Background: Impairment in pulmonary capacity due to pleural effusion compromises daily activity. Removal of fluid improves symptoms, but the impact, especially on exercise capacity, has not been determined. Methods: Twenty-five patients with unilateral pleural effusion documented by chest radiograph were included. The 6-min walk test, Borg modified dyspnea score, FVC, and FEV, were analyzed before and 48 h after the removal of large pleural effusions. Results: The mean fluid removed was 1,564 +/- 695 mL. After the procedure, values of FVC, FEV and 6-min walk distance increased (P<.001), whereas dyspnea decreased (P<.001). Statistical correlations (P<.001) between 6-min walk distance and FVC (r=0.725) and between 6-min walk distance and FEV, (r=0.661) were observed. Correlations also were observed between the deltas (prethoracentesis X postthoracentesis) of the 6-min walk test and the percentage of FVC (r=0.450) and of FEV, (r=0.472) divided by the volume of fluid removed (P<.05). Conclusion: In addition to the improvement in lung function after thoracentesis, the benefits of fluid removal are more evident in situations of exertion, allowing better readaptation of patients to routine activities. BACKGROUND AND OBJECTIVE: Experimental assessment of anticancer effect, normal tissue damage, and toxicity of intrathoracic mTHPC-mediated photodynamic therapy (PDT) combined to surgery in malignant pleural mesothelioma (MPM) bearing rats. STUDY DESIGN/MATERIALS AND METHODS: Six days after implantation of syngenic malignant mesothelioma cells in the left chest cavity of Fischer rats (n = 21) and 4 days after sensitization (0.1 mg/kg mTHPC), a left-sided pneumonectomy was performed, followed by intraoperative light delivery (652 nm, fluence 20 J/cm(2)), either by spherical illumination of the chest cavity (fluence rate 15 mW/cm(2)) or by focal illumination of a tumor area (fluence rate 150 mW/cm(2)). Controls comprised tumor-bearing untreated animals, tumor-bearing animals undergoing pneumonectomy, and tumor-bearing animals undergoing pneumonectomy and light delivery without sensitization or sensitization without light delivery. No thoracocentesis was performed during follow-up. RESULTS: An invasively growing sarcomatous type of mesothelioma was found in all animals at day 10, without tumor necrosis in control animals. PDT resulted in 0.5-1 mm deep inhomogeneous tumor necrosis after spherical, and in a 1-2 mm deep tumor necrosis after focal illumination. No injury to mediastinal organs was observed, neither after PDT with spherical nor with focal light delivery except focal interstitial lung fibrosis at the mediastinal area of the opposite lung. All animals with pneumonectomy followed by spherical PDT of the entire tumor-bearing chest cavity died within 72 hours whereas all other animals survived. All animals that died presented massive pleural effusion. CONCLUSIONS: PDT following pneumonectomy in mesothelioma bearing rats was technically feasible and allowed to study its effect on tumor and normal tissues. PDT-related tumor necrosis was observed after spherical and focal light delivery, however, pneumonectomy followed by PDT with spherical light delivery to the tumor-bearing chest cavity resulted in fatal complications.

Introdução: A pneumonia é uma das infecções mais comuns em crianças, sendo o derrame pleural uma complicação potencial, principalmente nos países em desenvolvimento, onde existem sérias limitações de recursos diagnóstico-terapêuticos. Objetivos: Conhecer o perfil das crianças portadoras de derrame pleural parapneumônico, analisando sua evolução a partir da terapêutica clínica e cirúrgica que são submetidas. Método: Estudo descritivo, transversal, de coleta prospectiva, no qual foram estudadas todas as crianças internadas em um hospital de referência de doenças infecciosas na Região Norte-Brasil com diagnóstico de derrame pleural parapneumônico, submetidas a procedimento cirúrgico para abordagem do mesmo, no período de outubro de 2010 a outubro de 2011. Resultados: A amostra foi composta por 46 crianças, com distribuição igual entre os sexos, predominância de menores de 3 anos (74%) e procedentes do interior do estado (54,3%). Parcela considerável (28%) possuía algum grau de inadequação do estado nutricional. A média de tempo de doença até à admissão na referida instituição foi de 16,9 dias, e todos os indivíduos eram provenientes de outro hospital. A duração média da internação hospitalar foi de 26,0 dias, e a do estado febril, de 9,8 dias. Foram utilizados 2,2 esquemas antimicrobianos, em média, por paciente e o Ceftriaxone foi o antibiótico mais comum. Diagnóstico etiológico só foi alcançado em um único caso. Em 22 crianças, (47,8%), havia empiema pleural, e elas apresentaram maior tempo de drenagem. Foi encontrada associação entre crianças com nanismo e cirurgias múltiplas (Teste G = 8,40; p= 0,040). A grande maioria das criança foi operada uma única vez (80,4%), e a drenagem torácica fechada foi a cirurgia mais realizada. A drenagem torácica aberta foi instalada em 24,0% dos pacientes. A toracotomia com descorticação foi realizada em 2 pacientes (4,0%). Todas as crianças da amostra obtiveram recuperação clínica e radiológica em até quatro meses após a alta hospitalar, e não houve óbito na amostra. Conclusão: A população estudada possui doença em estágio avançado e distúrbios nutricionais prevalentes, que podem influenciar na evolução. É necessária padronização da antibioticoterapia, e reconsiderar a drenagem torácica aberta no empiema pleural, a partir de novos estudos sobre o tema, principalmente na indisponibilidade da videotoracoscopia.

This study aimed to evaluate the sleep quality and impact of thoracentesis on sleep in patients with a large pleural effusion. Patients with large unilateral pleural effusion were evaluated by the Pittsburgh Sleep Quality Index (PSQI) questionnaire and dyspnea Borg scale. Full polysomnography (PSG) was performed on the night before and 36 h after thoracentesis. We studied 19 patients, 11 males and 8 females, age 55 +/- 18 years and body mass index of 26 +/- 5 kg/m(2). The baseline sleep quality was poor (PSQI = 9.1 +/- 3.5). Thoracentesis removed 1.624 +/- 796 mL of pleural fluid and resulted in a significant decrease in dyspnea Borg scale (2.3 +/- 2.1 vs. 0.8 +/- 0.9, p < 0.001). The PSG before and after thoracentesis showed no significant change in apnea-hypopnea index and sleep time with oxygen saturation < 90%. There was a significant improvement in sleep efficiency (76% vs. 81%, p = 0.006), decrease percent sleep stage 1 (16% vs. 14%, p = 0.002), and a trend improvement in total sleep time (344 +/- 92 vs. 380 +/- 69 min, p = 0.056) and percentage of rapid eye movement sleep (15% vs. 20%, p = 0.053). No significant changes occurred in six patients that performed two consecutive PSG before thoracentesis. The improvement in sleep quality was not associated with the volume of pleural fluid withdrawn or changes in dyspnea. Patients with large pleural effusion have poor subjective and objective sleep quality that improves after thoracentesis.
