The sonographic evaluation of thyroid glands in veterinary medicine presents challenges due to the complexity of the anatomical region, professional experience and type of ultrasonography equipment. The technique is considered a versatile diagnostic method that is noninvasive and has a low cost indicated in different clinical situations. Thyroid carcinoma is a malignant tumor that is often invasive and frequently metastatic to regional lymph nodes and lungs. The prognostic indicators for survival after surgery include tumor size, histological type, mobility and presence or absence of metastasis. The objective of the present report is to demonstrate the importance of ultrasound as a complementary method in the evaluation of thyroid carcinoma in dogs. At the Dr. Halim Atique Veterinary Hospital, an eight-year-old male Pit Bull was examined due to a history of firm painless swelling, approximately six inches in diameter, in the ventral cervical region, for about two months. The sonogram showed a nodular area, with defined and regular margins, and heterogeneous hypoechoic parenchyma, with areas of cavitation and swelling of the thyroid. Histopathology of the nodule was consistent with carcinoma. After thyreoidectomy and hormone replacemet, the patient is in good clinical condition.

Introduction: Previous studies had enlisted renal medullary carcinoma (RMC) as the seventh nephropathy in sickle cell disease (SCD). Clinical experience has contradicted this claim and this study is targeted at refuting or supporting this assumption. Objective: To estimate the prevalence of RMC and describe other renal complications in SCD. Materials and methods: 14 physicians (haematologists and urologists) in 11 tertiary institutions across the country were collated from patients’ case notes and hospital SCD registers. Results: Of the 3,596 registered sickle patients, 2 (0.056%) had been diagnosed with RMC over a ten year period, thereby giving an estimated prevalence rate of 5.6 per 100,000. The most common renal complication reported by the attending physicians was chronic kidney disease (CKD). The frequency of routine renal screening for SCD patients varied widely between centres – most were done at diagnosis, annually or bi-annually. Conclusion: The ten year prevalence of RMC in Nigerian SCD patients was determined to be 5.6 (estimated incidence of 0.56). RMC is not more common in SCD patients and therefore cannot be regarded as a “Seventh Sickle nephropathy”. Most of the managing physicians reported that the commonest nephropathy observed in their SCD patients was chronic kidney disease.

Somatostatin analogue scintigraphy represents a new technique employing radiolabelled peptides to detect specific receptor-bearing lesions. 111Indium diethylenetriaminopentaacetic acid-linked octreotide (111In-DTPA-D-Phe1 octreotide), also known as [111In]pentetreotide or OctreoScan, is now established in the management of patients with neuroendocrine gastrointestinal tract and pancreatic tumours, and has proved effective in localizing disease sites in lung, breast and medullary thyroid carcinomas, lymphomas, meningiomas and others. In these conditions (a) the imaging of all disease sites at a single sitting (in a proportion of patients) thereby making further investigations unnecessary, (b) the localization of otherwise unexpected metastatic deposits and (c) the detection of residual disease not found by other means suggest that [111In]pentetreotide may be a useful adjunct in the diagnostic evaluation of patients with somatostatin receptor-bearing tumours.

The endothelin axis is a group of signaling molecules and their receptors that have been implicated in vascularization of cancers, with their expression being observed to change in different cancer types. In this research, we examined the expression of endothelin 1 and endothelin receptor A at the protein and messenger RNA (mRNA) levels in 123 papillary thyroid carcinomas and 40 matched lymph nodes with metastatic papillary thyroid carcinomas. We found altered endothelin axis mRNA expression in several clinicopathologic parameters with increased endothelin 1 expression in thyroid papillary carcinoma showing stromal calcification, cancers in men, and primary cancers with lymph node metastases. Increased endothelin receptor A mRNA expression was noted in the larger cancers. There is a significant correlation between expression of endothelin receptor A and endothelin 1 in papillary thyroid carcinoma. Both endothelin receptor A and endothelin 1 mRNA expressions were significantly higher in metastatic carcinoma in the lymph node than in primary thyroid cancer. The metastatic carcinoma in the lymph node had increased expression compared with matched primary thyroid carcinoma. Expressions of endothelin 1 and endothelin receptor A were also documented as being high at the protein level. Our results indicate that in thyroid cancer, endothelin 1 and endothelin receptor A are associated with growth in advanced stages and lymph node metastases, likely through known angiogenic linkages. Targeting the endothelin axis may be useful in planning angiogenesis therapy for thyroid cancer.

The treatment of choice for these cases was surgical removal. In both cases the diagnostic was infiltrative follicular carcinoma of thyroid. The first animal did not present a good recovery after the surgery, and died four hours after the procedure. In the second case, the diagnosis was more precocious and antineoplastic chemotherapy was used after the surgery. At the time of submission of this manuscript, this animal had survived for foully months. Currently, there is a need to define the protocols of chemotherapy to avoid relapse and metastases, in order to increase the life expectancy in dogs with thyroid neoplasm.

Increased incidence of incidental cancer in patients operated for benign thyroid disease has been reported. We report our experience about incidental thyroid cancer (ITC) in order to better characterize this nosologic entity. Between 2001 and 2009 a total of 568 patients underwent surgery for benign thyroid disease. Patients with preoperative cytology undetermined or positive for malignancy were excluded. The most frequent indication for surgery was multinodular or diffuse nontoxic goiter. We performed total thyroidectomy in 499 cases and emithyroidectomy in 69 cases. Final histology revealed ITC in 53 patients (9.3%): 44 had papillary carcinoma (20 classic variant and 24 follicular variant), 4 follicular carcinoma, 4 medullary carcinoma and 1 primitive thyroid paraganglioma. The preoperative diagnosis was multinodular or diffuse goiter in 45 cases of ITC and uninodular goiter in 8 cases. We performed total thyroidectomy in 46 case, emithyroidectomy in 4 patients with past history of lobectomy, emithyroidectomy in 3 patients with following radicalization and central neck dissection. In 14 patients the tumor was multifocal and in 12 of these patients the tumor foci were bilateral. The lesion was a microcarcinoma in 34 cases. Mean diameter of the ITC was 1.14 cm. We retrospectively reconsidered the results of preoperative ultrasound examinations in relation to the exact position of the tumor in the specimens and we found a statistically significant association between echogenicity and papillary histotype. Twenty-six patients were followed up at our Hospital. The mean follow-up period was 38.2 months. A relapse was observed in 3/26 patients. Incidental thyroid cancer in patients operated for benign disease has its own surgical and oncological relevance. A correct preoperative assessment, with a careful selection of nodules for fine-needle aspiration cytology on the basis of ultrasound pattern, could better address the choice of surgical procedure. The non irrelevant incidence of incidental thyroid cancer, the eventuality of multifocality and bilaterality and the possible occurrence of relapse, support that total thyroidectomy without residuum is a valuable option for treating benign thyroid conditions such as multinodular goitre. When an incidental cancer is diagnosed after emithyroidectomy, a radicalization with central neck dissection could be considered. We suggest that natural history of papillary microtumors and the correct surgical approach for these lesions could be better defined with a more extensive use of “Porto proposal” criteria. A decreased radioiodine uptake is frequently detected in differentiated thyroid carcinomas (DTC) and is associated with high recurrence rate and reduced survival. We investigated the correlation between NIS mRNA expression levels in the primary tumor and patient outcome using a quantitative real-time RT-PCR method. NIS expression was decreased in 17 DTC (21.04 +/- 39.66 pg Eq) compared to four autoimune thyroid disease (180.51 +/- 92.63 pg Eq) and 14 normal tissues (75.71 +/- 66.98 pg Eq) (p < 0.0001). The 17 thyroid differentiated carcinoma patients were submitted to surgery complemented by radioiodine ablation and had at least 24 months of follow-up, under levothyroxine continued suppressive therapy. According to their outcome, we could characterize a group of papillary carcinoma patients with aggressive carcinomas, whose NIS mRNA levels were markedly lower than a group with non-aggressive carcinomas (0.62 +/- 0.79 versus 54.87 +/- 53.79: p < 0.005). We suggest that the quantification of NIS mRNA relative levels in the primary tumor may predict poor outcome. (C) 2003 Elsevier Ireland Ltd. All rights reserved.

Medullary breast cancer (MBC) is a rare, diagnostically difficult, pathological subtype. Despite being high grade, it has a good prognosis. MBC patients have an excess of BRCA1 germ-fine mutation and reliable identification of MBC could help to identify patients at risk of carrying germline BRCA1 mutations or in whom chemotherapy could be avoided. The aim of this study was therefore to improve diagnosis by establishing an MBC protein expression profile using immunohistochemistry (IHC) on tissue-microarrays (TMA). Using a series of 779 breast carcinomas ('EC' set), diagnosed initially as MBC, a double-reading session was carried out by several pathologists on all of the histological material to establish the diagnosis as firmly as possible using a 'medullary score'. Only MBCs with high scores, i.e. typical MBC (TMBC) (n = 44) and non-TMBC grade III with no or low scores (n = 160), were included in the IHC study. To validate the results obtained on this first set, a control series of TMBC (n = 17) and non-MBC grade III cases (n = 140) ('IPC' set) was studied. The expression of 18 proteins was studied in the 61 TMBCs and 300 grade III cases from the two sets. The global intra-observer concordance of the first reading for the diagnosis of TMBC was 94%, with almost perfect kappa (kappa) of 0.815. TMBC was characterized by a high degree of basal/myoepithelial differentiation. In multivariate analysis with logistic regression, TMBC was defined by the association of P-cadherin (R = 2.29), MIB1 > 50 (R = 3.80), ERBB2 negativity (R = 2.24) and p53 positivity (RR = 1.45). Aims: Cytokeratin (CK) 14, a myoepithelial marker, is also expressed in a proportion of breast carcinomas. There is evidence that these tumours show a differing metastatic pattern and clinical outcome from other invasive ductal carcinomas (IDCs) and may need different management. Currently, they are not identified in routine practice and no morphological guidelines exist to aid their identification. The aim of this study was to analyse the histological features of CK14+ IDC. Methods and results: A detailed histological review of 453 grade 3 IDCs revealed 88 (19.4%) that expressed CK14. Assessment was made independently by two pathologists using a standardized 'tick-box' proforma covering grade, architectural and cytological features. The results were analysed using logistic regression to identify features that predicted for basal phenotype. Concordance between the two pathologists was fair to good for most parameters (kappa 0.4-0.6). On multiple logistic regression, the basal phenotype was highly significantly associated with the presence of a central scar (P = 0.005), tumour necrosis (P < 0.0001), presence of spindle cells (P = 0.006) or squamous metaplasia (P < 0.0001), high total mitotic count (> 40 per 10 high-power field) (P = 0.0002) and high nuclear-cytoplasmic ratio (P = 0.0002). Conclusions: Specific morphological features are strongly associated with basal-like breast carcinoma. These could be used in routine diagnostic practice to identify this important subset of tumours.

Prostacyclin synthase and thromboxane synthase signaling via arachidonic acid metabolism affects a number of tumor cell survival pathways such as cell proliferation, apoptosis, tumor cell invasion and metastasis, and angiogenesis. However, the effects of these respective synthases differ considerably with respect to the pathways described. While prostacyclin synthase is generally believed to be anti-tumor, a pro-carcinogenic role for thromboxane synthase has been demonstrated in a variety of cancers. The balance of oppositely-acting COX-derived prostanoids influences many processes throughout the body, such as blood pressure regulation, clotting, and inflammation. The PGI2/TXA2 ratio is of particular interest in-vivo, with the corresponding synthases shown to be differentially regulated in a variety of disease states. Pharmacological inhibition of thromboxane synthase has been shown to significantly inhibit tumor cell growth, invasion, metastasis and angiogenesis in a range of experimental models. In direct contrast, prostacyclin synthase overexpression has been shown to be chemopreventive in a murine model of the disease, suggesting that the expression and activity of this enzyme may protect against tumor development. In this review, we discuss the aberrant expression and known functions of both prostacyclin synthase and thromboxane synthase in cancer. We discuss the effects of these enzymes on a range of tumor cell survival pathways, such as tumor cell proliferation, induction of apoptosis, invasion and metastasis, and tumor cell angiogenesis. As downstream signaling pathways of these enzymes have also been implicated in cancer states, we examine the role of downstream effectors of PGIS and TXS activity in tumor growth and progression. Finally, we discuss current therapeutic strategies aimed at targeting these enzymes for the prevention/treatment of cancer. © 2010 Elsevier B.V. All rights reserved.

O câncer de tireoide é a neoplasia maligna mais comum de glândula endócrina e tem sua incidência aumentada em vários países, inclusive o Brasil, nos últimos anos. Em crianças e adolescentes, o câncer de tireoide é raro, correspondendo a cerca de 2% dos casos, sendo o carcinoma papilífero o tipo histológico mais frequente. O prognóstico de crianças e adolescentes com câncer de tireoide é excelente, com taxas de sobrevida superiores a 95%. Contudo, quando diagnosticada, essa parcela da população apresenta características clínico-patológicas de doença avançada, como invasão de cápsula e extensão extratireoidiana, além de metástase linfonodal. A leptina é um hormônio produzido principalmente pelo tecido adiposo, participando da regulação da ingestão de alimentos e do gasto energético e tem sido associada a fator prognóstico de alguns tumores, como hematológicos, pancreático, câncer de mama e tireoide. Há relatos deste hormônio atuar como fator angiogênico e mitogênico. O objetivo deste estudo é avaliar a associação entre o carcinoma papilífero de tireoide em crianças e adolescentes e proteínas da via de sinalização da leptina, através de imunofluorescência para ObR (receptor de leptina), pJAK2 e pSTAT3, relacionando com dados que apontam para o prognóstico da doença. Foram incluídos no estudo 52 pacientes, sendo a expressão e o padrão de marcação avaliados por consenso entre dois patologistas. O perfil de marcação foi puntiforme e multifocal para o tecido não neoplásico adjacente como para o tecido tumoral. Foram atribuídos scores para a imunomarcação (1-10% e >10%) e analisadas possíveis associações entre ObR, pJAK2 e pSTAT3 com variáveis demográficas qualitativas ou características clínico-patológicas. Todos os casos apresentaram marcação relevante (>10%) no tecido não-neoplásico para as 3 proteínas, enquanto no tecido neoplásico, houve marcação em 53% dos casos para ObR, 47% para pJAK2 e 71% para pSTAT3. ObR foi 6,2 vezes mais frequente em pacientes sem metástase linfonodal. O tamanho do tumor foi inversamente relacionado com marcação relevante para pJAK2 e pSTAT3. Nossos achados mostram que ObR está relacionado a menos metástase linfonodal e pJAK-pSTAT3 com tamanho tumoral reduzido, sugerindo que a via de sinalização da leptina em câncer de tireoide de crianças e adolescentes pode contribuir para prevenir o aumento da tumorigênese.

Os tumores bem diferenciados da tireóide representam mais de 95% das neoplasias malignas da glândula. A identificação pré-operatória dos carcinomas papilíferos está bem estabelecida através dos métodos de Punção aspirativa por Agulha fina e Ultrassonografia, com quase 100% de acurácia, enquanto os tumores foliculares significam um dilema para o cirurgião, visto que os métodos existentes não conseguem determinar com eficiência o diagnóstico sendo que 60-80% dos casos operados são benignos. Dessa forma, com o intuito de se analisar alterações genômicas do tipo variações no número de cópias (CNVs) que possam diferenciar esse tumor de outros tipos de doenças da tireóide, foram analisados 13 pacientes com doença tireoidiana (3 bócios, 2 hiperplasias, 4 adenomas foliculares e 4 carcinomas foliculares ) mais 1 individuo sadio através do método de aCGH, para identificação de CNVs que pudessem determinar com eficiência a presença de carcinoma folicular. Os achados foram confrontados com dados de carcinomas papilífero clássico (4 pacientes) e variante folicular (2 pacientes). Foram encontradas 725 CNVs na amostra, 703 dos pacientes com patologia. Dentre estas foram selecionadas 18 regiões mais frequentes. Houve um padrão de amplificação maior em pacientes jovens com adenomas e deleção em pacientes mais velhos. Os pacientes com carcinoma apresentaram taxas de CNVs muito próximas. Os carcinomas foliculares apresentaram padrões exclusivos de alteração nos cromossomos 8 e 12. Concluímos, assim, que os carcinomas foliculares da tireoide são uma patologia com um padrão exclusivo de alterações, não havendo correlação de progressão tumoral a partir de adenomas foliculares, sendo que duas regiões -8p22 e 12p13.32-p13.33, estão presentes em 100% e 75% das amostras respectivamente, podendo ser fortes candidatos a marcadores desse tipo tumoral.
