BACKGROUND: Increasing levels of physical inactivity and sedentariness are contributing to the current overweight and obesity epidemic. In this paper, the findings of two recent studies are used to explore the relationships between sitting time ( in transport, work and leisure), physical activity and body mass index (BMI) in two contrasting samples of adult Australians. METHODS: Data on sitting time, physical activity, BMI and a number of demographic characteristics were compared for participants in two studies-529 women who were participants in a preschool health promotion project ('mothers'), and 185 men and women who were involved in a workplace pedometer study ('workers'). Relationships between age, number of children, physical activity, sitting time, BMI, gender and work patterns were explored. Logistic regression was used to predict the likelihood of being overweight or obese, among participants with different physical activity, sitting time and work patterns. Background: There is growing evidence suggesting that prolonged sitting has negative effects on people's weight, chronic diseases and mortality. Interventions to reduce sedentary time can be an effective strategy to increase daily energy expenditure. The purpose of this study is to evaluate the effectiveness of a six-month primary care intervention to reduce daily of sitting time in overweight and mild obese sedentary patients. Method/Design: The study is a randomized controlled trial (RCT). Professionals from thirteen primary health care centers (PHC) will randomly invite to participate mild obese or overweight patients of both gender, aged between 25 and 65 years old, who spend 6 hours at least daily sitting. A total of 232 subjects will be randomly allocated to an intervention (IG) and control group (CG) (116 individuals each group). In addition, 50 subjects with fibromyalgia will be included. Primary outcome is: (1) sitting time using the activPAL device and the Marshall questionnaire. The following parameters will be also assessed: (2) sitting time in work place (Occupational Sitting and Physical Activity Questionnaire), (3) health-related quality of life (EQ-5D), (4) evolution of stage of change (Prochaska and DiClemente's Stages of Change Model), (5) physical inactivity (catalan version of Brief Physical Activity Assessment Tool), (6) number of steps walked (pedometer and activPAL), (7) control based on analysis (triglycerides, total cholesterol, HDL, LDL, glycemia and, glycated haemoglobin in diabetic patients) and (8) blood pressure and anthropometric variables. All parameters will be assessed pre and post intervention and there will be a follow up three, six and twelve months after the intervention. A descriptive analysis of all variables and a multivariate analysis to assess differences among groups will be undertaken. Multivariate analysis will be carried out to assess time changes of dependent variables. All the analysis will be done under the intention to treat principle. Discussion: If the SEDESTACTIV intervention shows its effectiveness in reducing sitting time, health professionals would have a low-cost intervention tool for sedentary overweight and obese patients management.

Abstract Background: Little is known about how sitting time, alone or in combination with markers of physical activity (PA), influences mental well-being and work productivity. Given the need to develop workplace PA interventions that target employees’ health related efficiency outcomes; this study examined the associations between self-reported sitting time, PA, mental well-being and work productivity in office employees. Methods: Descriptive cross-sectional study. Spanish university office employees (n = 557) completed a survey measuring socio-demographics, total and domain specific (work and travel) self-reported sitting time, PA (International Physical Activity Questionnaire short version), mental well-being (Warwick-Edinburg Mental Well-Being Scale) and work productivity (Work Limitations Questionnaire). Multivariate linear regression analyses determined associations between the main variables adjusted for gender, age, body mass index and occupation. PA levels (low, moderate and high) were introduced into the model to examine interactive associations. Results: Higher volumes of PA were related to higher mental well-being, work productivity and spending less time sitting at work, throughout the working day and travelling during the week, including the weekends (p < 0.05). Greater levels of sitting during weekends was associated with lower mental well-being (p < 0.05). Similarly, more sitting while travelling at weekends was linked to lower work productivity (p < 0.05). In highly active employees, higher sitting times on work days and occupational sitting were associated with decreased mental well-being (p < 0.05). Higher sitting times while travelling on weekend days was also linked to lower work productivity in the highly active (p < 0.05). No significant associations were observed in low active employees. Conclusions: Employees’ PA levels exerts different influences on the associations between sitting time, mental well-being and work productivity. The specific associations and the broad sweep of evidence in the current study suggest that workplace PA strategies to improve the mental well-being and productivity of all employees should focus on reducing sitting time alongside efforts to increase PA.

Background: One of the major immediate and long-term health issues in modern society is the problem of overweight and obesity. This paper examines the role of the workplace in the problem by studying the association between occupational sitting time and overweight and obesity (body mass index [BMI] >= 25) in a sample of adult Australians in full-time employment. Methods: Data on age, gender, occupation, physical activity, occupational sitting time, and BMI were collected in September 2003 from a sample of 1579 adult men and women in full-time employment at the time of the survey. Logistic regression was used to examine the association between occupational sitting time and overweight and obesity. Results: Mean occupational sitting time was > 3 hours/day, and significantly higher in men (209 minutes) than in women (189 minutes, p =0.026). Univariate analyses showed significant associations between occupational sitting time and BMI of >= 25 in men but not in women. After adjusting for age, occupation, and physical activity, the odds ratio for BMI >= 25 was 1.92 (confidence interval: 1.17-3.17) in men who reported sitting for > 6 hours/day, compared with those who sat for < 45 minutes/day. Conclusions: Occupational sitting time was independently associated with overweight and obesity in men who were in full-time paid work. These results suggest that the workplace may play an important role in the growing problem of overweight and obesity. Further research is needed to clearly understand the association between sitting time at work and over-weight and obesity in women.

This study aimed to assess sitting time and number of steps taken each day, and the relationships between these variables, in a sample of working Australian adults. Workers (N = 185), wore a pedometer for 7 days and recorded the number of steps taken and time spent sitting each day. Average time spent sitting on weekdays was 9.4 (SD = 2.40) hr with about half spent sitting at work. Despite this, the average steps taken each day (M = 8,873, SD = 2,757) was higher on weekdays than on weekend days. There was a clear inverse relationship between sitting time at work and number of steps taken on weekdays, r = -.34, p < .001); those in the highest tertile for sitting time reported about 3,000 fewer daily steps. Workers in managerial and professional occupations reported more time sitting at work (M = 6.2 hr per day) and lower weekday step counts (M = 7,883, N = 43) than technical (M = 3.3 hr sitting at work and 10,731 weekday steps, N = 33) and blue collar workers (M = 1.6 hours sitting and 11,784 steps N = 11). The findings suggest those whose daily work involves long hours of sitting should be the focus of efforts to promote physical activity both within and outside the workplace.

OBJECTIVE: To investigate the prevalence and predictors of weight maintenance over time in a large sample of young Australian women. DESIGN: This population study examined baseline and 4y follow-up data from the cohort of young women participating in the Australian Longitudinal Study on Women's Health. SUBJECTS: A total of 8726 young women aged 18 - 23y at baseline. MEASURES: Height, weight and body mass index (BMI); physical activity; time spent sitting; selected eating behaviours (eg dieting, disordered eating, takeaway food consumption); cigarette smoking, alcohol consumption; parity; and socio-demographic characteristics. RESULTS: Only 44% of the women reported their BMI at follow-up to be within 5% of their baseline BMI (maintainers); 41% had gained weight and 15% had lost weight. Weight maintainers were more likely to be in managerial or professional occupations; to have never married; to be currently studying; and not to be mothers. Controlling for sociodemographic factors, weight maintainers were more likely to be in a healthy weight range at baseline, and to report that they spent less time sitting, and consumed less takeaway food, than women who gained weight. CONCLUSIONS: Fewer than half the young women in this community sample maintained their weight over this 4y period in their early twenties. Findings of widespread weight gain, particularly among those already overweight, suggest that early adulthood, which is a time of significant life changes for many women, may be an important time for implementing strategies to promote maintenance of healthy weight. Strategies which encourage decreased sitting time and less takeaway food consumption may be effective for encouraging weight maintenance at this life stage.

Background Little is known about the types of ‘sit less, move more’ strategies that appeal to office employees, or what factors influence their use. This study assessed the uptake of strategies in Spanish university office employees engaged in an intervention, and those factors that enabled or limited strategy uptake. Methods The study used a mixed method design. Semi-structured interviews were conducted with academics and administrators (n = 12; 44 ± 12 mean SD age; 6 women) at three points across the five-month intervention, and data used to identify factors that influenced the uptake of strategies. Employees who finished the intervention then completed a survey rating (n = 88; 42 ± 8 mean SD age; 51 women) the extent to which strategies were used [never (1) to usually (4)]; additional survey items (generated from interviewee data) rated the impact of factors that enabled or limited strategy uptake [no influence (1) to very strong influence (4)]. Survey score distributions and averages were calculated and findings triangulated with interview data. Results Relative to baseline, 67% of the sample increased step counts post intervention (n = 59); 60% decreased occupational sitting (n = 53). ‘Active work tasks’ and ‘increases in walking intensity’ were the strategies most frequently used by employees (89% and 94% sometimes or usually utilised these strategies); ‘walk-talk meetings’ and ‘lunchtime walking groups’ were the least used (80% and 96% hardly ever or never utilised these strategies). ‘Sitting time and step count logging’ was the most important enabler of behaviour change (mean survey score of 3.1 ± 0.8); interviewees highlighted the motivational value of being able to view logged data through visual graphics in a dedicated website, and gain feedback on progress against set goals. ‘Screen based work’ (mean survey score of 3.2 ± 0.8) was the most significant barrier limiting the uptake of strategies. Inherent time pressures and cultural norms that dictated sedentary work practices limited the adoption of ‘walk-talk meetings’ and ‘lunch time walking groups’. Conclusions The findings provide practical insights into which strategies and influences practitioners need to target to maximise the impact of ‘sit less, move more’ occupational intervention strategies.

Purpose Encouraging office workers to ‘sit less and move more’ encompasses two public health priorities. However, there is little evidence on the effectiveness of workplace interventions for reducing sitting, even less about the longer term effects of such interventions and still less on dual-focused interventions. This study assessed the short and mid-term impacts of a workplace web-based intervention (Walk@WorkSpain, W@WS; 2010-11) on self-reported sitting time, step counts and physical risk factors (waist circumference, BMI, blood pressure) for chronic disease. Methods Employees at six Spanish university campuses (n=264; 42±10 years; 171 female) were randomly assigned by worksite and campus to an Intervention (used W@WS; n=129; 87 female) or a Comparison group (maintained normal behavior; n=135; 84 female). This phased, 19-week program aimed to decrease occupational sitting time through increased incidental movement and short walks. A linear mixed model assessed changes in outcome measures between the baseline, ramping (8 weeks), maintenance (11 weeks) and followup (two months) phases for Intervention versus Comparison groups.A significant 2 (group) × 2 (program phases) interaction was found for self-reported occupational sitting (F[3]=7.97, p=0.046), daily step counts (F[3]=15.68, p=0.0013) and waist circumference (F[3]=11.67, p=0.0086). The Intervention group decreased minutes of daily occupational sitting while also increasing step counts from baseline (446±126; 8,862±2,475) through ramping (+425±120; 9,345±2,435), maintenance (+422±123; 9,638±3,131) and follow-up (+414±129; 9,786±3,205). In the Comparison group, compared to baseline (404±106), sitting time remained unchanged through ramping and maintenance, but decreased at follow-up (-388±120), while step counts diminished across all phases. The Intervention group significantly reduced waist circumference by 2.1cms from baseline to follow-up while the Comparison group reduced waist circumference by 1.3cms over the same period. Conclusions W@WSis a feasible and effective evidence-based intervention that can be successfully deployed with sedentary employees to elicit sustained changes on “sitting less and moving more”.

La mesure de l’activité physique des personnes âgées dans des études populationnelles est un défi. Peu des outils de mesure trouvés dans la littérature sont jugés suffisamment valides ou fiables pour être utilisés dans une étude internationale et longitudinale sur les déterminants de perte de mobilité de personnes âgées. Une étude pilote a été conduite en 2009 pour déterminer la meilleure forme d’évaluation. Les objectifs de ce mémoire sont l’étude des validité, fiabilité et capacité prédictive des données d’un accéléromètre (gold standard), de deux questionnaires choisis (PAQ, IPAQ 7d-recall). Le but est de combiner des éléments des questionnaires dans un seul pour une étude longitudinale projetée au Canada, au Brésil et en Colombie. Le PAQ évalue la fréquence et la durée d’activités de loisirs, l’IPAQ 7d-recall évalue la durée et l’intensité perçue des activités et le temps assis. La collecte de données s’est faite auprès de participants de Saint-Bruno (n=64) et de Santa Cruz (n=60), de 65 à 74 ans. Le PAQ, l’IPAQ-7d-recall et un test de performance (SPPB) ont été complétés puis des accéléromètres remis. À Saint-Bruno, la validité de critère pour l’IPAQ et le PAQ, et la validité de construit pour l’IPAQ sont bonnes. Le refus de l’accéléromètre à Santa Cruz a empêché les analyses de validité. Le PAQ présente une bonne fiabilité mais ajoute peu d’amélioration au pouvoir de prédiction de la dépense énergétique de l’IPAQ. La recherche populationnelle, voulant estimer la dépense énergétique par l’activité physique des personnes âgées, devrait utiliser des instruments mesurant l’intensité perçue (IPAQ) plutôt que les fréquence et durée d’activités concrètes (PAQ).

Introdução: A doença cardiovascular é uma das principais causas de incapacidade e diminuição da qualidade de vida. O grande investimento na atuação preventiva ou de reabilitação impõe um apelo especial à conjugação de esforços por parte de todos os interlocutores. Neste contexto, o objetivo do presente estudo centrou-se em avaliar o impacto de um Programa de Reabilitação na qualidade de vida e outros indicadores de saúde em indivíduos que possuam doença cardíaca, analisando a influência das variáveis sociodemográficas, antropométricas, clínicas, de qualidade de vida e de atividade física. Método: Recorrendo a um estudo de natureza quantitativa, do tipo prospetivo com características pré-experimentais, inquirimos 48 indivíduos portadores de patologia cardíaca, na sua maioria do género masculino (75%), com idades compreendidas entre os 26 e 87 anos (M= 57.90; Dp= 12.23), casados (81.2%), reformados (45,8%), com fatores de risco cardiovascular (87.5%), que se encontram com algum grau de limitação física para atividades quotidianas. O protocolo de pesquisa inclui, além de uma ficha sociodemográfica e clínica, instrumentos de medida aferidos e validados para a população portuguesa (Qualidade de Vida e Índice de Atividade Física), os quais foram aplicados antes e após a Fase II do Programa de Reabilitação Cardíaca, Resultados: Após implementação do Programa de Reabilitação Cardíaca, os resultados evidenciam uma melhoria estatisticamente significativa nos dados antropométricos (peso, IMC e PA), nas características analíticas (CT, LDL, TG, HDL e glicemia), nos dados hemodinâmicos (PAS, PAD, FE%), na prova de esforço (METs e %FC) e ainda na qualidade de vida (nos seus domínios emocional, físico, social e global) e no índice de atividade física (vigorosa, moderada, caminhada, METs e tempo sentado). Conclusão: A evidência dos resultados obtidos dá corpo à importância duma abordagem multidisciplinar nos programas de reabilitação cardíaca, realçando a necessidade de aumentar a taxa de referenciação para os centros existentes e a necessidade de criar novos centros, de forma a se poderem proporcionar cuidados considerados essenciais na recuperação pós-evento agudo e na prevenção da doença cardiovascular e cardíaca em geral. Objective: The aims of this study were to estimate average yearly weight gain in midage women and to identify the determinants of weight gain and gaining weight at double the average rate. Research Methods and Procedures: The study sample comprised 8071 participants (45 to 55 years old) in the Australian Longitudinal Study on Women's Health who completed mailed surveys in 1996, 1998, and 2001. Results: On average, the women gained almost 0.5 kg per year [average 2.42 kg (95% confidence interval, 2.29 to 2.54) over 5 years]. In multivariate analyses, variables associated with energy balance (physical activity, sitting time, and energy intake), as well as quitting smoking, menopause/hysterectomy, and baseline BMI category were significantly associated with weight gain, but other behavioral and demographic characteristics were not. After adjustment for all of the other biological and behavioral variables, the odds of gaining weight at about twice the average rate (> 5 kg over 5 years) were highest for women who quit smoking (odds ratio = 2.94; 95% confidence interval, 2.17, 3.96). There were also independent relationships between the odds of gaining > 5 kg and lower levels of habitual physical activity, more time spent sitting, energy intake (but only in women with BMI > 25 at baseline), menopause transition, and hysterectomy. Discussion: The average weight gain equates with an energy imbalance of only about 10 kcal or 40 kJ per day, which suggests that small sustained changes in the modifiable behavioral variables could prevent further weight gain.
