The aim of this study was to investigate the effect of adding whole-body vibration (WBV; frequency = 35 to 40 Hz; amplitude = 4 mm) to squat training on the T-cell proliferative response of elderly patients with osteoarthritis (OA) of the knee. This study was a randomized controlled trial in which the selected variables were assessed before and after 12 weeks of training. Twenty-six subjects (72 ± 5 years of age) were divided into three groups: 1) squat training with WBV (WBV, N = 8); 2) squat training without WBV (N = 10), and 3) a control group (N = 8). Women who were ≥60 years of age and had been diagnosed with OA in at least one knee were eligible. The intervention consisted of 12 uninterrupted weeks of squatting exercise training performed 3 times/week. Peripheral blood mononuclear cells were obtained from peripheral blood collected before and after training. The proliferation of TCD4+ and TCD8+ cells was evaluated by flow cytometry measuring the carboxyfluorescein succinimidyl ester fluorescence decay before and after the intervention (∆). The proliferative response of TCD4+ cells (P = 0.02, effect size = 1.0) showed a significant decrease (23%) in the WBV group compared to the control group, while there was no difference between groups regarding the proliferative response of TCD8+ cells (P = 0.12, effect size = 2.23). The data suggest that the addition of WBV to squat exercise training might modulate T-cell-mediated immunity, minimizing or slowing disease progression in elderly patients with OA of the knee.

The aim of the current study was to investigate the apoptosis of neurons, astrocytes and immune cells from human patients that were infected with rabies virus by vampire bats bite. Apoptotic neurons were identified by their morphology and immune cells were identified using double immunostaining. There were very few apoptotic neurons present in infected tissue samples, but there was an increase of apoptotic infiltrating CD4+ and TCD8+ adaptive immune cells in the rabies infected tissue. No apoptosis was present in NK, macrophage and astrocytes. The dissemination of the human rabies virus within an infected host may be mediated by viral escape of the virus from an infected cell and may involve an anti-apoptotic mechanism, which does not kill the neuron or pro-apoptosis of TCD4+ and TCD8+ lymphocytes and which allows for increased proliferation of the virus within the CNS by attenuation of the adaptive immune response. A cross-sectional study with internal comparison groups was conducted to describe sociodemographic characteristics, as well as verify the association between the type of antiretroviral treatment used and hyperglycemia and hyperlipidemia, with special attention to the use of HIV protease inhibitors. The data was obtained through an interview questionnaire, as well as blood and urine samples that were collected for the laboratory exams. A total of 418 patients were interviewed. 46 of these, however, met the exclusion criteria. The sample was therefore composed by 372 HIV positive patients, attended at the laboratory of the Correia Picanço State Hospital for the collection of blood, to estimate the HIV viral load and/or TCD4 cell counts from August to November 2000. The association between the variables was tested using the chi-square test and the p-value. A multiple logistic regression analysis was carried out to adjust for potential confounding factors. A greater frequency of patients with high glucose levels was observed among those making use of antiretroviral therapy without protease inhibitors, but the number of patients limited the comparisons. An association was verified between the total serum cholesterol level and the use of HIV protease inhibitors (p = 0.047) even after controlling for age. An association was also observed between the triglyceride levels and the use of HIV protease inhibitors, which remained after adjustment for age, sex and creatinine levels (p < 0.001). The levels of glucose and TSH, the presence of proteinuria and the practice of physical activity were not associated either with the levels of cholesterol or with the levels of tryglicerides thus they were not confounders of the associations described.

The reactivation of Chagas disease in HIV infected patients presents high mortality and morbidity. We present the case of a female patient with confirmed Chagasic meningoencephalitis as AIDS-defining illness. Interestingly, her TCD4+ lymphocyte cell count was 318 cells/mm3. After two months of induction therapy, one year of maintenance with benznidazol, and early introduction of highly active antiretroviral therapy (HAART), the patient had good clinical, parasitological and radiological evolution. We used a qualitative polymerase chain reaction for the monitoring of T. cruzi parasitemia during and after the treatment. We emphasize the potential value of molecular techniques along with clinical and radiological parameters in the follow-up of patients with Chagas disease and HIV infection. Early introduction of HAART, prolonged induction and maintenance of antiparasitic therapy, and its discontinuation are feasible, in the current management of reactivation of Chagas disease.

Neste trabalho, investigamos concentração da vitamina B12 e folato, considerando-se a influência dos genótipos da metilenotetrahidrofolato redutase, o perfil imunológico e a terapia antiretroviral utilizada na população brasileira portadora do HIV. Um grupo de 86 indivíduos portadores do HIV-1 e 29 doadores de sangue foram recrutados para compor a casuística. Entre os infectados pelo HIV-1, observou-se menor concentração de B12 no grupo com maior número de linfócitos TCD4+. Não encontramos diferença na distribuição genotípica para as mutações MTHFR C677T e A1298C entre infectados e não infectados pelo HIV-1. Indivíduos portadores do HIV, genótipo C677C, apresentaram concentrações menores de B12 em relação ao grupo controle de mesmo genótipo. A terapia antiretroviral não mostrou qualquer influência nos valores de folato e vitamina B12. Estudos adicionais são necessários para reavaliar a prevalência de menores concentrações de B12 e folato e de hiperhomocisteinemia na população portadora do HIV sob a ótica do uso de HAART e da melhoria na sobrevida dos pacientes.

Este trabalho objetiva a análise prospectiva das características clínicas e epidemiológicas que levam os pacientes adultos com HIV/AIDS a procurarem atendimento clínico de urgência em Pronto Atendimento do Hospital das Clínicas da Universidade Federal de Minas Gerais. Noventa e nove pacientes perfizeram 118 internações. A idade foi em média 39,4 anos. A relação homem e mulher foi de 1,35:1. O tempo desde o diagnóstico até a admissão situou-se de forma mais freqüente entre 0-5 anos em 40,4% dos casos. A terapêutica anti-retroviral era usada regularmente em 56,8% das admissões. A contagem de linfócitos T CD4+ foi inferior a 200 células/mm³ em 45,7% dos pacientes. As queixas mais freqüentes foram aumento da temperatura corpórea, diarréia, tosse e dispnéia. O aparelho respiratório foi o mais acometido. As doenças oportunistas mais freqüentes foram pneumocistose, pneumonia comunitária, síndrome diarréica, e candidiase oral. A demanda de internações de pacientes com HIV representou 2,8% das admissões, com tempo médio de permanência hospitalar de 4,6 dias. Os pacientes possuíam, em sua maioria, contagem de linfócitos TCD4+ baixa, quase metade não usava a terapia anti-retroviral altamente eficaz. Houve tendência à feminização. As doenças relacionadas à AIDS continuam sendo as mais freqüentes no nosso meio.

INTRODUCTION : In this study, clinical-laboratory and epidemiological characteristics are described for a group of 700 individuals with HIV (human immunodeficiency virus)/AIDS (acquired immunodeficiency syndrome) in the ART (antiretroviral therapy) era at a teaching hospital that provides a quaternary level of care, with an emphasis on opportunistic infections (OIs), co-infections and immune profile. METHODS : A retrospective cross-sectional study of AIDS cases was conducted from 1998 to 2008 by reviewing medical records from the Base Hospital/FUNFARME (Funda&#231;&#227;o Faculdade Regional de Medicina), S&#227;o Jos&#233; do Rio Preto, S&#227;o Paulo, Brazil. RESULTS: The individuals were 14 to 75 years of age, and 458 were males. Heterosexuals accounted for 31.1% of all patients. Eighty-three percent were on ART, and 33.8% of those presented difficulties with treatment adherence. OIs were analyzed from medical records, and Pneumocystis jiroveci pneumonia was the most prevalent, regardless of the LTCD4+ (TCD4+ Lymphocytes) levels. Individuals whose viral loads were &#8805;10,000 showed a 90% greater chance of neurotoxoplasmosis. For P. jiroveci pneumonia, neurotoxoplasmosis, esophageal candidiasis, pulmonary tuberculosis and neurocryptococcosis, the chances of infection were higher among patients with LTCD4+ levels below 200 cells/mm3. HIV/hepatitis C virus (HCV) and HIV/hepatitis B virus (HBV) co-infections were significantly associated with death. CONCLUSIONS : OIs remain frequent in the ART era even in populations where the access to medical care is considered satisfactory.

This study was designed to assess the effect of GB virus (GBV)-C on the immune response to human immunodeficiency virus (HIV) in chronically HIV-infected and HIV- hepatitis C virus (HCV)-co-infected patients undergoing antiretroviral therapy. A cohort of 159 HIV-seropositive patients, of whom 52 were HCV-co-infected, was included. Epidemiological data were collected and virological and immunological markers, including the production of interferon gamma (IFN-&#947;) and interleukin (IL)-2 by CD4, CD8 and T&#947;&#948; cells and the expression of the activation marker, CD38, were assessed. A total of 65 patients (40.8%) presented markers of GBV-C infection. The presence of GBV-C did not influence HIV and HCV replication or TCD4 and TCD8 cell counts. Immune responses, defined by IFN-&#947; and IL-2 production and CD38 expression did not differ among the groups. Our results suggest that neither GBV-C viremia nor the presence of E2 antibodies influence HIV and HCV viral replication or CD4 T cell counts in chronically infected patients. Furthermore, GBV-C did not influence cytokine production or CD38-driven immune activation among these patients. Although our results do not exclude a protective effect of GBV-C in early HIV disease, they demonstrate that this effect may not be present in chronically infected patients, who represent the majority of patients in outpatient clinics.

We have studied 65 HIV-1-infected untreated patients recruited in Caracas, Venezuela with TCD4 counts &gt; or =350/microl. The reverse transcriptase and protease sequences of the virus were sequenced, aligned with reference HIV-1 group M strains, and analyzed for drug resistance mutations. Most of the viruses were subtype B genotype in both the protease and RT genomic regions. Five of the 62 virus isolates successfully amplified showed evidence of recombination between protease and RT, with their protease region being non-B while their RT region was derived from subtype B. Four strains were found bearing resistance mutations either to NRTIs, NNRTIs, or PIs. The prevalence of HIV-1 isolates bearing resistance mutations was therefore above the 5% threshold of WHO.

The design of therapeutic cancer vaccines is aimed at inducing high numbers and potent T cells that are able to target and eradicate malignant cells. This calls for close collaboration between cells of the innate immune system, in particular dendritic cells (DCs), and cells of the adaptive immune system, notably CD4+ helper T cells and CD8+ cytotoxic T cells. Therapeutic vaccines are aided by adjuvants, which can be, for example, Toll¬like Receptor agonists or agents promoting the cytosolic delivery of antigens, among others. Vaccination with long synthetic peptides (LSPs) is a promising strategy, as the requirement for their intracellular processing will mainly target LSPs to professional antigen presenting cells (APCs), hence avoiding the immune tolerance elicited by the presentation of antigens by non-professional APCs. The unique property of antigen cross-processing and cross-presentation activity by DCs plays an important role in eliciting antitumour immunity given that antigens from engulfed dead tumour cells require this distinct biological process to be processed and presented to CD8+T cells in the context of MHC class I molecules. DCs expressing the XCR1 chemokine receptor are characterised by their superior capability of antigen cross- presentation and priming of highly cytotoxic T lymphocyte (CTL) responses. Recently, XCR1 was found to be also expressed in tissue-residents DCs in humans, with a simitar transcriptional profile to that of cross- presenting murine DCs. This shed light into the value of harnessing this subtype of XCR1+ cross-presenting DCs for therapeutic vaccination of cancer. In this study, we explored ways of adjuvanting and optimising LSP therapeutic vaccinations by the use, in Part I, of the XCLl chemokine that selectively binds to the XCR1 receptor, as a mean to target antigen to the cross-presenting XCR1+ DCs; and in Part II, by the inclusion of Q.S21 in the LSP vaccine formulation, a saponin with adjuvant activity, as well as the ability to promote cytosolic delivery of LSP antigens due to its intrinsic cell membrane insertion activity. In Part I, we designed and produced XCLl-(OVA LSP)-Fc fusion proteins, and showed that their binding to XCR1+ DCs mediate their chemoattraction. In addition, therapeutic vaccinations adjuvanted with XCLl-(OVA LSP)-Fc fusion proteins significantly enhanced the OVA-specific CD8+ T cell response, and led to complete tumour regression in the EL4-OVA model, and significant control of tumour growth in the B16.0VA tumour model. With the aim to optimise the co-delivery of LSP antigen and XCLl to skin-draining lymph nodes we also tested immunisations using nanoparticle (NP)-conjugated OVA LSP in the presence or absence of XCLl chemokine. The NP-mediated delivery of LSP potentiated the CTL response seen in the blood of vaccinated mice, and NP-OVA LSP vaccine in the presence of XCLl led to higher blood frequencies of OVA-specific memory-precursor effector cells. Nevertheless, in these settings, the addition XCLl to NP-OVA LSP vaccine formulation did not increase its antitumour therapeutic effect. In the Part II, we assessed in HLA-A2/DR1 mice the immunogenicity of the Melan-AA27L LSP or the Melan-A26. 35 AA27l short synthetic peptide (SSP) used in conjunction with the saponin adjuvant QS21, aiming to identify a potent adjuvant formulation that elicits a quantitatively and qualitatively strong immune response to tumour antigens. We showed a high CTL immune response elicited by the use of Melan-A LSP or SSP with QS21, which both exerted similar killing capacity upon in vivo transfer of target cells expressing the Melan-A peptide in the context of HLA-A2 molecules. However, the response generated by the LSP immunisation comprised higher percentages of CD8+T cells of the central memory phenotype (CD44hl CD62L+ and CCR7+ CD62L+) than those of SSP immunisation, and most importantly, the strong LSP+QS21 response was strictly CD4+T cell-dependent, as shown upon CD4 T cell depletion. Altogether, these results suggest that both XCLl and QS21 may enhance the ability of LSP to prime CD8 specific T cell responses, and promote a long-term memory response. Therefore, these observations may have important implications for the design of protein or LSP-based cancer vaccines for specific immunotherapy of cancer -- Les vacans thérapeutiques contre le cancer visent à induire une forte et durable réponse immunitaire contre des cellules cancéreuses résiduelles. Cette réponse requiert la collaboration entre le système immunitaire inné, en particulier les cellules dendrites (DCs), et le système immunitaire adaptatif, en l'occurrence les lymphocytes TCD4 hdper et CD8 cytotoxiques. La mise au point d'adjuvants et de molécules mimant un agent pathogène tels les ligands TLRs ou d'autres agents facilitant l'internalisation d'antigènes, est essentielle pour casser la tolérance du système immunitaire contre les cellules cancéreuses afin de générer une réponse effectrice et mémoire contre la tumeur. L'utilisation de longs peptides synthétiques (LSPs) est une approche prometteuse du fait que leur présentation en tant qu'antigénes requiert leur internalisation et leur transformation par les cellules dendrites (DCs, qui sont les mieux à même d'éviter la tolérance immunitaire. Récemment une sous-population de DCs exprimant le récepteur XCR1 a été décrite comme ayant une capacité supérieure dans la cross-présentation d'antigènes, d'où un intérêt à développer des vaccins ciblant les DCs exprimant le XCR1. Durant ma thèse de doctorat, j'ai exploré différentes approches pour optimiser les vaccins avec LSPs. La première partie visait à cibler les XCR1-DCs à l'aide de la chemokine XCL1 spécifique du récepteur XCR1, soit sou s la forme de protéine de fusion XCL1-OVA LSP-Fc, soit associée à des nanoparticules. La deuxième partie a consisté à tester l'association des LSPs avec I adjuvant QS21 dérivant d'une saponine dans le but d'optimiser l'internalisation cytosolique des longs peptides. Les protéines de fusion XCLl-OVA-Fc développées dans la première partie de mon travail, ont démontré leur capacité de liaison spécifique sur les XCRl-DCs associée à leur capacité de chemo-attractio. Lorsque inclues dans une mmunisation de souris porteuse de tumeurs établies, ces protéines de fusion XCL1-0VA LSP-Fc et XCLl-Fc plus OVA LSP ont induites une forte réponse CDS OVA spécifique permettant la complète régression des tumeurs de modèle EL4- 0VA et un retard de croissance significatif de tumeurs de type B16-0VA. Dans le but d'optimiser le drainage des LSPs vers es noyaux lymphatiques, nous avons également testé les LSPs fixés de manière covalente à des nanoparticules co- injectees ou non avec la chemokine XCL1. Cette formulation a également permis une forte réponse CD8 accompagnée d'un effet thérapeutique significatif, mais l'addition de la chemokine XCL1 n'a pas ajouté d'effet anti-tumeur supplémentaire. Dans la deuxième partie de ma thèse, j'ai comparé l'immunogénicité de l'antigène humain Melan A soit sous la forme d un LSP incluant un épitope CD4 et CD8 ou sous la forme d'un peptide ne contenant que l'épitope CD8 (SSP) Les peptides ont été formulés avec l'adjuvant QS21 et testés dans un modèle de souris transgéniques pour les MHC let II humains, respectivement le HLA-A2 et DR1. Les deux peptides LSP et SSP ont généré une forte réponse CD8 similaire assoc.ee a une capacité cytotoxique équivalente lors du transfert in vivo de cellules cibles présentant le peptide SSP' Cependant les souris immunisées avec le Melan A LSP présentaient un pourcentage plus élevé de CD8 ayant un Phénotype «centra, memory» (CD44h' CD62L+ and CCR7+ CD62L+) que les souris immunisées avec le SSP, même dix mois après I'immunisation. Par ailleurs, la réponse CD8 au Melan A LSP était strictement dépendante des lymphocytes CD4, contrairement à l'immunisation par le Melan A SSP qui n'était pas affectée. Dans l'ensemble ces résultats suggèrent que la chemokine XCL1 et l'adjuvant QS21 améliorent la réponse CD8 à un long peptide synthétique, favorisant ainsi le développement d'une réponse anti-tumeur mémoire durable. Ces observations pourraient être utiles au développement de nouveau vaccins thérapeutiques contre les tumeurs.

Chez les patients cancéreux, les cellules malignes sont souvent reconnues et détruites par les cellules T cytotoxiques du patient. C'est pourquoi, depuis plusieurs années, des recherches visent à produire des vaccins sensibilisant les cellules de l'immunité adaptative, afin de prévenir certains cancers. Bien que les vaccins ciblant les cellules T CD8+ (cytotoxiques) ont une efficacité in-vitro élevée, un vaccin pouvant cibler les cellules T CD8+ et CD4+ aurait une plus grande efficacité (1-3). En effet, les cellules T helper (CD4+) favorisent la production et la maintenance des cellules T CD8+ mémoires à longue durée de vie. Il existe un grand nombre de sous-types de cellules T CD4+ et leur action envers les cellules cancéreuses est différente. Par exemple, les lymphocytes Treg ont une activité pro-tumorale importante (4) et les lymphocytes Th1 ont une activité anti-tumorale (5). Cependant, le taux naturel des différents sous-types de cellules T CD4+ spécifiques aux antigènes tumoraux est variable. De plus, une certaine flexibilité des différents sous-types de cellules T CD4+ a été récemment démontrée (6). Celle-ci pourrait être ciblée par des protocoles de vaccination avec des antigènes tumoraux administrés conjointement à des adjuvants définis. Pour cela, il faut approfondir les connaissances sur le rôle des cellules T CD4+ spécifiques aux antigènes dans l'immunité anti-tumorale et connaître précisément la proportion des sous-types de cellules T CD4+ activées avant et après la vaccination. L'analyse des cellules T, par la cytométrie de flux, est très souvent limité par le besoin d'un nombre très élevé de cellules pour l'analyse de l'expression protéique. Or dans l'analyse des cellules T CD4+ spécifiques aux antigènes tumoraux cette technique n'est souvent pas applicable, car ces cellules sont présentes en très faible quantité dans le sang et dans les tissus tumoraux. C'est pourquoi, une approche basée sur l'analyse de la cellule T individuelle a été mise en place afin d'étudier l'expression du profil génétique des cellules T CD8+ et CD4+. (7,8) Méthode : Ce nouveau protocole (« single cell ») a été élaboré à partir d'une modification du protocole PCR-RT, qui permet la détection spécifique de l'ADN complémentaire (ADNc) après la transcription globale de l'ARN messager (ARNm) exprimé par une cellule T individuelle. Dans ce travail, nous optimisons cette nouvelle technique d'analyse pour les cellules T CD4+, en sélectionnant les meilleures amorces. Tout d'abord, des clones à profils fonctionnels connus sont générés par cytométrie de flux à partir de cellules T CD4+ d'un donneur sain. Pour cette étape d'optimisation des amorces, la spécificité des cellules T CD4+ n'est pas prise en considération. Il est, donc, possible d'étudier et de trier ces clones par cytométrie de flux. Ensuite, grâce au protocole « single cell », nous testons par PCR les amorces des différents facteurs spécifiques de chaque sous-type des T CD4+ sur des aliquotes issus d'une cellule provenant des clones générés. Nous sélectionnons les amorces dont la sensibilité, la spécificité ainsi que les valeurs prédictives positives et négatives des tests sont les meilleures. (9) Conclusion : Durant ce travail nous avons généré de l'ADNc de cellules T individuelles et sélectionné douze paires d'amorces pour l'identification des sous-types de cellules T CD4+ par la technique d'analyse PCR « single cell ». Les facteurs spécifiques aux cellules Th2 : IL-4, IL-5, IL-13, CRTh2, GATA3 ; les facteurs spécifiques aux cellules Th1 : TNF&#945;, IL-2 ; les facteurs spécifiques aux cellules Treg : FOXP3, IL-2RA ; les facteurs spécifiques aux cellules Th17 : RORC, CCR6 et un facteur spécifique aux cellules naïves : CCR7. Ces amorces peuvent être utilisées dans le futur en combinaison avec des cellules antigènes-spécifiques triées par marquage des multimères pMHCII. Cette méthode permettra de comprendre le rôle ainsi que l'amplitude et la diversité fonctionnelle de la réponse de la cellule T CD4+ antigène-spécifique dans les cancers et dans d'autres maladies. Cela afin d'affiner les recherches en immunothérapie oncologique. (8)

OBJETIVO: avaliar a proliferação de células T e a produção de citocinas em gestantes infectadas pelo HIV-1 e seu impacto na replicação viral in vitro. MÃTODOS: sangue periférico de 12 gestantes infectadas pelo HIV-1 e de seus neonatos, bem como de 10 gestantes HIV-1 negativas, foi colhido e a quantidade de linfócitos TCD4+ e TCD8+ periféricos foi avaliada por citometria de fluxo. Para obter plasma ou células mononucleares periféricas (PBMC), as amostras foram centrifugadas na ausência ou presença de um gradiente de Ficoll-Hypaque, respectivamente. As PBMC foram mantidas em cultura por sete dias na presença de fito-hemaglutinina mais IL-2 recombinante e a resposta linfoproliferativa de células T foi analisada pelo método de exclusão em azul de Trypan. Em alguns experimentos, as culturas foram mantidas na presença adicional de anticorpo anti-IL-10. Os plasmas e sobrenadantes das culturas de PBMC ativadas foram submetidos à análise da produção de citocinas, pelo método ELISA indireto, e a carga viral, detectada pelo RT-PCR. RESULTADOS: independente da carga viral plasmática, a resposta linfoproliferativa em culturas de células obtidas de gestantes infectadas pelo HIV foi inferior às amostras normais [4,2±0,37 vs 2,4±0,56 (x 10(6)) células/mL; p<0.005]. Tanto em gestantes normais quanto em pacientes com cargas virais baixas, os níveis de IL-10 foram mais elevados que os das gestantes com elevada replicação viral (9.790±3.224 vs 1.256±350 pg/mL; p=0.002). Níveis elevados de TNF-alfa no soro (7.200±2.440 vs 510±476 pg/mL) e nas culturas de células (21.350±15.230 vs 1.256±350 pg/mL) correlacionaram-se à carga viral plasmática elevada e a infecção neonatal. O bloqueio da IL-10 endógena com anticorpos anti-IL-10 aumentou a replicação do HIV-1 em cultura de células. CONCLUSÃO: nossos resultados sugerem que a IL-10 exerce influência negativa na replicação viral, o que provavelmente contribui para reduzir o risco de infecção vertical.

OBJETIVOS: Avaliar a prevalência de toxoplasmose, rubéola, citomegalovirose, hepatites B e C e sífilis (Torchs) em uma coorte de gestantes, bem como identificar os fatores sociodemográficos, clínicos e laboratoriais.MÃTODOS: Entre 1998 e 2013, foram atendidas 1.573 gestantes com sorologia positiva para o HIV em área metropolitana do Brasil, das quais 704 (44,8%) foram submetidas a algum dos testes sorológicos. Gestantes Torchs positivas (Gtp) foram consideradas aquelas com resultado positivo para uma dessas infecções, e gestantes Torchs negativas (Gtn) aquelas com resultados negativos para todas elas. As variáveis maternas investigadas foram: idade, estado civil, escolaridade, momento e forma de contágio da infeccção pelo HIV, contagem de linfócitos TCD4+, carga viral plasmática do HIV próxima ao parto e uso de terapia antirretroviral durante a gestação. As variáveis neonatais investigadas foram ocorrência de: transmissão vertical, prematuridade, baixo peso ao nascimento, complicações fetais, aborto e óbito fetal. Foram utilizadas razões de chance com intervalo de confiança de 95% para quantificar a associação entre as variáveis maternas e neonatais e a presença de Torchs.RESULTADOS: Entre as 704 gestantes, 70 (9,9%; IC95% 7,8-12,4) tinham alguma sorologia positiva para Torchs. Foram encontradas taxas: 1,5% (10/685) para a toxoplasmose; 1,3% (8/618) para rubéola; 1,3% (8/597) para citomegalovirose; 0,9% (6/653) para hepatite B e 3,7% (20/545) para hepatite C; e 3,8% (25/664) para sífilis. A transmissão vertical do HIV entre as gestantes Gtp foi 4,6% e de 1,2% entre as Gtn. As variáveis associadas à presença de Torchs na análise univariada foram: uso de terapia antirretroviral, transmissão vertical do HIV, baixo peso ao nascimento e complicações fetais.CONCLUSÃO: A prevalência das Torchs mostrou-se elevada para algumas infecções. Conclui-se que é importante manter o rastreamento de Torchs na gravidez, especialmente nas gestantes HIV positivas, para que se possa estabelecer diagnóstico e tratamento, e/ou medidas preventivas para evitar a transmissão materno-fetal.

Considerando a importância do uso do sangue do cordão umbilical como fonte potencial de células tronco hematopoiéticas e o uso do suíno doméstico (Sus scrofa) como modelo para pesquisas biomédicas em medicina regenerativa, e por outro lado, visando dar um contributo sobre a quantificação das subpopulações linfocitárias no sangue do cordão umbilical e periférico, objetivou-se quantificar as células CD4+, CD5+ e CD8+ nas amostras de sangue de suínos neonatos. Analisaram-se as amostras do sangue do cordão umbilical e periférico de 48 leitões de linhagem Topigs, provenientes de porcas hígidas, inseminadas artificialmente e de parto natural. Foram coletadas amostras de sangue do cordão umbilical e periférico no momento do nascimento, por meio de venopunção da veia umbilical e seio venoso retro-oftálmico, respectivamente. As quantificações imunofenotípicas de células CD4+, CD5+ e CD8+ foram obtidas por citometria de fluxo. Os valores médios obtidos para as contagens das células CD4+, CD5+ e CD8+ do sangue do cordão umbilical e periférico apresentaram-se inferiores aos reportados para o sangue periférico de suínos adultos, sugerindo um componente imunológico imaturo. A proporção CD4+:CD8+ obtida no sangue do cordão umbilical (3,2±1,2%) e no sangue periférico (3,2±1,7%) ilustrou a predominância dos linfócitos TCD4+ com relação aos TCD8+. A quantidade relativa de células CD4+ e CD8+ no sangue do cordão umbilical e periférico foi de 1,37±0,86% e 1,15±0,57%, respectivamente.

Le béryllium (Be) est un métal dont les propriétés physiques et mécaniques sont très recherchées, notamment dans les secteurs spatial, énergétique et électronique. Les principaux effets associés à lâexposition au Be sont la sensibilisation et la bérylliose chronique. La prévalence des effets associés au Be suggère que les risques sont, entre autres, fonction de sa spéciation. Par ailleurs, il semble que les particules fines constituent la fraction dâintérêt pour lâoccurrence de tels effets. Dans cette étude nous avons vérifié lâhypothèse que la forme chimique et la taille des particules du Be jouent un rôle majeur au niveau de la toxicité et de lâapparition dâeffets spécifiques à une exposition au Be. Les effets spécifiques se traduisent, entre autres, par la formation de granulomes inflammatoires pulmonaire, par la prolifération de lymphocytes TCD4+ et la production de cytokines de type Th1. Pour chacune des trois formes chimiques visées par la présente étude (le Be métallique ou Be, lâoxyde de Be ou BeO et lâalliage Be aluminium ou BeAl), la toxicité a été évaluée à la suite dâune exposition subchronique par inhalation oro-nasale à des particules fines (F) et totales (T). à cette fin, un modèle animal (souris) a été utilisé. Au total, 245 souris ont été utilisées. Elles ont été subdivisées en sept groupes de 35 souris. Un groupe a servi de contrôle, alors que chacun des six autres a été exposé soit à des particules fines soit à des particules totales, pour chacune des trois formes chimiques de Be (Be-F, Be-T, BeO-F, BeO-T, BeAl-F, BeAl-T). La durée dâexposition pour chacun des groupes sâest étendue sur 3 semaines, 5 jours par semaine, 6 heures par jour. Le niveau dâexposition des souris était de 250 µg/m3. Lâ˜urine des souris a été recueillie avant et durant lâexposition. Au moment du sacrifice, plusieurs tissus (poumon, rate, foie et reins) ainsi que des échantillons de sang ont été prélevés puis immédiatement congelés jusquâà leur analyse pour la détermination de leur teneur en Be. De plus, certains poumons et rates ont été analysés pour lâévaluation de la sensibilité immunologique et de l'inflammation pulmonaire. Cette étude dâexposition subchronique est la première étude murine qui étudie les effets toxiques de différentes tailles particulaires sur les changements pathologique et immunologique similaires à ceux observés chez lâhumain. Cette étude a permis de constater quâil existait des différences importantes au niveau de la toxicité du Be dâaprès les différentes tailles particulaires à lâétude. Ces différences seraient reliées au dépôt des particules de Be dans les voies respiratoires et également à la capacité des voies respiratoires à les éliminer totalement ou partiellement. La clairance respiratoire est fonction, notamment, du site de déposition et du caractère soluble ou non des particules. Cette recherche aura également permis de démontrer que les souris C3H/HeJ représentent un bon modèle pour lâétude des effets toxicologiques et immunologiques dâune exposition au Be. De plus, nos résultats démontrent que la sévérité des lésions pulmonaires causées par le Be, tel que lâinfiltration interstitielle de lymphocytes et la formation de granulomes non-caséeux, augmente avec le temps de résidence pulmonaire des particules de Be. Combinés à dâautres résultats, nos résultats contribueront à guider les actions de prévention relativement à lâexposition au Be, incluant éventuellement la révision de la valeur limite de lâexposition et possiblement lâétablissement de valeurs limites en fonction de la forme chimique et de la taille des particules.
