Plain radiography still accounts for the vast majority of imaging studies that are performed at multiple clinical instances. Digital detectors are now prominent in many imaging facilities and they are the main driving force towards filmless environments. There has been a working paradigm shift due to the functional separation of acquisition, visualization, and storage with deep impact in the imaging workflows. Moreover with direct digital detectors images are made available almost immediately. Digital radiology is now completely integrated in Picture Archiving and Communication System (PACS) environments governed by the Digital Imaging and Communications in Medicine (DICOM) standard. In this chapter a brief overview of PACS architectures and components is presented together with a necessarily brief account of the DICOM standard. Special focus is given to the DICOM digital radiology objects and how specific attributes may now be used to improve and increase the metadata repository associated with image data. Regular scrutiny of the metadata repository may serve as a valuable tool for improved, cost-effective, and multidimensional quality control procedures.

OBJETIVO: Apresentar a implementação de "toolkits" para visualização de imagens médicas no padrão DICOM e fazer uma revisão dos fundamentos e características deste padrão. É apresentado o VDTApplication para visualização das imagens locais e remotas, e o VDTApplet, que possibilita a visualização das imagens utilizando um navegador. MATERIAIS E MÉTODOS: Os "toolkits" foram implementados utilizando a linguagem de programação Java. Para seu desenvolvimento foram consideradas as variações do padrão DICOM, tornando-o um sistema genérico, capaz de ler e abrir imagens geradas por diferentes modalidades de diversos fabricantes. As ferramentas foram avaliadas qualitativamente por médicos, considerando a interface do sistema, a qualidade das imagens e o ajuste da imagem. RESULTADOS: A avaliação foi feita com base em conceitos de 1 (muito ruim) a 5 (muito bom) para cada item, sendo os resultados: 5 para interface do sistema, 4 para qualidade das imagens e 3 para ajuste da imagem. Baseando-se nos conceitos obtidos pode-se classificar as ferramentas como boas. CONCLUSÃO: As ferramentas são práticas, amigáveis e deverão ser incorporadas ao projeto PACS ("picture archiving and communication system") do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto - Universidade de São Paulo.

OBJETIVO: Integração dos Sistemas de Informação em Radiologia (RIS - "Radiology Information System") e de Arquivamento e Comunicação de Imagens (PACS - "Picture Archiving and Communication System") no Serviço de Radiodiagnóstico do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo, para possibilitar a consulta remota de laudos e imagens associadas. MATERIAIS E MÉTODOS: A integração RIS/PACS implementada é feita em tempo real, no momento da consulta, utilizando tecnologias "web" e técnicas de programação para "intranet/internet". RESULTADOS: A aplicação "web" permite a consulta pela "intranet" do hospital dos laudos de exames e imagens associadas através de nome, sobrenome, número de registro hospitalar dos pacientes ou por modalidade, dentro de um determinado período. O visualizador possibilita que o usuário navegue pelas imagens, podendo realizar algumas funções básicas como "zoom", controle de brilho e contraste e visualização de imagens lado a lado. CONCLUSÃO: A integração RIS/PACS diminui o risco de inconsistências, através da redução do número de interfaces entre bases de dados com grande redundância de informação, proporcionando um ambiente de trabalho rápido e seguro para consulta de laudos radiológicos e visualização de imagens associadas.

OBJETIVO: Pesquisar visualizadores de imagens médicas gratuitos disponíveis na internet capazes de funcionar como cliente PACS (picture archiving and communication system) e avaliar suas principais funções e a viabilidade do uso em computadores pessoais. MATERIAIS E MÉTODOS: Foi feita pesquisa, no Google e em sites especializados, por programas gratuitos disponíveis para o Windows. Foram encontrados cerca de 70, sendo 11 capazes de funcionar como cliente PACS, e selecionados seis destes para análise: ClearCanvas Workstation, KPACS, Onis, Synedra View Personal, Mito e Tudor DicomViewer. Com base nas necessidades dos autores, 16 funções foram avaliadas. RESULTADOS: Dos seis programas avaliados, dois possuem 10 das 16 funções avaliadas e um possui apenas duas. Três realizam MPR (reconstrução multiplanar), um realiza MIP (reconstrução por projeção de intensidade máxima), dois realizam VR (renderizações volumétricas), dois funcionam como servidor PACS, dois geram CDs, um realiza fusão de imagens, três permitem utilizar múltiplos monitores e apenas um não é compatível com Windows 7. CONCLUSÃO: Diversos programas gratuitos estão disponíveis e não existe nenhum completo. Cabe ao usuário analisar e selecionar o programa que melhor se enquadra nas suas necessidades, porém, os programas Onis, Synedra e ClearCanvas se destacam, cada um com suas peculiaridades. É totalmente viável o uso de programas gratuitos para o dia-a-dia do radiologista.

Contexte : Les premières implantations PACS (Picture Archiving and Communication System) remontent aux années 80. Le rehaussement récent du PACS au sein d’un centre hospitalier universitaire a permis d’étudier l’introduction d’innovations dans une technologie mature. Objectif : Dans un premier temps, effectuer une revue de la littérature sur les études évaluant la perceptions des utilisateurs de l’implantation des PACS. Deuxièmement, évaluer dans une étude de cas si le niveau d’alignement entre les tâches et la technologie prédit l’utilisation et la perception des bénéfices. Méthodes : Il y a eu une revue systématique de la littérature afin d’identifier les études sur la perception des utilisateurs du PACS. Une adaptation du modèle TTF (Task-Technology-Fit) développé par Goodhue (1995) a servi de base théorique pour l’étude de cas effectuée par questionnaire auto-administré. Résultats : Les modèles recensés dans la revue systématique sont mieux adaptés à des nouvelles implantions PACS qu’à des systèmes matures en évolution. Il y a une corrélation significative entre le niveau de l’alignement, tel que perçu par les utilisateurs, et la perception des bénéfices nets, ainsi que l’utilisation de la technologie. L’étude a démontré un écart entre la perception des utilisateurs et celle des développeurs interviewés. Conclusions : Cette étude supporte la validité du concept alignement tâche-technologie dans l’analyse d’un rehaussement PACS. Il est important de maintenir une interaction soutenue entre développeurs et utilisateurs, qui va audelà de la phase initiale de développement.

Smart healthcare is a complex domain for systems integration due to human and technical factors and heterogeneous data sources involved. As a part of smart city, it is such a complex area where clinical functions require smartness of multi-systems collaborations for effective communications among departments, and radiology is one of the areas highly relies on intelligent information integration and communication. Therefore, it faces many challenges regarding integration and its interoperability such as information collision, heterogeneous data sources, policy obstacles, and procedure mismanagement. The purpose of this study is to conduct an analysis of data, semantic, and pragmatic interoperability of systems integration in radiology department, and to develop a pragmatic interoperability framework for guiding the integration. We select an on-going project at a local hospital for undertaking our case study. The project is to achieve data sharing and interoperability among Radiology Information Systems (RIS), Electronic Patient Record (EPR), and Picture Archiving and Communication Systems (PACS). Qualitative data collection and analysis methods are used. The data sources consisted of documentation including publications and internal working papers, one year of non-participant observations and 37 interviews with radiologists, clinicians, directors of IT services, referring clinicians, radiographers, receptionists and secretary. We identified four primary phases of data analysis process for the case study: requirements and barriers identification, integration approach, interoperability measurements, and knowledge foundations. Each phase is discussed and supported by qualitative data. Through the analysis we also develop a pragmatic interoperability framework that summaries the empirical findings and proposes recommendations for guiding the integration in the radiology context.

When patients enter our emergency room with suspected multiple injuries, Statscan provides a full body anterior and lateral image for initial diagnosis, and then zooms in on specific smaller areas for a more detailed evaluation. In order to examine the possible role of Statscan in the management of multiply injured patients we implemented a modified ATLS((R)) algorithm, where X-ray of C-spine, chest and pelvis have been replaced by single-total a.p./lat. body radiograph. Between 15 October 2006 and 1 February 2007 143 trauma patients (mean ISS 15+/-14 (3-75)) were included. We compared the time in resuscitation room to 650 patients (mean ISS 14+/-14 (3-75)) which were treated between 1 January 2002 and 1 January 2004 according to conventional ATLS protocol. The total-body scanning time was 3.5 min (3-6 min) compared to 25.7 (8-48 min) for conventional X-rays, The total ER time was unchanged 28.7 min (13-58 min) compared to 29.1 min (15-65 min) using conventional plain radiography. In 116/143 patients additional CT scans were necessary. In 98/116 full body trauma CT scans were performed. In 18/116 patients selective CT scans were ordered based on Statscan findings. In 43/143 additional conventional X-rays had to be performed, mainly due to inadequate a.p. views of fractured bones. All radiographs were transmitted over the hospital network (Picture Archiving and Communication System, PACS) for immediate simultaneous viewing at different places. The rapid availability of images for interpretation because of their digital nature and the reduced need for repeat exposures because of faulty radiography are also felt to be strengths.

Introdução: A Medicina Nuclear (MN) é uma modalidade imagiológica em grande expansão na atualidade, tornando-se especialmente importante em pediatria. Considerando a elevada radiosensibilidade das crianças, torna-se crucial otimizar os vários procedimentos dentro desta modalidade. Objetivo: Determinar Níveis de Referência de Diagnóstico (NRD’s) para procedimentos de MN Pediátrica num dos centros de excelência nacional e comparar os valores obtidos com a literatura internacional. Material e Métodos: Foi realizada uma análise retrospetiva dos cabeçalhos Digital Imaging and Communications in Medicine (DICOM) de Tomografia por Emissão de Positões (PET-CT) disponíveis no Picture Archiving and Communication System (PACS), de modo a recolher os valores de CT Dose Index (CTDIvol-mGy), Dose Length Product (DLP-mGy.cm), atividade administrada, o género, peso, altura e idade foram recolhidos; De modo a obter os valores de dose relativos a outros procedimentos de MN foram analisados documentos em formato papel. Considerando a idade dos doentes foi estabelecida a categorização etária de forma a determinar os NRD’s locais com base no percentil 75 dos valores de dose. Resultados: Os NRD’s para a atividade administrada em recém-nascidos (0 anos) para a Cintigrafia Renal e Cistografia Direta foi de 26MBq (0,70 e 0,69mCi, respetivamente). Na Renocintigrafia, o valor obtido foi 30MBq (0,80mCi). Para o exame de PET-CT corpo inteiro, os NRD’s para a atividade administrada, CTDIvol e DLP, para os 15 anos foi de 296MBq, 3,23mGy e de 396,79mGy.cm, respectivamente. Para a Cintigrafia do esqueleto o valor obtido para os 15 anos foi de 684MBq (17,5mCi). Conclusões: Após a comparação com os resultados publicados em outros estudos verificaram-se algumas diferenças, sendo os valores de NRD locais obtidos acima dos níveis apresentados.

When an elementary proposition is instituted, the pictorial relations establish a contact between the proposition and the fact. This seems to commit the Tractarian project with a psychological view, but this is not the case because the point of view of the Tractatus Logico-Philosophicus is concerned with the conditions of possibility of the representation. In this paper my objective is twofold. First, I show that the thought plays a basic function in the institution of a picture. Second, I show that the fact that the thought plays an important role in the institution of a picture does not commit the Tractarian project with an inquiry of psychological nature.

We used event-related functional magnetic resonance imaging (fMRI) to investigate neural responses associated with the semantic interference (SI) effect in the picture-word task. Independent stage models of word production assume that the locus of the SI effect is at the conceptual processing level (Levelt et al. [1999]: Behav Brain Sci 22:1-75), whereas interactive models postulate that it occurs at phonological retrieval (Starreveld and La Heij [1996]: J Exp Psychol Learn Mem Cogn 22:896-918). In both types of model resolution of the SI effect occurs as a result of competitive, spreading activation without the involvement of inhibitory links. These assumptions were tested by randomly presenting participants with trials from semantically-related and lexical control distractor conditions and acquiring image volumes coincident with the estimated peak hemodynamic response for each trial. Overt vocalization of picture names occurred in the absence of scanner noise, allowing reaction time (RT) data to be collected. Analysis of the RT data confirmed the SI effect. Regions showing differential hemodynamic responses during the SI effect included the left mid section of the middle temporal gyrus, left posterior superior temporal gyrus, left anterior cingulate cortex, and bilateral orbitomedial prefrontal cortex. Additional responses were observed in the frontal eye fields, left inferior parietal lobule, and right anterior temporal and occipital cortex. The results are interpreted as indirectly supporting interactive models that allow spreading activation between both conceptual processing and phonological retrieval levels of word production. In addition, the data confirm that selective attention/response suppression has a role in resolving the SI effect similar to the way in which Stroop interference is resolved. We conclude that neuroimaging studies can provide information about the neuroanatomical organization of the lexical system that may prove useful for constraining theoretical models of word production. In the picture-word interference task, naming responses are facilitated when a distractor word is orthographically and phonologically related to the depicted object as compared to an unrelated word. We used event-related functional magnetic resonance imaging (fMRI) to investigate the cerebral hemodynamic responses associated with this priming effect. Serial (or independent-stage) and interactive models of word production that explicitly account for picture-word interference effects assume that the locus of the effect is at the level of retrieving phonological codes, a role attributed recently to the left posterior superior temporal cortex (Wernicke's area). This assumption was tested by randomly presenting participants with trials from orthographically related and unrelated distractor conditions and acquiring image volumes coincident with the estimated peak hemodynamic response for each trial. Overt naming responses occurred in the absence of scanner noise, allowing reaction time data to be recorded. Analysis of this data confirmed the priming effect. Analysis of the fMRI data revealed blood oxygen level-dependent signal decreases in Wernicke's area and the right anterior temporal cortex, whereas signal increases were observed in the anterior cingulate, the right orbitomedial prefrontal, somatosensory, and inferior parietal cortices, and the occipital lobe. The results are interpreted as supporting the locus for the facilitation effect as assumed by both classes of theoretical model of word production. In addition, our results raise the possibilities that, counterintuitively, picture-word interference might be increased by the presentation of orthographically related distractors, due to competition introduced by activation of phonologically related word forms, and that this competition requires inhibitory processes to be resolved. The priming effect is therefore viewed as being sufficient to offset the increased interference. We conclude that information from functional imaging studies might be useful for constraining theoretical models of word production. (C) 2002 Elsevier Science (USA).

In the two-Higgs-doublet model (THDM), generalized-CP transformations (phi(i) -> X-ij phi(*)(j) where X is unitary) and unitary Higgs-family transformations (phi(i) -> U-ij phi(j)) have recently been examined in a series of papers. In terms of gauge-invariant bilinear functions of the Higgs fields phi(i), the Higgs-family transformations and the generalized-CP transformations possess a simple geometric description. Namely, these transformations correspond in the space of scalar-field bilinears to proper and improper rotations, respectively. In this formalism, recent results relating generalized CP transformations with Higgs-family transformations have a clear geometric interpretation. We will review what is known regarding THDM symmetries, as well as derive new results concerning those symmetries, namely how they can be interpreted geometrically as applications of several CP transformations.
