Objectives. This paper explores the use of regression models for estimating health status of schizophrenic patients, from a Bayesian perspective. Our aims are: 1- To obtain a set of values of health states of the EQ-5D based on self-assessed health from a sample of schizophrenic patients. 2- To analyse the differences in the health status and in patients’ perceptions of their health status between four mental-health districts in Spain. Methods. We develop two linear models with dummy variables. The first model seeks to obtain an index of the health status of the patients using a VAS as a dependent variable and the different dimensions of EQ-5D as regressors. The second model allows to analyse the differences between the self-assessed health status in the different geographic areas and also the differences between the patients’ self-assessed health states, irrespective of their actual health state, in the different geographic areas. The analysis is done using Bayesian approach with Gibbs sampling (computer program WinBUGS 1.4). Data concerning self-assessed EQ-5D with VAS from four geographic areas of schizophrenic patients were obtained for the purposes of this analysis. Results. We obtained the health status index for this sample and analysed the differences for this index between the four geographic areas. Our study reveals variables that explain the differences in patients’ health status and differences in their health states assessment. We consider four possible scenarios.

The main objective of this ex post facto study is to compare the differences in cognitive functions and their relation to schizotypal personality traits between a group of unaffected parents of schizophrenic patients and a control group. A total of 52 unaffected biological parents of schizophrenic patients and 52 unaffected parents of unaffected subjects were assessed in measures of attention (Continuous Performance Test- Identical Pairs Version, CPT-IP), memory and verbal learning (California Verbal Learning Test, CVLT) as well as schizotypal personality traits (Oxford-Liverpool Inventory of Feelings and Experiences, O-LIFE). The parents of the patients with schizophrenia differ from the parents of the control group in omission errors on the Continuous Performance Test- Identical Pairs, on a measure of recall and on two contrast measures of the California Verbal Learning Test. The associations between neuropsychological variables and schizotpyal traits are of a low magnitude. There is no defined pattern of the relationship between cognitive measures and schizotypal traits

In the human brain, cortical GABAergic interneurons represent an important population of local circuit neurons responsible for the intrinsic modulation of neuronal information and have been supposed to be involved in the pathophysiology of schizophrenia. We conducted a quantitative study on the differentiated three-dimensional morphological structure of two types of parvalbumin-immunoreactive interneurons in the anterior cingulate cortex (ACC) of schizophrenic patients versus controls. While type A interneurons ('small bipolar cells') showed a significant reduction of their soma size in schizophrenics, type B interneurons ('small multipolar cells') of schizophrenic patients exhibited a marked decrease in the extent of their dendritic system. These results further support the assumption of a considerable significance of the ACC, an important limbic relay centre, for the etiopathogenesis of schizophrenic psychoses.

Time domain analysis of electroencephalography (EEG) can identify subsecond periods of quasi-stable brain states. These so-called microstates assumingly correspond to basic units of cognition and emotion. On the other hand, Global Field Synchronization (GFS) is a frequency domain measure to estimate functional synchronization of brain processes on a global level for each EEG frequency band [Koenig, T., Lehmann, D., Saito, N., Kuginuki, T., Kinoshita, T., Koukkou, M., 2001. Decreased functional connectivity of EEG theta-frequency activity in first-episode, neuroleptic-naive patients with schizophrenia: preliminary results. Schizophr Res. 50, 55-60.]. Using these time and frequency domain analyzes, several previous studies reported shortened microstate duration in specific microstate classes and decreased GFS in theta band in drug naïve schizophrenia compared to controls. The purpose of this study was to investigate changes of these EEG parameters after drug treatment in drug naïve schizophrenia. EEG analysis was performed in 21 drug-naive patients and 21 healthy controls. 14 patients were reevaluated 2-8 weeks (mean 4.3) after the initiation of drug administration. The results extended findings of treatment effect on brain functions in schizophrenia, and imply that shortened duration of specific microstate classes seems a state marker especially in patients with later neuroleptic responsive, while lower theta GFS seems a state-related phenomenon and that higher gamma GFS is a trait like phenomenon.

Numerous studies have found deficits in premorbid IQ in schizophrenic patients, but it is not clear whether this deficit is shared by (a) patients with other functional psychoses, and (b) relatives of these patients. Ninety-one schizophrenic patients, 66 affective psychotic patients (29 schizoaffective and 37 manic or depressed), and 50 normal control subjects were administered the National Adult Reading Test (NART) which provides an estimate of premorbid IQ. The NART was also completed by 85 first-degree relatives of schizophrenic patients and by 65 first-degree relatives of affective psychotic patients. After adjustments were made for sex, social class, ethnicity and years of education, schizophrenic patients had significantly lower premorbid IQ than their relatives, the affective psychotic patients and controls. Manic and depressed patients had significantly lower NART scores than their first-degree relatives, but schizoaffective patients did not, and neither group differed significantly from controls. There was no significant difference in premorbid IQ between patients who had experienced obstetric complications (OC +) and those who had not (OC -). Both OC + and OC - schizophrenic patients differed significantly from their relatives, but the disparity was greatest between OC + patients and their relatives. Bien que les troubles cognitifs soient un aspect essentiel de la schizophrénie, le dysfonctionnement des systèmes émotionnels y est également considéré comme un élément très important de cette maladie d’autant plus que plusieurs régions du cerveau sont concernées par la régulation émotionnelle. Le principal objectif du présent travail était d’explorer, en imagerie par résonnance magnétique fonctionnelle (IRMf), l’effet de la ziprasidone sur les différentes réponses neuronales à l’affichage de stimuli émotionnels au niveau de la région préfrontale,particulièrement dans le cortex cingulaire antérieur [CCA], le cortex orbito-frontal [COF] et le cortex préfrontal dorso-latéral [CPFDL]. Nous avons examiné les activations cérébrales, chez des patients souffrants de schizophrénie avant et après médication à la ziprasidone, en leur présentant des séries d’images émotionnellement chargées (négatives, neutres et positives) associées à différentes instructions quand aux types d’images qu’ils devaient sélectionner (négatives,neutres et positives). Nous avons analysé les différents changements d’activation (avant et après médication) essentiellement pour les valences extrêmes des stimuli (positives et négatives), ensuite nous avons regardé l’effet du type d’instruction sur ces changements. L’échantillon comprenait 13 patients atteints de schizophrénie et 15 témoins sains. Nous avons également effectué une évaluation clinique des symptômes dépressifs, positifs et négatifs de la maladie ainsi que des mesures biochimiques et de poids avant et après 16 semaines de médication. Malgré l’absence de changement significatif sur les mesures cliniques (PANSS et Dépression) avant et après une moyenne de 14.3 semaines de médication à la ziprasidone, plusieurs régions préfrontales (CCA, COF, CPDL) ont sensiblement accru leur réponse aux stimuli positifs par rapport aux stimuli négatifs. En outre, dans les régions habituellement impliquées dans le contrôle cognitif (CCA et CPFDL), cette tendance s'est accentuée lorsque les patients ont été invités à ne sélectionner que les stimuli négatifs (effet du type d’instruction). Nous avons également trouvé plusieurs similitudes dans le fonctionnement préfrontal (à la fois dans le volume et la force d'activation) entre les contrôles sains et les patients après médication en tenant compte du type d’instruction plus que de la valence émotionnelle des images. Pour conclure, les résultats de la présente étude suggèrent que le traitement antipsychotique avec la ziprasidone améliore le fonctionnement cognitif lié au traitement de l'information émotionnelle dans le cortex préfrontal chez les patients souffrant de schizophrénie. Étant donné le mécanisme d'action neuro-pharmacologique de la ziprasidone (plus d'affinité pour la sérotonine que pour les récepteurs de la dopamine dans le cortex préfrontal), nous pensons que nos résultats démontrent que le contrôle cognitif et la régulation des réactions face à des stimuli émotionnellement chargés dans la schizophrénie sont liés à une plus forte concentration de dopamine dans les voies préfrontales.

Objective: The objective was to evaluate the cardiovascular profile of first-episode psychosis patients in Sao Paulo, Brazil, an issue that has not been sufficiently explored in low-/middle-income countries. Method: A cross-sectional study was performed 1 to 3 years after an initial, larger survey that assessed first-episode psychosis in sao Paulo. We evaluated cardiovascular risk factors and lifestyle habits using standard clinical examination and laboratory evaluation. Results: Of 151 contacted patients, 82 agreed to participate (mean age=35 years; 54% female). The following diagnoses were found: 20.7% were obese, 29.3% had hypertension, 39.0% had dyslipidemia, 19.5% had metabolic syndrome, and 1.2% had a >20% 10-year risk of coronary heart disease based on Framingham score. Also, 72% were sedentary, 25.6% were current smokers, and 7.3% reported a heavy alcohol intake. Conclusion: Compared to other samples, ours presented a distinct profile of higher rates of hypertension and diabetes (possibly due to dietary habits) and lower rates of smoking and alcohol intake (possibly due to higher dependence on social support). Indirect comparison vs. healthy, age-matched Brazilians revealed that our sample had higher frequencies of hypertension, diabetes and metabolic syndrome. Therefore, we confirmed a high cardiovascular risk in first-episode psychosis in Brazil. Transcultural studies are needed to investigate to which extent lifestyle contributes to such increased risk. Background: The association between suicidal behavior and quality of life (QoL) in bipolar disorder (BD) is poorly understood. Worse QoL has been associated with suicide attempts and suicidal ideation in schizophrenic patients, but this relationship has not been investigated in BD. This study tested whether a history of suicide attempts was associated with poor QoL in a well-characterized sample of patients with BD, as has been observed in other psychiatric disorders and in the general population. Methods: One hundred eight patients with Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition BD type I (44 with previous suicide attempts, 64 without previous suicide attempts) were studied. Quality of life was assessed using the World Health Organization's Quality of Life Instrument Short Version. Depressive and manic symptoms were assessed using the Hamilton Depression Rating Scale-17 items and the Young Mania Rating Scale. Results: Patients with BD and previous suicide attempts had significantly lower scores in all the 4 domains of the World Health Organization's Quality of Life Instrument Short Version scale than did patients with BD but no previous suicide attempts (physical domain P=.001; psychological domain P <.0001; social domain P=.001, and environmental domain P=.039). In the euthymic subgroup (n=70), patients with previous suicide attempts had significantly lower scores only in the psychological and social domains (P=.020 and P=.004). Limitations: This was a cross-sectional study, and no causal associations can be assumed. Conclusions: Patients with BD and a history of previous suicide attempts seem to have a worse QoL than did patients who never attempted suicide. Poorer QoL might be a marker of poor copying skills and inadequate social support and be a risk factor for suicidal behavior in BD. Alternatively, poorer QoL and suicidal behavior might be different expressions of more severe BD. (C) 2012 Elsevier Inc. All rights reserved.

About half of all schizophrenic patients have a co-occurring substance use disorder, leading to poorer social and functional outcomes than obtained in non-abusing patients. To improve outcomes, integrated treatments have been designed that address the two conditions simultaneously. Results are, however, conflicting because the available effect studies are hampered by various methodological issues, among which are heterogeneous patient samples.
