Introduction: We evaluated the role of cardiovascular autonomic changes in hemodynamics at rest and in response to exercise in streptozotocin-induced diabetic rats. Methods: Male Wistar rats were divided into nondiabetic (ND, n = 8) and diabetic (D, n = 8) groups. Arterial pressure signals were recorded in the basal state and after atropine or propranolol injections at rest, during exercise and during recovery. Results: At rest, vagal tonus was reduced in D (37 +/- 3 bpm) in comparison with the ND group (61 +/- 9 bpm). Heart rate during exercise was lower in D in relation to ND rats associated with reduced vagal withdrawal in the D group. The D rats had an increase in vagal tonus in the recovery period (49 +/- 6 bpm). Conclusions: Exercise-induced hemodynamic adjustment impairment in diabetic rats was associated with reduced cardiac vagal control. The vagal dysfunction was attenuated after aerobic exercise, reinforcing the positive role of this approach in the management of cardiovascular risk in diabetics. Muscle Nerve 46: 96101, 2012

A frequência cardíaca (FC) no repouso (FC Rep), no exercício máximo (FC Max) e após o exercício (FC Rec) traz importantes informações para a saúde, e parte dessas respostas é modulada pelo tônus vagal cardíaco (TVC), que também oferece proteção cardíaca. Para uma melhor compreensão dos aspectos prognósticos da FC e de suas interações com o TVC, foram realizados três estudos: dois retrospectivos e um ensaio clínico randomizado. O primeiro testou se o TVC, estimado utilizando o índice vagal cardíaco (IVC), contribui para a FC Max (% do previsto: 208-0,7 x idade (anos)) em 1000 indivíduos saudáveis (39 14 anos; 719 homens). Regressão linear identificou que TVC explica apenas 1% da variabilidade da FC Max (% do previsto), com erro padrão da estimativa alto (~ 6,3%), indicando potencial papel complementar clínico para essas duas variáveis relacionadas ao exercício. O segundo estudo verificou se a análise de mortalidade utilizando FC de reserva (FC Res) e FC Rec de forma combinada descriminaria melhor a mortalidade que a análise de um destes itens e forma isolada. Dados de FC Res e FC Rec de 1.476 indivíduos (41 a 79 anos, 937 homens) foram calculados e divididos em quintis, os quais somados forneceram categorias de 2 a 10, produzindo um gradiente da FC (FC Grad) e refletindo a magnitude dos transientes iniciais e finais do exercício máximo. Análises de sobrevida foram realizadas usando os quintis mais baixos (Q1) dos escores do Grad FC, FC Res e FC Rec. Em um seguimento médio de 7,3 anos, 44 participantes morreram (3,1%). Houve uma tendência inversa entre os escores do Grad FC e a taxa de mortalidade (p<0,05), que passou de 1,2% para 13,5%, respectivamente, para os escores 10 e 2. Uma pontuação no Grad FC de 2 foi melhor preditor de mortalidade por todas as causas, quando comparado ao Q1 da FC Res e da FC Rec, com riscos relativos ajustados pela idade de 3,53 (p=0,01); 2,52 (p<0,05) e 2,57(p<0,05), respectivamente. Conclusão: Grad FC é um preditor de risco de mortalidade por todas as causas com desempenho superior ao das medidas isoladas de FC Res e FC Rec. Por último verificou-se a hipótese do aumento do TVC em participantes de um programa de exercício supervisionado (PES) com IVC baixo (≤ 1,30), através de um treinamento específico, utilizando a transição repouso-exercício no protocolo denominado treinamento vagal (TV). Estes foram randomizados num delineamento cruzado (duas etapas de oito semanas), com ou sem três sessões semanais de TV. Houve discreta melhora no IVC em 16 semanas (1,19 vs 1,22; p=0,02) dos 44 pacientes (64% homens; 65,5 11,4 anos) que finalizaram o estudo, mas não se pôde afirmar que a diferença no IVC se deveu ao período em que foi realizado o TV (p=0,36). Portanto, 16 semanas de PES, incluindo oito semanas com TV, aumentam a resposta vagal à transição repouso-exercício, embora não tenha sido possível atribuir os resultados exclusivamente ao TV. We examined the role of physiological regulation (heart rate, vagal tone, and salivary cortisol) in short-term memory in preterm and full-term 6-month-old infants. Using a deferred imitation task to evaluate social learning and memory recall, an experimenter modeled three novel behaviors (removing, shaking, and replacing a glove) on a puppet. Infants were tested immediately after being shown the behaviors as well as following a 10-min delay. We found that greater suppression of vagal tone was related to better memory recall in full-term infants tested immediately after the demonstration as well as in preterm infants tested later after a 10-min delay. We also found that preterm infants showed greater coordination of physiology (i.e., tighter coupling of vagal tone, heart rate, and cortisol) at rest and during retrieval than full-term infants. These findings provide new evidence of the important links between changes in autonomic activity and memory recall in infancy. They also raise the intriguing possibility that social learning, imitation behavior, and the formation of new memories are modulated by autonomic activity that is coordinated differently in preterm and full-term infants.

Fond : Le substrat de fibrillation auriculaire (FA) vagale et celui secondaire à remodelage par tachycardie auriculaire (RTA) partagent beaucoup des caractéristiques : période réfractaire efficace (PRE) réduite, hétérogénéité accrue de PRE et quelques mécanismes moléculaires communs. Cette étude a comparé les 2 substrats à une abréviation comparable de PRE. Méthodes : Chez chacun de 6 chiens de groupe de stimulation vagal (SV), les paramètres de stimulation cervicale bilatérale de nerves vagaux ont été ajustés pour produire la même PRE moyenne (calculé à 8 sites des oreillettes gauche et droite) avec 6 chiens de groupe de RTA assorti à sexe et poids. Des paramètres électrophysiologiques, la durée moyenne de la fibrillation auriculaire (DAF) et les fréquences dominantes (FD) locales ont étés calculés. Résultats : En dépit des PREs assorties (SV: 80±12msec contre RTA: 79±12msec) la DAF était plus longue (*), l’hétérogénéité de conduction était plus élevée (*), la FD était plus rapide (*) et la variabilité de FD plus grande (*) chez les chiens SV. Les zones de maximum FD qui reflètent les zones d’origine de FA étaient à côté de ganglions autonomes chez les chiens SV. Conclusions : Pour un PRE atriale comparable, la FA secondaire à SV est plus rapide et plus persistante que la FA avec un substrat de RTA. Ces résultats sont consistants avec des modèles de travail suggérant que l'hyperpolarisation SV-induite contribue de façon important à la stabilisation et à l'accélération des rotors qui maintiennent la FA. La similitude de la distribution de FD du groupe vagal avec la distribution des lésions d’ablation après cartographie des électrogrammes atriales fragmentés suggère des nouvelles techniques d’ablation. La distribution des FD entre le SV et le RTA fournit de nouvelles idées au sujet de possible rémodelage neuroreceptorial et indique des différences importantes entre ces substrats de FA superficiellement semblables.

La fibrillation auriculaire est le trouble du rythme le plus fréquent chez l'homme. Elle conduit souvent à de graves complications telles que l'insuffisance cardiaque et les accidents vasculaires cérébraux. Un mécanisme neurogène de la fibrillation auriculaire mis en évidence. L'induction de tachyarythmie par stimulation du nerf médiastinal a été proposée comme modèle pour étudier la fibrillation auriculaire neurogène. Dans cette thèse, nous avons étudié l'activité des neurones cardiaques intrinsèques et leurs interactions à l'intérieur des plexus ganglionnaires de l'oreillette droite dans un modèle canin de la fibrillation auriculaire neurogène. Ces activités ont été enregistrées par un réseau multicanal de microélectrodes empalé dans le plexus ganglionnaire de l'oreillette droite. L'enregistrement de l'activité neuronale a été effectué continument sur une période de près de 4 heures comprenant différentes interventions vasculaires (occlusion de l'aorte, de la veine cave inférieure, puis de l'artère coronaire descendante antérieure gauche), des stimuli mécaniques (toucher de l'oreillette ou du ventricule) et électriques (stimulation du nerf vague ou des ganglions stellaires) ainsi que des épisodes induits de fibrillation auriculaire. L'identification et la classification neuronale ont été effectuées en utilisant l'analyse en composantes principales et le partitionnement de données (cluster analysis) dans le logiciel Spike2. Une nouvelle méthode basée sur l'analyse en composante principale est proposée pour annuler l'activité auriculaire superposée sur le signal neuronal et ainsi augmenter la précision de l'identification de la réponse neuronale et de la classification. En se basant sur la réponse neuronale, nous avons défini des sous-types de neurones (afférent, efférent et les neurones des circuits locaux). Leur activité liée à différents facteurs de stress nous ont permis de fournir une description plus détaillée du système nerveux cardiaque intrinsèque. La majorité des neurones enregistrés ont réagi à des épisodes de fibrillation auriculaire en devenant plus actifs. Cette hyperactivité des neurones cardiaques intrinsèques suggère que le contrôle de cette activité pourrait aider à prévenir la fibrillation auriculaire neurogène. Puisque la stimulation à basse intensité du nerf vague affaiblit l'activité neuronale cardiaque intrinsèque (en particulier pour les neurones afférents et convergents des circuits locaux), nous avons examiné si cette intervention pouvait être appliquée comme thérapie pour la fibrillation auriculaire. Nos résultats montrent que la stimulation du nerf vague droit a été en mesure d'atténuer la fibrillation auriculaire dans 12 des 16 cas malgré un effet pro-arythmique défavorable dans 1 des 16 cas. L'action protective a diminué au fil du temps et est devenue inefficace après ~ 40 minutes après 3 minutes de stimulation du nerf vague.

Voltage-gated potassium (Kv) channels are essential components of neuronal excitability. The Kv3.4 channel protein is widely distributed throughout the central nervous system (CNS), where it can form heteromeric or homomeric Kv3 channels. Electrophysiological studies reported here highlight a functional role for this channel protein within neurons of the dorsal vagal nucleus (DVN). Current clamp experiments revealed that blood depressing substance (BDS) and intracellular dialysis of an anti-Kv3.4 antibody prolonged the action potential duration. In addition, a BDS sensitive, voltage-dependent, slowly inactivating outward current was observed in voltage clamp recordings from DVN neurons. Electrical stimulation of the solitary tract evoked EPSPs and IPSPs in DVN neurons and BDS increased the average amplitude and decreased the paired pulse ratio, consistent with a presynaptic site of action. This presynaptic modulation was action potential dependent as revealed by ongoing synaptic activity. Given the role of the Kv3 proteins in shaping neuronal excitability, these data highlight a role for homomeric Kv3.4 channels in spike timing and neurotransmitter release in low frequency firing neurons of the DVN.

Este trabalho teve o objetivo de avaliar as possíveis alterações eletrocardiográficas de cães submetidos a duas condutas anestésicas, indicando aquela que produz menores alterações no eletrocardiograma (ECG). Foram utilizados 40 cães com idade igual ou superior a cinco anos, 24 machos e 16 fêmeas, com peso entre 5 e 42kg, oriundos de clínicas particulares da cidade de São Paulo e do canil da Polícia Militar do Estado de São Paulo, sem sintomas cardiovasculares e triados para tartarectomia. Todos os cães foram submetidos a dois ECG: um antes da anestesia e outro imediatamente antes de se iniciar o procedimento cirúrgico. As características eletrocardiográficas avaliadas foram: ritmo, duração da onda P, do complexo QRS e dos intervalos PR e QT, amplitude das ondas P e R e alterações do segmento ST e da onda T; avaliou-se também o índice de tono vagal (ITV). Os cães foram divididos em: grupo 1, que recebeu atropina, levomepromazina, tiopental e halotano (ALTH) e grupo 2, que recebeu atropina, tiletamina e zolazepam (ATZ). Os resultados mostraram que a associação ALTH produziu algumas alterações discretas no ECG dos cães avaliados, caracterizadas por modificações qualitativas no segmento ST, na onda T e no ritmo cardíaco, reduzindo significativamente o ITV, com tendência à bradicardia e hipóxia. Já a associação ATZ, além das alterações já citadas, produziu diminuição da duração dos intervalos PR e QT, bem como aumentou a freqüência cardíaca, com tendência à taquicardia e hipóxia.

Autonomic control of heart rate variability and the central location of vagal preganglionic neurones (VPN) were examined in the rattlesnake ( Crotalus durissus terrificus), in order to determine whether respiratory sinus arrhythmia (RSA) occurred in a similar manner to that described for mammals. Resting ECG signals were recorded in undisturbed snakes using miniature datalogging devices, and the presence of oscillations in heart rate (f(H)) was assessed by power spectral analysis (PSA). This mathematical technique provides a graphical output that enables the estimation of cardiac autonomic control by measuring periodic changes in the heart beat interval. At fH above 19 min(-1) spectra were mainly characterised by low frequency components, reflecting mainly adrenergic tonus on the heart. By contrast, at f(H) below 19 min(-1) spectra typically contained high frequency components, demonstrated to be cholinergic in origin. Snakes with a f(H) > 19 min(-1) may therefore have insufficient cholinergic tonus and/or too high an adrenergic tonus acting upon the heart for respiratory sinus arrhythmia ( RSA) to develop. A parallel study monitored f(Hd) simultaneously with the intraperitoneal pressures associated with lung inflation. Snakes with a fH < 19 min(-1) exhibited a high frequency (HF) peak in the power spectrum, which correlated with ventilation rate (f(V)). Adrenergic blockade by propranolol infusion increased the variability of the ventilation cycle, and the oscillatory component of the f(H) spectrum broadened accordingly. Infusion of atropine to effect cholinergic blockade abolished this HF component, confirming a role for vagal control of the heart in matching f(H) and f(V) in the rattlesnake. A neuroanatomical study of the brainstem revealed two locations for vagal preganglionic neurones (VPN). This is consistent with the suggestion that generation of ventilatory components in the heart rate variability (HRV) signal are dependent on spatially distinct loci for cardiac VPN. Therefore, this study has demonstrated the presence of RSA in the HRV signal and a dual location for VPN in the rattlesnake. We suggest there to be a causal relationship between these two observations.

The vagus is clearly of primary importance in the regulation of reptilian cardiorespiratory systems. Vagal control of pulmonary blood flow and cardiac shunts provides reptiles with an additional means of regulating arterial oxygen levels that is not present in endothermic vertebrates (birds and mammals). Within a given species, there exists a clear correlation between withdrawal of vagal tone on the cardiovascular system and elevated metabolic rate. Undisturbed and resting reptiles are normally characterised by high vagal tone, low pulmonary blood flow and large right-left (R-L) cardiac shunts. The low oxygen levels that result from the large R-L shunt may serve to regulate metabolism. However, when metabolism is increased by temperature, exercise or digestion, the R-L cardiac shunt is reduced, which serves to increase oxygen delivery. This response is partially elicit ed by reduction of vagal tone. Interspecies comparisons reveal a similar pattern. Thus, species that are able to sustain the highest metabolic rates possess the highest degree of anatomical ventricular separation and, therefore, less cardiac shunting. It is interesting to note that when cardiac shunts occur in mammals, due for example to developmental defects, they are associated with reduced maximal metabolic rates and impaired exercise tolerance. It appears, therefore, that full separation of ventricular blood flows was a prerequisite for the evolution of high aerobic metabolic rates and exercise stamina in mammals and birds.
