In the framework of health services research sponsored by the Swiss National Science Foundation, a research was undertaken of the activity of the large majority of the public health nurses working in the Swiss cantons of Vaud and Fribourg (total population 700,000). During one week, 130 nurses gathered, with a specially devised instrument, data on 4165 patient visits. Studying the duration of the contacts, one has distinguished contact duration per se (DC), duration of the travel time preceding the contact (DD), and total duration in relation with the contact (DTC-addition of the first two). It was noted that the three durations increased significantly with patient age (as regard travel time, this is explained by the higher proportion of home visits in higher age groups, as compared with visits at a health center). Examined according to location of the visit, contact duration per se (without travel) is higher for visits at home and in nursing homes than for those taking place at a health center. Looked at in respect to the care given (technical care, or basic nursing care, or both simultaneously), our data show that the provision of basic nursing care (alone or with technical care) doubles contact duration (from 20 to 42-45'). The analyses according to patient age shows that, at an advanced age (beyond 80 years particularly), there is an important increase of the visits where both types of care are given. However, contact duration per se shows a significant raise with age only for the group "technical care only"; it can be demonstrated that this is due to the fact that older patients require more complex technical acts (e.g., bladder care, as compared with simpler acts such as injection). A model of the relationships between patient age and contact duration is proposed: it is because of the increase in the proportions of home visits, of visits including basic nursing care, and of more complex technical acts that older persons require more of the working time of public health nurses.

Flexitime : between autonomy and constraints. A case study in SwitzerlandBy looking at how a new regulation is translated into everyday practices, this dissertation explores through a specific case study the degree of autonomy gained by wage-earners with the introduction of flexible working schedules. The guiding hypothesis is that by introducing procedural rules, flexitime opens the space for more daily negotiations, therefore reinforcing the effects of power relations inherent to employment relationships. The goal is to understand, through a sociological approach, how employees experience a form of working time that transfers responsibility for time management to them, and howthey integrate work-related constraints with their life outside the workplace. The first part of the dissertation sets up the context of the case study. It offers a definition of flexibility by situating it in the broader history of work time, as well as in relation to various organizational forms and cultural transformations. An international literature review and a focus on the Swiss case are offered. In the second part, the focus is narrowed to a specificSwiss firm specialized in mail-order, where a system of individualized management of annual work time has been introduced. By combining a quantitative and qualitative approach, it is possible to analyze determinants of the practices internal to the firm anddeterminants related to employees themselves, as well as the way in which employees articulate these two orders of constraints. The results show that the implementation of flexible working time is not affecting daily negotiation practices so much as it is creating a set of informal rules. The autonomy ofwage-earners is expressed first and foremost through their capacity to produce, negotiate, and legitimate these rules. The intraindividual level has proven to be central for the social regulation of flexible working time. It is not so much a question of legitimation, but rather the process of institutionalization nurtured by the energy invested by wage-earners in their personal quest for a compromise between their various roles, identities, and aspirations. It is this individualized regulation that is ensuring the success of the system under study.

Résumé I (Pratiques Pédagogiques)- Ce compte-rendu du stage réalisé pour ma deuxième année de master rapporte le résultat de l’observation des cours donnés à trois élèves de niveaux différents par le professeur de harpe de l’Ecole de Musique « Nossa Senhora do Cabo » à Linda-a-Velha, commune de Oeiras, située près de Lisbonne. Grâce à l’activité du professeur, et par le suivi de l’évolution de ses élèves tout au long de l’année scolaire 2012-2013, tant sur le plan technique que sur le plan musical, j’ai pu participer à toutes les étapes de leur apprentissage et retrouver quelques principes pédagogiques fondamentaux. Ainsi, j’ai constaté la nécessité d’une organisation didactique solide dans la définition d’objectifs, la planification du travail, le choix des méthodes d’étude, mais souple par la régulation des rythmes d’apprentissage et des techniques d’acquisition. La métacognition est aussi une notion composante essentielle de la pratique du professeur, dont un des grands objectifs est de développer chez ses élèves la capacité de se prendre en charge seul. J’ai également apprécié l’importance de l’aspect relationnel intrinsèque à toute situation d’apprentissage, ainsi que celle de la connaissance des théories de la motivation, atout important permettant d’agir au niveau psychologique sur les élèves et d’obtenir à plus ou moins long terme des changements comportementaux influents sur la qualité de ces apprentissages. J’ai enfin essayé de dégager différents types d’approches pédagogiques possibles, parmi les stratégies observées chez le professeur, ainsi que d’après quelques éléments de réflexion personnelle.

Le travail en situations d'urgence, notamment médicale, comporte de nombreux facteurs de risque pour la santé mentale. Cependant, des études récentes suggèrent que la reconnaissance de même que certains aspects relatifs au sens du travail pourraient permettre aux individus de préserver leur équilibre psychique malgré la présence de stresseurs importants.Nous avons effectué une étude portant sur la santé au travail pour des ambulanciers travaillant en Suisse romande. Dans un premier temps, une phase qualitative centrée sur l'observation du travail réel a été effectuée. Les observations ont été effectuées dans 11 services pour un total de 416 heures d'observation. Dans un second temps, un questionnaire a été développé et administré à l'ensemble des ambulanciers de Suisse romande.Notre étude a notamment permis de confirmer l'importance de la reconnaissance et du sens du travail pour les ambulanciers.
