The antimicrobial susceptibility of 176 unusual non-fermentative gram-negative bacilli (NF-GNB) collected from Latin America region through the SENTRY Program between 1997 and 2002 was evaluated by broth microdilution according to the National Committee for Clinical Laboratory Standards (NCCLS) recommendations. Nearly 74% of the NF-BGN belonged to the following genera/species: Burkholderia spp. (83), Achromobacter spp. (25), Ralstonia pickettii (16), Alcaligenes spp. (12), and Cryseobacterium spp. (12). Generally, trimethoprim/sulfamethoxazole (MIC50, < 0.5 µg/ml) was the most potent drug followed by levofloxacin (MIC50, 0.5 µg/ml), and gatifloxacin (MIC50, 1 µg/ml). The highest susceptibility rates were observed for levofloxacin (78.3%), gatifloxacin (75.6%), and meropenem (72.6%). Ceftazidime (MIC50, 4 µg/ml; 83.1% susceptible) was the most active beta-lactam against B. cepacia. Against Achromobacter spp. isolates, meropenem (MIC50, 0.25 µg/ml; 88% susceptible) was more active than imipenem (MIC50, 2 µg/ml). Cefepime (MIC50, 2 µg/ml; 81.3% susceptible), and imipenem (MIC50, 2 µg/ml; 81.3% susceptible) were more active than ceftazidime (MIC50, >16 µg/ml; 18.8% susceptible) and meropenem (MIC50, 8 µg/ml; 50% susceptible) against Ralstonia pickettii. Since selection of the most appropriate antimicrobial agents for testing and reporting has not been established by the NCCLS for many of NF-GNB species, results from large multicenter studies may help to guide the best empiric therapy.

Emerging resistance phenotypes and antimicrobial resistance rates among pathogens recovered from community-acquired urinary tract infections (CA-UTI) is an increasing problem in specific regions, limiting therapeutic options. As part of the SENTRY Antimicrobial Surveillance Program, a total of 611 isolates were collected in 2003 from patients with CA-UTI presenting at Latin American medical centers. Each strain was tested in a central laboratory using Clinical Laboratory Standard Institute (CLSI) broth microdilution methods with appropriate controls. Escherichia coli was the leading pathogen (66%), followed by Klebsiella spp. (7%), Proteus mirabilis (6.4%), Enterococcus spp. (5.6%), and Pseudomonas aeruginosa (4.6%). Surprisingly high resistance rates were recorded for E. coli against first-line orally administered agents for CA-UTI, such as ampicillin (53.6%), TMP/SMX (40.4%), ciprofloxacin (21.6%), and gatifloxacin (17.1%). Decreased susceptibility rates to TMP/SMX and ciprofloxacin were also documented for Klebsiella spp. (79.1 and 81.4%, respectively), and P. mirabilis (71.8 and 84.6%, respectively). For Enterococcus spp., susceptibility rates to ampicillin, chloramphenicol, ciprofloxacin, and vancomycin were 88.2, 85.3, 55.9, and 97.1%, respectively. High-level resistance to gentamicin was detected in 24% of Enterococcus spp. Bacteria isolated from patients with CA-UTI in Latin America showed limited susceptibility to orally administered antimicrobials, especially for TMP/SMX and fluoroquinolones. Our results highlight the need for developing specific CA-UTI guidelines in geographic regions where elevated resistance to new and old compounds may influence prescribing decisions.

INTRODUCCIÓN: Los carbapenémicos (CP) son una de las últimas líneas de tratamiento para infecciones por microorganismos multirresistentes (MDR), especialmente Gram-negativos productores de betalactamasas de espectro extendido. Es creciente la preocupación a nivel mundial por el aumento de aislamientos resistentes a CP, en EEUU hasta 60% de las infecciones nosocomiales son causadas por bacterias MDR. En la Unión Europea, cerca de 25.000 pacientes mueren anualmente por esta causa. En Latinoamérica hay una tendencia creciente en las tasas de resistencia.OBJETIVO: Identificar y describir factores protectores o de riesgo, relacionados con colonización o infección por Gram negativos resistentes a CP en pacientes adultos hospitalizados, mediante una revisión sistemática de la literatura.MÉTODOS: Revisión sistemática de literatura, búsqueda de estudios observacionales analíticos en las bases de datos PubMed, Embase, Scopus, BVS, Scielo y búsqueda de literatura gris, publicados desde el 01/01/2004 al 15/04/2015. Se evalúo la calidad de los estudios con escala Newcastle-Ottawa y FLC Osteba. RESULTADOS: Se seleccionaron 36 estudios de alta calidad, diseño de casos y controles. Los factores de riesgo estadísticamente significativos observados son estancia en UCI OR:36.46, insuficiencia renal aguda OR:6.23, diálisis OR:10.80 ventilación mecánica OR:17.5, cateterismo vesical OR:14.3, uso de carbapenémicos OR:18,52,quinolonas OR17.30, cefepime OR:28.05, glicopéptidos OR:19.1; metronidazol OR:4.17, p:0.03, colistina OR:12.1, linezolid OR:7 CONCLUSIÓN: Pese a que hay alta heterogeneidad en las variables incluidas en los estudios, se encontró que los factores de riesgo principales para adquirir GNR-CP en pacientes hospitalizados son: antecedente de insuficiencia renal aguda y diálisis, ventilación mecánica, cateterismo vesical, estancia en UCI y uso previo de antibióticos carbapenémicos, quinolonas, cefepime, glicopéptidos, metronidazol, linezolid y colistina. No se hallaron factores protectores. factores de riesgo

Se realizó un estudio descriptivo, retrospectivo; se usó la base de datos de los aislamientos microbiológicos documentados en las UCI de la Fundación Santa fe de Bogotá para el año 2014. La prevalencia de bacterias resistentes en los aislamientos de la FSFB no es baja, por lo que se requiere una terapia empírica acertada acorde con la flora local. Se requieren estudios analíticos para evaluar factores asociados al desarrollo de gérmenes multi resistentes y mortalidad por sepsis

In a hospital environment, these bacteria can be spread by insects such as ants, which are characterized by high adaptability to the urban environment. Staphylococcus is a leading cause of hospital infection. In Europe, Latin America, USA and Canada, the group of coagulase negative staphylococci (CoNS) is the second leading cause of these infections, according to SENTRY (antimicrobial surveillance program- EUA). In this study, we investigated the potential of ants (Hymenoptera: Formicidae) as vehicle mechanics of Staphylococcus bacteria in a public hospital, in Natal-RN. The ants were collected, day and night, from June 2007 to may 2008, in the following sectors: hospitals, laundry, kitchen, blood bank. The ants were identified according to the identification key of Bolton, 1997. For the analysis of staphylococci, the ants were incubated in broth Tryptic Soy Broth (TSB) for 24 hours at 35 º C and then incubated on Mannitol Salt Agar. The typical colonies of staphylococci incubated for 24 hours at 35 ° C in Tryptic Soy Agar for the characterization tests (Gram stain, catalase, susceptibility to bacitracin and free coagulase). The identification of CoNS was performed through biochemical tests: susceptibility to novobiocin, growth under anaerobic conditions, presence of urease, the ornithine decarboxylation and acid production from the sugars mannose, maltose, trehalose, mannitol and xylose. The antimicrobial susceptibility examined by disk-diffusion technique. The technique of Polymerase Chain Reaction was used to confirm the presence of mecA gene and the ability to produce biofilm was verified by testing in vitro using polystyrene inert surface, in samples of resistant staphylococci. Among 440 ants, 85 (19.1%) were carrying coagulase-negative staphylococci (CoNS) of the species Staphylococcus saprophyticus (17), Staphylococcus epidermidis (15), Staphylococcus xylosus (13), Staphylococcus hominis hominis (10), Staphylococcus lugdunensis (10), Staphylococcus warneri (6), Staphylococcus cohnii urealyticum (5), Staphylococcus haemolyticus (3), Staphylococcus simulans (3), Staphylococcus cohnii cohnii (2), and Staphylococcus capitis (1). No Staphylococcus aureus was found. Among the isolates, 30.58% showed resistance to erythromycin. Two samples of CoNS (2.35%), obtained from the ant Tapinoma melanocephalum collected in the post-surgical female ward, S. Hominis hominis and S. lugdunensis harbored the mecA gene and were resistant to multiple antibiotics, and the specie S. hominis hominis even showed to be a biofilm producer. This study proves that ants act as carriers of multidrug-resistant coagulase-negative Staphylococci and biofilm producers and points to the risk of the spreading of pathogenic microorganisms by this insect in the hospital environment

This study aimed to assess knowledge of nursing professionals (nurses, technicians and nursing assistants) on the Project Sentinel Hospitals in a state hospital in the interior of and identify the occurrence of under-reporting of hospital products and the reasons that prevented the team to notify them. This is a study of a descriptive and exploratory, held in a public hospital in the interior of which serves only patients of the Unified Health System (SUS). The study sample consisted of 245 nurses. Data analysis revealed that the majority of nurses and nursing assistants refers knowledge about Project Sentinel Hospitals. In relation to the Practical nurses less than half know Hospitals Project Sentry. Knowledge of the four spheres of Hospitals Project Sentinel is higher among nurses. Among the practical nurses and nursing assistants prevailing knowledge in the area of Pharmacovigilance. The nurses have more knowledge about the process of notification and are major notifiers. Technicians and nursing assistants are those who have greater interest in learning content as a whole Hospitals Project Sentry. The nurses were the professionals who witnessed the most under-and under-reporting associated with the fear, lack of knowledge on the subject, insecurity and lack of time. It can be concluded that the study identified the knowledge of nurses on Project Sentinel Hospitals and the procedure for notification of technical defects and adverse events related to health products as well as identify the presence of under-reporting among professionals. In addition, the study shows the importance and need for greater disclosure of the activities developed by Project Sentinel Hospitals among nursing professionals through meetings, lectures, brochures, among other outlets

La tesi è il risultato di un tirocinio, della durata di cinque mesi, svolto presso l'Azienda 'Aliva', che si occupa di sistemi di facciate. Lo scopo della tesi è la realizzazione di un nuovo sistema di fissaggio a scomparsa di lastre in vetro. Tutta la ricerca è scaturita dalla richiesta dell'azienda per realizzare un nuovo prodotto di fissaggio di lastre di grandi dimensioni in vetro con impatto visivo molto ridotto. Il sistema unisce due tecnologie: - sistema meccanico; - sistema adesivo strutturale di alte performance. Partendo da dei test di laboratorio, la tesi consisterà nell'effettuare le verifiche agli elementi finiti, con l'ausilio del software Strauss7, di lastre in vetro stratificato e vetro camera.

Analisi riguardante la tenacizzazione della matrice di laminati compositi. Lo scopo è quello di aumentare la resistenza alla frattura di modo I e, a tal proposito, sono stati modificati gli interstrati di alcuni provini tramite l’introduzione di strati, di diverso spessore, di nanofibre in polivinilidenfluoruro (PVDF). La valutazione di tale metodo di rinforzo è stata eseguita servendosi di dati ottenuti tramite prove sperimentali svolte in laboratorio direttamente dal sottoscritto, che si è occupato dell’elaborazione dei dati servendosi di tecniche e algoritmi di recente ideazione. La necessità primaria per cui si cerca di rinforzare la matrice risiede nel problema più sentito dei laminati compositi in opera da molto tempo: la delaminazione. Oltre a verificare le proprietà meccaniche dei provini modificati sottoponendoli a test DCB, si è utilizzata una tecnica basata sulle emissioni acustiche per comprendere più approfonditamente l’inizio della delaminazione e i meccanismi di rottura che si verificano durante le prove. Quest’ultimi sono illustrati servendosi di un algoritmo di clustering, detto Fuzzy C-means, tramite il quale è stato possibile identificare ogni segnale come appartenente o meno ad un determinato modo di rottura. I risultati mostrano che il PVDF, applicato nelle modalità esposte, è in grado di aumentare la resistenza alla frattura di modo I divenendo contemporaneamente causa di un diverso modo di propagazione della frattura. Infine l’elaborato presenta alcune micrografie delle superfici di rottura, le quali appoggiano i risultati ottenuti nelle precedenti fasi di analisi.

No âmbito do Relatório Científico Final do Trabalho de Investigação Aplicada, o presente trabalho com o título “Os Recursos Cinotécnicos e a sua necessidade no Exército Português” estuda o emprego operacional atual, fruto das valências cinotécnicas existentes e as tarefas operacionais que podem, pelos referidos recursos, ser apoiadas. O autor considera também as características físicas do cão, as implicações logísticas e financeiras e a doutrina que concorrem para o referido emprego operacional, elencando lacunas existentes que, quando colmatadas poderão concorrer para um emprego operacional eficiente dos recursos cinotécnicos do Exército Português. Para obtenção de dados foi conduzida análise documental e bibliográfica e efetuados inquéritos por entrevista a comandantes de Batalhão/ Grupo com meios cinotécnicos na sua orgânica ou em apoio aos mesmos, aos comandantes das unidades cinotécnicas do Exército Português e dos restantes ramos das Forças Armadas e Forças de Segurança, ainda a comandantes da Força de Operações Especiais. O objetivo deste trabalho passa por avaliar de que forma podem os recursos cinotécnicos do Exército Português contribuir para o melhor desempenho das suas unidades operacionais. Para a consecução do mesmo o autor fez uso do método indutivo onde, com base no emprego operacional junto dos Paraquedistas e da Polícia do Exército, generalizou o que poderia ser profícuo para todas as unidades operacionais do Exército Português, bem como todas as lacunas que devem ser colmatadas para apoio ao mesmo. No âmbito da doutrina é importante o fornecimento aos comandantes das unidades apoiadas informação sobre as capacidades, limitações e regras de segurança relativos ao emprego dos binómios em apoio a cada tarefa das tipologias de operações, fornecendo informações precisas e detalhadas sobre o que cada tipo de cão consegue fazer, durante quanto tempo e em que condições ambientais pois, mesmo havendo atualmente a PDE 0-20-18, sendo a cinotecnia uma área muito técnica, a falta da referida doutrina pode se transformar numa condicionante ao emprego dos binómios. A referida PDE, atualmente, assume-se como um potenciador do emprego dos recursos cinotécnicos. No âmbito logístico pode ser pensada uma rede logística própria tendo em conta o emprego operacional e a projeção dos binómios, explícitando desde a forma de aquisição dos cães até ao suporte à sua projeção, contudo deve-se pesar, por outro lado, se o emprego dos binómios em treinos operacionais ou em missões assume um peso significativo na manobra logística global de modo a exigir criação de uma rede própria. As características naturais do cão são um potenciador da ação e sentimento de segurança, podendo, ainda, ser usadas para colmatar a falta de efetivos. Quanto ao emprego operacional importa salientar a deteção de explosivos e estupefacientes, os cães de sentinela, binómios de guarda e binómios de exploração (que em simultâneo executam patrulhas de segurança).
