Loimaan seudun liikenneturvallisuussuunnitelma on laadittu seudun kuntien ja Varsinais-Suomen ELY-keskuksen yhteistyönä. Liikenneturvallisuussuunnitelman tavoitteena on edistää kokonaisvaltaisella tavalla nykyistä vastuullisempaa ja turvallisuushakuisempaa liikkumista seudulla. Suunnitelmassa esitetyillä toimenpiteillä pyritään merkittävästi vähentämään seudulla tapahtuvien liikennekuolemien ja liikenteessä loukkaantuneiden määrää. Tavoitteena on myös edistää nykyistä kestävämpää liikkumista, jolla terveys- ja ympäristövaikutusten ohella on myönteinen vaikutus myös liikenneturvallisuuskehitykseen. Loimaan seudulla tapahtuu vuosittain keskimäärin viisi kuolemaan ja 70 loukkaantumiseen johtanutta onnettomuutta. Asukaslukuun suhteutettuna Loimaan seudulla tapahtuu selvästi enemmän henkilövahinko-onnettomuuksia kuin maakunnassa tai koko maassa keskimäärin. Myös liikenneonnettomuuksien vakavuusaste on maakunnan ja koko maan keskitasoa korkeampi. Kulkutavoittain tarkasteltuna korostuvat erityisesti henkilöauto-onnettomuudet. Liikennekuolemista peräti kolme neljäsosaa on tapahtunut autoilijoille. Ikäryhmittäin tarkasteltuna suurin onnettomuusriski on 15–24-vuotiailla, jotka muodostavat seudun liikenteen uhreista 39 prosenttia (ikäryhmän osuus seudun väestöstä on 11 prosenttia). Liikenneonnettomuuksista aiheutuu seudun kunnille vuosittain lähes kahdeksan miljoonan euron kustannukset – yli 200 euroa asukasta kohden. Nykytilan kuvausten pohjalta ja valtakunnalliset linjaukset huomioiden seudun liikenneturvallisuustyölle on asetettu pitkän aikavälin visio, määrälliset liikenneonnettomuuksien vähentämistavoitteet sekä lähivuosien toimintaa suuntaavat kärkitehtävät. Tavoitteiden mukaan seudun liikennekuolemien määrän tulee vähentyä nykyisestä viidestä hengestä1-2 henkeen vuoteen 2020 mennessä. Loukkaantumisten määrän tulee vähentyä nykyisestä 95 henkilöstä vähintään 71 henkilöön vuoteen 2020 mennessä (-25 %). Suunnitelmassa asetettujen tavoitteiden saavuttamiseen pyritään monipuolisen ja toimien vaikuttavuuden perusteella kohdennetun toimenpidejoukon keinoin sekä kehittämällä eri toimijoiden välistä yhteistyötä ja työnjakoa toimenpiteiden edistämisessä. Suunnitelmassa on esitetty 14 kärkitehtäväkokonaisuutta, joihin toiminta tulee vaikuttavuuden perusteella ensisijaisesti kohdistaa. Suunnitelman vaikutustenarvioinnin perusteella voidaan todeta, että keinot asetettujen tavoitteiden saavuttamiseksi ovat olemassa, mutta tavoitteisiin pääseminen edellyttää erittäin suuria panostuksia kaikilla liikenneturvallisuustyön osa-alueilla, ja erityisesti liikennekäyttäytymiseen ja yhteistyöhön kohdistuvien toimenpiteiden osalta. Liikennekuolemien puolittamisen tavoite nykytasosta vuoteen 2020 on tehtyjen tarkastelujen perusteella mahdollista. Myös loukkaantumisten määrän vähentäminen on saavutettavissa esitetyin toimenpitein.

Recent research has shown that University students with a history of self-reported mild head injury (MHI) are more willing to endorse moral transgressions associated with personal, relative to impersonal, dilemmas (Chiappetta & Good, 2008). However, the terms 'personal' and 'impersonal' in these dilemmas have functionally confounded the 'intentionality' of the transgression with the 'personal impact' or 'outcome' of the transgression. In this study we used a modified version of these moral dilemmas to investigate decision-making and sympathetic nervous system responsivity. Forty-eight University students (24 with MHI, 24 with no-MHI) read 24 scenarios depicting moral dilemmas varying as a function of 'intentionality' of the act (deliberate or unintentional) and its 'outcome' (physical harm, no physical harm, non-moral) and were required to rate their willingness to engage in the act. Physiological indices of arousal (e.g., heart rate - HR) were recorded throughout. Additionally, participants completed several neurocognitive tests. Results indicated significantly lowered HR activity at baseline, prior to, and during (but not after) making a decision for each type of dilemma for participants with MHI compared to their non-injured cohort. Further, they were more likely than their cohort to authorize personal injuries that were deliberately induced. MHI history was also associated with better performance on tasks of cognitive flexibility and attention; while students' complaints of postconcussive symptoms and their social problem solving abilities did not differ as a function of MHI history. The results provide subtle support for the hypothesis that both emotional and cognitive information guide moral decision making in ambiguous and emotionally distressing situations. Persons with even a MHI have diminished physiological arousal that may reflect disruption to the neural pathways of the VMPFC/OFC similar to those with more severe injuries.

An extended research project, funded by the Thomas Pocklington Trust, and carried out by the Research Group for Inclusive Environments (RGIE) at The University of Reading, has examined the lighting found in the homes of people who are visually impaired (VIP). This paper will summarise the results of this substantive study. All the surveyed homes have been occupied by people with sight loss, some of the dwellings were shared with sighted partners. There are several safety issues concerning domestic lighting where inadequate provision may contribute to the incidence of personal injuries occurring in the home. Qualitative and quantitative data from questionnaires, photometric surveys and faceto- face interviews have been obtained from 57 homes. The nature and extent of the visual impairment of each study participant has been identified. This paper will identify important findings from the study, including: a range of areas and tasks within the home that visually impaired people find inadequately lit; the variability of illuminance provided for task lighting and general lighting; and how effective visually impaired people find a selection different lighting systems to those that they commonly use. The research team are able to offer preliminary design guidance for lighting the homes of people who are visually impaired. These will be summarised in the paper.

The ankle sprains represent the most common injuries in sports and basketball. In this sense, the use of ankle bracing and strength capacity analysis of the ankle evertor and invertor muscles, have been suggested as preventive measures and important tools for identifying risk factors associated with ankle sprains. However, questions still persist as to effect of the use ankle bracing on biomechanical variables related to the stability of the ankle. For this reason, this study aims to analyze the effect of the use of ankle bracing on peak torque (PT) of ankle evertor and invertor muscles and on eccentric evertor/concentric invertor torque ratio (EVEECC/INVCON), during the basketball match-play simulation. Ten healthy college basketball players, without mechanics or functional ankle instability performed a laboratory-based protocol representative of work rates observed during basketball match-play, in two different situations, with and without use of ankle bracing. The test was composed of a succession of intermittent physical effort equally distributed in four periods of 10 minutes each, considering the mechanical and physiological demands of a basketball match-play. Prior to the start of the trial (Evaluation 1) and after 2° (Evaluation 2) and 4° (Evaluation 3) periods, the subjects performed five maximal isokinetic concentric and eccentric contractions of ankle invertor and evertor muscles, separated by two minutes rest, at 60 °/s and 120 °/s. After testing for normality of data distribution with the Shapiro-Wilk test, was used the ANOVA repeated measures for two factors and post-hoc Bonferroni test for comparison of variables between assessments. Was adopted p < 0.05. There was no significant difference for PT and EVEECC/INVCON torque ratio between assessments. The energetic performance of landfill biogas (LB) and biodigester biogas (BB) from municipal waste was examined in consumption tests. These tests were performed in situ at a gas generation plant associated with a landfill facility in Madrid (Spain) and following the standard UNE-EN 30-2-1 (1999). The jets of a domestic cooker commonly used for natural gas (NG) or liquefied petroleum gas (LPG) were modified to operate with the biogases produced at the facility. The working pressures best suited to the tested gases, i.e., to avoid flashback and flame lift, and to ensure the stability and correct functioning of the flame during combustion, were determined by trial and error. Both biogases returned optimum energetic performance for the transfer of heat to water in a metallic recipient (as required by the above standard) at a supply pressure of 10 mbar. Domestic cookers are normally supplied with NG at a pressure of 20 mbar, at which pressure the energetic performance of G20 reference gas was higher than that of both biogases (52.84% compared to 38.06% and 49.77% respectively). Data concerning these issues involving also unexplored feedstock are required for the correct conversions of domestic cookers in order to avoid risks of serious personal injuries or property damages.

PURPOSE This study aimed to examine the work-related impact of open hand injuries, specifically, the amount of lost work days subsequent to the injury and factors associated with work-related rehabilitation. PATIENTS AND METHODS We retrospectivley included consecutive patients with acute hand injuries who were operated between 2008 and 2009 in the Division of Hand Surgery (n=435) at the Department of Orthopaedic, Plastic and Hand Surgery. Information was obtained from the medical records and via a self-reported questionnaire sent out in 2011. Patients younger than 18 or older than 65 years, as well as the unemployed were excluded from the study. Descriptive group analysis was used to establish statistical relationships between time off work (TOW) and possible influencing variables. Multiple linear regression was applied to analyse the impact of injury, personal and/or work-related factors on TOW. RESULTS The sample included 290 patients with a mean age of 38.9 (SD 13.2) years of whom 98.6% returned to work after a median absence of 45.5 days. Univariate analysis demonstrated an association of length of absence from work with socio-demographic, clinical and work-related factors. Multiple regression analysis indicated that the location of injury, the number of injured regions, the need for secondary surgery, age, and the type of occupation were independently associated with TOW. CONCLUSION Most factors associated with TOW after traumatic hand injuries could not be influenced. Possible interventions should probably target improved injury prevention, optimal clinical treatment and rehabilitation starting early after injury. Whether improvements in communication and enhancement of cooperation between the treatment teams, the workplace and the insurance carrier may support a staged and earlier return to work remains to be investigated.

The Post-Fordist welfare state thesis locates contemporary social welfare change within a wider analysis of the transformation of capitalist accumulation regimes. Whilst this analysis is useful in directing attention to macro socio-economic change, it has for the most part contained three shortcomings. First, the Post-Fordist thesis has overemphasized the role of historical 'breaks' in the development of social welfare as it purportedly passes from Fordism to Post-Fordism. Second, the thesis has assumed a degree of convergence between welfare states as a result of global economic forces. In doing so, it has underemphasized the mediating impact of existing institutional arrangements within nations. Third, the thesis has assumed, rather than demonstrated, the specific changes which are alleged to be taking place in various fields of social welfare. As a consequence, aspects of continuity in social welfare have been neglected. These three lacunae are addressed through a comparative analysis of developments in the personal social services in Australia and Britain. Services to older people are employed as the specific context of comparison in relation to three dimensions of measuring transformation along a Post-Fordist trajectory: a shift from a unitary economy to a mixed economy of service provision; changes in the model of service delivery and consumption; and strengthening the governance function of the central state. This comparative analysis suggests the need for refinement of the Post-Fordist welfare state thesis concerning the restructuring of social welfare and its impact on the personal social services.

The Women's Health Australia (WHA) project is a longitudinal study of several cohorts of Australian women, which aims to examine the relationships between biological, psychological, social and lifestyle factors and women's physical health, emotional well-being, and their use of and satisfaction with health care. Using the Medicare database as a sampling frame (with oversampling of women from rural and remote areas), 106,000 women in the three age groups 18-23, 45-50 and 70-75 were sent an invitation to participate and a 24-page self-complete questionnaire. Reminder letters, a nation-wide publicity campaign, information brochures, a freecall number for inquiries, and the option of completing the questionnaire by telephone in English or in the respondent's own language, were used to encourage participation. Statutory regulations precluded telephone follow-up of non-respondents. Response rates were 41% (N = 14,792), 54% (N = 14,200) and 36% (N = 12,614) for the three age groups. Comparison with Australian census data indicated that the samples are reasonably representative of Australian women in these age groups, except fur a somewhat higher representation of women who are married or in a defacto relationship, and of women with post-school education. The most common reason for non-participation was lack of interest or time. Personal circumstances, objections to the questionnaire or specific items in it, and concerns about confidentiality were the other main reasons. Recruitment of three representative age-group cohorts of women, and the maintenance of these cohorts over a number of years, will provide a valuable opportunity to examine associations over time between aspects of women's lives and their physical and emotional health and well-being.

Australia's Great Barrier Reef is one of the world's most popular scuba diving destinations. Unfortunately, a series of recent diving injuries and deaths has tarnished the region's safety record. In particular, media attention surrounding the disappearance of American divers Thomas and Eileen Lonergan has focused attention on dive operators' legal responsibilities and the consequences of failing to discharge their duty of care to customers. This paper briefly examines the relevant Australian law for recreational diving operations, and reviews risk management strategies that may reduce or prevent the occurrence of future problems. This is an overview of the first burden of disease and injury studies carried out in Australia. Methods developed for the World Bank and World Health Organization Global Burden of Disease Study were adapted and applied to Australian population health data. Depression was found to be the top-ranking cause of non-fatal disease burden in Australia, causing 8% of the total years lost due to disability in 1996. Mental disorders overall were responsible for nearly 30% of the non-fatal disease burden. The leading causes of total disease burden (disability-adjusted life years [DALYs]) were ischaemic heart disease and stroke, together causing nearly 18% of the total disease burden. Depression was the fourth leading cause of disease burden, accounting for 3.7% of the total burden. Of the 10 major risk factors to which the disease burden can be attributed, tobacco smoking causes an estimated 10% of the total disease burden in Australia, followed by physical inactivity (7%).

Objective: To determine the number of overseas visitors admitted to Queensland hospitals for water-related injuries over three years, the causes of their injuries, the resulting conditions treated, and the type of hospitals to which they were admitted. Design: Retrospective analysis of admissions of overseas visitors to Queensland hospitals over the three financial years 1995/96, 1996/97 and 1997/98. Patients: 296 overseas visitors admitted for water-related injuries, identified from hospital records by their usual place of residence. Main outcome measures: Number of admissions, causes of injuries, conditions treated. and bed days occupied by these patients at different types of hospitals (metropolitan, regional and rural public hospitals, and private hospitals). Results: The 296 overseas visitors accounted for a total of 596 separate admissions, many of these the result of patients with decompression illness being admitted several times to a regional hospital hyperbaric chamber for treatment as day patients. The largest number of injuries involved the use of diving equipment. The main conditions treated were decompression illness (54.7%), fractures and dislocations (15.5%), and drowning and non-fatal submersion (14.9%). Overall, overseas visitors admitted to hospital following a water-related incident occupied 1215 bed days; 90% of these admissions were to regional hospitals. Conclusions: The main reason for admission of overseas visitors is for decompression illness, suggesting that the prevention of injuries among scuba divers requires further coordinated efforts by health and tourism authorities.

Because higher-than-average turnover rates for nurses who work in remote and rural areas are the norm, the authors conducted a study to identify professional and personal factors that influenced rural nurses' decisions to resign. Using a mail survey, the authors gathered qualitative and quantitative data from nurses who had resigned from rural and remote areas in Queensland, Australia. Their findings, categorized into professional and rural influences, highlight the importance of work force planning strategies that capitalize on the positive aspects of rural and remote area practice, to retain nurses in nonmetropolitan areas.

This study examined the impact of computer and assistive device use on the employment status and vocational modes of people with physical disabilities in Australia. A survey was distributed to people over 15 years in age with physical disabilities living in the Brisbane area. Responses were received from 82 people, including those with spinal cord injuries, cerebral palsy and muscular dystrophy. Of respondents 46 were employed, 22 were unemployed, and 12 were either students or undertaking voluntary work. Three-quarters of respondents used a computer in their occupations, while 15 used assistive devices. Using logistic regression analysis it was found that gender, education, level of computer skill and computer training were significant predictors of employment outcomes. Neither the age of respondent nor use of assistive software were significant predictors. From information obtained in this study guidelines for a training programme designed to maximize the employability of people with physical disabilities were developed.
