Patients with penetrating eye injuries are a very heterogeneous group both medically and economically. Since 2009, treatment involving sutures for open eye injuries and cases requiring amniotic membrane transplantation (AMT) were allocated to DRG C01B of the German diagnosis-related group system. However, given the significant clinical differences between these treatments, an inhomogeneity of costs to performance is postulated. This analysis describes case allocation problems within the G-DRG C01B category and presents solutions.

Background: The work environment of cocoa farmers exposes them to several ocular hazards that predispose them to eye diseases and injuries. However, the extent of ocular injuries and health seeking patterns following these injuries are unknown among cocoa farmers in Ghana. Objectives: To determine the prevalence of ocular injuries and health seeking behaviour following injury among cocoa farmers in Ghana. Methods: Five hundred and fifty six participants were recruited through simple random sampling using a multistage approach from four cocoa growing districts in Ghana. A structured questionnaire was used to collect relevant information such as demography, ocular hazards and injuries experienced. An ocular examination was also conducted to assess the eye health of the participants. Descriptive and regression statistics were used to analyze the data. The rate of ocular injuries was calculated by using the number of injuries reported that resulted in lost work time/days divided by the number of worker years at risk of injury (sum of years worked in cocoa farms for all the participants). Results: The rate of ocular injuries was 11.3/1000 worker years (95% CI: 9.4 - 31) which led to lost work time of 37.3/1000 worker years (95% CI: 34.1 - 40.8). The major causes of ocular injury were plants/branches (n=73, 51.1%), chemicals (n=27, 18.9%), cocoa pod/husk (n=14, 9.8%) and occurred mostly during weeding, harvesting and chemical spraying. Few (n=34, 6.1%) participants reported the use of ocular protection. Fifty-five (38.5%) participants visited the local chemical shops, while 37 (25.9%) visited hospitals/clinics for ocular treatment of their injuries. Conclusion: There is a high rate of ocular injuries among cocoa farmers who make insufficient use of appropriate eye care services. There is the need for eye health education among cocoa farmers in Ghana. Keywords:

Purpose: To review the epidemiology of serious ocular trauma presenting to Cairns Base Hospital, from the far north Queensland health districts. Methods: A retrospective study of cases from January 1995 to November 2002 inclusive. Cases were analysed with respect to demographics, cause and nature of injury, method of transport and time to and type of ophthalmic treatment, and visual outcomes. Results: There were 226 cases identified, including 71 open-globe and 155 closed-globe injuries. The annual rate of injury was 3.7 per 100 000 for open-globe and 11.8 per 100 000 in total. The Aboriginal and Torres Strait Islander population from the far north Queensland districts showed a disproportionate incidence, with 38% of the total number of injuries, despite representing only 12.3% of the population. Assault in the Aboriginal and Torres Strait Islander population resulted in 69.6% of injuries in men and 75.8% of injuries in women. Of all assaults 76.2% were alcohol-related. The majority (71.5%) of injuries in the Caucasian population were due to accidental blunt and sharp trauma. In total, 77.4% of injuries occurred in men, with an average age of 31 years. Of all open and closed injuries in the study, a final visual acuity of 6/12 or better was achieved in 47.8% of eyes and a final visual acuity of 6/60 or less occurred in 17.7% of patients, 20.8% patients were lost to follow up. In total, 14.1% of open injuries required enucleation/evisceration. Conclusions: The incidence of ocular trauma in far north Queensland is equal to other Australian populations. However, there is a disproportionately high incidence in the Aboriginal and Torres Strait Islander population. Alcohol-related assault is a significant cause of visual loss in the Aboriginal and Torres Strait Islander population. Closed-globe injuries are more common than open globe; however, the latter have poorer visual prognosis. Initial visual acuity of all injuries correlated with final visual acuity.

In a recent outbreak of human ocular injuries that occurred in the town of Araguatins, at the right bank of Araguaia river, state of Tocantins, Brazil, along the low water period of 2005, two patients (8 and 12-year-old boys) presented inferior adherent leukoma in the left eye (OS), and peripherical uveites, with snowbanking in the inferior pars plana. The third one (13-year-old girl) showed posterior uveites in OS, also with snowbanking. Histopathological analysis of lensectomy material from the three patients and vitrectomy from the last one revealed several silicious spicules (gemmoscleres) of the freshwater sponges Drulia uruguayensis and D. ctenosclera. This work brings material evidences, for the first time in the literature, that freshwater sponge spicules may be a surprising new etiological agent of ocular pathology.

ABSTRACTObjective:the study has the intention of evaluate the accuracy of computed tomography for the diagnosis of cervical lesions on penetrating neck trauma and also identify the most frequent mechanisms of trauma. Most injured structures, determine the age range and the most prevalent sex.Methods: observational descriptive retrospective study executed by the systematic retrospective review of medical records of all patients victims of penetrating neck trauma that went through surgery and CT scans, admitted into Hospital do Trabalhador, between January 2009 and December 2013.Results:the final sample was of 30 patients, 96.7% of the male sex, the median age was of 28 years old. Most patients suffered injuries by gun (56.7%) and 33,3% suffered stab wounds. The most stricken area of the neck was Zone II (77.8%) and the left side (55.2%). Regarding the structures injured, the CT showed 6.7% lesions on airways but the surgery showed 40% of damaged, with a value of p=0.002. As to damages of the esophagus and pharynx the CT detected 10% of lesions, while surgery found 30% of lesions, therefore with a significant value of p=0.013. As for the analysis the CT showed reliable. As for the analysis of vascular damage, the CT showed to be, in most cases, reliable to the findings during the surgical act.Conclusion:besides the great use of CT for the diagnosis of penetrating neck injuries we can say that this is an exam with low accuracy for the diagnosis of lesions of aerodigestive tract, therefore it is important a clinical correlation for a good diagnosis.as for the vascular lesions and of other structures, the CT had high sensibility and specificity, thus a good exam to be used in overall.

ABSTRACTObjective:to evaluate the safety and effectiveness of non-operative management (NOM) of liver injury, being the only abdominal injury, from gunshot wounds to the abdomen.Methods:patients who had liver damage diagnosed as single abdominal injury caused by PAF in the right thoracoabdominal region, hemodynamically stable were studied. All underwent examination with computed tomography. Were analyzed: age, gender, levels of trauma, hemodynamic condition and the abdominal examination on admission, the results of the CT scan, the extra-abdominal lesions found, the serum levels of hemoglobin, clinical course, complications, length of hospital stay, outpatient treatment and death.Results:during the study period 169 patients, treated non-operatively, presented liver gunshot wounds. Of these, only 28 patients (16.6%) had liver injury as the only abdominal injury and consequently met the inclusion criteria for this study. The average age was 27.7 years and 25 patients (89.2%) were male. The overall average of verified trauma scores were: RTS 7.45, ISS 10.9, and TRISS 98.7%. The most frequent injuries were grade II and grade III (85.7%). Complications occurred in only one patient who presented a progressive decline in hemoglobin. He underwent a CT scan which showed blush in the liver parenchyma. An arteriography was performed, which showed a successfully embolized arteriovenous fistula. There were no deaths in the patient sample. The average hospital stay was 5.3 days.Conclusion:isolated hepatic injury in gunshot abdominal trauma is uncommon. However, the NOM protocol for this type of injury is safe and has low morbidity. This approach should only be followed in institutions with adequate infrastructure, where an experienced and cohesive team is able to follow a specific protocol, with rigorous periodic evaluation of its results.

Obiective. This study assesses the descriptive epidemiology of children with eye injuries presenting to the Emergency Department of a non exclusive Paediatric University Hospital - First Division of Ophthalmology of “Sapienza” University of Rome - over a period of 12 years. Study Design. A retrospective long term study of 12 years. Participants. All paediatric patients (up to 14 years of age) presenting with ocular injuries and hospitalized. Methods. It was analyzed the incidence of the ocular trauma among males and females. The situation in which the trauma occurred, type of trauma (contusive or perforating), the presence of endo-bulbar foreign bodies, visual acuity outcome. Results. There were 203 patients who presented to the Emergency Department in the period examined. Contusive traumas were 130 (90 males, 40 females). The perforating trauma were 73 (63 males, 10 females). The presence of an endo-bulbar foreign body was registered in 10 patients. A detailed analysis of the causes of the trauma is therefore provided. We evidence that males were almost exclusively involved in sport traumas (60 males versus 2 females), and in second instance accidental trauma is almost equally divided between two genders. Conclusions. Our retrospective study presents the paediatric cases of a non exclusive Paediatric University Hospital where 3% of ocular traumas requiring hospitalization were in children. Therefore our data could be useful in order to bring about the necessary preventive measures to minimize paediatric eye injuries.

OBJECTIVE: This study seeks to identify practices of self-medication in the treatment of ocular emergencies. We examine patients' use of both homemade preparations and manufactured products before seeking specialized care. MATERIALS AND METHODS: We conducted a cross-sectional analytic survey of consecutive patients seen in the ophthalmology emergency room of a teaching hospital. RESULTS: The sample included 561 subjects, 51.3% males and 48.7% females, with a mean age of 39.8 years. Prior to seeking emergency care, 40.5% reported self-medicating; 29.4% used a homemade preparation (13.9% referred to an industrialized product like boric acid as a homemade preparation), and 11.1% used a manufactured product. The most frequently used products included a boric acid solution (53.3%), a normal saline solution (35.7%), herbal infusions (6.1%) and breast milk (4.8%). Viral conjunctivitis was the most frequent diagnosis (24.4%), followed by the presence of a corneal foreign body (7.4%). No significant differences were found in the self-treatment of ocular injuries according to gender (p = 0.95), level of education (p = 0.21) or age (p = 0.14). In addition, self-medication practices were not related to the medically judged severity of the condition. CONCLUSION: Patients often attempt to treat conditions that require ophthalmologic emergency care by self-medicating with homemade or manufactured products. The most widely used products include boric acid, normal saline, leaf infusions and breast milk. This behavior occurs independently of educational level, gender, age or the nature of the ocular condition. Self-medication is a culturally driven practice that is used even in cases of acute ocular injuries.

Introducción El manejo de las heridas abdominales penetrantes ha variado en los últimos 30 años, la laparotomía mandatoria es la conducta más usada en el Hospital Occidente De Kennedy a pesar presentar complicaciones y aumentar las tasas de morbimortalidad. Existen diferentes conductas selectivas del trauma abdominal penetrante como son: valoraciones físicas seriadas, ultrasonografía, laparoscopia, TAC de triple contraste y lavado peritoneal; sin embargo no se ha definido cuál de estos métodos es el más eficiente para el diagnóstico. El objetivo es determinar cual es el método diagnostico más efectivo para evaluar el trauma abdominal penetrante dependiendo del sitio topográfico de la lesión. Métodos: Revisión sistemática de la literatura en revistas indexadas a nivel mundial y literatura gris colombiana. Se escogieron artículos entre 1990 y 2010 que suministraran la mejor evidencia. Se evaluó su calidad metodológica de acuerdo a las recomendaciones desarrolladas por la Asociación Médica Americana en JAMA 1993. Adicionalmente se utilizó el SCORE de calidad metodológico publicado en la revista Chilena de Cirugía 2003. Los estudios incluidos fueron organizados en una tabla de evidencia donde se tuvo en cuenta los siguientes puntos: fecha de publicación, revista, autor, nivel de evidencia y desenlace principal. Resultados: En total se encontraron 98 artículos, según los criterios de elegibilidad se seleccionaron 57 y se adiciono 2 guías de manejo clínico. A pesar de la heterogeneidad de los estudios se encontró concordancia en 17 artículos en cuanto la disminución de complicaciones, tiempo de estancia hospitalaria y disminución de laparotomías negativas con el uso de la TAC de triple contraste y la laparoscopia diagnostica. Conclusión: No existe un juicio para determinar que metodología diagnóstica es la más eficiente en el manejo conservador, pero resulta evidente que la tendencia actual del manejo del trauma abdominal está dirigida a seleccionar y a enfocar al paciente de forma individual. Las alternativas más efectivas hasta el momento ha considerar son la TAC de triple contraste y la laparoscopia diagnostica en las heridas en flancos, dorso y región toracoabdominal respectivamente.

Objetivó: Caracterizar los pacientes con heridas cardiacas penetrantes grado II a VI, describir las características del trauma, tratamiento quirúrgico, evolución clínica e identificar los factores asociados a un desenlace. Metodología: Se diseñó un estudio de asociación en 308 pacientes que ingresaron a cirugía con diagnóstico de herida penetrante de corazón entre enero de 1999 y octubre de 2009. Se excluyeron 68 casos. La serie analizada incluyó 240 pacientes con heridas cardiacas. Se analizaron variables demográficas, clínicas, quirúrgicas y de evolución, tabulados en EXCEL® y analizados en SPSS 20®. Resultados: El promedio de edad fue 27.8 años, principalmente hombres (96%), lesiones por arma cortopunzante 93% y un 7% por proyectil arma de fuego. El estado hemodinámico al ingreso (según Ivatury) fue normal 44%; Shock profundo 34%; Agónicos 18% y 3% fatales. El 67% (n=161) presentaron taponamiento cardiaco. Los grados de lesión cardiaca según la clasificación OIS-AAST fueron: grado II 33%, grado III 13%, grado IV 29%, grado V 22% y grado VI 3%. La ventana pericárdica fue el método diagnóstico confirmatorio de lesión en 63% y las incisiones de abordaje quirúrgico fueron la esternotomía 63% y la toracotomía anterolateral 35%. La mortalidad fue 15% (n=36). Las diferencias en mortalidad entre el estado hemodinámico al inicio de cirugía, mecanismo de lesión y grado de herida, demostraron ser estadísticamente significativas (valor de p<0.001). Conclusiones: El estado hemodinámico y las heridas por arma de fuego son factores asociados a mortalidad. La ventana pericárdica subxifoidea favorece la preferencia y buenos resultados de la esternotomía como vía de abordaje quirúrgico.

OBJETIVO: Avaliar as formas de tratamento empregadas e os principais aspectos relacionados à morbidade e à mortalidade dos ferimentos cardíacos.. MÉTODOS: Estudo retrospectivo de 102 doentes com lesão cardíaca, atendidos nos dois prontos socorros de Manaus (Pronto Socorro Municipal 28 de Agosto e Hospital Pronto Socorro Dr. João Lúcio P. Machado) no período de janeiro de 1998 a junho de 2006. RESULTADOS: Dos 102 doentes, 95,1% eram homens; a média de idade foi 27 anos; ferimentos por arma branca representaram 81,4% dos casos, contra 18,6% por arma de fogo; cardiorrafia foi realizada em 98,1% dos casos. As câmaras cardíacas atingidas foram: VD: 43,9% (36,2% isoladamente e 7,7% associada a outras câmaras); VE: 37,2%; AD: 8,5% e AE: 10,4%, com mortalidades específicas de 21%, 23%, 22% e 45%, respectivamente. Lesões de duas câmaras associadas alcançaram mortalidade de 37,5%, sendo 20% para VD+AD, 100% para VD+VE e zero para VD+AE. O pulmão correspondeu a 33,7% de 89 lesões associadas. Os tempos médios de cirurgia e de internação foram de 121 minutos e 8,2 dias, respectivamente. Cerca de 22,5% complicaram representando 41 complicações. A mortalidade foi 28,4%. Lesões grau IV e V corresponderam a 55% e 41% dos casos, com mortalidade específica de 26% e 15%, respectivamente. Todos os doentes com lesão grau VI morreram. CONCLUSÃO: O ferimentos cardíacos por arma branca estiveram associados a menor mortalidade, as lesões cardíacas grau IV estiveram associadas à maior mortalidade e um menor tempo operatório esteve associado à maior gravidade e mortalidade.

La ecografía básica cardiaca (ECB) es una herramienta útil en la Unidad de Cuidados intensivos al facilitar la realización de ciertas intervenciones. No se ha definido el número de repeticiones necesarias para obtener un nivel de competencia adecuado. La evidencia encontrada indica un número mínimo de cincuenta repeticiones, para alcanzar cierto grado de habilidad.

The purpose of this study was to determine, for penetrating injuries (gunshot, stab) of the chest/abdomen, the impact on fatality of treatment in trauma centers and shock trauma units compared with general hospitals. Medical records of all cases of penetrating injury limited to chest/abdomen and admitted to and discharged from 7 study facilities in Baltimore city 1979-1980 (n = 581) were studied: 4 general hospitals (n = 241), 2 area-wide trauma centers (n = 298), and a shock trauma unit (n = 42). Emergency center and transferred cases were not studied. Anatomical injury severity, measured by modified Injury Severity Score (mISS), was a significant prognostic factor for death, as were cardiovascular shock (SBP $\le$ 70), injury type (gunshot vs stab), and ambulance/helicopter (vs other) transport. All deaths occurred in cases with two or more prognostic factors. Unadjusted relative risks of death compared with general hospitals were 4.3 (95% confidence interval = 2.2, 8.4) for shock trauma and 0.8 (0.4, 1.7) for trauma centers. Controlling for prognostic factors by logistic regression resulted in these relative risks: shock trauma 4.0 (0.7, 22.2), and trauma centers 0.8 (0.2, 3.2). Factors significantly associated with increased risk had the following relative risks by multiple logistic regression: SBP $\le$ 70 (RR = 40.7 (11.0, 148.7)), highest mISS (42 (7.7, 227)), gunshot (8.4 (2.1, 32.6)), and ambulance/helicopter transport (17.2 (1.3, 228.1)). Controlling for age, race, and gender did not alter results significantly. Actual deaths compared with deaths predicted from a multivariable model of general-hospital cases showed 3.7 more than predicted deaths in shock trauma (SMR = 1.6 (0.8, 2.9)) and 0.7 more than predicted deaths in area-wide trauma centers (SMR = 1.05 (0.6, 1.7)). Selection bias due to exclusion of transfers and emergency center cases, and residual confounding due to insufficient injury information, may account for persistence of adjusted high case fatality in shock trauma. Studying all cases prospectively, including emergency center and transferred cases, is needed. ^
