) do que os controles (1,0±0,12, p< 0,001). Pacientes com doença de Chagas com e sem disfunção ventricular esquerda podem apresentar resposta cronotrópica deprimida, manifesta por menor inclinação do índice cronotrópico-metabólico.

INTRODUCTION: The septal position is an alternative site for cardiac pacing (CP) that is potentially less harmful to cardiac function. METHODS: Patients with Chagas disease without heart failure submitted to permanent pacemaker (PP) implantation at the Clinics Hospital of the Triângulo Mineiro Federal University (UFTM), were selected from February 2009 to February 2010. The parameters analyzed were ventricular remodeling, the degree of electromechanical dyssynchrony (DEM), exercise time and VO2 max during exercise testing (ET) and functional class (NYHA). Echocardiography was performed 24 to 48h following implantation and after one year follow-up. The patients were submitted to ET one month postprocedure and at the end of one year. RESULTS: Thirty patients were included. Patient mean age was 59±13 years-old. Indication for PP implantation was complete atrioventricular (AV) block in 22 (73.3%) patients and 2nd degree AV block in the other eight (26.7%). All patients were in NYHA I and no changes occurred in the ET parameters. No variations were detected in echocardiographic remodeling measurements. Intraventricular dyssynchrony was observed in 46.6% of cases and interventricular dyssynchrony in 33.3% of patients after one year. CONCLUSIONS: The findings of this work suggest that there is not significant morphological and functional cardiac change following pacemaker implantation in septal position in chagasic patients with normal left ventricular function after one year follow-up. Thus, patients may remain asymptomatic, presenting maintenance of functional capacity and no left ventricular remodeling.

OBJETIVO: Avaliar se a capacidade de exercício está relacionada à dimensão atrial esquerda (DAE) em pacientes com endomiocardiofibrose biventricular. MÉTODOS: Estudaram-se 38 pacientes sendo 25 mulheres, com idade média 37,5 ± 11,5 anos (variação de 11 a 59 anos), todos em ritmo sinusal, divididos nos grupos A (12 pacientes) e B (26 pacientes) de acordo com a classe funcional da NYHA na internação. Todos os pacientes foram submetidos à ergoespirometria para a obtenção do consumo máximo de oxigênio (VO2 max) e tiveram a dimensão atrial esquerda determinada pela ecocardiografia. RESULTADOS: VO2 max de 21,8±4,8 ml.kg-1.min-1 e 13,7±3,5 ml.kg.-1. min-1, e dimensão atrial esquerda de 3,7±0,7cm e 4,4± 0,7cm para os grupos A e B, respectivamente. Foi encontrada correlação significativa e inversa entre VO2max e a DAE nos grupos estudados. CONCLUSÃO: O aumento da dimensão do átrio esquerdo acha-se associado ao comprometimento da capacidade de exercício em pacientes com endomiocardiofibrose. Nossos achados levam a permitir a utilização da dimensão atrial esquerda para estimar um índice de capacidade funcional, mais complexo e difícil de avaliar, como o VO2max.

OBJETIVO: Comparar a tolerância ao exercício de crianças e adolescentes submetidos a teste ergométrico (TE) em esteira com os protocolos de Bruce ou em rampa, e descrever a velocidade e a inclinação alcançadas com o protocolo em rampa, para auxiliar na orientação do exercício com esse protocolo. MÉTODOS: Estudo observacional, tipo série de casos, com controle histórico, de 1.006 crianças e adolescentes entre 4 e 17 anos submetidos a TE entre outubro de 1986 e fevereiro de 2003, que concluíram um dos dois protocolos. Foram excluídos os que tiveram o TE interrompido por outras causas que não cansaço físico, os que estavam em uso de medicações que interferiam na freqüência cardíaca (FC) e aqueles com limitações físicas à realização do exercício. Na análise estatística dos dados foi adotado nível de significância para p < 0,05 e intervalo de confiança de 95%. RESULTADOS: O tempo de exercício próximo a 10 minutos no protocolo em rampa foi significativamente maior no protocolo de Bruce. A FC máxima alcançada foi superior a 180 bpm nos dois protocolos. A inclinação foi pouco maior nas meninas mais jovens com o protocolo de Bruce, e a velocidade e o consumo máximo de oxigênio (VO2 max) foram maiores em todas as faixas etárias nos que realizaram o protocolo em rampa. CONCLUSÃO: A velocidade e a inclinação alcançadas com o protocolo em rampa podem ser utilizadas como referência para auxiliar na orientação do exercício no TE com o protocolo em rampa, que mostrou tolerância ao esforço superior à do protocolo de Bruce.
