Les études sur les milieux de vie et la santé ont traditionnellement porté sur le seul quartier de résidence. Des critiques ont été émises à cet égard, soulignant le fait que la mobilité quotidienne des individus n’était pas prise en compte et que l’accent mis sur le quartier de résidence se faisait au détriment d’autres milieux de vie où les individus passent du temps, c’est-à-dire leur espace d’activité. Bien que la mobilité quotidienne fasse l’objet d’un intérêt croissant en santé publique, peu d’études se sont intéressé aux inégalités sociales de santé. Ceci, même en dépit du fait que différents groupes sociaux n’ont pas nécessairement la même capacité à accéder à des milieux favorables pour la santé. Le lien entre les inégalités en matière de mobilité et les inégalités sociales de santé mérite d’être exploré. Dans cette thèse, je développe d'abord une proposition conceptuelle qui ancre la mobilité quotidienne dans le concept de potentiel de mobilité. Le potentiel de mobilité englobe les opportunités et les lieux que les individus peuvent choisir d’accéder en convertissant leur potentiel en mobilité réalisée. Le potentiel de mobilité est façonné par des caractéristiques individuelles (ex. le revenu) et géographiques (ex. la proximité des transports en commun), ainsi que par des règles régissant l’accès à certaines ressources et à certains lieux (ex. le droit). Ces caractéristiques et règles sont inégalement distribuées entre les groupes sociaux. Des inégalités sociales en matière de mobilité réalisée peuvent donc en découler, autant en termes de l'ampleur de la mobilité spatiale que des expositions contextuelles rencontrées dans l'espace d'activité. Je discute de différents processus par lesquels les inégalités en matière de mobilité réalisée peuvent mener à des inégalités sociales de santé. Par exemple, les groupes défavorisés sont plus susceptibles de vivre et de mener des activités dans des milieux défavorisés, comparativement à leurs homologues plus riches, ce qui pourrait contribuer aux différences de santé entre ces groupes. Cette proposition conceptuelle est mise à l’épreuve dans deux études empiriques. Les données de la première vague de collecte de l’étude Interdisciplinaire sur les inégalités sociales de santé (ISIS) menée à Montréal, Canada (2011-2012) ont été analysées. Dans cette étude, 2 093 jeunes adultes (18-25 ans) ont rempli un questionnaire et fourni des informations socio-démographiques, sur leur consommation de tabac et sur leurs lieux d’activités. Leur statut socio-économique a été opérationnalisé à l’aide de leur plus haut niveau d'éducation atteint. Les lieux de résidence et d'activité ont servi à créer des zones tampons de 500 mètres à partir du réseau routier. Des mesures de défavorisation et de disponibilité des détaillants de produits du tabac ont été agrégées au sein des ces zones tampons. Dans une première étude empirique je compare l'exposition à la défavorisation dans le quartier résidentiel et celle dans l'espace d’activité non-résidentiel entre les plus et les moins éduqués. J’identifie également des variables individuelles et du quartier de résidence associées au niveau de défavorisation mesuré dans l’espace d’activité. Les résultats démontrent qu’il y a un gradient social dans l’exposition à la défavorisation résidentielle et dans l’espace d’activité : elle augmente à mesure que le niveau d’éducation diminue. Chez les moins éduqués les écarts dans l’exposition à la défavorisation sont plus marquées dans l’espace d’activité que dans le quartier de résidence, alors que chez les moyennement éduqués, elle diminuent. Un niveau inférieur d'éducation, l'âge croissant, le fait d’être ni aux études, ni à l’emploi, ainsi que la défavorisation résidentielle sont positivement corrélés à la défavorisation dans l’espace d’activité. Dans la seconde étude empirique j'étudie l'association entre le tabagisme et deux expositions contextuelles (la défavorisation et la disponibilité de détaillants de tabac) mesurées dans le quartier de résidence et dans l’espace d’activité non-résidentiel. J'évalue si les inégalités sociales dans ces expositions contribuent à expliquer les inégalités sociales dans le tabagisme. J’observe que les jeunes dont les activités quotidiennes ont lieu dans des milieux défavorisés sont plus susceptibles de fumer. La présence de détaillants de tabac dans le quartier de résidence et dans l’espace d’activité est aussi associée à la probabilité de fumer, alors que le fait de vivre dans un quartier caractérisé par une forte défavorisation protège du tabagisme. En revanche, aucune des variables contextuelles n’affectent de manière significative l’association entre le niveau d’éducation et le tabagisme. Les résultats de cette thèse soulignent l’importance de considérer non seulement le quartier de résidence, mais aussi les lieux où les gens mènent leurs activités quotidiennes, pour comprendre le lien entre le contexte et les inégalités sociales de santé. En discussion, j’élabore sur l’idée de reconnaître la mobilité quotidienne comme facteur de différenciation sociale chez les jeunes adultes. En outre, je conclus que l’identification de facteurs favorisant ou contraignant la mobilité quotidienne des individus est nécessaire afin: 1 ) d’acquérir une meilleure compréhension de la façon dont les inégalités sociales en matière de mobilité (potentielle et réalisée) surviennent et influencent la santé et 2) d’identifier des cibles d’intervention en santé publique visant à créer des environnements sains et équitables.

The fundamental diseconomy in the large metropolitan areas is the significant and sustained increase in travel time incurred by its inhabitants daily. Since a portion of the energy consumed in the daily mobility does not necessarily translate into individual and collective wealth, entropic systems have a feature which, as discussed for the case of Bogotá, does not have metropolitan institutions with the ability to redirect.

Emergent phenomena such as urban sprawl, travel intensification and loss of cohesion in contemporary metropolises, impose stronger constraints on its inhabitants. Among them, travel and location capabilities become a fundamental factor of social integration and a multiplier of income inequalities. The simultaneous analysis of housing-travel efforts and accessibility to urban opportunities in Greater Santiago shows that these dimensions are closely related and exert an important influence on spatial mobility and inequalities among its inhabitants. Furthermore, a theoretical model of displacements, considering income and location, confirms the importance of proximity and non-motorized transport in order to optimize daily mobility strategies of households. Overall, the empirical and theoretical results presented show the need to implement coordinated planning strategies between the housing and transport sectors, addressing not only travel acceleration, but mainly the consistency between accommodation and opportu  ties location. The creation of such planning tools could be a more sustainable alternative than current growth trends in Greater Santiago.

In this thesis, we extend some ideas of statistical physics to describe the properties of human mobility. By using a database containing GPS measures of individual paths (position, velocity and covered space at a spatial scale of 2 Km or a time scale of 30 sec), which includes the 2% of the private vehicles in Italy, we succeed in determining some statistical empirical laws pointing out "universal" characteristics of human mobility. Developing simple stochastic models suggesting possible explanations of the empirical observations, we are able to indicate what are the key quantities and cognitive features that are ruling individuals' mobility. To understand the features of individual dynamics, we have studied different aspects of urban mobility from a physical point of view. We discuss the implications of the Benford's law emerging from the distribution of times elapsed between successive trips. We observe how the daily travel-time budget is related with many aspects of the urban environment, and describe how the daily mobility budget is then spent. We link the scaling properties of individual mobility networks to the inhomogeneous average durations of the activities that are performed, and those of the networks describing people's common use of space with the fractional dimension of the urban territory. We study entropy measures of individual mobility patterns, showing that they carry almost the same information of the related mobility networks, but are also influenced by a hierarchy among the activities performed. We discover that Wardrop's principles are violated as drivers have only incomplete information on traffic state and therefore rely on knowledge on the average travel-times. We propose an assimilation model to solve the intrinsic scattering of GPS data on the street network, permitting the real-time reconstruction of traffic state at a urban scale.

The need of decarbonization of urban mobility is one of the main priorities for all countries to achieve greenhouse gas (GHG) emissions reduction targets. In general, the transport modes which have experienced the most growth in recent years tend to be the most polluting. Most efforts have been focused on the vehicle efficiency improvements and vehicle fleet renewal; nevertheless more emphasis should be placed on strategies related to the management of urban mobility and modal share. Research of individual travel which analyzes CO2 emissions and car and public transport share in daily mobility will enable better assessments of the potential of urban mobility measures introduced to limit GHG emissions produced by transport in cities. This paper explores the sustainability impacts of daily mobility in Spain using data from two National Travel Surveys (NTSs) (2000 and 2006) and includes a method by which to estimate the CO2 emissions associated with each journey and each surveyed individual. The results demonstrate that in the 2000 to 2006 period, there has been an increase in daily mobility which has led to a 17% increase in CO2 emissions. When separated by transport mode, cars prove to be the main contributor to that increase, followed by public transport. More focus should be directed toward modal shift strategies which not only take the number of journeys into account but also consider distance. The contributions of this paper have potential applications in the assessment of current and future urban transport policies.

The climate change impacts of daily mobility in Spain are explored with data from two national travel surveys in 2000 and 2006, and a method for estimating the CO2 emissions associated with each journey and each surveyed individual is provided. The results demonstrate that from 2000 to 2006, daily mobility has increased and has led to a 17% increase in CO2 emissions. When these results are separated by transport mode, cars prove to be the main contributor to that increase, followed by public transport. More focus should be directed toward modal shift strategies, which take into account not only the number of journeys but also the distance traveled. These contributions have potential applications in the assessment of current and future urban transport policies related to low-carbon urban transportation.

Ce mémoire porte sur l’étude du développement d’un sentiment d’appartenance à l’échelle d’un quartier dans un contexte de croissance de la mobilité des individus. À travers ce travail, il s’agit d’observer les formes que prend ce sentiment alors que les limites du territoire quotidien ne cessent d’être repoussées et que les individus sont de moins en moins ancrés de façon permanente sur un territoire. Dès lors, la pertinence du quartier est remise en cause. L’étude a pris pour cadre d’analyse l’arrondissement Plateau-Mont-Royal en raison de son caractère atypique et exemplaire en matière de forte mobilité de ses résidants et de la perception d’un sentiment d’appartenance territoriale marqué. Afin d’alimenter la réflexion, des entretiens ont été réalisés auprès de dix résidants de l’arrondissement recrutés lors d’un rendez-vous citoyen. Ces rencontres ont permis de comprendre que la mobilité des résidants ne répond pas à la tangente de la croissance observée. La flexibilité et la liberté d’avoir le choix de quitter son lieu de résidence sont des aptitudes valorisées, mais l’ancrage demeure prisé. Cette divergence s’inscrit dans une démarche particulière; celle de vivre selon un mode de vie où l’espace du quartier est priorisé à travers la mobilité quotidienne. Dans ce contexte, le sentiment d’appartenance se développe en direction d’un mode de vie, honorant un ensemble de valeurs, plutôt qu’à un territoire. Selon cette approche, l’arrondissement Plateau-Mont-Royal joue le second rôle; il représente le catalyseur et la vitrine de ce mode de vie, facilitant ainsi le regroupement des individus sous des valeurs communes.

Este estudio trata sobre la evolución de la movilidad cotidiana en dos municipios que ilustran la diversidad de las periferias populares de Bogotá: Soacha está integrado al mercado residencial y laboral de Bogotá, mientras Madrid conserva cierta autonomía. Ambos han entrado hoy día en una fase de “maduración urbana”, que se traduce en una diversificación de su composición demográfica, en una consolidación de lo construido, en un cierto arraigo residencial de sus habitantes y en un mejoramiento de la oferta local de empleos y servicios, entre los que se encuentra el de educación. El análisis de los censos realizados por el Departamento Administrativo Nacional de Estadísticas (DANE) en 1993 y 2005 y de unas encuestas específicas sobre movilidad (1993, 2009) muestra una relativa homogeneización de la composición social de los municipios, la cual contrasta con el recrudecimiento de las desigualdades de movilidad cotidiana, particularmente en Soacha. Estas desigualdades nos solo reflejan las jerarquías sociales, s  o también los efectos del género y de la edad, pues se observan diferencias importantes entre individuos de un mismo hogar.

In Metropolitan Area of Mexico City, most of urban displacements happen through semi formal public transportation: small and medium capacity vehicles operated by small private enterprises, through a concession scheme. This kind of public transportation has been playing a major role in the Mexican capital. On one hand, it has been one of the conditions for urbanization to be possible. On the other hand, despite its uncountable deficiencies, public transportation has allowed for a long time the whole population to be able to move within this huge metropolis. However, that important function with regards to integration has now reached its limits in the most recent suburbs of the city, where a new mode of urbanization is taking place, based on massive production of very big social housing gated settlements. Public transportation tends to constitute here a factor of exclusion and households meet with important difficulties for their daily mobility

Without transparent reporting, readers cannot judge the reliability and validity of trial findings nor extract information for systematic reviews. Recent methodological analyses indicate that inadequate reporting and design are associated with biased estimates of treatment effects. Such systematic error is seriously damaging to RCTs, which are considered the gold standard for evaluating interventions because of their ability to minimise or avoid bias. A group of scientists and editors developed the CONSORT (Consolidated Standards of Reporting Trials) statement to improve the quality of reporting of RCTs. It was first published in 1996 and updated in 2001. The statement consists of a checklist and flow diagram that authors can use for reporting an RCT. Many leading medical journals and major international editorial groups have endorsed the CONSORT statement. The statement facilitates critical appraisal and interpretation of RCTs. During the 2001 CONSORT revision, it became clear that explanation and elaboration of the principles underlying the CONSORT statement would help investigators and others to write or appraise trial reports. A CONSORT explanation and elaboration article was published in 2001 alongside the 2001 version of the CONSORT statement. After an expert meeting in January 2007, the CONSORT statement has been further revised and is published as the CONSORT 2010 Statement. This update improves the wording and clarity of the previous checklist and incorporates recommendations related to topics that have only recently received recognition, such as selective outcome reporting bias. This explanatory and elaboration document-intended to enhance the use, understanding, and dissemination of the CONSORT statement-has also been extensively revised. It presents the meaning and rationale for each new and updated checklist item providing examples of good reporting and, where possible, references to relevant empirical studies. Several examples of flow diagrams are included. BACKGROUND Panic disorder is characterised by the presence of recurrent unexpected panic attacks, discrete periods of fear or anxiety that have a rapid onset and include symptoms such as racing heart, chest pain, sweating and shaking. Panic disorder is common in the general population, with a lifetime prevalence of 1% to 4%. A previous Cochrane meta-analysis suggested that psychological therapy (either alone or combined with pharmacotherapy) can be chosen as a first-line treatment for panic disorder with or without agoraphobia. However, it is not yet clear whether certain psychological therapies can be considered superior to others. In order to answer this question, in this review we performed a network meta-analysis (NMA), in which we compared eight different forms of psychological therapy and three forms of a control condition. OBJECTIVES To assess the comparative efficacy and acceptability of different psychological therapies and different control conditions for panic disorder, with or without agoraphobia, in adults. SEARCH METHODS We conducted the main searches in the CCDANCTR electronic databases (studies and references registers), all years to 16 March 2015. We conducted complementary searches in PubMed and trials registries. Supplementary searches included reference lists of included studies, citation indexes, personal communication to the authors of all included studies and grey literature searches in OpenSIGLE. We applied no restrictions on date, language or publication status. SELECTION CRITERIA We included all relevant randomised controlled trials (RCTs) focusing on adults with a formal diagnosis of panic disorder with or without agoraphobia. We considered the following psychological therapies: psychoeducation (PE), supportive psychotherapy (SP), physiological therapies (PT), behaviour therapy (BT), cognitive therapy (CT), cognitive behaviour therapy (CBT), third-wave CBT (3W) and psychodynamic therapies (PD). We included both individual and group formats. Therapies had to be administered face-to-face. The comparator interventions considered for this review were: no treatment (NT), wait list (WL) and attention/psychological placebo (APP). For this review we considered four short-term (ST) outcomes (ST-remission, ST-response, ST-dropouts, ST-improvement on a continuous scale) and one long-term (LT) outcome (LT-remission/response). DATA COLLECTION AND ANALYSIS As a first step, we conducted a systematic search of all relevant papers according to the inclusion criteria. For each outcome, we then constructed a treatment network in order to clarify the extent to which each type of therapy and each comparison had been investigated in the available literature. Then, for each available comparison, we conducted a random-effects meta-analysis. Subsequently, we performed a network meta-analysis in order to synthesise the available direct evidence with indirect evidence, and to obtain an overall effect size estimate for each possible pair of therapies in the network. Finally, we calculated a probabilistic ranking of the different psychological therapies and control conditions for each outcome. MAIN RESULTS We identified 1432 references; after screening, we included 60 studies in the final qualitative analyses. Among these, 54 (including 3021 patients) were also included in the quantitative analyses. With respect to the analyses for the first of our primary outcomes, (short-term remission), the most studied of the included psychological therapies was CBT (32 studies), followed by BT (12 studies), PT (10 studies), CT (three studies), SP (three studies) and PD (two studies).The quality of the evidence for the entire network was found to be low for all outcomes. The quality of the evidence for CBT vs NT, CBT vs SP and CBT vs PD was low to very low, depending on the outcome. The majority of the included studies were at unclear risk of bias with regard to the randomisation process. We found almost half of the included studies to be at high risk of attrition bias and detection bias. We also found selective outcome reporting bias to be present and we strongly suspected publication bias. Finally, we found almost half of the included studies to be at high risk of researcher allegiance bias.Overall the networks appeared to be well connected, but were generally underpowered to detect any important disagreement between direct and indirect evidence. The results showed the superiority of psychological therapies over the WL condition, although this finding was amplified by evident small study effects (SSE). The NMAs for ST-remission, ST-response and ST-improvement on a continuous scale showed well-replicated evidence in favour of CBT, as well as some sparse but relevant evidence in favour of PD and SP, over other therapies. In terms of ST-dropouts, PD and 3W showed better tolerability over other psychological therapies in the short term. In the long term, CBT and PD showed the highest level of remission/response, suggesting that the effects of these two treatments may be more stable with respect to other psychological therapies. However, all the mentioned differences among active treatments must be interpreted while taking into account that in most cases the effect sizes were small and/or results were imprecise. AUTHORS' CONCLUSIONS There is no high-quality, unequivocal evidence to support one psychological therapy over the others for the treatment of panic disorder with or without agoraphobia in adults. However, the results show that CBT - the most extensively studied among the included psychological therapies - was often superior to other therapies, although the effect size was small and the level of precision was often insufficient or clinically irrelevant. In the only two studies available that explored PD, this treatment showed promising results, although further research is needed in order to better explore the relative efficacy of PD with respect to CBT. Furthermore, PD appeared to be the best tolerated (in terms of ST-dropouts) among psychological treatments. Unexpectedly, we found some evidence in support of the possible viability of non-specific supportive psychotherapy for the treatment of panic disorder; however, the results concerning SP should be interpreted cautiously because of the sparsity of evidence regarding this treatment and, as in the case of PD, further research is needed to explore this issue. Behaviour therapy did not appear to be a valid alternative to CBT as a first-line treatment for patients with panic disorder with or without agoraphobia.
