This paper analyses some aspects of the trajectory of the Argentinian physician and sociologist Juan César García (1932-1984) in the field of Latin American Social Medicine. Three dimensions constituting his basic orientations are highlighted: the elaboration of systematic and reflective social thought; a critical attitude in questioning teaching and professional practices; a commitment to the institutionalization and dissemination of health knowledge.

Social medicine is a medicine that seeks to understand the impact of socio-economic conditions on human health and diseases in order to improve the health of a society and its individuals. In this field of medicine, determining the socio-economic status of individuals is generally not sufficient to explain and/or understand the underlying mechanisms leading to social inequalities in health. Other factors must be considered such as environmental, psychosocial, behavioral and biological factors that, together, can lead to more or less permanent damages to the health of the individuals in a society. In a time where considerable progresses have been made in the field of the biomedicine, does the practice of social medicine in a primary care setting still make sense?

La médecine sociale est une médecine qui cherche à comprendre l'impact des conditions socio-économiques sur la santé humaine et les maladies, dans la perspective d'améliorer l'état de santé d'une société et de ses individus. Dans ce domaine, la détermination du statut socio-économique des individus ne suffit généralement pas à elle seule pour expliquer et comprendre les mécanismes qui sous-tendent les inégalités sociales de santé. D'autres facteurs doivent être pris en considération, tels que les facteurs environnementaux, psychosociaux, comportementaux et biologiques, facteurs qui peuvent conduire de manière synergique à des atteintes plus ou moins durables de l'état de santé des individus d'une société. A une époque où les connaissances, les compétences et les moyens à disposition en biomédecine ont fait des progrès considérables, la pratique de la médecine sociale en cabinet a-t-elle encore sa place en 2013?

This article aims to discuss the concepts of Social Determination of Health and Social Determinants of Health, by establishing a comparison between each of their guiding perspectives and investigating their implications on the development of health policies and health actions. We propose a historical and conceptual reflection, highlighting the Theory on the Social Production of Health, followed by a debate on the concepts, with a comparative approach among them.

En este documento se realiza una descripción de la producción científica colombiana en las disciplinas de medicina social y medicina básica para la base de datos Thompson ISI en el periodo comprendido entre 1975 y 2005. La caracterización de medicina social permite observar una baja producción de artículos internacionales de alto impacto propios de la disciplina, pues los artículos más citados siempre se encontraban clasificados en más de una disciplina; caso contrario al de medicina básica. De otra parte, se observa una alta concentración de los artículos de medicina social en publicaciones relacionadas con salud pública, siendo éstas una mayoría significativa dentro de la disciplina. A su vez, en las dos disciplinas es posible identificar una alta concentración de las citaciones en pocos artículos, siendo mayor en medicina social, reflejando un menor impacto promedio de las publicaciones en esta disciplina. Por último, los documentos más citados en medicina social se caracterizan por su interdisciplinariedad, colaboración internacional, interacción entre instituciones públicas y privadas, al igual que métodos no convencionales.
René Sand (1877-1953) and His Contribution to International Social Work, IASSW-President 1946 – 1953

The extraordinary significance of the life and work of René Sand lies in his central position as a mediator, promoter and coordinator of social work on an increasingly international level during the interwar-period and it can hardly be overestimated. To approach the achievements of Sand’s life and work you have to work archaeologically as he does not seem to have left any traces in the literature on social history. In Germany, even within the field of social work his name is hardly known. His biographical sketch and his importance for the development of the profession of social work have fallen into oblivion. The situation is a little different in the French-speaking countries where a biography has been published (compare Anciaux 1988a, b, c) which contains a detailed record of Sand’s writings. Altogether this lack of interest is regrettable because it doesn’t consider that René Sand is exemplary and in some parts fundamental to the emergence of professional social work in the 1920s in Belgium and Europe. Professional social work was established by a consequent international orientation and an emancipation from neighbouring fields such as social medicine and hygiene. Therefore it is a rewarding task to draw attention to this pioneer of social work and make the public appreciate his work. I want to emphasize explicitly that in this portrait Sand’s achievements concerning social work will be the main focus, even if this is an inevitable reduction of his accomplishments in the field of medicine and social medicine.

ANTECEDENTES: La Escuela de Medicina es una de las primeras de la Universidad de Cuenca en ser fundadas, no obstante el desarrollo de la Medicina Social ha sido paulatino cobrando impulso desde la década de los sesenta. Es un tema poco investigado y reportado. La enseñanza de la Medicina Social se relaciona con los cambios surgidos en la práctica de la Medicina con la ciudad y la llegada de profesores formados en el extranjero que enseñaban Higiene y Salud Pública. La Medicina Social parte desde una Medicina caritativa cuyo objetivo era el buen morir de las personas y no se centraba en su curación; para luego desarrollarse la privatización de los servicios de salud, también se produce la intervención del estado y los municipios en el área médica y social, posteriormente existe la desaparición de la Medicina Caritativa realizada por religiosas y médicos de la facultad, para surgir en una tendencia hacia un Sistema de Salud basado en el concepto de la salud como mercancía Posteriormente existe un enfoque principal en la Medicina que abarca la prevención, promoción y el desarrollo de la Atención Primaria en Salud.

Aim: To describe changes in leisure time and occupational physical activity status in an urban Mediterranean population-based cohort, and to evaluate sociodemographic, health-related and lifestyle correlates of such changes. Methods: Data for this study come from the Cornellè Health Interview Survey Follow-Up Study, a prospective cohort study of a representative sample (n¿=¿2500) of the population. Participants in the analysis reported here include 1246 subjects (567 men and 679 women) who had complete data on physical activity at the 1994 baseline survey and at the 2002 follow-up. We fitted Breslow-Cox regression models to assess the association between correlates of interest and changes in physical activity. Results: Regarding leisure time physical activity, 61.6% of cohort members with ¿sedentary¿ habits in 1994 changed their status to ¿light/moderate¿ physical activity in 2002, and 70% who had ¿light/moderate¿ habits in 1994 did not change their activity level. Regarding occupational physical activity, 74.4% of cohort members who were ¿active¿ did not change their level of activity, and 64.3% of participants with ¿sedentary¿ habits in 1994 changed to ¿active¿ occupational physical activity. No clear correlates of change in physical activity were identified in multivariate analyses. Conclusion: While changes in physical activity are evident in this population-based cohort, no clear determinants of such changes were recognised. Further longitudinal studies including other potential individual and contextual determinants are needed to better understand determinants of changes in physical activity at the population level.
