Objective: To assess the effect of reduced skin exposure in preterm infants receiving overhead phototherapy treatment on total serum bilirubin (TSB). Methods: Randomized controlled trial. Preterm infants (>1500 g birthweight and less than or equal to 36 weeks gestation) were randomized to being nursed either partially clothed with only disposable nappies and in posturally supported positions (n = 30) or naked without postural support (n = 29). Primary outcome was mean TSB percentage change at 24 h of completed conventional overhead phototherapy treatment (irradiance of 6 muWcm(-2)/nm at a wavelength of 425-475 nm). The incidence of rebound jaundice, number of infants continuing to receive phototherapy treatment at 24 h periods, parental stress, mother-infant interaction and mean TSB percentage change at 24 h of completed conventional overhead phototherapy treatment were examined. Results: Mean TSB percentage change at 24 h of completed treatment for the partially clothed group was 15.4% (+/-18) and for the naked group 19% (+/-15) (mean difference 3.6% 95% CI -5.1, 12.3). No other outcomes were significantly affected by reduced skin exposure to overhead phototherapy treatment. Conclusion: Our results show no statistically significant difference in TSB level change using either nursing practice.

OBJECTIVES: Skin notations are used as a hazard identification tool to flag chemicals associated with a potential risk related to transdermal penetration. The transparency and rigorousness of the skin notation assignment process have recently been questioned. We compared different approaches proposed as criteria for these notations as a starting point for improving and systematizing current practice. METHODS: In this study, skin notations, dermal acute lethal dose 50 in mammals (LD(50)s) and two dermal risk indices derived from previously published work were compared using the lists of Swiss maximum allowable concentrations (MACs) and threshold limit values (TLVs) from the American Conference of Governmental Industrial Hygienists (ACGIH). The indices were both based on quantitative structure-activity relationship (QSAR) estimation of transdermal fluxes. One index compared the cumulative dose received through skin given specific exposure surface and duration to that received through lungs following inhalation 8 h at the MAC or TLV. The other index estimated the blood level increase caused by adding skin exposure to the inhalation route at kinetic steady state. Dermal-to-other route ratios of LD(50) were calculated as secondary indices of dermal penetrability. RESULTS: The working data set included 364 substances. Depending on the subdataset, agreement between the Swiss and ACGIH skin notations varied between 82 and 87%. Chemicals with a skin notation were more likely to have higher dermal risk indices and lower dermal LD(50) than chemicals without a notation (probabilities between 60 and 70%). The risk indices, based on cumulative dose and kinetic steady state, respectively, appeared proportional up to a constant independent of chemical-specific properties. They agreed well with dermal LD(50)s (Spearman correlation coefficients -0.42 to -0.43). Dermal-to-other routes LD(50) ratios were moderately associated with QSAR-based transdermal fluxes (Spearman correlation coefficients -0.2 to -0.3). CONCLUSIONS: The plausible but variable relationship between current skin notations and the different approaches tested confirm the need to improve current skin notations. QSAR-based risk indices and dermal toxicity data might be successfully integrated in a systematic alternative to current skin notations for detecting chemicals associated with potential dermal risk in the workplace. 90Y-labelled radiopharmaceuticals offer promising prospects for radionuclide therapies of tumours, e.g. radioimmunotherapies (RIT), (EANM, 2007), peptide receptor radiotherapies (PRRT), (Otte et al., 1998), and selective internal radiotherapies (SIRT), (Salem and Thurston, 2006). 90Y, an almost pure high-energy beta radiation emitter (Eβ,max = 2.28 MeV), is a favourable radionuclide for therapeutic purposes. However, when preparing and performing these therapies, high activities of 90Y (>1 GBq) are to be manipulated and technicians, physicians and nurses may receive high skin exposures to the hands. If radiation protection standards are low, the exposure of staff can exceed the annual skin dose limit of 500 mSv. Within a particular work package (WP4) of the ORAMED project, comprehensive measurements in nuclear medicine departments of several hospitals in 6 European countries were carried out. The study focussed on 90Y-labelled substances such as Zevalin® and DOTATOC to achieve a representative database on staff exposure. This paper summarises the most important results and conclusions for individual monitoring of skin exposure of staff.

Ageing and skin exposure to UV radiation induces production and activation of matrix metalloproteinases (MMPs) and human neutrophil elastase (HNE). These enzymes are known to break down the extracellular matrix (ECM) which leads to wrinkle formation. Here, we demonstrated the potential of a solid-in-oil nanodispersion containing a competitive inhibitor peptide of HNE mixed with hyaluronic acid (HA), displaying 158 nm of mean diameter, to protect the skin against the ageing effects. Western blot analysis demonstrated that activation of MMP-1 in fibroblasts by HNE treatment is inhibited by the solid-in-oil nanodispersion containing the peptide and HA. The results clearly demonstrate that solid-in-oil nanodispersion containing the HNE inhibitor peptide is a promising strategy for anti-ageing effects. This effect can be seen particularly by ECM regulation by affecting fibroblasts. The formulation also enhances the formation of thicker bundles of actin filaments.

Professional cleaning is a basic service occupation with a wide variety of tasks carried out in all kind of different sectors and workplaces by a large workforce. One important risk for cleaning workers is the exposure to chemical substances that are present in cleaning products.Monoethanolamine was found to be often present in cleaning products such as general purpose cleaners, bathroom cleaners, floor cleaners and kitchen cleaners. Monoethanolamine can injure the skin, and exposure to monoethanolamine was associated to asthma even when the air concentrations were low. It is a strong irritant and known to be involved in sensitizing mechanisms. It is very likely that the use of cleaning products containing monoethanolamine gives rise to respiratory and dermal exposures. Therefore there is a need to further investigate the exposures to monoethanolamine for both, respiratory and dermal exposure.The determination of monoethanolamine has traditionally been difficult and analytical methods available are little adapted for occupational exposure assessments. For monoethanolamine air concentrations, a sampling and analytical method was already available and could be used. However, a method to analyses samples for skin exposure assessments as well as samples of skin permeation experiments was missing. Therefore one main objective of this master thesis was to search an already developed and described analytical method for the measurement of monoethanolamine in water solutions, and to set it up in the laboratory. Monoethanolamine was analyzed after a derivatisation reaction with o-pthtaldialdehyde. The derivated fluorescing monoethanolamine was then separated with high performance liquid chromatography and detection took place with a fluorescent detector. The method was found to be suitable for qualitative and quantitative analysis of monoethanolamine. An exposure assessment was conducted in the cleaning sector to measure the respiratory and dermal exposures to monoethanolamine during floor cleaning. Stationary air samples (n=36) were collected in 8 companies and samples for dermal exposures (n=12) were collected in two companies. Air concentrations (Mean = 0.18 mg/m3, Standard Deviation = 0.23 mg/m3, geometric Mean = 0.09 mg/m3, Geometric Standard Deviation = 3.50) detected were mostly below 1/10 of the Swiss 8h time weighted average occupational exposure limit. Factors that influenced the measured monoethanolamine air concentrations were room size, ventilation system and the concentration of monoethanolamine in the cleaning product and amount of monoethanolamine used. Measured skin exposures ranged from 0.6 to 128.4 mg/sample. Some cleaning workers that participated in the skin exposure assessment did not use gloves and had direct contact with the solutions containing the cleaning product and monoethanolamine. During the entire sampling campaign, cleaning workers mostly did not use gloves. Cleaning workers are at risk to be regularly exposed to low air concentrations of monoethanolamine. This exposure may be problematic if a worker suffers from allergic reactions (e.g. Asthma). In that case a substitution of the cleaning product may be a good prevention measure as several different cleaning products are available for similar cleaning tasks. Currently there are no occupational exposure limits to compare the skin exposures that were found. To prevent skin exposures, adaptations of the cleaning techniques and the use of gloves should be considered. The simultaneous skin and airborne exposures might accelerate adverse health effects. Overall the risks caused by exposures to monoethanolamine are considered as low to moderate when the cleaning products are used correctly. Whenever possible, skin exposures should be avoided. Further research should consider especially the dermal exposure routes, as very high exposures might occur by skin contact with cleaning products. Dermatitis but also sensitization might be caused by skin exposures. In addition, new biomedical insights are needed to better understand the risks of the dermal exposure. Therefore skin permeability experiments should be considered.

La substitution est une méthode de prévention primaire qui permet l’élimination à la source des dangers pour les travailleurs. Une des étapes de la démarche est la comparaison des options afin de procéder au choix final. Divers indices de comparaison, basés sur des paramètres physicochimiques, sanitaires et environnementaux des substances, permettent de guider ce choix. Toutefois, aucune évaluation de ces indices n’a été effectuée dans le cas de la substitution des solvants. Une recherche de développement a été entreprise afin de proposer une méthodologie améliorée de comparaison des solvants. La démarche d’analyse de substitution et la comparaison des options de remplacement à l’aide du rapport de danger de vapeur (« Vapour Hazard Ratio », VHR) ont été appliquées à un cas réel de substitution de solvants en entreprise. Trois indices de potentiel de surexposition (IPS) (VHR, « Måleteknisk Arbejdshygiejnisk Luftbehov » (MAL) et « SUBstitution FACtor » (SUBFAC)) et trois indices globaux de hiérarchisation des dangers (indice air (ψiair), « Indiana Relative Chemical Hazard Score » (IRCHS) et « Final Hazard Score » (FHS)) ont été évalués et comparés à partir de listes de 56 et 67 solvants respectivement. La problématique de la non-idéalité des mélanges a aussi été considérée par rapport aux IPS par l’évaluation et la comparaison de 50 mélanges de solvant. Une méthodologie d’établissement d’une valeur limite d’exposition (VLE), pour les solvants n’en possédant pas, a été développée par modélisation de type relations quantitatives propriété-propriété (QPPR). La modélisation QPPR des VLE, effectuée sur une liste de 88 solvants possédant une VLE, a été effectuée à partir des coefficients de partage octanol:air, octanol:eau, sang:air et des constantes métaboliques. L’étude de cas a montré que l’utilisation du VHR facilitait la comparaison des options, bien qu’elle puisse se heurter à l’absence de VLE. Les indices VHR et SUBFAC ont été identifiés comme des méthodes très proches, caractérisées par une forte corrélation (R=0,99) alors que l’indice MAL se distingue des deux autres IPS par une perte d’information sur la volatilité résultant en une corrélation plus faible avec le VHR (R=0,75). L’impact de la non idealité, évalué par le calcul de coefficients d’activité sur une série de 50 mélanges, a permis d’établir que les ratios entre les indices VHR corrigés et non corrigés variaient entre 0,57 et 2,7, suggérant un facteur de sécurité de cinq lors de la comparaison de mélanges. Les analyses de corrélation et de sensibilité ont montré que les indices de hiérarchisation des dangers différaient de façon importante sur leur prise en compte de paramètres comme la volatilité, les VLE, l’exposition cutanée, l’inflammabilité, la cancérogénicité et les divers paramètres environnementaux. Aucun de ces indices ne peut être recommandé pour la substitution des solvants. Deux modèles QPPR ont été développés afin de prédire des VLE et des VHR, et 61 % et 87 % des VHR prédits variaient respectivement d’un facteur maximal de deux et de cinq par rapport aux VHR calculés. Nos résultats mènent à proposer une démarche améliorée de comparaison en deux étapes. Après un tri selon des critères prioritaires de santé, de sécurité et d’environnement, la comparaison devrait se baser sur le calcul du VHR tout en considérant d’autres paramètres selon la situation concrète de l’entreprise ou du procédé. La comparaison devra tenir compte de la non-idéalité pour les mélanges, et de VLE estimées pour les solvants n’en possédant pas.

Techniques for the coherent generation and detection of electromagnetic radiation in the far infrared, or terahertz, region of the electromagnetic spectrum have recently developed rapidly and may soon be applied for in vivo medical imaging. Both continuous wave and pulsed imaging systems are under development, with terahertz pulsed imaging being the more common method. Typically a pump and probe technique is used, with picosecond pulses of terahertz radiation generated from femtosecond infrared laser pulses, using an antenna or nonlinear crystal. After interaction with the subject either by transmission or reflection, coherent detection is achieved when the terahertz beam is combined with the probe laser beam. Raster scanning of the subject leads to an image data set comprising a time series representing the pulse at each pixel. A set of parametric images may be calculated, mapping the values of various parameters calculated from the shape of the pulses. A safety analysis has been performed, based on current guidelines for skin exposure to radiation of wavelengths 2.6 µm–20 mm (15 GHz–115 THz), to determine the maximum permissible exposure (MPE) for such a terahertz imaging system. The international guidelines for this range of wavelengths are drawn from two U.S. standards documents. The method for this analysis was taken from the American National Standard for the Safe Use of Lasers (ANSI Z136.1), and to ensure a conservative analysis, parameters were drawn from both this standard and from the IEEE Standard for Safety Levels with Respect to Human Exposure to Radio Frequency Electromagnetic Fields (C95.1). The calculated maximum permissible average beam power was 3 mW, indicating that typical terahertz imaging systems are safe according to the current guidelines. Further developments may however result in systems that will exceed the calculated limit. Furthermore, the published MPEs for pulsed exposures are based on measurements at shorter wavelengths and with pulses of longer duration than those used in terahertz pulsed imaging systems, so the results should be treated with caution.

Objetivou-se com este trabalho avaliar a eficiência da cabina do trator, de dois tipos de ponta de pulverização e de duas posições da barra do pulverizador montado em trator em aplicações de herbicidas na cultura de cana-de-açúcar, como medidas de proteção coletiva para a atividade de tratorista, separadamente ou combinadas; e classificar a segurança dessas condições de trabalho com as 46 recomendações de herbicidas registradas. As exposições dérmica e respiratória do tratorista foram quantificadas em aplicações com o pulverizador equipado com barra traseira ou central, associadas com pontas com indução de ar, modelo Turbo TeeJet Air Induction® (TTI-11004VP), e sem indução de ar, modelo Turbo Floodjet® (TF-VP3), e o trator sem e com cabina. Foram calculadas as margens de segurança (MS) para 46 recomendações de aplicação de herbicidas nessas condições de trabalho. Pelos valores de MS calculados, as condições de trabalho foram classificadas como seguras (MS > 1) ou inseguras (MS < 1). A condição de trabalho mais segura para o tratorista é a associação de pulverizador de barra central, trator com cabina e pontas TTI. Nas aplicações com o pulverizador de barra central sem a cabina, das 46 recomendações de herbicidas, são seguras para o tratorista as de imazapyr, trifloxysulfuron-sodium, imazapic, glyphosate, amicarbazone, hexazinone, sulfentrazone, clomazone, oxadiazon, isoxaflutole, pendimethalin, flazasulfuron, tebuthiuron, ethoxysulfuron e acetoclor. Com o uso da cabina, também são seguras as de metribuzin e s-metalochlor. Nas aplicações com o pulverizador de barra traseira sem a cabina, das 46 recomendações de herbicidas, são seguras as de imazapyr, trifloxysulfuron-sodium, imazapic, glyphosate, amicarbazone, hexazinone, sulfentrazone, clomazone, oxadiazon, isoxaflutole, pendimethalin, flazasulfuron e tebuthiuron. Com a associação da cabina, também são seguras para o tratorista a aplicação do ethoxysulfuron com as pontas TF e a do metribuzin e s-metalochlor com as pontas TTI.

The purpose of this study envisaged the quantification of skin and respiratory exposures occasioned by work conditions during pesticide spraying of citrus fruits using hand gun sprayers; the evaluation of the efficiency of individual and group protection measures for the workers; the determination of workers' most exposed body regions; and the classification of work conditions, with and without the tested work-safety protection measures as recommended for the registered pesticides used to control the main pests and diseases that attack these types of trees and fruits. The AZR protection equipment proved to be the most efficient for the tractor driver, when spraying using pistol sprayers. The two sets of individual protection equipment that were checked also proved to be efficient. The most exposed regions of the tractor driver's body were the thighs, the front of the legs, the feet and hands. The most exposed regions of the individual sprayer working on foot were the hands and feet.

Expression of metallothionein, an antioxidant induced by a variety of stimuli including ultraviolet light, was quantitated by immunohistochemistry in the skin of males aged over 50 who had known short- and long-term exposures to sunlight. Skin punch biopsies were taken from two sites in each subject: the hand in all subjects and a range of other sites matched to patients with a previously excised primary melanoma. Metallothionein expression (strongest in the basal layers of the epidermis and primarily nuclear) was associated with both short- and long-term exposure to sunlight. A plateau of staining intensity was reached after 3 h sun exposure, within the previous 3 d before biopsy. Expression was also elevated in the nonexposed skin sites of subjects who had recent sun exposure, indicating a systemic response to exposure of remote sites. Using the skin of the hand to normalize responses to chronic exposure between individuals, the systemically modulated response to sunlight was significantly greater on the unexposed back than on other sites. The possibility of ultraviolet-induced cytokines selectively modifying the response of skin on a site-specific basis was investigated. The circulating leukocytes, but not lymphocytes, of two individuals exposed to 1 minimal erythema dose whole-body solar-simulated ultraviolet showed increased interleukin-6 mRNA 4 h after exposure. Interleukin-6 was not directly induced in these cell populations 4 h after ultraviolet A or ultraviolet B irradiation ex vivo . Leukocytes may therefore contribute to and amplify the systemic effects of ultraviolet-induced interleukin-6 and metallothionein expression.
