Cultured melanoma cells release soluble factors that influence immune responses. Screening of a cDNA library with anti-sera from a melanoma patient identified an immunoreactive plaque, which encoded heavy-chain ferritin (H-ferritin), Previous studies have drawn attention to the immunosuppressive effects of this molecule and prompted further studies on its biochemical and functional properties in human melanoma, These studies demonstrated, firstly, that H-ferritin appeared to be secreted by melanoma cells, as shown by immunoprecipitation of a 21.5 kDa band from supernatants. It was also detected in extracts of melanoma cells by Western blotting as 43 and 64 kDa dimers and trimers of the 21.5 kDa fraction. Secondly, flow-cytometric analysis of H- and light-chain ferritin (L-ferritin) expression on melanoma showed a wide variation in L-ferritin expression and consequently of the ratio of H- to L-ferritin expression. Suppression of mitogenic responses of lymphocytes to anti-CD3 showed a correlation with the ratio of H- to L-ferritin in the supernatants and was specific for H-ferritin, as shown by inhibition studies with a monoclonal antibody (MAb) against H-ferritin, Similar results were obtained with H- and L-ferritin from other sources. Suppression of mitogenic responses of lymphocytes to anti-CD3 by H-ferritin was inhibited using a MAb against IL-IO, which suggested that the immunosuppressive effect of H-ferritin was mediated by IL-IO, Assays of cytokine production from anti-CD3-stimulated lymphocytes showed that H-ferritin markedly increased production of IL-10 and IFN-gamma and had only slight effects on IL-2 and IL-4 production, Our results suggest that melanoma cells may be a major source of H-ferritin and that production of the latter may account for some of the immunosuppressive effects of melanoma, (C) 2001 Wiley-Liss. Background:¦Hirschsprung's disease (HSCR) is a congenital malformation of the enteric nervous system due to the¦arrest of migration of neural crest cells to form the myenteric and submucosal plexuses. It leads to an anganglionic intestinal segment, which is permanently contracted causing intestinal obstruction. Its incidence is approximately 1/5000 birth, and males are more frequently affected with a male/female ratio of 4/1. The diagnosis is in most cases made within the first year of life. The rectal biopsy of the mucosa and sub-mucosa is the diagnostic gold standard.¦Purpose:¦The aim of this study was to compare two surgical approaches for HSCR, the Duhamel technique and the transanal endorectal pull-through (TEPT) in term of indications, duration of surgery, duration of hospital stay, postoperative treatment, complications, frequency of enterocolitis and functional outcomes.¦Methods:¦Fifty-nine patients were treated for HSCR by one of the two methods in our department of pediatric¦surgery between 1994 and 2010. These patients were separated into two groups (I: Duhamel, II: TEPT), which were compared on the basis of medical records. Statistics were made to compare the two groups (ANOVA test). The first group includes 43 patients and the second 16 patients. It is noteworthy that twenty-four patients (about 41% of all¦patients) were referred from abroad (Western Africa). Continence was evaluated with the Krickenbeck's score.¦Results:¦Statistically, this study showed that operation duration, hospital stay, postoperative fasting and duration of postoperative antibiotics were significantly shorter (p value < 0.05) in group II (TEPT). But age at operation and length of aganglionic segment showed no significant difference between the two groups. The continence follow-up showed generally good results (Krickenbeck's scores 1; 2.1; 3.1) in both groups with a slight tendency to constipation in group I and soiling in group II.¦Conclusion:¦We found two indications for the Duhamel method that are being referred from a country without¦careful postoperative surveillance and/or having a previous colostomy. Even if the Duhamel technique tends to be replaced by the TEPT, it remains the best operative approach for some selected patients. TEPT has also proved some advantages but must be followed carefully because, among other points, of the postoperative dilatations. Our postoperative standards, like digital rectal examination and anal dilatations seem to reduce the occurrence of complications like rectal spur and anal/anastomosis stenosis, respectively in the Duhamel method and the TEPT technique.

El tema del narcotráfico ha sido ampliamente tratado, así como el caso de las drogas en Colombia, pero la afectación de dicho problema no ha sido analizada desde la República Dominicana y mucho menos desde la relación bilateral entre esta y Colombia. Aunque el tema es de gran relevancia en la agenda internacional, así como en la agenda interna de cada uno de estos Estados, no es el tema principal en la relación bilateral, donde los asuntos comerciales tienen mayor importancia, aún cuando hay ciertos mecanismos que buscan eliminar el tráfico ilegal de estupefacientes. En esta investigación, se busca dar un diagnóstico de las relaciones bilaterales y de aquellos instrumentos existentes, específicamente aquellos implementados desde la acogida internacional del término responsabilidad compartida en el año 1998 hasta el año 2010, para determinar la efectividad de los mismos y de aquellos factores que no son precisamente resultantes de las relaciones bilaterales pero que sí afectan de una u otra manera el tráfico de drogas ilegales entre estos dos países. Así, se buscará encontrar las debilidades, en los instrumentos bilaterales entre Colombia y República Dominicana y hacer recomendaciones para hacerlos más efectivos.

Immuntherapien stellen basierend auf gut charakterisierten, tumorspezifischen Antigenen ein vielversprechendes Konzept in der Tumor-Therapie dar. Für das kutane T-Zell Lymphom (CTCL) sind bislang nur wenige tumorassoziierte Antigene bekannt. In dieser Arbeit konnten ein mögliches Onkogen (PAR-3ta), ein weiteres, putativ Virus-induziertes Antigen (se57-1), neue Antigene und ihre Splicing-Varianten (se70-2, cTAGE-1/2, cTAGE-5), sowie lymphozytenspezifische Differenzierungsantigene (se20-10-Familie) für das CTCL gefunden werden. Auf Grund des Cancer-Testis spezifischen Expressionsprofils kommen die Antigene cTAGE-1 (46% CTCL-Expression), cTAGE-1B (31%) und cTAGE-5A (44%) für eine CTCL-Immuntherapie in Frage. Einige bekannte, spezifische Tumorantigene konnten auch im CTCL nachgewiesen werden: MAGE-A9 (21%), GAGE 3-7b (46%) und MUC-1 (44%). Die Immunogenität konnte mittels der SEREX-Technologie durch reaktive Antikörper gegen rekombinantes MAGE-A9 in 22% der getesteten CTCL-Seren, gegen GAGE in 16% und gegen cTAGE-5A in 15% bewiesen werden. Die cTAGE-mRNAs bilden eine neue Cancer-Testis Antigenfamilie. Die Multigenfamilie ist durch Retrotransposition eines aktiven Gens (cTAGE-5) entstanden, wobei 7 aktive, transkribierte und 7 nicht transkribierte Pseudogene unterschieden werden müssen. Die aktiven Gene cTAGE-1, cTAGE-1B und cTAGE-5A werden tumorspezifisch aus den Chromosomen 18p11.2 und 14 gespliced. Die Testis-CTCL spezifischen mRNAs können häufig auch in anderen Tumoren gefunden werden. Die Antigene cTAGE-1, cTAGE-5, MAGE-A9 und GAGE 3-7b gehören zu den ersten spezifischen Zielstrukturen des CTCLs, die somit für mögliche Immuntherapien in Betracht kommen.
