Approximately one-third of stroke patients experience depression. Stroke also has a profound effect on the lives of caregivers of stroke survivors. However, depression in this latter population has received little attention. In this study the objectives were to determine which factors are associated with and can be used to predict depression at different points in time after stroke; to compare different depression assessment methods among stroke patients; and to determine the prevalence, course and associated factors of depression among the caregivers of stroke patients. A total of 100 consecutive hospital-admitted patients no older than 70 years of age were followed for 18 months after having their first ischaemic stroke. Depression was assessed according to the Diagnostic and Statistical Manual of Mental Disorders (DSM-III-R), Beck Depression Inventory (BDI), Hamilton Rating Scale (HRSD), Visual Analogue Mood Scale (VAMS), Clinical Global Impression (CGI) and caregiver ratings. Neurological assessments and a comprehensive neuropsychological test battery were performed. Depression in caregivers was assessed by BDI. Depressive symptoms had early onsets in most cases. Mild depressive symptoms were often persistent with little change during the 18-month follow-up, although there was an increase in major depression over the same time interval. Stroke severity was associated with depression especially from 6 to 12 months post-stroke. At the acute phase, older patients were at higher risk of depression, and a higher proportion of men were depressed at 18 months post-stroke. Of the various depression assessment methods, none stood clearly apart from the others. The feasibility of each did not differ greatly, but prevalence rates differed widely according to the different criteria. When compared against DSM-III-R criteria, sensitivity and specificity were acceptable for the CGI, BDI, and HRSD. The CGI and BDI had better sensitivity than the more specific HRSD. The VAMS seemed not to be a reliable method for assessing depression among stroke patients. The caregivers often rated patients depression as more severe than did the patients themselves. Moreover, their ratings seemed to be influenced by their own depression. Of the caregivers, 30-33% were depressed. At the acute phase, caregiver depression was associated with the severity of the stroke and the older age of the patient. The best predictor of caregiver depression at later follow-up was caregiver depression at the acute phase. The results suggest that depression should be assessed during the early post-stroke period and that the follow-up of those at risk of poor emotional outcome should be extended beyond the first year post-stroke. Further, the assessment of well-being of the caregivers of stroke patients should be included as a part of a rehabilitation plan for stroke patients.

La protection de l’environnement est un enjeu capital de la société contemporaine. Suite à la révolution industrielle, la contamination de l’environnement a pris divers chemins pour se retrouver dans notre eau, notre atmosphère et, de manière parfois moins évidente, dans nos sols. Considérant le nombre de sites contaminés répertoriés par le ministère du Développement durable, de l’Environnement et des Parcs, on peut s’interroger sur l’efficacité des dispositions prévues à la section IV.2.1 de la Loi sur la qualité de l’environnement qui prévoit des pouvoirs d’ordonnance de caractérisation et de réhabilitation pouvant viser de manière rétroactive non seulement le pollueur et celui ayant permis la contamination, mais également, dans certains cas, le gardien, à quelque titre que ce soit, du terrain. En 2003, le cadre réglementaire en matière d’ordonnances de décontamination a fait l’objet d’une réforme majeure, dont les grandes lignes sont rapportées dans la première partie de cette étude. Toutefois, l’application de ces mesures relève d’un pouvoir de nature discrétionnaire pour le ministre, cette discrétion faisant l’objet de développements dans la deuxième partie de notre mémoire. Le nombre d’ordonnances rendues par le ministre en matière de décontamination des sites est si peu élevé qu’on ne peut éviter de traiter, dans la dernière partie de notre étude, de l’éventuelle responsabilité de l’État en lien avec la contamination des sols, considérant les principes de développement durable et surtout, d’équité intergénérationnelle qui, selon nous, devraient se refléter dans l’application des pouvoirs d’ordonnance du ministre de l’Environnement.

In order for the projects of recovery of degraded areas to be successful, it is necessary to have a perfect recovery of the soil where the revegetation will be implanted as an initial action in the recovery of the whole process. The use of native forest species fully adapted to these types of terrain is another aspect of great importance, once the non-selection of these species, even if abundant in the surrounding areas, as it is in our case, implies great mortality of individuals during the planting and their low fixation during the process. The establishment of a monitoring program that contemplates the advancements obtained in the soil, the vegetation and the return of wild animals also collaborate in the evaluation of the success of the process. And, finally, the effective participation of the mining company, accepting and applying the techniques tested and indicated by research, even if, initially, the return time is longer than expected, also guarantees the success of the process. The mining company not only implemented a partnership with important universities in Brazil to obtain solutions for the environmental problems but also applied the developed techniques and the monitoring program. In the present work, we have attempted to summarize important aspects to evaluate the advancements in the rehabilitation plan for those areas, being here presented some results of the monitoring of areas under different levels of recovery, in accordance with the techniques adopted. Biological parameters of the soil were used to verify the efficiency of these different techniques in the recovery process. This work is part of the monitoring program of areas in rehabilitation by the mining company, implemented as of 1999 and in partnership with universities. The microbial activity was determined through the quantification of the carbon and nitrogen microbial biomass (BMC and BMN) and the activity of the dehydrogenase evaluated in the mining floor and tailing areas in different levels of soil preparation and planting of native species. The analysis of the parameters studied revealed that the preparation of the soil, following the three years proposed by the methodology, was important for the success in establishing the rehabilitation process. Some of the areas analyzed already show some parameters with values close or superior to those found in the capoeira (secondary forest), the latter being the non-treated area. © 2010 WIT Press.

The growing demand for physical rehabilitation processes can result in the rising of costs and waiting lists, becoming a threat to healthcare services’ sustainability. Telerehabilitation solutions can help in this issue by discharging patients from points of care while improving their adherence to treatment. Sensing devices are used to collect data so that the physiotherapists can monitor and evaluate the patients’ activity in the scheduled sessions. This paper presents a software platform that aims to meet the needs of the rehabilitation experts and the patients along a physical rehabilitation plan, allowing its use in outpatient scenarios. It is meant to be low-cost and easy-to-use, improving patients and experts experience. We show the satisfactory results already obtained from its use, in terms of the accuracy evaluating the exercises, and the degree of users’ acceptance. We conclude that this platform is suitable and technically feasible to carry out rehabilitation plans outside the point of care.

RATIONALE: A key objective of A Very Early Rehabilitation Trial is to determine if the intervention, very early mobilisation following stroke, is cost-effective. Resource use data were collected to enable an economic evaluation to be undertaken and a plan for the main economic analyses was written prior to the completion of follow up data collection. AIM AND HYPOTHESIS: To report methods used to collect resource use data, pre-specify the main economic evaluation analyses and report other intended exploratory analyses of resource use data. SAMPLE SIZE ESTIMATES: Recruitment to the trial has been completed. A total of 2,104 participants from 56 stroke units across three geographic regions participated in the trial. METHODS AND DESIGN: Resource use data were collected prospectively alongside the trial using standardised tools. The primary economic evaluation method is a cost-effectiveness analysis to compare resource use over 12 months with health outcomes of the intervention measured against a usual care comparator. A cost-utility analysis is also intended. STUDY OUTCOME: The primary outcome in the cost-effectiveness analysis will be favourable outcome (modified Rankin Scale score 0-2) at 12 months. Cost-utility analysis will use health-related quality of life, reported as quality-adjusted life years gained over a 12 month period, as measured by the modified Rankin Scale and the Assessment of Quality of Life. DISCUSSION: Outcomes of the economic evaluation analysis will inform the cost-effectiveness of very early mobilisation following stroke when compared to usual care. The exploratory analysis will report patterns of resource use in the first year following stroke.

El objetivo de este trabajo es la elaboración de un plan de mercadeo para la empresa Nobel Biocare en el sector de estética dental. Para cumplir este objetivo se realizó un análisis de la situación actual de la empresa, específicamente en Bogotá, siendo la ciudad donde se maneja el 40% de su mercado. Para el presente análisis se evaluó la competencia, reconociendo sus ventajas y desventajas en el mercado. También se clasificaron los tipos de clientes y la importancia que estos tienen para la compañía. Se realizaron encuestas a los clientes de Nobel Biocare, por medio de las cuales se pudo concluir una positiva percepción de la compañía. Asimismo se analizaron las tres líneas de productos principales de Nobel Biocare diseñándose una estrategia para cada uno de estos y su posicionamiento en el mercado.
