Embora a dor perioperatória seja multifactorial, existem dois factores principais sobre os quais se pode actuar em termos terapêuticos: a sensibilização central - pelo uso de analgésicos opióides - e o estímulo nociceptivo periférico - pelo uso de bloqueios de nervos periféricos. O uso destas técnicas para anestesia e analgesia de pós-operatório é cada vez mais frequente em cirurgia ortopédica devido à qualidade dos bloqueios sensitivo e motor que produz e às vantagens que apresenta relativamente ao alívio da dor e à mobilização precoce no pós-operatório. Apresentamos um caso clínico em que foi realizado um bloqueio contínuo do nervo ciático para correcção cirúrgica de Pé Cavum neurológico numa criança com Atraso Intelectual e Síndrome de Hiperactividade, permitindo uma analgesia perioperatória eficaz com efeitos secundários mínimos, e um regresso rápido ao ambiente familiar.

A técnica de analgesia multimodal, por meio da infusão contínua de fármacos, pode ser empregada para diminuir a incidência de sensibilização central durante a anestesia. Avaliaram-se as características cardiorrespiratórias, durante o procedimento de artroscopia de joelho, em cães anestesiados com isofluorano e monitorados por meio do índice biespectral, submetidos à infusão contínua de morfina ou fentanil, associada à lidocaína e cetamina. Utilizaram-se 16 cães adultos, machos ou fêmeas, os quais foram distribuídos aleatoriamente em dois grupos, denominados MLK - que recebeu morfina (3,3μg/kg/min), lidocaína (50μg/kg/min) e cetamina (10μg/kg/min) ou FLK - em que foi substituída a morfina pelo fentanil (0,03μg/kg/min). Os cães foram pré-tratados com levomepromazina (0,5mg/kg IV), induzidos à anestesia com propofol (5mg/kg) e mantidos com isofluorano, ajustando-se a concentração para obterem-se valores de índice biespectral entre 55 e 65. As mensurações da frequência cardíaca (FC), dos parâmetros eletrocardiográficos (ECG), das pressões arteriais sistólica (PAS), diastólica (PAD) e média (PAM), da tensão de dióxido de carbono expirado (EtCO2), da saturação de oxi-hemoglobina (SpO2), da frequência respiratória (FR) e da temperatura esofágica (T) iniciaram-se 30 minutos após a indução (M0) e continuaram após o início da infusão das soluções, em intervalos de 15 minutos (M15 a M75). Diferenças entre os grupos foram registradas para duração do complexo QRS (M60), para FC e T, entre M30 e M75, com MLK apresentando médias maiores que FLK, que registrou médias maiores que MLK para a SpO2 (M60), para os intervalos QT (M30 e M75) e RR (M0, M60 e M75). Concluiu-se que o emprego de morfina ou fentanil, associados à lidocaína e cetamina, promove efeitos semelhantes e não compromete as características avaliadas.

O presente relatório está inserido no âmbito da unidade curricular do Estágio Curricular do Mestrado Integrado em Medicina Veterinária da Universidade de Évora, sob orientação da Drª Sónia Lucena e coorientação da Drª Ângela Martins. Encontra-se dividido em duas partes, a primeira é referente à casuística acompanhada no Hospital Veterinário da Arrábida e Centro de Reabilitação Animal da Arrábida durante o período de cinco meses de estágio. Na casuística são expostos de forma breve os casos clínicos e procedimentos acompanhados durante o período de estágio. Na segunda parte é desenvolvida uma monografia intitulada “Maneio da dor crónica” acompanhada por uma descrição de um caso clínico assistido durante o estágio. A dor crónica é considerada uma dor patológica, deixando de ser um mecanismo de defesa do organismo e passando a ser considerada uma doença. É de difícil tratamento e requer um esforço multidisciplinar; ABSTRACT: Small Animal Medicine This report is conducted under the context of the curricular internship course unit of integrated master’s degree in Veterinary Medicine at the University of Évora under the supervision of Dr. Sónia Lucena and cosupervision of Dr. Ângela Martins. The first component refers to the assisted cases followed in Hospital Veterinário da Arrábida and Centro de Reabilitação Animal da Arrábida during the five months of the practice period. In the followed cases part it is explored briefly some clinical cases and procedures accompanied during that practice period. The second component consists of a monograph titled “Chronic pain management” accompanied with a description of a clinical case followed seen during the practice period. Chronic pain is considered a pathological pain, ceasing to be a defense mechanism of the body and to be considered a disease. It is difficult to treat and requires a multidisciplinary effort.

JUSTIFICATIVA E OBJETIVOS: As pesquisas recentes têm focalizado a plasticidade bioquímica e estrutural do sistema nervoso decorrente da lesão tissular. Os mecanismos envolvidos na transição da dor aguda para crônica são complexos e envolvem a interação de sistemas receptores e o fluxo de íons intracelulares, sistemas de segundo mensageiro e novas conexões sinápticas. O objetivo deste artigo foi discutir os novos mecanismos que envolvem a sensibilização periférica e central. CONTEÚDO: A lesão tissular provoca aumento na resposta dos nociceptores, chamada de sensibilização ou facilitação. Esses fenômenos iniciam-se após a liberação local de mediadores inflamatórios e a ativação de células do sistema imune ou de receptores específicos no sistema nervoso periférico e central. CONCLUSÕES: As lesões do tecido e dos neurônios resultam em sensibilização de nociceptores e facilitação da condução nervosa central e periférica.

Pharmacological treatments currently used in ethanol addiction are inefficient, requiring new drugs for this purpose. The pro-drug N-Acetylcysteine (N-Ac) has shown efficacy in the treatment of addiction to cocaine and nicotine in preclinical research and clinical pilot studies. When administered, N-Ac is subsequently converted to cysteine, and cystine, which has an action on the central nervous system. However, there are few data about the possible application of N-Ac in the ethanol addiction. Behavioral sensitization is the gradual increase of the psychostimulant effects induced by repeated administration of drugs of abuse, including ethanol, and its development has been linked to important neuroadaptations in addiction. These neuroadaptations occur in neural circuits that mediate the reinforcing properties of these drugs and may involve several proteins. The ΔFosB protein accumulates in neurons after repeated activation and mediates long lasting changes in response to drugs of abuse. These changes are manifested mainly in the nucleus accumbens (Acb) and prefrontal cortex (PFC) neurons. The aim of the study was to evaluate the effects of N-Ac treatment in the development of ethanol behavioral sensitization and in alterations in the ΔFosB protein in mice. Swiss male mice were exposed to a standardized behavioral sensitization protocol to the experimental conditions of the laboratory and treated with N-Ac. At the end of behavioral sensitization procedure, animals were euthanized and their brains removed for ΔFosB quantification by Western blotting. Two experiments of behavioral sensitization were performed to the standardization of the protocol. The first, although effective in demonstrating the development of behavioral sensitization, was not effective in allowing the evaluation of the expression of the behavioral sensitization. Os elevados índices de incidência e mortalidade por câncer do colo do útero e de mama no Brasil justificam a implantação de estratégias efetivas de controle dessas doenças que incluam ações de promoção à saúde, prevenção e detecção precoce, tratamento e de cuidados paliativos, quando esses se fizerem necessários. enfatizando a atenção integral à saúde da mulher. Diante dessa perspectiva, realizou-se uma intervenção em saúde entre os meses de abril e agosto de 2015 na Unidade de Saúde Central de Cambará do Sul/RS objetivando a melhoria das ações de prevenção e controle dos cânceres de colo de útero e de mama entre as mulheres com idade entre 25 e 64 anos e entre 50 e 69 anos, respectivamente. A estimativa previa um total de 813 mulheres entre 25 e 64 anos e 305 mulheres entre 50 e 69 anos para a área de abrangência. Para guiar o cuidado foi adotado o protocolo do Ministério da Saúde Controle dos cânceres do colo do útero e da mama de 2013. Para a coleta de dados foi utilizada a ficha-espelho específica disponibilizada pelo curso. Assim, ao longo da intervenção, obtemos os seguintes resultados: no eixo monitoramento e avaliação foi possível monitorar a situação de saúde de 152 (18,7%) mulheres entre 25 e 64 anos que foram avaliadas e realizaram o exame citopatológico e 55 (18,0%) mulheres passaram pela avaliação clínica das mamas e para as quais também solicitamos exame mamográfico. No eixo organização e gestão do serviço, melhoramos a qualidade dos registros, pois adotamos uma ficha específica para cadastramento e acompanhamento das mulheres bem como realizar exame citopatológico com qualidade das amostras, solicitação do exame mamográfico para as idades preconizadas, exame clínico de mamas, registro na ficha-espelho, avaliação dos sinais de alerta para câncer de colo e de mama, orientações aos fatores de risco para 100% das mulheres acompanhadas. Ainda, organizamos por meio de uma agenda todos os atendimentos, aliados aos atendimentos de demanda espontânea. Já no eixo engajamento público foi possível trabalhar a sensibilização da comunidade sobre a importância da equipe estar desenvolvendo este trabalho de priorização no cuidado para as mulheres entre 25 e 69 anos por meio de atividade em grupos de educação em saúde na comunidade. E, ainda no eixo da qualificação da prática clínica foi possível capacitar todos os profissionais da equipe tendo como base o protocolo de Saúde da mulher do Ministério da Saúde, 2013. Conclui-se que a intervenção proporcionou às mulheres um entendimento mais amplo sobre os fatores de riscos e sobre as consequências dessas duas doenças, também aumentou o conhecimento de todas as mulheres e comunidade em geral quanto à importância de ações preventivas, uma vez que as mudanças no estilo de vida podem ser essenciais para a prevenção e que a detecção precoce também é primordial. Pretendemos dar continuidade às ações iniciadas com a intervenção ampliando para outros grupos prioritários, com objetivo de atingir outras necessidades da comunidade.

The control of energy homeostasis relies on robust neuronal circuits that regulate food intake and energy expenditure. Although the physiology of these circuits is well understood, the molecular and cellular response of this program to chronic diseases is still largely unclear. Hypothalamic inflammation has emerged as a major driver of energy homeostasis dysfunction in both obesity and anorexia. Importantly, this inflammation disrupts the action of metabolic signals promoting anabolism or supporting catabolism. In this review, we address the evidence that favors hypothalamic inflammation as a factor that resets energy homeostasis in pathological states.

Assessment of central blood pressure (BP) has grown substantially over recent years because evidence has shown that central BP is more relevant to cardiovascular outcomes than peripheral BP. Thus, different classes of antihypertensive drugs have different effects on central BP despite similar reductions in brachial BP. The aim of this study was to investigate the effect of nebivolol, a β-blocker with vasodilator properties, on the biochemical and hemodynamic parameters of hypertensive patients. Experimental single cohort study conducted in the outpatient clinic of a university hospital. Twenty-six patients were recruited. All of them underwent biochemical and hemodynamic evaluation (BP, heart rate (HR), central BP and augmentation index) before and after 3 months of using nebivolol. 88.5% of the patients were male; their mean age was 49.7 ± 9.3 years and most of them were overweight (29.6 ± 3.1 kg/m2) with large abdominal waist (102.1 ± 7.2 cm). There were significant decreases in peripheral systolic BP (P = 0.0020), diastolic BP (P = 0.0049), HR (P < 0.0001) and central BP (129.9 ± 12.3 versus 122.3 ± 10.3 mmHg; P = 0.0083) after treatment, in comparison with the baseline values. There was no statistical difference in the augmentation index or in the biochemical parameters, from before to after the treatment. Nebivolol use seems to be associated with significant reduction of central BP in stage I hypertensive patients, in addition to reductions in brachial systolic and diastolic BP.

To investigate central auditory processing in children with unilateral stroke and to verify whether the hemisphere affected by the lesion influenced auditory competence. 23 children (13 male) between 7 and 16 years old were evaluated through speech-in-noise tests (auditory closure); dichotic digit test and staggered spondaic word test (selective attention); pitch pattern and duration pattern sequence tests (temporal processing) and their results were compared with control children. Auditory competence was established according to the performance in auditory analysis ability. Was verified similar performance between groups in auditory closure ability and pronounced deficits in selective attention and temporal processing abilities. Most children with stroke showed an impaired auditory ability in a moderate degree. Children with stroke showed deficits in auditory processing and the degree of impairment was not related to the hemisphere affected by the lesion.

Infections of the central nervous systems (CNS) present a diagnostic problem for which an accurate laboratory diagnosis is essential. Invasive practices, such as cerebral biopsy, have been replaced by obtaining a polymerase chain reaction (PCR) diagnosis using cerebral spinal fluid (CSF) as a reference method. Tests on DNA extracted from plasma are noninvasive, thus avoiding all of the collateral effects and patient risks associated with CSF collection. This study aimed to determine whether plasma can replace CSF in nested PCR analysis for the detection of CNS human herpesvirus (HHV) diseases by analysing the proportion of patients whose CSF nested PCR results were positive for CNS HHV who also had the same organism identified by plasma nested PCR. In this study, CSF DNA was used as the gold standard, and nested PCR was performed on both types of samples. Fifty-two patients with symptoms of nervous system infection were submitted to CSF and blood collection. For the eight HHV, one positive DNA result-in plasma and/or CSF nested PCR-was considered an active HHV infection, whereas the occurrence of two or more HHVs in the same sample was considered a coinfection. HHV infections were positively detected in 27/52 (51.9%) of the CSF and in 32/52 (61.5%) of the plasma, difference not significant, thus nested PCR can be performed on plasma instead of CSF. In conclusion, this findings suggest that plasma as a useful material for the diagnosis of cases where there is any difficulty to perform a CSF puncture.

In this study, we hypothesized that blunting of the natriuresis response to intracerebroventricularly (i.c.v.) microinjected cholinergic and adrenergic agonists is involved in the development of hypertension in spontaneously hypertensive rats (SHR). We evaluated the effect of i.c.v. injection of cholinergic and noradrenergic agonists, at increasing concentrations, and of muscarinic cholinergic and α1 and α2-adrenoceptor antagonists on blood pressure and urinary sodium handling in SHR, compared with age-matched Wistar Kyoto rats (WR). We confirmed that CCh and NE microinjected into the lateral ventricle (LV) of conscious rats leads to enhanced natriuresis. This response was associated with increased proximal and post-proximal sodium excretion accompanied by an unchanged rate of glomerular filtration. We showed that cholinergic-induced natriuresis in WR and SHR was attenuated by previous i.c.v. administration of atropine and was significantly lower in the hypertensive strain than in WR. In both groups the natriuretic effect of injection of noradrenaline into the LV was abolished by previous local injection of an α1-adrenoceptor antagonist (prazosin). Conversely, LV α2-adrenoceptor antagonist (yohimbine) administration potentiated the action of noradrenaline. The LV yohimbine pretreatment normalized urinary sodium excretion in SHR compared with age-matched WR. In conclusion, these are, as far as we are aware, the first results showing the importance of interaction of central cholinergic and/or noradrenergic receptors in the pathogenesis of spontaneous hypertension. These experiments also provide good evidence of the existence of a central adrenergic mechanism consisting of α1 and α2-adrenoceptors which works antagonistically on regulation of renal sodium excretion.

Triatomine surveillance in rural areas, artificial ecotypes, and natural ecotopes of the cities of Caturama, Ibipitanga, Macaúbas, and Seabra in the south-central region of the Brazilian state of Bahia was carried out between 2008 and 2013. Natural infection by Trypanosoma cruzi was evaluated in the specimens collected to monitor vectors of Chagas disease. A total of 1,357 specimens were collected, and four species were identified: Triatoma sordida (83%), Triatoma lenti (16.4%), Triatoma pseudomaculata (0.5%), and Panstrongylus geniculatus (0.1%). Triatoma sordida was found in four cities, only 0.7% in intradomiciliary environments. Triatoma lenti was found only in Macaúbas; 8.5% were found in intradomiciliary environments, 88.3% in peridomiciliary environments, and 3.1% in sylvatic environments. Natural infection by T. cruzi was 0.5% for T. sordida and 3.1% T. lenti. All of these cases were found in peridomiciliary environments of Macaúbas. As the results show, triatomines were found in intradomiciliary environments in three cities that were surveyed in the south-central region of the state of Bahia. Thus, an epidemiologic survey should be performed to avoid the risk of transmission to the population.

Oligodendrocytes and Schwann cells are engaged in myelin production, maintenance and repairing respectively in the central nervous system (CNS) and the peripheral nervous system (PNS). Whereas oligodendrocytes act only within the CNS, Schwann cells are able to invade the CNS in order to make new myelin sheaths around demyelinated axons. Both cells have some limitations in their activities, i.e. oligodendrocytes are post-mitotic cells and Schwann cells only get into the CNS in the absence of astrocytes. Ethidium bromide (EB) is a gliotoxic chemical that when injected locally within the CNS, induce demyelination. In the EB model of demyelination, glial cells are destroyed early after intoxication and Schwann cells are free to approach the naked central axons. In normal Wistar rats, regeneration of lost myelin sheaths can be achieved as early as thirteen days after intoxication; in Wistar rats immunosuppressed with cyclophosphamide the process is delayed and in rats administered cyclosporine it may be accelerated. Aiming the enlightening of those complex processes, all events concerning the myelinating cells in an experimental model are herein presented and discussed.
