The present study was undertaken to determine the importance of socioeconomic factors (family income), urban or rural family location, parity, maternal age, the presence of maternal and fetal pathologies as well as placental abnormalities on the weight and gestational age of 566 newborns. The highest incidence of newborns with low birth weight for gestational age was significantly more frequent in urban populations when the mothers were from low socio-economic levels. In mothers from low socioeconomic levels infants with low birth weight for gestational age were seen in greater proportion among primaparas and had a tendency to be higher in mothers aged less than 20 years; prematurity was highest in mothers ≥ 30 years old and significantly higher from the 8th gestation on. Maternal and fetal pathologies emphasized these characteristics and placental pathologies were not correlated with the distribution of weight and length of gestation. In mothers of high socioeconomic levels age and parity were not correlated with weight and gestational age of the newborns.

Study the semi-quantitative and quantitative technique in the diagnosis of catheter-related infections in newborns and to determine oxacillin resistance in Staphylococcus isolated. It was analyzed 353 catheter tips from 273 newborns in the Neonatal Unit of Hospital FMB. To confirm the diagnosis of infection, were analyzed the clinical data of newborns, the presence of at least one positive blood culture and growth of ≥ 1000 CFU/mL on quantitative culture and/or ³15 UFC on semiquantitative culture, with the same microorganism isolation (species and drug sensitivity) in blood culture and no other focus of infection except the catheter. The disk diffusion technique was used to check similarity of strains and resistance to oxacillin. Of the 353 tips analyzed, 39 were included in this study as the inclusion criteria. The semiquantitative culture was positive in 26 (66.7%) catheters and quantitative culture was positive in 24 (61.5%). Of 273 patients, 19 (6.9%) had a diagnosis of catheter-related bloodstream Infection (CR-BSI). Of the 19 episodes of CR-BSI, S. epidermidis was the predominant etiological agent (84.2%). The resistance to the antibiotic methicillin was found in 14 (73.7%) strains of Staphylococcus. The semiquantitative method was more sensitive (79%) compared with the quantitative method (63%). The use of antibiotics may have influenced the sensitivity of the quantitative method as the microorganisms present in the lumen are exposed to higher concentrations of antibiotics administered via the catheter

The concern with infant mortality has been a priority in public policies, especially for Brazil to achieve the Millennium Development Goal number four: reducing child mortality by 75% by the year 2015. It is known that prematurity has an intimate relationship with mortality of children under one year and therefore it is necessary an effective intervention in risk factors linked to premature births. To evaluate the profile of mothers and newborn babies living in Botucatu-SP, in the period 2001 to 2009, focusing on prematurity. A quantitative study, retrospective, descriptive, epidemiological, from the Information System Newborn Alive. The prevalence of preterm births was 15.1%, and low birth weight 14.7%. Among infants, the Apgar score below 7 at 1 and at 5 minutes was 13.1% and 2.4% respectively. Considering the total of pregnant women studied, 20.3% were adolescents and 10.3% were aged 35 years or more and most (63.2%) had eight or more years of school approval. The profile of mothers of premature infants shows that 23.6% were teenagers, 14.1% had at least 35 years and 60.4% had at least primary education. With regard to premature deliveries, caesarean sections and 58.4% were between the total newborns, this type of delivery was achieved in 46.6% of cases. Prematurity was associated with an Apgar score below seven in the first and fifth minutes of life, presence of malformation and multiple pregnancy. Among maternal variables, prematurity was associated with extremes of maternal age, maternal education, history of stillbirth, primiparas, white and presence of the mother's partner. With regard to delivery was associated with cesarean section and outside the hospital. This study assessed maternal characteristics associated with prematurity and point out how relevant the pregnancies at the extremes of age and those that occur in women with low education level, indicating the interference... The Hypertension Arterial Gestationis is a of largest complications to the pregnant women, a time that is associated with to high risk of morbimortalidade fetal and maternal ;the term If referred the levels pressure equal or above of 140mmhg to the pressure systolic and of 90mmhg to the pressure diastolic (1).Hypertension in pregnancy can be classified into gestational hypertension, chronic hypertension, preeclampsia and eclampsia(3). This study aimed to calculate and analyze the cost of care of newborns of hypertensive mothers hospitalized in rooming, nursery and the neonatal intensive care unit (Neonatal UTI). It’s a study of exploratory, descriptive and quantitative data analysis, in newborns of mothers with hypertension, who underwent prenatal care in HCFMB, from January 1 to 31 in December 2010. The data analysis showed that the cost of care for newborn in rooming was R$ 38.62 for the control group and groups of hypertensive mothers were R$ 19.93 to R$ 37.38. The costs of care to the newborn in the nursery were R$ 1,781.81 for the control group and groups of hypertensive mothers were R$ 680.03 to 7544.10. The costs for the newborn who Neonatal UTI were R$ 7,468.60 for the control group and groups of hypertensive mothers were R$ 5,228.02 to R $ 18,372.75. The total costs of care for newborn in rooming, nursery and Neonatal UTI were R$ 916.15 for the control group, R$ 1,385.98 for the HAC group, R$ 327.23 for the group HAS, R$ 3,896.57 for the group of preeclampsia and R$ 6,326.54 for the group of eclampsia. Considerations It can be concluded that the costs of mothers with preeclampsia and eclampsia were higher, being conditions with increased risk of maternal-fetal morbidity / mortality, requiring care in intensive care unit and longer stay in hospital

The main objective of the presented study is the development of a predictive interval type-2 fuzzy inference system in order to estimate the mortality risk for a newborn, to be used as an auxiliary tool for decision making in medical centers where there is a lack of professionals for this purpose and, afterwards, to compare its performance to a type-1 fuzzy system. The input variables were chosen due to their acquisition ‘simplicity, not involving any invasive tests, such as blood tests or other specific tests. The variables are easily obtained in the first few minutes of life: birth weight, gestational age at delivery, 5-minute Apgar score and previous report of stillbirth. Databases from the DATASUS were used to validate the model. 1351 records from the city of São José dos Campos, a mid-sized city in the São Paulo state’s countryside, were considered in this study. Finally, an analysis using the ROC curve was performed to estimate the model’s accuracy

A infecção pulmonar de etiologia bacteriana é um dos principais problemas que levam a morbi-mortalidade na fibrose cística (FC). Staphylococcus aureus se destaca como um dos micro-organismos mais frequentes e com um agravante para a terapêutica quando se apresentam resistentes à oxacilina (MRSA). Amostras MRSA podem ser classificadas tanto genotipicamente quanto fenotipicamente em MRSA adquiridas na comunidade (CA-MRSA) ou adquiridas no hospital (HA-MRSA). Fenotipicamente, essa classificação é muito controversa, podendo se basear em critérios epidemiológicos ou ainda pelo perfil de susceptibilidade aos antimicrobianos. Por outro lado, a classificação genotípica consiste na determinação dos cassetes cromossômicos (SCCmec), local de inserção do gene mecA (que confere resistência a meticilina). Atualmente são reconhecidos 11 tipos de SCCmec, sendo os de tipo I ao III e VIII relacionados ao genótipo HA-MRSA e IV ao XI ao genótipo CA-MRSA. Classicamente CA-MRSA é capaz de produzir a toxina Panton-Valentine leukocidin (PVL), codificada pelos genes luk-S e luk-F que está associada à pneumonia necrotizante e infecções de tecidos moles em pacientes com FC com quadros de exacerbação pulmonar. No Brasil, raros são os trabalhos envolvendo caracterização de SCCmec em amostras de pacientes com FC. Diante disso, este estudo teve como objetivo principal a caracterização dos tipos de SCCmec e ainda a determinação do perfil de susceptibilidade a antimicrobianos em uma população de MRSA recuperada de pacientes com FC assistidos em dois centros de tratamento no Rio de Janeiro, Hospital Universitário Pedro Ernesto (HUPE) e Instituto Fernandes Figueira (IFF). Foram estudadas 108 amostras de MRSA isoladas do período de 2008 a 2010, sendo 94 oriundas de 28 pacientes adultos atendidos no IFF e 14 de 2 pacientes adultos atendidos no HUPE. Foram encontradas altas taxas de resistência para os antimicrobianos oxacilina, cefoxitina e eritromicina. Todas as amostras foram sensíveis à vancomicina e a linezolida quando determinada as Concentrações Inibitórias Mínimas (CIM). Através da técnica de PCR foi possível a tipificação dos SCCmec em 82,4% das amostras, sendo 64% destas compatíveis ao genótipo CA-MRSA. Não houve diferença estatística nas taxas de susceptibilidade aos antimicrobianos entre as amostras CA-MRSA e HA-MRSA. Foram encontrados os SCCmec dos tipos I, III, IV e V, sendo os tipos I e IV os mais frequentes. O gene que codifica a toxina PVL foi encontrado em 34,2% das amostras e foi observado em amostras CA-MRSA e HA-MRSA. Nosso estudo se destaca por apresentar um alto percentual de amostras CA-MRSA e ainda por ser o primeiro do país a detectar a presença do gene que codifica a toxina PVL em pacientes com FC. Além disso, de forma inédita na literatura, encontramos o gene luk-S, em amostras classificadas como HA-MRSA em pacientes com FC. Os poucos estudos nacionais, bem como as diferenças encontradas entre trabalhos, refletem a necessidade de conhecimento mais aprimorado do MRSA envolvido nas infecções pulmonares dos pacientes com FC.
