Objectives: Pneumothorax is a frequent complication during mechanical ventilation. Electrical impedance tomography (EIT) is a noninvasive tool that allows real-time imaging of regional ventilation. The purpose of this study was to 1) identify characteristic changes in the EIT signals associated with pneumothoraces; 2) develop and fine-tune an algorithm for their automatic detection; and 3) prospectively evaluate this algorithm for its sensitivity and specificity in detecting pneumothoraces in real time. Design: Prospective controlled laboratory animal investigation. Setting: Experimental Pulmonology Laboratory of the University of Sao Paulo. Subjects: Thirty-nine anesthetized mechanically ventilated supine pigs (31.0 +/- 3.2 kg, mean +/- SD). Interventions. In a first group of 18 animals monitored by EIT, we either injected progressive amounts of air (from 20 to 500 mL) through chest tubes or applied large positive end-expiratory pressure (PEEP) increments to simulate extreme lung overdistension. This first data set was used to calibrate an EIT-based pneumothorax detection algorithm. Subsequently, we evaluated the real-time performance of the detection algorithm in 21 additional animals (with normal or preinjured lungs), submitted to multiple ventilatory interventions or traumatic punctures of the lung. Measurements and Main Results: Primary EIT relative images were acquired online (50 images/sec) and processed according to a few imaging-analysis routines running automatically and in parallel. Pneumothoraces as small as 20 mL could be detected with a sensitivity of 100% and specificity 95% and could be easily distinguished from parenchymal overdistension induced by PEEP or recruiting maneuvers, Their location was correctly identified in all cases, with a total delay of only three respiratory cycles. Conclusions. We created an EIT-based algorithm capable of detecting early signs of pneumothoraces in high-risk situations, which also identifies its location. It requires that the pneumothorax occurs or enlarges at least minimally during the monitoring period. Such detection was operator-free and in quasi real-time, opening opportunities for improving patient safety during mechanical ventilation.

Spontaneous pneumothorax is a frequent complication in advanced lung disease of cystic fibrosis. We describe a case of a complicated pneumothorax in a 21-year old-woman with cystic fibrosis who was effectively treated with the application of biological glue via a minithoracotomy. Introduction: Les complications dues à la cocaïne inhalée se¦rencontrent de plus en plus fréquemment dans les services d'urgence¦et dans la pratique des médecins de premier recours. La survenue de¦complications pulmonaires aiguës de type pneumothorax est¦méconnue.¦Vignette clinique: Un patient de 25 ans consulte aux urgences en¦raison d'une gêne pharyngée, une odynodysphagie et une dysphonie¦en progression depuis 12 h. Il avoue une prise de cocaïne sniffée à 5¦reprises dans les 6 heures précédant l'apparition des symptômes. Des¦crépitations sous-cutanées sont mise en évidence à la palpation du¦creux sus-claviculaire droit, remontant jusqu'à la base du cou. La¦radiographie thoracique confirme un emphysème sous-cutané des¦creux sus-claviculaires (fig. 1). Le patient bénéficie d'une¦oxygénothérapie et d'une observation, avec une évolution¦spontanément favorable.¦Discussion: Après une prise de cocaïne par inhalation profonde, des¦manoeuvres de Valsalva intenses répétées contre glotte fermée sont¦pratiquées, afin d'augmenter la quantité de substance absorbée et donc¦ses effets. Ceci engendre un gradient de pression entre les alvéoles et¦l'interstitium pulmonaire, entraînant une augmentation de la pression¦intra-alvéolaire, puis une rupture des alvéoles avec libération d'air à¦travers les tissus péri-bronchiques dans le médiastin et les tissus¦sous cutanés. En cas de rupture dans la cavité pleurale, il en résulte¦un pneumothorax. Un tabagisme actif est souvent associé, laissant¦suspecter un effet favorisant du tabac sur la survenue des lésions¦pulmonaires. Un traitement conservateur associant antalgie et¦oxygénothérapie s'avère en général suffisant. En présence d'une¦augmentation de la taille du pneumothorax ou de signes de mise¦sous tension, la pose d'un drain thoracique est indiquée. L'arrêt de la¦consommation de cocaïne et du tabac doit être encouragé.¦Conclusion: Un emphysème sous-cutané ou un pneumothorax¦spontané chez un jeune patient doivent faire suspecter une éventuelle¦consommation de cocaïne. Les questions concernant la prise de¦cocaïne ou d'autres substances (alcool, amphétamines, etc.), ainsi¦que son mode d'utilisation et sa fréquence, doivent être formulées¦clairement et réitérées à plusieurs reprises. Le risque de pneumothorax¦existe également lors de consommation occasionnelle et peut entraîner¦des complications pulmonaires importantes.

Cocaine use, often not recognized, is a frequent cause of consultation in the emergency room or by primary care physicians. The use of cocaine causes numerous cardiovascular and pulmonary side effects. In this context, the occurrence of a pneumothorax represents a specific complication, often misunderstood by primary care physicians. We describe here three cases of patients who suffered from subcutaneous emphysema and pneumothorax after taking cocaine and emphasize the importance of always keeping in mind the possibility of illicit substance use in such cases, especially among young and healthy patients.

Many routes have been described for percutaneous adrenal gland biopsy. They require either a complex non-axial path or a long hydrodissection or even pass through an organ thereby increasing complications. We describe here an approach using an artificially-induced carbon dioxide (CO2) pneumothorax, performed as an outpatient procedure in a 57-year-old woman. Under local anaesthesia, 200 ml of CO2 was injected in the pleural space through a Veress needle under computed tomography fluoroscopy, to clear the lung parenchyma from the biopsy route. Using this technique, transthoracic adrenal biopsy can be performed under simple local anaesthesia as an safely outpatient procedure.

Birt-Hogg-Dube syndrome refers to a dermatologic syndrome, consisting of small papular skins lesion distributed on the scalp, forehead, face and neck, which is autosomal dominantly inherited. Subsequently patients may develop concomitant renal and thoracic pathology. We report the case of a patient with Birt-Hogg-Dube syndrome diagnosed after spontaneous pneumothorax.

Tracheobronchial aspergillosis is a rare entity mainly observed in immune-compromised patients or those who have undergone transplantation. It may cause airway ulcerations or bleeding. We report the case of a 17-year-old patient receiving chemotherapy for acute lymphoblastic leukemia who presented with right-sided tension pneumothorax. Chest tube drainage revealed a massive air leak without reexpansion of the lung, and bronchoscopy showed a 15- × 15-mm defect of the distal trachea related to aspergillosis infection. The defect was closed by an intrathoracic transposition of a pedicled latissimus dorsi muscle flap, which was sutured into the debrided defect followed by temporary endotracheal stenting and antifungal medication.
