Almost 10% of all births are preterm and 2.2% are stillbirths globally. Recent research has suggested that environmental factors may be a contributory cause to these adverse birth outcomes. The authors examined the relationship between ambient temperature and preterm birth and stillbirth in Brisbane, Australia between 2005 and 2009 (n = 101,870). They used a Cox proportional hazard model with live birth and stillbirth as competing risks. They also examined if there were periods of the pregnancy where exposure to high temperatures had a greater effect. Exposure to higher ambient temperatures during pregnancy increased the risk of stillbirth. The hazard ratio for stillbirth was 0.3 at 12 °C relative to the reference temperature at 21 °C. The temperature effect was greatest for fetuses of less than 36 weeks of gestation. There was an association between higher temperature and shorter gestation, as the hazard ratio for live birth was 0.96 at 15 °C and 1.02 at 25 °C. This effect was greatest at later gestational ages. The results provide strong evidence of an association between increased temperature and increased risk of stillbirth and shorter gestations.

Objective To examine the extent to which the odds of birth, pregnancy, or adverse birth outcomes are higher among women aged 28 to 36 years who use fertility treatment compared with untreated women. Design Prospective, population-based. Setting Not applicable. Patient(s) Participants in the ALSWH born in 1973 to 1978 who reported on their infertility and use of in vitro fertilization (IVF) or ovulation induction (OI). Intervention(s) Postal survey questionnaires administered as part of ALSWH. Main Outcome Measure(s) Among women treated with IVF or OI and untreated women, the odds of birth outcomes estimated by use of adjusted logistic regression modeling. Result(s) Among 7,280 women, 18.6% (n = 1,376) reported infertility. Half (53.0%) of the treated women gave birth compared with 43.8% of untreated women. Women with prior parity were less likely to use IVF compared with nulliparous women. Women using IVF or OI, respectively, were more likely to have given birth after treatment or be pregnant compared with untreated women. Women using IVF or OI were as likely to have ectopic pregnancies, stillbirths, or premature or low birthweight babies as untreated women. Conclusion(s) More than 40% of women aged 28–36 years reporting a history of infertility can achieve births without using treatment, indicating they are subfertile rather than infertile.

Objectives We aimed to use simple clinical questions to group women and provide their specific rates of miscarriage, preterm delivery, and stillbirth for reference. Further, our purpose was to describe who has experienced particularly low or high rates of each event. Methods Data were collected as part of the Australian Longitudinal Study on Women's Health, a national prospective cohort. Reproductive histories were obtained from 5806 women aged 31–36 years in 2009, who had self-reported an outcome for one or more pregnancy. Age at first birth, number of live births, smoking status, fertility problems, use of in vitro fertilisation (IVF), education and physical activity were the variables that best separated women into groups for calculating the rates of miscarriage, preterm delivery, and stillbirth. Results Women reported 10,247 live births, 2544 miscarriages, 1113 preterm deliveries, and 113 stillbirths. Miscarriage was correlated with stillbirth (r = 0.09, P<0.001). The calculable rate of miscarriage ranged from 11.3 to 86.5 miscarriages per 100 live births. Women who had high rates of miscarriage typically had fewer live births, were more likely to smoke and were more likely to have tried unsuccessfully to conceive for ≥12 months. The highest proportion of live preterm delivery (32.2%) occurred in women who had one live birth, had tried unsuccessfully to conceive for ≥12 months, had used IVF, and had 12 years education or equivalent. Women aged 14–19.99 years at their first birth and reported low physical activity had 38.9 stillbirths per 1000 live births, compared to the lowest rate at 5.5 per 1000 live births. Conclusion Different groups of women experience vastly different rates of each adverse pregnancy event. We have used simple questions and established reference data that will stratify women into low- and high-rate groups, which may be useful in counselling those who have experienced miscarriage, preterm delivery, or stillbirth, plus women with fertility intent.

Chapter 5: Fertility and infertility. p52-70. This section describes patterns of fertility across Surveys 1 to 4 among the cohort of women who were born in 1973-1978. This section includes the examination of pregnancy outcomes including both live births and pregnancy losses (stillbirths, miscarriages, terminations and ectopic pregnancies). This section also examines the prevalence of self-reported problems with fertility and whether these women sought advice and/or treatment. As women age they are more likely to experience infertility and, with little other data available, the ALSWH provides an important opportunity to examine this problem and the related use of health services. 1. Pregnancy losses are common. Half of the women who report a pregnancy outcome at Survey 4 have experienced a pregnancy loss. 2. More than one third (39%) of women who have experienced a live birth by Survey 4 have also experienced a pregnancy loss. 3. For every ten women aged 28-33 years in 2006: four women had not had been pregnant, five women had a live birth (with or without a recognised pregnancy loss), and one woman had a recognised pregnancy loss only. 4. Among women who had tried to conceive or had been pregnant, one-in-six had experienced infertility. (i.e. tried unsuccessfully to get pregnant for 12 months or more) 5. The most significant factors associated with having infertility, seeking advice and using treatment were: polycystic ovary syndrome, endometriosis and miscarriage. 6. Of the women who reported infertility, two-thirds sought advice but only half used treatment. 7. Most of the women who used fertility treatment had used low cost and non-invasive methods.

Background The population exposed to potentially hazardous substances through inappropriate and unsafe management practices related to disposal and recycling of end-of-life electrical and electronic equipment, collectively known as e-waste, is increasing. We aimed to summarise the evidence for the association between such exposures and adverse health outcomes. Methods We systematically searched five electronic databases (PubMed, Embase, Web of Science, PsycNET, and CINAHL) for studies assessing the association between exposure to e-waste and outcomes related to mental health and neurodevelopment, physical health, education, and violence and criminal behaviour, from Jan 1, 1965, to Dec 17, 2012, and yielded 2274 records. Of the 165 full-text articles assessed for eligibility, we excluded a further 142, resulting in the inclusion of 23 published epidemiological studies that met the predetermined criteria. All studies were from southeast China. We assessed evidence of a causal association between exposure to e-waste and health outcomes within the Bradford Hill framework. Findings We recorded plausible outcomes associated with exposure to e-waste including change in thyroid function, changes in cellular expression and function, adverse neonatal outcomes, changes in temperament and behaviour, and decreased lung function. Boys aged 8–9 years living in an e-waste recycling town had a lower forced vital capacity than did those living in a control town. Significant negative correlations between blood chromium concentrations and forced vital capacity in children aged 11 and 13 years were also reported. Findings from most studies showed increases in spontaneous abortions, stillbirths, and premature births, and reduced birthweights and birth lengths associated with exposure to e-waste. People living in e-waste recycling towns or working in e-waste recycling had evidence of greater DNA damage than did those living in control towns. Studies of the effects of exposure to e-waste on thyroid function were not consistent. One study related exposure to e-waste and waste electrical and electronic equipment to educational outcomes. Interpretation Although data suggest that exposure to e-waste is harmful to health, more well designed epidemiological investigations in vulnerable populations, especially pregnant women and children, are needed to confirm these associations. Background: Many European countries including Ireland lack high quality, on-going, population based estimates of maternal behaviours and experiences during pregnancy. PRAMS is a CDC surveillance program which was established in the United States in 1987 to generate high quality, population based data to reduce infant mortality rates and improve maternal and infant health. PRAMS is the only on-going population based surveillance system of maternal behaviours and experiences that occur before, during and after pregnancy worldwide.Methods: The objective of this study was to adapt, test and evaluate a modified CDC PRAMS methodology in Ireland. The birth certificate file which is the standard approach to sampling for PRAMS in the United States was not available for the PRAMS Ireland study. Consequently, delivery record books for the period between 3 and 5 months before the study start date at a large urban obstetric hospital [8,900 births per year] were used to randomly sample 124 women. Name, address, maternal age, infant sex, gestational age at delivery, delivery method, APGAR score and birth weight were manually extracted from records. Stillbirths and early neonatal deaths were excluded using APGAR scores and hospital records. Women were sent a letter of invitation to participate including option to opt out, followed by a modified PRAMS survey, a reminder letter and a final survey.Results: The response rate for the pilot was 67%. Two per cent of women refused the survey, 7% opted out of the study and 24% did not respond. Survey items were at least 88% complete for all 82 respondents. Prevalence estimates of socially undesirable behaviours such as alcohol consumption during pregnancy were high [>50%] and comparable with international estimates.Conclusion: PRAMS is a feasible and valid method of collecting information on maternal experiences and behaviours during pregnancy in Ireland. PRAMS may offer a potential solution to data deficits in maternal health behaviour indicators in Ireland with further work. This study is important to researchers in Europe and elsewhere who may be interested in new ways of tailoring an established CDC methodology to their unique settings to resolve data deficits in maternal health.

OBJECTIVE: The purpose of this article is to present the specific public health indicators recently developed by EUROCAT that aim to summarize important aspects of the public health impact of congenital anomalies in a few quantitative measures. METHODS: The six indicators are: (1) congenital anomaly perinatal mortality, (2) congenital anomaly prenatal diagnosis prevalence, (3) congenital anomaly termination of pregnancy, (4) Down syndrome livebirth prevalence, (5) congenital anomaly pediatric surgery, and (6) neural tube defects (NTD) total prevalence. Data presented for this report pertained to all cases (livebirths, fetal deaths, or stillbirths after 20 weeks of gestation and terminations of pregnancy for fetal anomaly [TOPFA]) of congenital anomaly from 27 full member registries of EUROCAT that could provide data for at least 3 years during the period 2004 to 2008. Prevalence of anomalies, prenatal diagnosis, TOPFA, pediatric surgery, and perinatal mortality were calculated per 1000 births. RESULTS: The overall perinatal mortality was approximately 1.0 per 1000 births for EUROCAT registries with almost half due to fetal and the other half due to first week deaths. There were wide variations in perinatal mortality across the registries with the highest rates observed in Dublin and Malta, registries in countries where TOPFA are illegal, and in Ukraine. The overall perinatal mortality across EUROCAT registries slightly decreased between 2004 and 2008 due to a decrease in first week deaths. The prevalence of TOPFA was fairly stable at about 4 per 1000 births. There were variations in livebirth prevalence of cases typically requiring surgery across the registries; however, for most registries this prevalence was between 3 and 5 per 1000 births. Prevalence of NTD decreased by about 10% from 1.05 in 2004 to 0.94 per 1000 in 2008. CONCLUSION: It is hoped that by publishing the data on EUROCAT indicators, the public health importance of congenital anomalies can be clearly summarized to policy makers, the need for accurate data from registries emphasized, the need for primary prevention and treatment services highlighted, and the impact of current services measured.

L’asthme maternel complique environ 3,4% à 12,4% des grossesses dans les pays développés ce qui en fait une des maladies chroniques les plus fréquentes pouvant engendrer de sérieux problèmes médicaux chez la mère et le fœtus. D’autre part, un taux relativement important de femmes enceintes, soit 4 à 7%, utilisent des médicaments anti-asthmatiques. La mortinaissance, la mortalité néonatale et/ou la mortalité périnatale sont les issues de grossesses les plus dramatiques pour l’enfant et la famille. Toutefois, l’effet de l’asthme et de l’utilisation des corticostéroïdes inhalés (CSI) pendant la grossesse sur ces complications a été inadéquatement évalué. La majorité des études qui ont évalué ces associations souffraient d’un manque de puissance statistique et/ou d’une absence ou d’un ajustement inadéquat pour les variables potentiellement confondantes. Les travaux présentés dans cette thèse ont donc pour objectif d’évaluer le risque de mortalité périnatale chez les femmes asthmatiques comparativement aux femmes non- asthmatiques. Cette thèse vise également à évaluer si les femmes asthmatiques exposées aux CSI courent plus de risque de mortalité périnatale que les femmes asthmatiques non exposées et si le risque de mortalité périnatale varie en fonction de la dose quotidienne de CSI utilisée par la mère pendant la grossesse. À l’aide du croisement de trois bases de données administratives du Québec, une large cohorte de femmes asthmatiques et non-asthmatiques qui ont eu au moins une grossesse entre 1990 et 2002 a été construite (n=41 142). À partir de cette cohorte, deux cohortes de grossesses ont été constituées. Les deux premières études présentées dans cette thèse sont basées sur toute la cohorte alors que la dernière étude est basée uniquement sur les grossesses de femmes asthmatiques. Une étude de cohorte a d’abord été réalisée afin d’évaluer l’effet de l’asthme maternel sur le risque de mortalité périnatale permettant l’ajustement pour les variables provenant des bases de données administratives. Afin de mieux estimer le risque de mortalité périnatale chez les femmes asthmatiques une étude de cohorte comprenant deux phases d’échantillonnage a ensuite été réalisée à l’aide d’informations additionnelles sur le tabagisme, l’utilisation de drogue illicite et l’histoire de mortinaissances, colligées à partir du dossier médical de la mère. Finalement, le risque de mortalité périnatale chez les femmes asthmatiques qui ont utilisé des CSI pendant la grossesse et le risque de mortalité périnatale en fonction de la dose moyenne quotidienne de CSI consommée par la mère pendant la grossesse ont été investigués à l’aide d’une étude de cohorte à deux phases d’échantillonnage chez les femmes asthmatiques uniquement. Nous avons premièrement observé que l’asthme pendant la grossesse pourrait augmenter le risque de mortalité périnatale due à l’augmentation du risque de bébés de petits poids et de bébés prématurés chez les femmes asthmatiques (OR=1,30; IC 95%: 1,05-1,57). Toutefois, après avoir ajusté pour le tabagisme pendant la grossesse, le risque relatif de mortalité périnatale a diminué à 12% et l’association n’est pas demeurée statistiquement significative (OR= 1,12; IC 95%: 0,87-1,45). Finalement, l’utilisation de CSI pendant la grossesse, lorsque la dose n’a pas été considérée, n’a pas été associé à une augmentation significative du risque de mortalité périnatale (OR= 1,07; IC 95% : 0,70-1,61) et un effet protecteur non-significatif de l’utilisation de doses de CSI de 250 ug ou moins par jour a été observé (OR=0,89; IC 95%: 0,55 -1,44). Toutefois, les femmes qui ont pris des doses >250 ug/jour avaient un risque accru de mortalité périnatale de 52%, mais cette association n’était pas statistiquement significative (OR=1,52; IC 95%: 0,62-3,76). Cette augmentation du risque pourrait toutefois résulter d’un ajustement imparfait pour la sévérité et le contrôle de l’asthme (les femmes asthmatiques qui ont utlisé >250 ug/jour sont susceptibles d’avoir un asthme plus sévère ou inadéquatement maîtrisé). Les conclusions de nos travaux qui sont plutôt rassurantes pourront contribuer à une meilleure prise en charge des femmes enceintes asthmatiques, à aider les médecins dans la prescription de CSI pendant la grossesse et à rassurer les femmes enceintes souffrant d’asthme et les femmes enceintes qui doivent utiliser des CSI. Toutefois, des études supplémentaires sont nécessaires afin de pouvoir conclure que l’utilisation de doses plus élevées de CSI (>250 ug/jour) pendant la grossesse sont sécuritaires.

Introducción. La sífilis congénita es un problema de salud pública en nuestro País debido a que supone la adquisición de un padecimiento crónico con consecuencias muy graves y un alto costo humano, social y económico. Por esta razón, OPS, UNICEF y otras organizaciones han propuesto impulsar la Iniciativa regional para la eliminación de la sífilis congénita en América Latina y el Caribe con una meta de 0.5 casos o menos por 1000 nacidos vivos incluyendo mortinatos para el año 2015. En Colombia se ha apoyado esta iniciativa, pero a pesar de los esfuerzos realizados por diferentes sectores involucrados en el seguimiento y control de este evento, no ha sido eliminado, por el contrario la incidencia va en aumento pasando de 0.90 casos por 1.000 nacidos vivos en 1998 a 2.1 en 2009. Metodología. Se realizó un estudio descriptivo retrospectivo de fuente secundaria utilizando los datos suministrados por el Sivigila para los años 2008–2009. Resultados. Se calcula una incidencia en 2008 de 2.5 y en el año siguiente 2.1 casos por 1000 nacidos vivos. De los casos notificados el 98.2% y 97.4% respectivamente nacieron vivos, el 71.6% y 73.0% recibieron tratamiento completo y se evidencia que el 66.2% y 65.1% de las madres de niños notificados como casos de sífilis congénita asistieron al control prenatal. Discusión. Se plantea la discordancia de estos resultados con respecto a la meta planteada, la no notificación de los mortinatos y abortos, el tratamiento inadecuado y las deficiencias en el control prenatal.

This study compares the infant mortality profiles of 128 infants from two urban and two rural cemetery sites in medieval England. The aim of this paper is to assess the impact of urbanization and industrialization in terms of endogenous or exogenous causes of death. In order to undertake this analysis, two different methods of estimating gestational age from long bone lengths were used: a traditional regression method and a Bayesian method. The regression method tended to produce more marked peaks at 38 weeks, while the Bayesian method produced a broader range of ages and were more comparable with the expected "natural" mortality profiles. At all the sites, neonatal mortality (28-40 weeks) outweighed post-neonatal mortality (41-48 weeks) with rural Raunds Furnells in Northamptonshire, showing the highest number of neonatal deaths and post-medieval Spitalfields, London, showing a greater proportion of deaths due to exogenous or environmental factors. Of the four sites under study, Wharram Percy in Yorkshire showed the most convincing "natural" infant mortality profile, suggesting the inclusion of all births (i.e., stillbirths and unbaptised infants).
