The study presented in this paper reviewed 9,358 accidents which occurred in the U.S. construction industry between 2002 and 2011, in order to understand the relationships between the risk factors and injury severity (e.g. fatalities, hospitalized injuries, or non-hospitalized injuries) and to develop a strategic prevention plan to reduce the likelihood of fatalities where an accident is unavoidable. The study specifically aims to: (1) verify the relationships among risk factors, accident types, and injury severity, (2) determine significant risk factors associated with each accident type that are highly correlated to injury severity, and (3) analyze the impact of the identified key factors on accident and fatality occurrence. The analysis results explained that safety managers’ roles are critical to reducing human-related risks—particularly misjudgement of hazardous situations—through safety training and education, appropriate use of safety devices and proper safety inspection. However, for environment-related factors, the dominant risk factors were different depending on the different accident types. The outcomes of this study will assist safety managers to understand the nature of construction accidents and plan for strategic risk mitigation by prioritizing high frequency risk factors to effectively control accident occurrence and manage the likelihood of fatal injuries on construction sites.

A number of Intelligent Transportation Systems (ITS) were used with an advanced driving simulator to assess its influence on driving behavior. Three types of ITS interventions namely, Video in-vehicle (ITS1), Audio in-vehicle (ITS2), and On-road flashing marker (ITS3) were tested. Then, the results from the driving simulator were used as inputs for a developed model using a traffic micro-simulation (Vissim 5.4) in order to assess the safety interventions. Using a driving simulator, 58 participants were required to drive through a number of active and passive crossings with and without an ITS device and in the presence or absence of an approaching train. The effect of driver behavior changing in terms of speed and compliance rate was greater at passive crossings than at active crossings. The difference in speed of drivers approaching ITS devices was very small which indicates that ITS helps drivers encounter the crossings in a safer way. Since the current traffic simulation was not able to replicate a dynamic speed change or a probability of stopping that varies based on different ITS safety devices, some modifications of the current traffic simulation were conducted. The results showed that exposure to ITS devices at active crossings did not influence the drivers’ behavior significantly according to the traffic performance indicators used, such as delay time, number of stops, speed, and stopped delay. On the other hand, the results of traffic simulation for passive crossings, where low traffic volumes and low train headway normally occur, showed that ITS devices improved overall traffic performance.

It deals with a large-sized press that, despite being endowed with a light curtain in areas of access to the pressing zone, did not meet legal requirements. The safety devices gave rise to an illusion of safety, permitting activation of the machine when a worker was still found within the operational zone. Preventive interventions must stimulate the tailoring of systems to the characteristics of workers, minimizing the creation of traps and encouraging safety policies and practices that replace judgments of behaviors that participate in accidents by analyses of reasons that lead workers to act in that manner.

This study has the purpose of determining the barriers and facilitators to nurses' acceptance of the Johnson and Johnson Protectiv®* Plus IV catheter safety needle device and implications for needlestick injuries at St. Luke's Episcopal Hospital, Houston, Texas. A one-time cross-sectional survey of 620 responding nurses was conducted by this researcher during December, 2000. The study objectives were to: (1) describe the perceived (a) organizational and individual barriers and facilitators and (b) acceptance of implementation of the IV catheter device; (2) examine the relative importance of these predictors; (3) describe (a) perceived changes in needlestick injuries after implementation of the device; (b) the reported incidence of injuries; and (c) the extent of underreporting by nurses; and (4) examine the relative importance of (a) the preceding predictors and (b) acceptance of the device in predicting perceived changes in needlestick injuries. Safety climate and training were evaluated as organizational factors. Individual factors evaluated were experience with the device, including time using it and frequency of use, and background information, including nursing unit, and length of time as a nurse in this hospital and in total nursing career. The conceptual framework was based upon the safety climate model. Descriptive statistics and multiple and logistic regression were utilized to address the study objectives. ^ The findings showed widespread acceptance of the device and a strong perception that it reduced the number of needlesticks. Acceptance was notably predicted by adequate training, appropriate time between training and device use, solid safety climate, and short length of service, in that order. A barrier to acceptance was nurses' longtime of use of previous needle technologies. Over four-fifths of nurses were compliant in always using the device. Compliance had two facilitators: length of time using device and, to a lesser extent, safety climate. Rates of compliance tended to be lower among nurses in units in which the device was frequently used. ^ High quality training and an atmosphere of caring about nurse safety stand out as primary facilitators that other institutions would need to adopt in order to achieve maximum success in implementing safety programs involving utilization of new safety devices. ^

Heavy-vehicle driving involves a challenging work environment and a high crash rate. We investigated the associations of sleepiness, sleep disorders, and work environment (including truck characteristics) with the risk of crashing between 2008 and 2011 in the Australian states of New South Wales and Western Australia. We conducted a case-control study of 530 heavy-vehicle drivers who had recently crashed and 517 heavy-vehicle drivers who had not. Drivers' crash histories, truck details, driving schedules, payment rates, sleep patterns, and measures of health were collected. Subjects wore a nasal flow monitor for 1 night to assess for obstructive sleep apnea. Driving schedules that included the period between midnight and 5:59 am were associated with increased likelihood of crashing (odds ratio = 3.42, 95% confidence interval: 2.04, 5.74), as were having an empty load (odds ratio = 2.61, 95% confidence interval: 1.72, 3.97) and being a less experienced driver (odds ratio = 3.25, 95% confidence interval: 2.37, 4.46). Not taking regular breaks and the lack of vehicle safety devices were also associated with increased crash risk. Despite the high prevalence of obstructive sleep apnea, it was not associated with the risk of a heavy-vehicle nonfatal, nonsevere crash. Scheduling of driving to avoid midnight-to-dawn driving and the use of more frequent rest breaks are likely to reduce the risk of heavy-vehicle nonfatal, nonsevere crashes by 2–3 times.

Seat belts are effective safety devices used to protect car occupants from severe injuries and fatalities during road vehicle accidents. Despite the proven effectiveness of seat belts, seat belt use rates are quite low, especially in developing countries, such as Turkey. The general aim of the present study was to investigate a large variety of factors related to seat belt use among Turkish car occupants using different perspectives and methods and therefore, to contribute to the design of effective seat belt use interventions for increasing seat belt use rates in Turkey. Five sub-studies were conducted within the present study. In the first sub-study, environmental (e.g., road type) and psycho-social factors (e.g., belt use by other car occupants) related to the seat belt use of front-seat occupants were investigated using observation techniques. Being male, of a young age, and traveling on city roads were the main factors negatively related to seat belt use. Furthermore, seat belt use by the drivers and front-seat passengers was highly correlated and a significant predictors of each other. In the second sub-study, the motivations of the car occupants for seat belt use and non-use were investigated using interview techniques. Situational conditions, such as traveling on city roads and for short distances, and not believing in the effectiveness and relevance of seat belt use for safety, were the most frequently reported reasons for not using a seat belt. Safety, habit and avoiding punishment were among the most frequently reported reasons for using a seat belt. In the third sub-study, the Theory of Planned Behavior (TPB) and the Health Belief Model (HBM) were applied to seat belt use using Structural Equation Modeling techniques. The TPB model showed a good fit to the data, whereas the HBM showed a poor fit to the data. Within the TPB model, attitude and subjective norm were significant predictors of intentions to use a seat belt on both urban and rural roads. In the fourth sub-study, seat belt use frequency and motivations for seat belt use among taxi drivers were investigated and compared between free-time and work-time driving using a survey. The results showed that taxi drivers used seat belts more when driving a private car in their free-times compared to when driving a taxi during their work-times. The lack of a legal obligation to use a seat belt in city traffic and fear of being attacked or robbed by the passengers were found as two specific reasons for not using a seat belt when driving a taxi. Lastly, in the fifth sub-study, the relationship of seat belt use to driver and health behaviors was investigated using a survey. Although seat belt use was related both to health and driver behaviors, factor analysis results showed that it grouped with driver behaviors. Based on the results of the sub-studies, a tentative empirical model showing different predictors of seat belt use was proposed. According to the model, safety and normative motivations and perceived physical barriers related to seat belt use are the three important predictors of seat belt use. Pesquisa realizada em um Hospital Universitário do Estado do Rio de Janeiro, através de uma abordagem quantitativa descritiva, com objetivo de identificar os fatores de riscos ambientais presentes nas situações de trabalho dos profissionais de enfermagem, a partir da observação sistemática dos locais de trabalho pelos profissionais de saúde e segurança do trabalho e dos chefes de enfermagem de clínicas de um Hospital Universitário, visando gerar resultados que possam trazer a discussão, os riscos ocupacionais aos quais estão expostos os profissionais de enfermagem, seu conhecimento a respeito destes riscos e sua atuação na identificação e ação sobre os mesmos. A população foi composta por treis profissionais de saúde e segurança no trabalho e trinta enfermeiros chefes de unidade de internação. Para a coleta de dados foi utilizado um questionário fechado proposto no Guia de Avaliação de Riscos nos Locais de Trabalho de Boix e Vogel (1997) e adaptado para aplicação em estabelecimentos de saúde por Mauro (2001). Os dados foram analisados através do software Statical Package for the Social Sciences (SPSS) versão 15.0. Os resultados evidenciaram que os fatores de riscos ocupacionais de maior relevância do estudo foram: os sistemas inadequados de prevenção de incêndio, de saída de emergência e dispositivos e instruções de segurança e manutenção preventiva inadequada, exposição à riscos biológicos, desenho arquitetônico dos locais de trabalho inadequado, distribuição inadequada de pessoal e conhecimento ergonômico insuficiente do trabalhador. Estes fatores atuam de forma direta ou indireta nos locais de trabalho, propiciando aos profissionais um ambiente desfavorável para a realização das atividades, o que pode comprometer a sua saúde e vida profissional. Concluiu-se que os profissionais enfermeiros no cargo de gestores, em sua maioria, não possuem a visibilidade sobre os fatores de riscos aos quais eles próprios e a equipe sob sua gerência encontram-se expostos, mesmo porque desempenham suas tarefas quase em sua integralidade com alto risco de acidentes e doenças. O estudo proporcionou melhor compreensão dos fatores de risco presentes no ambiente, suas repercussões no processo de trabalho de enfermagem e na saúde dos profissionais, da importância da inserção e comprometimento dos gestores sobre os fatores de risco no ambiente de trabalho e da ergonomia participativa na análise e prevenção de riscos ocupacionais.

O potencial eólico do Brasil, de vento firme e com viabilidade econômica de aproveitamento, é de 143 GW. Isso equivale ao dobro de toda a capacidade da geração já instalada no país. No Brasil, a energia eólica tem uma sazonalidade complementar à energia hidrelétrica, porque os períodos de melhor condição de vento coincidem com os de menor capacidade dos reservatórios. O projeto desenvolvido neste trabalho nasceu de uma chamada pública do FINEP, e sob os auspícios do recém criado CEPER. Ao projeto foi incorporado um caráter investigativo, de contribuição científica original, resultando em um produto de tecnologia inovadora para aerogeradores de baixa potência. Dentre os objetivos do projeto, destacamos a avaliação experimental de turbinas eólicas de 5000 W de potência. Mais especificamente, dentro do objetivo geral deste projeto estão incluídas análise estrutural, análise aerodinâmica e análise de viabilidade de novos materiais a serem empregados. Para cada uma das diferentes áreas de conhecimento que compõem o projeto, será adotada a metodologia mais adequada. Para a Análise aerodinâmica foi realizada uma simulação numérica preliminar seguida de ensaios experimentais em túnel de vento. A descrição dos procedimentos adotados é apresentada no Capítulo 3. O Capítulo 4 é dedicado aos testes elétricos. Nesta etapa, foi desenvolvido um banco de testes para obtenção das características específicas das máquinas-base, como curvas de potência, rendimento elétrico, análise e perdas mecânicas e elétricas, e aquecimento. Este capítulo termina com a análise crítica dos valores obtidos. Foram realizados testes de campo de todo o conjunto montado. Atualmente, o aerogerador de 5kW encontra-se em operação, instrumentado e equipado com sistema de aquisição de dados para consolidação dos testes de confiabilidade. Os testes de campo estão ocorrendo na cidade de Campos, RJ, e abrangeram as seguintes dimensões de análise; testes de eficiência para determinação da curva de potência, níveis de ruído e atuação de dispositivos de segurança. Os resultados esperados pelo projeto foram atingidos, consolidando o projeto de um aerogerador de 5000W.

Esta dissertação analisa o significado das Unidades de Polícia Pacificadora (UPP) enquanto técnica de controle incorporada ao espaço, usando como pano de fundo a fragmentação do tecido sociopolítico-espacial em curso na cidade do Rio de Janeiro que, junto com o biopoder Foucaultiano, propicia a formação das UPPs como novos dispositivos de segurança protetores da Raça Pura e assim, possam legitimar o poder do Estado. Além disso, a presente dissertação observa o significado da favela para a cidade do Rio de Janeiro e a regulação de poder dentro dela no período Pós-UPP, em especial, a forma de atuação dos principais agentes reguladores desse poder: traficantes de drogas armados; moradores e policiais. Finalmente, mede o grau de violência na Santa Marta antes e depois da UPP, usando os dados do Instituto de Segurança Pública (ISP) e entrevistas realizadas durante a pesquisa além de estabelecer alguns limites e possibilidades dessa nova política no que tange a prevenção da violência, usando três categorias de análise: redistributiva; institucionalista e culturalista.

Une préoccupation grandissante face aux risques biologiques reliés aux maladies infectieuses est palpable tant au niveau international que national ou provincial. Des maladies émergentes telles que le SRAS ou la grippe A/H1N1 ont amené une prise en charge des risques pandémiques et à l’élaboration de mesures d’urgence pour maîtriser ces risques : développer une culture de sécurité est devenu une priorité de recherche de l’OMS. Malgré tout, peu d’écrits existent face à cette volonté de sécuriser la santé et le bien-être par toute une série de dispositifs au sein desquels les discours occupent une place importante en matière de culture de sécurité face aux risques biologiques. Une réflexion sociopolitique était nécessaire pour les infirmières qui sont aux premières loges en dispensant des soins à la population dans une perspective de prévention et de contrôle des infections (PCI) dans laquelle elles se spécialisent. Dès lors, ce projet avait pour buts d’explorer la perception du risque et de la sécurité face aux maladies infectieuses auprès des infirmières cliniciennes et gestionnaires québécoises; d’explorer plus spécifiquement l'existence ou l'absence de culture de sécurité dans un centre de santé et de services sociaux québécois (CSSS); et d’explorer les discours en présence dans le CSSS en matière de sécurité et de risques biologiques face aux maladies infectieuses et comment ces discours de sécurité face aux risques biologiques se traduisent dans le quotidien des infirmières. Les risques biologiques sont perçus comme identifiables, mesurables et évitables dans la mesure où les infirmières appliquent les mesures de préventions et contrôle des infections, ce qui s’inscrit dans une perspective positiviste du risque (Lupton, 1999). La gestion de ces risques se décline au travers de rituels de purification et de protection afin de se protéger de toute maladie infectieuse. Face à ces risques, une culture de sécurité unique est en émergence dans le CSSS dans une perspective de prévention de la maladie. Toutefois, cette culture de sécurité désirée est confrontée à une mosaïque de cultures qui couvrent différentes façons d’appliquer ou non les mesures de PCI selon les participants. La contribution de cette recherche est pertinente dans ce nouveau domaine de spécialité que constituent la prévention et le contrôle des infections pour les infirmières québécoises. Une analyse critique des relations de pouvoir tel qu’entendu par Foucault a permis de soulever les questions de surveillance infirmière, de politique de l’aveu valorisée, de punition de tout écart à l’application rigoureuse des normes de PCI, de contrôle de la part des cadres infirmiers et d’assujettissement des corps relevant des mécanismes disciplinaires. Elle a permis également de documenter la présence de dispositifs de sécurité en lien avec la tenue de statistiques sur les patients qui sont répertoriés en tant que cas infectieux, mais également en termes de circulation des personnes au sein de l’établissement. La présence d’un pouvoir pastoral est perceptible dans la traduction du rôle d’infirmière gestionnaire qui doit s’assurer que ses équipes agissent de la bonne façon et appliquent les normes de PCI privilégiées au sein du CSSS afin de réguler les taux d’infections nosocomiales présents dans l’établissement. En cas de non-respect des mesures de PCI touchant à l’hygiène des mains ou à la vaccination, l’infirmière s’expose à des mesures disciplinaires passant de l’avertissement, la relocalisation, l’exclusion ou la suspension de l’emploi. Une culture du blâme a été décrite par la recherche d’un coupable au sein de l’institution, particulièrement en temps de pandémie. Au CSSS, l’Autre est perçu comme étant à l’origine de la contamination, tandis que le Soi est perçu comme à l’abri de tout risque à partir du moment où l’infirmière respecte les normes d’hygiène de vie en termes de saines habitudes alimentaires et d’activité physique. Par ailleurs, les infirmières se doivent de respecter des normes de PCI qu’elles connaissent peu, puisque les participantes à la recherche ont souligné le manque de formation académique et continue quant aux maladies infectieuses, aux risques biologiques et à la culture de sécurité qu’elles considèrent pourtant comme des sujets priorisés par leur établissement de santé. Le pouvoir produit des effets sur les corps en les modifiant. Cette étude ethnographique critique a permis de soulever les enjeux sociopolitiques reliés aux discours en présence et de mettre en lumière ce que Foucault a appelé le gouvernement des corps et ses effets qui se capillarisent dans le quotidien des infirmières. Des recherches ultérieures sont nécessaires afin d’approfondir ce champ de spécialité de notre discipline infirmière et de mieux arrimer la formation académique et continue aux réalités infectieuses cliniques.

Entre la publication de La volonté de savoir en 1976 et les derniers ouvrages publiés en 1984, un certain silence recouvre l’œuvre de Michel Foucault. Mais cet apparent mutisme s’estompe aujourd’hui, grâce à la publication et à la réception progressive de ses cours — notamment ceux professés au Collège de France en 1977-1978. Ils nous permettent en effet de porter un regard nouveau sur le parcours philosophique de Foucault, éclairant tout le pan d’une œuvre qui allait par la suite s’enrichir d’un êthos éthico-critique. En ce sens, il nous a semblé important de revenir sur quelques unes des notions-clés qui conditionneront ses derniers travaux. Ainsi, nous nous proposons à la fois d’esquisser la genèse du biopouvoir ­ qui s’articule autour du « concept » capital de population ­ et de décrire les traits généraux des dispositifs de sécurité : thèmes qui offrent une résonance toute contemporaine et qui permettent de problématiser le présent de notre avenir.
