Terveydenhuollon toimintaympäristö on muutoksessa 2010-luvulla lainsäädännön ja palvelujärjestelmän muuttuessa. Strategisen johtamisen kautta organisaatiot pyrkivät arvioimaan ja hallitsemaan toimintaympäristössään tapahtuvia muutoksia. Strateginen ohjaus tukee strategian toteuttamista organisaatiossa. Strateginen ohjaus sisältää strategian toteutumisen arvioinnin strategisten suorituskykymittarien avulla, jolloin huomiota kiinnitetään siihen, eteneekö organisaatio kohti asettamiaan tavoitteita. Organisaatioissa käytetään usein runsaasti resursseja suorituskyvyn arvioimiseen. Organisaation toiminnasta kertova arviointitieto tuottaa lisäarvoa organisaatiolle vain, mikäli tietoa kyetään hyödyntämään päätöksenteossa. Tämän tutkimuksen tavoitteena oli lisätä teoreettista tietoa strategisen ohjauksen tueksi kuvaamalla, miten case-organisaatiossa johtotehtävissä toimivat hyödyntävät strategisten suorituskykymittarien tuottamaa seurantatietoa päätöksenteossaan. Tutkimuksessa selvitettiin, miten strategiset suorituskykymittarit asetetaan, miten niiden tuottamaa tietoa siirretään ja miten saatua tietoa hyödynnetään case-organisaatiossa. Tutkimuksen päätutkimuskysymyksenä oli: Miten strategisten suorituskykymittarien tuottaman seurantatiedon hallintaan panostamalla tuetaan päätöksentekoa case-organisaatiossa? Tutkimus toteutettiin laadullisena tapaustutkimuksena, jossa kohdeorganisaationa toimi Tyks-Sapa liikelaitos, joka on julkinen sairaanhoidollisia palveluita tuottava terveydenhuoltoalan liikelaitos. Teoriatiedon perusteella tiedon hallintaan kuuluu tietotarpeiden määrittäminen, tiedon hankinta ja siirtäminen sekä tiedon hyödyntäminen toiminnan muuttamiseen. Kirjallisuuden ja tutkimushaastattelujen perusteella tiedon hyödyntäminen päätöksenteossa edellyttää, että strategisten suorituskykymittarien tuottama tieto vastaa johtajan tietotarpeita. Haastattelujen perusteella eksplisiittisen tiedon siirtämistä tukevat johdon sähköiset tietojärjestelmät. Haastattelujen ja teoriatiedon pohjalta näyttäisi kuitenkin siltä, että strategisten suorituskykymittarien tuottamaa eksplisiittistä tietoa tulisi tulkita vuorovaikutuksessa muiden kanssa, jotta kerätty data muuntuu päätöksentekoa ohjaavaksi strategiseksi tiedoksi. Tutkimuksen tulosten mukaan strategisten suorituskykymittarien tehtävänä on tuottaa tietoa johtajien päätöksenteon tueksi, mutta he tarvitsevat myös muita tietolähteitä päätöksenteossaan. Johtopäätöksenä voidaan todeta, että tämän tutkimuksen tulosten ja aiemman teoriatiedon valossa näyttäisi siltä, että kiinnittämällä huomiota strategisten suorituskykymittarien tuottaman seurantatiedon hallintaan, voidaan tukea kerätyn tiedon hyödyntämistä päätöksenteossa.

A field study of the immune response to the shed acute phase antigen (SAPA) of Trypanosoma cruzi was carried out in the locality of Mizque, Cochabamba department, Bolivia. Schoolchildren (266), with an average of 8.6 ± 3.6 years, were surveyed for parasitological and serological diagnosis, as well as antibodies directed against SAPA using the corresponding recombinant protein in ELISA. The antibodies against SAPA were shown in 82% of patients presenting positive serological diagnosis (IgG specific antibodies). The positive and negative predictive values were 0.88. Antibodies anti-SAPA were shown in 80.8% of the chagasic patients in the initial stage of the infection (positive IgM serology and/or positive buffy coat (BC) test) and in 81.4% of the patients in the indeterminate stage of the infection (positive IgG serology with negative BC and IgM tests). These results show that the anti-SAPA response is not only present during the initial stage of the infection (few months) but extends some years after infection

A cross section of a human population (501 individuals) selected at random, and living in a Bolivian community, highly endemic for Chagas disease, was investigated combining together clinical, parasitological and molecular approaches. Conventional serology and polymerase chain reaction (PCR) indicated an active transmission of the infection, a high seroprevalence (43.3%) ranging from around 12% in < 5 years to 94.7% in > 45 years, and a high sensitivity (83.8%) and specificity of PCR. Abnormal ECG tracing was predominant in chagasic patients and was already present among individuals younger than 13 years. SAPA (shed acute phase antigen) recombinant protein and the synthetic peptide R-13 were used as antigens in ELISA tests. The reactivity of SAPA was strongly associated to Trypanosoma cruzi infection and independent of the age of the patients but was not suitable neither for universal serodiagnosis nor for discrimination of specific phases of Chagas infection. Anti-R-13 response was observed in 27.5% only in chagasic patients. Moreover, anti-R13 reactivity was associated with early infection and not to cardiac pathology. This result questioned previous studies, which considered the anti-R-13 response as a marker of chronic Chagas heart disease. The major clonets 20 and 39 (belonging to Trypanosoma cruzi I and T. cruzi II respectively) which circulate in equal proportions in vectors of the studied area, were identified in patients' blood by PCR. Clonet 39 was selected over clonet 20 in the circulation whatever the age of the patient. The only factor related to strain detected in patients' blood, was the anti-R-13 reactivity: 37% of the patients infected by clonet 39 (94 cases) had anti-R13 antibodies contrasting with only 6% of the patients without clonet 39 (16 cases).

Fueled by ever-growing genomic information and rapid developments of proteomics–the large scale analysis of proteins and mapping its functional role has become one of the most important disciplines for characterizing complex cell function. For building functional linkages between the biomolecules, and for providing insight into the mechanisms of biological processes, last decade witnessed the exploration of combinatorial and chip technology for the detection of bimolecules in a high throughput and spatially addressable fashion. Among the various techniques developed, the protein chip technology has been rapid. Recently we demonstrated a new platform called “Spacially addressable protein array” (SAPA) to profile the ligand receptor interactions. To optimize the platform, the present study investigated various parameters such as the surface chemistry and role of additives for achieving high density and high-throughput detection with minimal nonspecific protein adsorption. In summary the present poster will address some of the critical challenges in protein micro array technology and the process of fine tuning to achieve the optimum system for solving real biological problems.

Campylobacter rectus is an important periodontal pathogen in humans. A surface-layer (S-layer) protein and a cytotoxic activity have been characterized and are thought to be its major virulence factors. The cytotoxic activity was suggested to be due to a pore-forming protein toxin belonging to the RTX (repeats in the structural toxins) family. In the present work, two closely related genes, csxA and csxB (for C. rectus S-layer and RTX protein) were cloned from C. rectus and characterized. The Csx proteins appear to be bifunctional and possess two structurally different domains. The N-terminal part shows similarity with S-layer protein, especially SapA and SapB of C. fetus and Crs of C. rectus. The C-terminal part comprising most of CsxA and CsxB is a domain with 48 and 59 glycine-rich canonical nonapeptide repeats, respectively, arranged in three blocks. Purified recombinant Csx peptides bind Ca2+. These are characteristic traits of RTX toxin proteins. The S-layer and RTX domains of Csx are separated by a proline-rich stretch of 48 amino acids. All C. rectus isolates studied contained copies of either the csxA or csxB gene or both; csx genes were absent from all other Campylobacter and Helicobacter species examined. Serum of a patient with acute gingivitis showed a strong reaction to recombinant Csx protein on immunoblots.
