The correction of bone defects can be performed using autogenous or alloplastic materials, such as beta-tricalcium phosphate (β-TCP). This study compared the changes in bone volume (CBV) after maxillary sinus lifting using autogenous bone (n=12), autogenous bone associated with β-TCP 1:1 (ChronOS; DePuy Synthes, Paoli, CA, USA) (n=9), and β-TCP alone (n=11) as grafting material, by means of cone beam computed tomography (CBCT). CBV was evaluated by comparing CBCT scans obtained in the immediate postoperative period (5-7 days) and at 6 months postoperative in each group using OsiriX software (OsiriX Foundation, Geneva, Switzerland). The results showed an average resorption of 45.7±18.6% for the autogenous bone group, 43.8±18.4% for the autogenous bone+β-TCP group, and 38.3±16.6% for the β-TCP group. All bone substitute materials tested in this study presented satisfactory results for maxillary sinus lifting procedures regarding the maintenance of graft volume during the healing phase before the insertion of implants, as assessed by means of CBCT.

AimTo describe the early healing within the void obtained after the elevation of the sinus mucosa and simultaneous implant installation without the use of any grafting material in monkeys.Material and methodsImplants were installed simultaneously with the elevation of the maxillary sinus using the lateral approach in eight monkeys without the use of grafting material. The healing after 4, 10, 20 and 30 days was evaluated in the area distal to the implants. Paraffin sections were prepared and analyzed using qualitative histological methods.ResultsThe healing process was initiated by the formation of a coagulum and followed by a provisional matrix and woven bone. Subsequently, a parallel-fiber bone replaced woven bone. The dimension of the elevated area shrank during the healing process. Sprouts of woven bone, present to a moderate extent after 4 days, were more numerous after 10 and 20 days. Newly formed bone originated from the sinus walls and septa, while there was no evidence of participation of the Schneiderian membrane in this process. After 30 days, the window access appeared to be closed by a layer of newly formed trabecular bone.ConclusionsThe coagulum that filled the void distal to the implant after simultaneous elevation of the sinus floor gave rise to newly formed bone. However, the void occupied by the coagulum shrank substantially. This work aimed to analyze clinically and histologically the allogen bone graft behavior at 6 and 9 months. A leukoderm, female, 55 years old patient sought dental care for oral rehabilitation with dental implants and implant supported fixed prosthesis in the maxilla. Bilateral sinus lifting procedure were performed in a same patient, the analysis were made after 6 and 9 months, respectively. At 6 months, there was lack of a better bone remodeling in the region, associated to the presence of fibrous connective tissue within the collected tissue, showing us that this is not the best stage of tissue maturation to place dental implants. The 9 months period showed an improvement, with a more organized bone tissue surrounding particles of homogenous bone, what possibly had increased implant stability at the time of surgery. There is no doubt about the increase of clinical applications of FFBA, but there are no studies available regarding the standardization of time to load implants inserted in allografts. So papers with long-term monitoring are necessary to eliminate questions that still remain to be answered.

The implant has high success rates and predictability in this decade. However, bone availability is a factor that may limit the installation technique of dental implants in subsequent rehabilitation of the maxillary arch has been proposed technique’s maxillary sinus lifting when it presents itself pneumatized, for installation of dental implants The aim of this paper is to present two clinical cases using the surgical technique and rehabilitation to the patient with surgery maxillary sinus lifting with the use of a biomaterial alone or in combination with autogenous bone and the prosthetic resolution radiographic and clinical follow-up of three years.

Sinus floor augmentation has been established as a predictable technique to overcome maxillary bone loss. Antral cystic lesions may lead to intrasurgical complications and should be accurately diagnosed. However, antral pseudocysts have recently been described not to be contraindicated for sinus-grafting procedures. The current article sought to report clinical, cone beam computed tomographic, and histologic results of a maxillary sinus floor augmentation, performed with piezoelectric surgery, in the presence of a large antral pseudocyst. Success of graft maturation was confirmed with histologic analysis, which also indicated the absence of inflammatory infiltration in the tissue evaluated. On the basis of our findings, it is possible to perform a predictable treatment based on sinus floor augmentation in the presence of antral pseudocysts. Graft maturation can also be achieved 6 months after sinus-lifting surgeries.

设计了一种能够使蛇形机器人运动更灵巧、奇异点更少和运动能力更强的机构 ,对具有三个自由度的新型蛇形机器人单元进行了改进 ,在单元上增加被动轮机构 ,使其具有万向机构的特点。该单元不仅能够用被动轮驱动机器人运动 ,而且增加了类似于主动轮的驱动机构 ,克服了被动轮驱动能力弱的缺点 ,增强了机器人的运动能力。在分析非完整约束的基础上 ,对蛇形机器人的运动学和冗余度进行分析 ,提出了控制该类蛇形机器人运动的分解矩阵方法和分组交替运动法。

Piezosurgery is a new and modern technique of bone surgery in implantology. Selective cutting is possible for different ultrasonic frequencies acting only in hard tissues (mineralized), saving vital anatomical structures. With the piezoelectric osteotomy technique, receptor site preparation for implants, autogenous bone graft acquistition (particles and blocks), osteotomy for alveolar bone crest expansion, maxillary sinus lifting, and dental implant removal can be performed accurately and safely, providing excellent clinical and biological results, especially for osteocyte viability. The aim of this review was, through literature review, to present clinical applications of piezosurgery in implant dentistry and outline their advantages and disadvantages over conventional surgical systems. Moreover, this study addressed the biological aspects related to piezosurgery that differentiate it from those of bone tissue approaches. Overall, piezosurgery enables critical operations in simple and fully executable procedures; and effectively, areas that are difficult to access have less risk of soft tissue and neurovascular tissue damage via piezosurgery.

The purpose of this literature review is to present the state-of-the-art on the application of tissue engineering techniques in sinus lifts procedures, reporting the influence of these techniques in increasing bone height and volume in the maxillary sinus, and the osseointegration of the implants installed in the grafted areas. PubMed and Scopus databases were searched using the keywords “scaffold” OR “engineered tissue” AND “sinus augmention” OR “sinus floor elevation”. In the initial search 463 articles were selected, of which 19 were selected abstracts reading, with nine papers selected by the end, for evaluation of the application of tissue engineering techniques in sinus lift procedures. Despite the positive results of tissue engineering procedures in sinus floor elevation reported in the selected articles, further studies are still needed, for a better standardization of experimental models and materials used, leading to definitive conclusions about the effects of the application of tissue engineering procedures on bone formation in maxillary sinus lifting procedures.

The aim of this study was to evaluate the clinical, tomographic, and histological similarity within the use of HA+β-TCP (Boneceramic) alone or associated with enamel matrix derived (EMD) proteins in sinus lifting procedures with subsequent dental implant placement. We selected two patients with residual bony ridges between 3 mm and 5 mm in height in need of bone grafts. Six months later, eight implants were installed (two implants at each site). Biopsies were made, and the tissue was processed and stained with HE solution. Tomographic evaluation showed bone height gain up to 10.0 mm on average. Histology showed new bone formation for both groups, with similar bone density and trabecular interconnectivity. It can be concluded that the HA+β-TCP graft isolated or associated to EMD provided bone height gain in sinus lifting procedures, with similar clinical, tomographic and histological characteristics.

Implant dentistry is a dental specialty which presents great predictability in the rehabilitation at posterior, partially edentulous maxillary areas. Early tooth loss results in significant jaw remodeling. The maxillary sinus lifting followed by implant placement is a predictable technique initially described in 1980. Since then, several different techniques have been investigated varying filling materials and the management of complications in order to provide effective guidance in the rehabilitation of these patients. The current study evaluated ten patients who underwent sinus lifting before implant placement and crown installation. First, a retrospective analysis of the medical records was conducted to obtain information about possible postoperative complications. Clinical and radiographic analyses were performed at baseline and 180 days after surgeries. The sinus lifting with immediate implant placement provided satisfactory outcomes and can be considered a safe procedure. Treatment predictability was demonstrated in 90% of patients and for 86.96% of implants placed. It is important to highlight knowledge of anatomical structures at this area, the use of delicate surgical techniques, and strict patient follow-up.

Maxillary sinus lifting surgery is considered an excellent option for patient rehabilitation in cases of maxillary sinus pneumatization. This longitudinal study was performed in 41 patients to evaluate 51 maxillary sinus lifting procedures concomitant to autogenous bone grafted from mandibular ramus. Of the 107 implants installed, 28 were immediate fixations. Success index achieved regarding to bone graft was of 98.04%. The implants installed immediately or after bone grafting resulted in success rates of 92.86% and 94.59% respectively, with no statistically significant differences between them. Surgeries for bone grafting and immediate installation are considered a reliable treatment alternative provided that both anatomical structures and tissue physiology are kept in good conditions, with appropriate indication and techniques for good implant stability.

O periodonto é uma unidade biológica e funcional, constituída pela gengiva, pelo cemento, pelo ligamento periodontal e pelo osso alveolar. O seu processo de cicatrização envolve mecanismos fisiológicos complexos que requerem a ação dos fatores de crescimento, péptidos oriundos da desgranulação das plaquetas. Neste sentido surge o Plasma Rico em Plaquetas como um produto autólogo, obtido a partir da centrifugação do sangue do próprio paciente e que visa melhorar a cicatrização dos tecidos após procedimentos enquadrados na Medicina Dentária. Esta revisão bibliográfica baseou-se numa pesquisa realizada na base de dados MEDLINE, via pubmed. Foram utilizadas as palavras-chave “plasma rich in growth factors”, “platelet-rich plasma”, “oral surgery”, “dental implants”, “sinus lift”, “third molar surgery” e “bone regeneration”. Após leitura de 40 artigos, foram descartados 9 pela sua pouca relevância no contexto. O objetivo é avaliar a efetividade da aplicação de plasma rico em plaquetas na regeneração dos tecidos periodontais em situações clínicas como alvéolos pós-extracionais, cirurgias de implantes, cirurgias de elevação do seio maxilar e na regeneração óssea. A efetividade em tecidos moles parece ser consensual. A efetividade no tecido ósseo é alvo de contradição entre os diversos autores, concluindo-se que há necessidade de mais estudos randomizados e controlados para que se possa responder à questão com evidência científica suficiente.
