Introducción: Conocer y diagnosticar las variaciones más frecuentes de la vasculatura renal es de gran importancia para la planificación de la nefrectomía laparoscópica en el donante y para la reconstrucción vascular en el trasplante renal. De igual forma, considerar las variaciones vasculares −especialmente las del sistema venoso− es indispensable en reconstrucción vascular debido a la gran proporción de variaciones venosas asociadas a aneurismas de la aorta abdominal; además, es ideal en el estudio de condiciones clínicas tales como el síndrome de congestión pélvica y la hematuria. Metodología: Se trata de una revisión de la bibliografía sobre la proporción, diagnóstico, procedimientos quirúrgicos y síndromes clínicos asociados a las variaciones de la vasculatura renal, basada en el material encontrado con la siguiente estrategia de búsqueda: “Renal Artery/abnormalities”[Mesh] OR Renal Veins/abnormalities”[Mesh] AND “surgery”[Mesh] OR “transplantation”[Mesh] OR “radiography”[Mesh] “Kidney Pelvis/abnormalities”[Mesh] AND “Kidney Pelvis/blood supply”[Mesh]. Esta estrategia se modificó de acuerdo con las bases de datos: MEDLINE/PubMed, MEDLINE OVID, SCIENCEDIRECT, HINARI y LILACS. Desarrollo: Se revisó el origen y los tipos más frecuentes de variaciones de la vasculatura renal. Se investigó sobre las implicaciones quirúrgicas y los síndromes clínicos asociados.

Millon describes the normal personality by means of adaptation styles that are effective in normal environments and personality disorders such as unadapted operating styles. To operacionalize his theoretical model, Millon has built several instruments, including the Millon Clinical Multiaxial Inventory III (MCMI-III), wich consists of a self report inventory composed by 175 true or false response items, containing four verification scales, and others scales wich evaluates 14 personality patterns and 10 clinical syndromes. The Substance Dependence scale (T) is placed along with Clinical Syndromes scales. This research is justified by the lack of a Brazilian instrument to assess personality psychopathological aspects, and aims to translate and semantically adapt the MCMI-III to the Brazilian context, checking validity elements of the Substance Dependence scale, and developing a computer application for assisting the evaluation of assessment results. To this intent, 2.588 individuals data was collected, male and female, aged between 18 and 85 years, characterized as belonging to a clinical or non-clinical group, who took part in the survey via the internet or in person. Respondents completed the MCMI-III, a socio-demographic questionnaire and a subgroup also answered to the Goldberg General Health Questionnaire (GHQ). Besides descriptive statistics, we performed the analysis using the Student t test, principal components analysis and internal consistency. Despite difficulties related to translating very specific English terms, the assessment by judges, experts on Millon´s theory, and the back translation, attested the adequacy of the Brazilian version. Factorial analysis indicated the grouping of translated T scale items into three factors (social activities prejudice, lack of impulse control, and oppositional behavior), by presenting a single item on a fourth factor (apparently related to seeking pleasurable stimuli). The Cronbach alpha for this set of items was 0,82, indicating an acceptable scale reliability. The data analysis resulted in distinction of scores between clinical and non-clinical groups and between men and women; the relationship between high scores on the scale T and the other scales; scores of drug users according to the declared used substance; and the relationship between high scores on T and the verification of disorder or risk on GHQ mental health factor, indicating the instrument´s adequate sensistivity in identifying psychopathologies and the relationship between the different disorders or psychopathological personality patterns. Although further studies are necessary to develop the scores transformation factors, the computerized correction tool was adequate.

This study has as main objectives to translate and to adapt the MCMI-III to brazilian Portuguese, as well as investigate and analyze the involved modifications in the Alcohol Dependence Scale concerning to the results obtained on the validity and on the process of adaptation to Brazil. The inventory was translated and, posteriorly, applied on people with different reading levels to certify that the items are understandable to public in general, from diverse places of the country, divided into clinical and non-clinical groups. Were evaluated 2855 subjects between the ages of 18 and 85 years old, male and female, resident and dwellers of Brazilian cities. The application methods were face-to-face and computerized. Results showed that the clinical group presented significant differences between the means in comparison to the non-clinical group. Through the application of the General Health Questionnaire were developed studies related to the achievement of convergent validity and its results pointed to the relation between the instrument scores and the MCMI-III. The Alcohol Dependence Scale analysis indicated that people who reported abusive use of alcohol had highest scores, indicating adequacy of the instrument on identifying manifestation of disorders and syndromes. Nevertheless, further studies are necessary to the establishment of normative patterns to the Brazilian sample

A úlcera gástrica figura como uma importante causa de desconforto abdominal em eqüinos jovens. de acordo com a localização das lesões na mucosa gástrica, a presença ou ausência de sinais clínicos e possíveis complicações resultantes de sua ocorrência, quatro síndromes clínicas são freqüentemente descritas em potros: 1) Úlceras assintomáticas ou silenciosas; 2) Úlceras sintomáticas ou ativas; 3) Úlceras perfuradas; e 4) Obstruções gástrica ou duodenal. Com o objetivo de se verificar a distribuição de lesões gástricas (úlceras e/ou erosões) e descamações do epitélio aglandular no estômago de eqüinos jovens assim como uma possível relação entre as alterações mencionadas (lesão/descamação) sessenta potros da raça Quarto de Milha não-portadores de sinais clínicos compatíveis com úlceras gástricas foram submetidos à gastroscopia. Os potros foram divididos em quatro faixas etárias de 15 animais cada uma, sendo: 1 a 30 dias, 31 a 60 dias, 61 a 90 dias e 91 a 120 dias de idade. As lesões ocorreram em ordem decrescente de freqüência nas regiões aglandular próximo ao margo plicatus ao longo da curvatura maior, aglandular próximo à cárdia ao longo da curvatura menor, fundo glandular e aglandular e antro. As descamações do epitélio aglandular ocorreram de forma similar nas regiões de fundo e margo plicatus. Não houve associação entre a ocorrência de lesões e descamações.

Dentre as inúmeras patologias que acometem os eqüinos jovens, as úlceras gástricas situam-se como uma das mais importantes. MURRAY et al. (1987) trabalhando com potros clinicamente normais, constatou uma prevalência de úlceras da ordem de 51%. O mesmo autor realizando gastroscopia em 45 equinos com idade entre 1 e 24 anos, observou a presença de úlceras em 93% dos animais (MURRAY, 1988). de etiologia não completamente definida, acredita-se que inúmeros fatores estejam envolvidos na gênese das úlceras gastroduodenais, sendo o estresse um dos mais importantes. Um desequilíbrio entre os fatores de agressão e proteção da mucosa gástrica é sugerido, onde uma vez ocorrendo o predomínio dos fatores de agressão, a formação das úlceras estaria determinada. de acordo com o tipo, presença ou ausência de sinais clínicos, localização das lesões na mucosa gástrica e possíveis complicações de sua ocorrência, quatro síndromes clínicas são reconhecidas em potros: 1) úlceras assintomáticas ou silenciosas; 2) úlceras sintomáticas ou ativas; 3) úlceras perfuradas e, 4) obstrução gástrica ou duodenal. O diagnóstico deve ser baseado na história clínica, sinais clínicos, resposta à terapia e principalmente nos achados do exame gastroscópico. O tratamento pode ser feito utilizando-se drogas inibidoras da secreção ácida, protetores de mucosas, anti-ácidos, análogos da prostaglandina e estimulantes de motilidade intestinal. Quando indicado emprega-se o tratamento cirúrgico.

O diagnóstico clínico da amebíase intestinal continua sendo meramente presuntivo; o diagnóstico de certeza depende sempre de confirmação laboratorial. Com o objetivo de correlacionar os achados clínicos com a pesquisa do coproantígeno GIAP de Entamoeba histolytica, por teste imunoenzimático, foram estudados 105 pacientes de ambos os sexos, com idade entre 13 e 18 anos, provenientes de demanda passiva do Serviço Ambulatorial de Clínica Médica da Polícia Militar. A prevalência de amebíase intestinal encontrada por coproscopia foi de 13,33% (14/105) e por ELISA 24,76% (26/105). Houve diferença estatisticamente significativa na prevalência desta protozoose quando os métodos foram comparados (p<0,05-McNemar). Entre os enteroparasitas detectados por métodos coproscópicos de rotina destacaram-se: Endolimax nana com 61,90% (65/105), Blastocystis hominis 28,57% (30/105), Entamoeba coli 18,10% (19/105) e Giardia amblia em 5,71 (06/105). Entre os helmintos, os mais prevalentes foram o Trichiuris.trichiura com 4,76% (5/105) e de Ascaris lumbricoides em 3,81% (4,105). A prevalência dessas parasitoses na população estudada foi compatível com a casuística regional. No estudo da sintomatologia dos pacientes com teste de ELISA positivo, 73,08% (19/26) relataram um ou mais sintomas sugestivos de amebíase intestinal, observando-se cólicas intestinais em 46,15% (12/26), diarréia sem elementos anormais em 42,31% (11/26), tenesmo em 3,85% (1/26) e constipação intestinal em 11,54% (3/26). A presença desses sintomas quando comparada com os casos clinicamente idênticos, porém com teste de ELISA não apresentaram significância estatística (p<0,05-McNemar). Quanto a diarréia mucossanguinolenta, esta foi referida por 4,76% de apresentação das síndromes clínicas e sua amplitude de diagnósticos diferenciais, aliados às possibilidades de equívocos que os métodos diagnósticos usualmente empregados podem fornecer. Recomenda-se a inclusão do teste de ELISA no diagnóstico da amebíase intestinal como um recurso indispensável na clínica, embora ele não dispense o exame coproparasitológico, por ser capaz de indetificar somente um patógeno, a E. histolyca.

Os flebovírus (família Bunyaviridae; gênero Phlebovirus), possuem importância considerável em saúde publica, pois podem causar uma variedade de síndromes clínicas. Na região amazônica brasileira até o momento já foram isolados 23 flebovírus sendo que quatro se destacam por terem sido isolados de humanos: Virus Alenquer, Virus Candiru, Virus Morumbi e Virus Serra Norte. Estes mais o Virus Itaituba, isolado de Didelphis marsupialis, fazem parte do Complexo Candiru (grupo Candiru) e foram utilizados para estudos de caracterização genética e de infecção experimental em hamsters dourados (Mesocricetus auratus). Os Hamsters mostraram-se suscetíveis a infecção por esses flebovírus, apesar de não terem apresentado sinais de doença. A análise histopatológica demonstrou aspectos lesionais no fígado, nos rins, no baço, nos pulmões e no sistema nervoso central, sendo as lesões mais intensas no fígado comprovadas pela presença dos antígenos virais empregando a técnica de imunohistoquímica. A análise das seqüências nucleotídicas parciais dos segmentos PRNA e MRNA obtidas para os cinco flebovírus estudados mostraram maior similaridade genética entre si do que com outros membros do gênero Phlebovirus. Sendo as mesmas geneticamente mais relacionadas ao Virus Punta Toro. Filogeneticamente, independentemente do segmento genômico analisado, os cinco flebovírus constituem um grupo monofilético, sendo que a análise pelo método de máxima verossimilhança demonstrou diferentes origens evolutivas para os segmentos de RNA o que sugere a ocorrência de rearranjo genético em natureza entre estes cinco flebovírus amazônicos.

Systemic Lupus Erythematosus (SLE) is a chronic autoimmune disease, rare, multisystem, with a very heterogeneous clinical and serological manifestations standard. The patient, in addition to suffering injuries on his physical and physiological functioning, may also face a number of psychosocial problems. Research indicates that SLE can cause significant damage to the psychological realm, especially with the presence of anxiety and depression. In 1999, the American College of Rheumatology (ACR), proposed the establishment of 19 neuropsychiatric clinical syndromes attributed to SLE. Depression lies between mood disorders and is one of the most common psychiatric manifestations in this group, being found more frequently in these patients than in the general population. Studies also suggest that social support plays an important role in the development of coping strategies, in SLE management and depression. This study has as main objective verify the association between depressive symptoms and perceived social support in patients with SLE. The specific objectives turned to: investigte the prevalence of depressive symptoms; investigate the perceived social support and verify if there is an association between depression, social support and sociodemographic variables. We used a sociodemographic questionnaire, the Beck Depression Scale, and the Perceived Social Support Scale. The analysis was performed through descriptive and inferential statistics. The final sample could count with 79 SLE women, with an average age of 35.7 years. 44 (55.7%) of the participants were married. Only 6 (7.59%) had completed higher education and 32 (40.51%) have not finished high school. Seventy-one (89.87%) had an income below three minimum salaries and 71 (89.87) practiced a religion, and the Catholic (67.71%) was the most mentioned by them. Of the total sample, 37 (46.74%) had been diagnosed SLE more than 7 years before, and 25 (31.65%) had the disease for more than 10 years. Only 19 (24.05%) had some work activity. Forty-two of them (53.17%) had depressive symptoms levels from mild to severe, and 51 (64.46%) reported pain levels of 5, or above. The study found a significant association between depressive symptoms and pain (p = 0.013) and depressive symptoms and work activity (p = 0.02). When we examined the perception of social support, the results showed high levels among participants. Using the Spearman correlation test we found a strong correlation between depressive symptoms and social support (p= 0,000037). It means that the higher the frequency of support, the lower the score of depression. These findings are relevant because depressive symptoms in patients with SLE have a multicausal and multifactorial character and may remain unnoticed, since many of them are confused with the manifestations of the disease. This fact requires a careful assessment from professionals, not only in the clinical setting, but also considering other psychosocial reasons, that may be influencing the emergence or worsening of symptoms. These results also corroborate other studies, which not only confirm the predictive role of social support in the physical wellbeing, but also in the psychological.

A hipertensão arterial está entre as causas mais freqüentes de morte materna. Entre os tipos presentes na gravidez destacam-se as manifestações específicas, isto é, a pré-eclâmpsia e a hipertensão gestacional, definidas clinicamente por aumento dos níveis da pressão arterial após a 20ª semana de gestação, associado (pré-eclâmpsia) ou não (hipertensão gestacional) à proteinúria. Na fase inicial a doença é assintomática, porém, quando não tratada ou não se interrompe a gestação, sua evolução natural é desenvolver as formas graves, como a eclâmpsia e a síndrome HELLP. Eclâmpsia é definida pela manifestação de uma ou mais crises convulsivas tônico-clônicas generalizadas e/ou coma, em gestante com hipertensão gestacional ou pré-eclâmpsia, na ausência de doenças neurológicas. Pode ocorrer durante a gestação, durante o trabalho de parto e no puerpério imediato. É comumente precedida pelos sinais e sintomas de eclâmpsia iminente (distúrbios do sistema nervoso central, visuais e gástricos). A associação de hemólise, plaquetopenia e disfunção hepática já era relatada na literatura na década de cinqüenta. em 1982, Weinstein reuniu estas alterações sob o acrônimo de HELLP, significando hemólise (H), aumento de enzimas hepáticas (EL) e plaquetopenia (LP), e denominou-as de síndrome HELLP. A literatura diverge em relação aos valores dos parâmetros que definem a síndrome. Sibai et al. (1986) propuseram sistematização dos padrões laboratoriais e bioquímicos para o diagnóstico da mesma, que foi adotada pelo Ministério da Saúde do Brasil. As manifestações clínicas podem ser imprecisas, sendo comuns queixas como dor epigástrica, mal-estar geral, inapetência, náuseas e vômitos. O diagnóstico precoce é, eminentemente, laboratorial e deve ser pesquisado de maneira sistemática nas mulheres com pré-eclâmpsia grave/eclâmpsia e/ou dor no quadrante superior direito do abdome. Diferenciar a síndrome HELLP de outras ocorrências, com manifestações clínicas e/ou laboratoriais semelhantes, não é tarefa fácil. O diagnóstico diferencial é particularmente difícil para doenças como púrpura trombocitopênica trombótica, síndrome hemolítico-urêmica e fígado gorduroso agudo da gravidez, devido à insuficiente história clínica e à semelhança dos aspectos fisiopatológicos. O conhecimento da fisiopatologia da pré-eclâmpsia, o diagnóstico precoce e a atuação precisa no momento adequado nas situações complicadas pela eclâmpsia e/ou síndrome HELLP permitem melhorar o prognóstico materno e perinatal.

To establish guidelines based on cientific evidences for the management of cryopyrin associated periodic syndromes. The Guideline was prepared from 4 clinical questions that were structured through Pico (Patient, Intervention or indicator, Comparison and Outcome), to search in key primary scientific information databases. After defining the potential studies to support the recommendations, these were graduated considering their strength of evidence and grade of recommendation. 1215 articles were retrieved and evaluated by title and abstract; from these, 42 articles were selected to support the recommendations. 1. The diagnosis of Caps is based on clinical history and clinical manifestations, and later confirmed by genetic study. Caps may manifest itself in three phenotypes: FCAS (mild form), MWS (intermediate form) and Cinca (severe form). Neurological, ophthalmic, otorhinolaryngological and radiological assessments may be highly valuable in distinguishing between syndromes; 2. The genetic diagnosis with NLRP3 gene analysis must be conducted in suspected cases of Caps, i.e., individuals presenting before 20 years of age, recurrent episodes of inflammation expressed by a mild fever and urticaria; 3. Laboratory abnormalities include leukocytosis and elevated serum levels of inflammatory proteins; 4. Targeted therapies directed against interleukin-1 lead to rapid remission of symptoms in most patients. However, there are important limitations on the long-term safety. None of the three anti-IL-1β inhibitors prevents progression of bone lesions.

To establish guidelines based on scientific evidence for the management of periodic fever, aphthous stomatitis, pharyngitis and adenitis (PFAPA) syndrome. The Guideline was prepared from 5 clinical questions that were structured through PICO (Patient, Intervention or indicator, Comparison and Outcome), to search in key primary scientific information databases. After defining the potential studies to support the recommendations, these were graduated considering their strength of evidence and grade of recommendation. 806 articles were retrieved and evaluated by title and abstract; from these, 32 articles were selected to support the recommendations. 1. PFAPA is a diagnosis of exclusion established on clinical grounds, and one must suspect of this problem in children with recurrent and periodic febrile episodes of unknown origin, or with recurrent tonsillitis interspersed with asymptomatic periods, especially in children in good general condition and with preservation of weight and height development; 2. Laboratory findings are nonspecific. Additional tests do not reveal pathognomonic changes; 3. The evidence supporting an indication for surgical treatment (tonsillectomy with or without adenoidectomy), is based on two non-blinded randomized clinical trials with small numbers of patients; 4. The use of prednisone at the onset of fever in patients with PFAPA proved to be an effective strategy. There is still need for more qualified evidence to support its use in patients with PFAPA; 5. Despite promising results obtained in studies with IL-1ß inhibitors, such studies are limited to a few case reports.

En los últimos años, las Guías Clínicas basadas en la revisión sistemática de la evidencia científica se han convertido en un vehículo esencial para la transferencia efectiva del conocimiento médico a la atención primaria. Sin embargo, su difusión no ha sido todo lo efectiva que hubiera sido deseable. En este contexto aparece Aide Portal (www.aide-portal.ehu.es), el primer portal de habla hispana de Guías Clínicas Informa-tizadas ejecutables. Aide Portal facilita la asimilación del conocimiento contenido en guías clínicas, ya que ofrece múltiples funcionalidades como la particularización de las recomendaciones proporcionadas por las guías a pacientes concretos, la conexión con sistemas de Historia Clínica Electrónica o el acceso a informa-ción terminológica, bibliográfica y farmacológica en tiempo real. En este informe detalla paso a paso cómo extraer el máximo rendimiento de todas las funcionalidades que ofrece Aide Portal, y resultará de especial ayuda a aquellos profesionales del ámbito sanitario que deseen familiarizarse con el mismo.
