Kallikrein-kinin system exerts cardioprotective effects against pathological hypertrophy. These effects are modulated mainly via B(2) receptor activation. Chronic physical exercise can induce physiological cardiac hypertrophy characterized by normal organization of cardiac structure. Therefore, the aim of this work was to verify the influence of kinin B(2) receptor deletion on physiological hypertrophy to exercise stimulus. Animals were submitted to swimming practice for 5 min or for 60 min, 5 days a week, during 1 month and several cardiac parameters were evaluated. Results showed no significantly difference in heart weight between both groups, however an increased left ventricle weight and myocyte diameter were observed after the 60 min swimming protocol, which was more pronounced in B(2)(-/-) mice. In addition, sedentary B(2)(-/-) animals presented higher left ventricle mass when compared to wild-type (WT) mice. An increase in capillary density was observed in exercised animals, however the effect was less pronounced in B(2)(-/-) mice. Collagen, a marker of pathological hypertrophy, was increased in B(2)(-/-) mice submitted to swimming protocol, as well as left ventricular thickness, suggesting that these animals do not respond with physiological hypertrophy for this kind of exercise. In conclusion, our data suggest an important role for the kinin B(2) receptor in physiological cardiac hypertrophy. (c) 2007 Elsevier B.V. All rights reserved.

L’hypertension pulmonaire (HP) est une maladie dont l’étiologie est inconnue et qui entraîne ultimement une défaillance du ventricule droit (VD) et le décès. L’HP peut être induite chez le rat par la la monocrotaline (MCT), un alcaloïde pyrrolizidique extrait de la plante Crotalaria Spectabilis, causant des lésions à l’endothélium des artères pulmonaires, menant à un épaississement de ces dernières et à une augmentation de la résistance vasculaire. Ceci à pour conséquence de causer une hypertrophie du VD, de l’inflammation, une dysfonction endothéliale NO-dépendante des artères coronariennes et une augmentation des peptides natriurétiques circulants. Objectif: Nous avons testé l’hypothèse selon laquelle l’étiopathologie de l’HP impliquerait le récepteur à ocytocine (OTR) dû à son implication fonctionnelle avec les cytokines inflammatoires et la libération du peptide natriurétique atrial (ANP) et du NO. Méthodes: Des rats mâles Sprague-Dawley pesant 220-250g reçurent une seule injection sous-cutanée de MCT (60 mg/kg). 6 à 7 semaines (46±1 jours) suivant l’injection, les rats furent sacrifiés et l’expression génique et protéique fut déterminée par PCR en temps réel et par western blot, respectivement, dans le VD et le ventricule gauche (VG) Résultats: Les rats traités au MCT démontrèrent une augmentation significative du VD. Une hypertrophie du VD était évidente puisque le ratio du VD sur le VG ainsi que le poids du septum étaient près de 77% plus élevés chez les rats traités au MCT que chez les rats contrôles. Le traitement au MCT augmenta l’expression génique d’ANP (3.7-fois dans le VG et 8-fois dans le VD) ainisi que le NP du cerveau (2.7-fois dans le VG et 10-fois dans le VD). Les transcrits de trois récepteurs de NP augmentèrent significativement (0.3-2 fois) seulement dans le VD. L’expression protéique de la NO synthase (iNOS) fut également augmentée de façon sélective dans le VD. Par contre, les transcripts de NOS endothéliale et de NOS neuronale étaient plus élevés (0.5-2 fold) dans le VG. L’ARNm et l’expression protéique d’OTR furent diminués de 50% dans le VD, tandis qu’une augmentation de l’expression des cytokines IL-1β and IL-6 fut observée. L’ARNm de Nab1, un marqueur d’hypertrophie pathologique, fut augmentée de deux-fois dans le VD. Conclusion: L’augmentation d’expression génique de NP dans le VD des rats traités au MCT est associée à une augmentation des transcripts du récepteur NP, suggérant une action locale de NP dans le VD durant l’HP. L’expression d’OTR est atténuée dans le VD, possiblement par des cytokines inflammatoires puisque le promoteur du gène de l’OTR contient de multiples éléments de réponse aux interleukines. Diminuer l’expression d’OTR dans le VD durant l’hypertension pulmonaire pourrait influencer de manière positive la fonction cardiaque car l’OTR régule la contractilité et le rythme cardiaque. AIM It is unknown how the heart distinguishes various overloads, such as exercise or hypertension, causing either physiological or pathological hypertrophy. We hypothesize that alpha-calcitonin-gene-related peptide (αCGRP), known to be released from contracting skeletal muscles, is key at this remodelling. METHODS The hypertrophic effect of αCGRP was measured in vitro (cultured cardiac myocytes) and in vivo (magnetic resonance imaging) in mice. Exercise performance was assessed by determination of maximum oxygen consumption and time to exhaustion. Cardiac phenotype was defined by transcriptional analysis, cardiac histology and morphometry. Finally, we measured spontaneous activity, body fat content, blood volume, haemoglobin mass and skeletal muscle capillarization and fibre composition. RESULTS While αCGRP exposure yielded larger cultured cardiac myocytes, exercise-induced heart hypertrophy was completely abrogated by treatment with the peptide antagonist CGRP(8-37). Exercise performance was attenuated in αCGRP(-/-) mice or CGRP(8-37) treated wild-type mice but improved in animals with higher density of cardiac CGRP receptors (CLR-tg). Spontaneous activity, body fat content, blood volume, haemoglobin mass, muscle capillarization and fibre composition were unaffected, whereas heart index and ventricular myocyte volume were reduced in αCGRP(-/-) mice and elevated in CLR-tg. Transcriptional changes seen in αCGRP(-/-) (but not CLR-tg) hearts resembled maladaptive cardiac phenotype. CONCLUSIONS Alpha-calcitonin-gene-related peptide released by skeletal muscles during exercise is a hitherto unrecognized effector directing the strained heart into physiological instead of pathological adaptation. Thus, αCGRP agonists might be beneficial in heart failure patients.

In hypertension, left ventricular (LV) hypertrophy develops as an adaptive mechanism to compensate for increased afterload and thus preserve systolic function. Associated structural changes such as microvascular disease might potentially interfere with this mechanism, producing pathological hypertrophy. A poorer outcome is expected to occur when LV function is put in jeopardy by impaired coronary reserve. The aim of this study was to evaluate the role of coronary reserve in the long-term outcome of patients with hypertensive dilated cardiomyopathy. Between 1996 and 2000, 45 patients, 30 of them male, with 52 +/- 11 years and LV fractional shortening <30% were enrolled and followed until 2006. Coronary flow velocity reserve was assessed by transesophageal Doppler of the left anterior descending coronary artery. Sixteen patients showed >= 10% improvement in LV fractional shortening after 17 +/- 6 months. Coronary reserve was the only variable independently related to this improvement. Total mortality was 38% in 10 years. The Cox model identified coronary reserve (hazard ratio = 0.814; 95% CI = 0.72-0.92), LV mass, low diastolic blood pressure, and male gender as independent predictors of mortality. In hypertensive dilated cardiomyopathy, coronary reserve impairment adversely affects survival, possibly by interfering with the improvement of LV dysfunction. J Am Soc Hypertens 2010;4(1):14-21. Aims - Glycogen synthase kinase 3 (GSK-3) signalling is implicated in the growth of the heart during development and in response to stress. However, its precise role remains unclear. We set out to characterize developmental growth and response to chronic isoproterenol (ISO) stress in knockin (KI) mice lacking the critical N-terminal serines, 21 of GSK-3 and 9 of GSK-3 respectively, required for inactivation by upstream kinases. Methods and results - Between 5 and 15 weeks, KI mice grew more rapidly, but normalized heart weight and contractile performance were similar to wild-type (WT) mice. Isolated hearts of both genotypes responded comparably to acute ISO infusion with increases in heart rate and contractility. In WT mice, chronic subcutaneous ISO infusion over 14 days resulted in cardiac hypertrophy, interstitial fibrosis, and impaired contractility, accompanied by foetal gene reactivation. These effects were all significantly attenuated in KI mice. Indeed, ISO-treated KI hearts demonstrated reversible physiological remodelling traits with increased stroke volume and a preserved contractile response to acute adrenergic stimulation. Furthermore, simultaneous pharmacological inhibition of GSK-3 in KI mice treated with chronic subcutaneous ISO recapitulated the adverse remodelling phenotype seen in WT hearts. Conclusion - Expression of inactivation-resistant GSK-3/does not affect eutrophic myocardial growth but protects against pathological hypertrophy induced by chronic adrenergic stimulation, maintaining cardiac function and attenuating interstitial fibrosis. Accordingly, strategies to prevent phosphorylation of Ser-21/9, and consequent inactivation of GSK-3/, may enable a sustained cardiac response to chronic-agonist stimulation while preventing pathological remodelling. © 2010 The Author.

Il y a 4 isoforme de p38 : α, β, δ, and γ. MK5, à l'origine identifié comme étant un régulateur de PRAK (Regulated/Activated Protein Kinase), est maintenant connu pour être activée par la protéine kinase p38 (qui est un mitogène activé par la protéine kinase, MAPK). Cette dernière est impliquée dans les mécanismes de fibrose et d'apoptose pendant l'hypertrophie cardiaque. De plus, MK5 est également activée par les MAPKs atypiques; ERK3 et ERK4. Bien qu’elles soient fortement exprimées dans le coeur, le rôle physiologique de MK5 et ERK3 demeure inconnu. Par conséquent, nous avons étudié l'effet de la constriction aortique transversale (TAC) – induisant un surcharge chronique de pression chez les souris hétèrozygotes knockout pour MK5 (MK5+/-) ou ERK3 (ERK3+/-) et pour leurs types sauvages (MK5+/+ et ERK3+/+). Deux sem post-TAC; le ratio de poids du coeur/poids corporel a été augmenté chez les 2 souris MK5+/- et MK5+/+. L'échocardiographie de la trans-thoracique démontre que la surcharge de pression a altéré la fonction diastolique du ventricule gauche chez MK5+/+, mais pas chez la souris MK5+/-. De plus, nous avons observé moins de dépôt de collagène, évalué par une coloration au trichrome de Masson, 2 et 3 sem post-TAC chez les souris MK5+/-. Parallèlement, le niveau de l’ARNm de collagène type1 alpha-1 a été significativement diminué dans les coeurs des souris MK5+/-, 2 et 3 sem post-TAC. De même, ERK3, mais pas ERK5 ni p38α, co-IP avec MK5 dans les 2 modèles des coeurs TAC; aigus ou chroniques. En revanche, l’ajout exogénique de GST-MK5 a abaissé ERK4 et p38α, mais pas ERK3 dans les lysâtes de coeur de souris. Par contre, GST-ERK3 et GST-p38α ne démontrent aucune co-IP avec MK5. Ces données suggèrent que dans le coeur seul ERK3, et non ERK4 ou p38α, est capable d’interagir avec, et réguler MK5. A niveau physiologique MK5 interagit entièrement avec ERK3 et par conséquent MK5 n’est pas disponible pour lier les protéines exogéniques. Les souris hétérozygotes pour ERK3 (ERK3+/-) ont également démontré une réduction ou une absence de collagène et une faible expression d’ARNm du collagène type1 alpha1, 3 sem post-TAC. Ces résultats démontrent un important rôle pro-fibrotique de la signalisation MK5-ERK3 pendant une surcharge chronique de pression.Nous avons également démontré 5 variant d'épissage de (MK5.1-5), y compris la forme originale (MK5.1). MK5.2 et MK5.5 subissent une délétion de 6 paires de base dans l’exon 12 : MK5.3 manque l'exon 12 : MK5.4 et MK5.5 manquent les exons 2-6. L'expression des ARNm des différents variant d'épissage a été vérifiée par PCR en temps réel (qPCR). Bien que l’expression est ubiquitaire, l'abondance relative de chaque variant était tissu-spécifique (coeur, rein, pancréas, muscle squelettique, poumon, foie, et cerveau). En plus, l'abondance relative des variant d’épissage varie pendant la surcharge de pression et le développement postnatal du coeur. En outre, l'immunofluorescence a indiqué que MK5.1-5.3 se localise au noyau alors que MK5.4-5.5 est situé au niveau cytoplasmic dans les cellules HEK 293 non stimulées. Suite à une stimulation avec l'anisomycin, un activateur de p38 MAPK, MK5.1-5.3 se translocalise du noyau au cytoplasme alors qu’une petite fraction de MK5.4-5.5 translocalise vers le noyau. Ces variant d'épissage peuvent diversifier la signalisation de MK5-ERK3 dans coeur, mais leur rôle exact oblige des recherches supplémentaires. Excepté l’isoforme δ, toutes les isoformes de p38 sont exprimées dans le coeur et la forme α est considérée comme étant l'isoforme dominante. L’analyse par qPCR et immunobuvardage de type western ont démontré que p38α et p38γ sont les deux isoformes prédominantes alors que p38β et p38δ sont exprimées aux mêmes niveaux dans le coeur de rat adulte. L'immunofluorescence a démontré que p38α et p38γ se trouvent dans le cytoplasme et le noyau. Cependant, suite à la surcharge par TAC, p38γ s'est accumulé dans noyau tandis que la distribution de p38α est demeurée inchangée. Ainsi, l'abondance de p38γ et sa translocalisation nucléaire suite à la surcharge de pression indique un rôle potentiel dans l'expression génique pendant le remodelage cardiaque. En conclusion, nous avons mis en évidence pour la première fois un rôle pro-fibrotique pour la signalisation MK5-ERK3 pendant une surcharge chronique de pression. D'ailleurs, les niveaux comparables d'expression de p38γ avec p38α, et la localisation différentielle de p38γ pendant la surcharge aiguë ou chronique de pression suggèrent différents rôles possibles pour ces isoformes pendant le remodelage hypertrophique cardiaque.

Cardiac hypertrophy and dilatation can result from stimulation of signal transduction pathways mediated by heterotrimeric G proteins, especially Gq, whose α subunit activates phospholipase Cβ (PLCβ). We now report that transient, modest expression of a hemagglutinin (HA) epitope-tagged, constitutively active mutant of the Gq α subunit (HAα*q) in hearts of transgenic mice is sufficient to induce cardiac hypertrophy and dilatation that continue to progress after the initiating stimulus becomes undetectable. At 2 weeks, HAα*q protein is expressed at less than 50% of endogenous αq/11, and the transgenic hearts are essentially normal morphologically. Although HAα*q protein declines at 4 weeks and is undetectable by 10 weeks, the animals develop cardiac hypertrophy and dilatation and die between 8 and 30 weeks in heart failure. As the pathology develops, endogenous αq/11 rises (2.9-fold in atria; 1.8-fold in ventricles). At 2 weeks, basal PLC activity is increased 9- to 10-fold in atria but not ventricles. By 10 weeks, it is elevated in both, presumably because of the rise in endogenous αq/11. We conclude that the pathological changes initiated by early, transient HAα*q expression are maintained in part by compensatory changes in signal transduction and other pathways. Cyclosporin A (CsA) prevents hypertrophy caused by activation of calcineurin [Molkentin, J. D., Lu, J.-R., Antos, C. L., Markham, B., Richardson, J., Robbins, J., Grant, S. R. & Olson, E. N. (1998) Cell 93, 215–228]. Because HAα*q acts upstream of calcineurin, we hypothesized that HAα*q might initiate additional pathways leading to hypertrophy and dilatation. Treating HAα*q mice with CsA diminished some, but not all, aspects of the hypertrophic phenotype, suggesting that multiple pathways are involved.

To investigate the physiological roles of gp130 in detail and to determine the pathological consequence of abnormal activation of gp130, transgenic mice having continuously activated gp130 were created. This was carried out by mating mice from interleukin 6 (IL-6) and IL-6 receptor (IL-6R) transgenic lines. Offspring overexpressing both IL-6 and IL-6R showed constitutive tyrosine phosphorylation of gp130 and a downstream signaling molecule, acute phase response factor/signal transducer and activator of transcription 3. Surprisingly, the distinguishing feature of such offspring was hypertrophy of ventricular myocardium and consequent thickened ventricular walls of the heart, where gp130 is also expressed, in adulthood. Transgenic mice overexpressing either IL-6 or IL-6R alone did not show detectable myocardial abnormalities. Neonatal heart muscle cells from normal mice, when cultured in vitro, enlarged in response to a combination of IL-6 and a soluble form of IL-6R. The results suggest that activation of the gp130 signaling pathways leads to cardiac hypertrophy and that these signals might be involved in physiological regulation of myocardium.

Background—The molecular mechanisms underlying similarities and differences between physiological and pathological left ventricular hypertrophy (LVH) are of intense interest. Most previous work involved targeted analysis of individual signaling pathways or screening of transcriptomic profiles. We developed a network biology approach using genomic and proteomic data to study the molecular patterns that distinguish pathological and physiological LVH. Methods and Results—A network-based analysis using graph theory methods was undertaken on 127 genome-wide expression arrays of in vivo murine LVH. This revealed phenotype-specific pathological and physiological gene coexpression networks. Despite >1650 common genes in the 2 networks, network structure is significantly different. This is largely because of rewiring of genes that are differentially coexpressed in the 2 networks; this novel concept of differential wiring was further validated experimentally. Functional analysis of the rewired network revealed several distinct cellular pathways and gene sets. Deeper exploration was undertaken by targeted proteomic analysis of mitochondrial, myofilament, and extracellular subproteomes in pathological LVH. A notable finding was that mRNA–protein correlation was greater at the cellular pathway level than for individual loci. Conclusions—This first combined gene network and proteomic analysis of LVH reveals novel insights into the integrated pathomechanisms that distinguish pathological versus physiological phenotypes. In particular, we identify differential gene wiring as a major distinguishing feature of these phenotypes. This approach provides a platform for the investigation of potentially novel pathways in LVH and offers a freely accessible protocol (http://sites.google.com/site/cardionetworks) for similar analyses in other cardiovascular diseases.

Markers of increased oxidative stress are known to be elevated following acute myocardial infarction and in the context of chronic left ventricular hypertrophy or heart failure, and their levels may correlate with the degree of contractile dysfunction or cardiac deficit. An obvious pathological mechanism that may account for this correlation is the potential deleterious effects of increased oxidative stress through the induction of cellular dysfunction, energetic deficit or cell death. However, reactive oxygen species have several much more subtle effects in the remodelling or failing heart that involve specific redox-regulated modulation of signalling pathways and gene expression. Such redox-sensitive regulation appears to play important roles in the development of several components of the phenotype of the failing heart, for example cardiomyocyte hypertrophy, interstitial fibrosis and chamber remodelling. In this article, we review the evidence supporting the involvement of reactive oxygen species and redox signalling pathways in the development of cardiac hypertrophy and heart failure, with a particular focus on the NADPH oxidase family of superoxide-generating enzymes which appear to be especially important in redox signalling.

BACKGROUND: The development of heart failure is associated with changes in the size, shape, and structure of the heart that has a negative impact on cardiac function. These pathological changes involve excessive extracellular matrix deposition within the myocardial interstitium and myocyte hypertrophy. Alterations in fibroblast phenotype and myocyte activity are associated with reprogramming of gene transcriptional profiles that likely requires epigenetic alterations in chromatin structure. The aim of our work was to investigate the potential of a currently licensed anticancer epigenetic modifier as a treatment option for cardiac diseases associated with hypertension-induced cardiac hypertrophy and fibrosis.

METHODS AND RESULTS: The effects of DNA methylation inhibition with 5-azacytidine (5-aza) were examined in a human primary fibroblast cell line and in a spontaneously hypertensive rat (SHR) model. The results from this work allude to novel in vivo antifibrotic and antihypertrophic actions of 5-aza. Administration of the DNA methylation inhibitor significantly improved several echocardiographic parameters associated with hypertrophy and diastolic dysfunction. Myocardial collagen levels and myocyte size were reduced in 5-aza-treated SHRs. These findings are supported by beneficial in vitro effects in cardiac fibroblasts. Collagen I, collagen III, and α-smooth muscle actin were reduced in a human ventricular cardiac fibroblast cell line treated with 5-aza.

CONCLUSION: These findings suggest a role for epigenetic modifications in contributing to the profibrotic and hypertrophic changes evident during disease progression. Therapeutic intervention with 5-aza demonstrated favorable effects highlighting the potential use of this epigenetic modifier as a treatment option for cardiac pathologies associated with hypertrophy and fibrosis.

Premature dropout from treatments for pathological gambling is potentially of significant importance, if it occurs before substantial progress has been made in addressing the problem. A systematic review of current research on dropout from psychological treatments for pathological gambling identified 12 studies from five countries. Dropout ranged from 14% to 50%, with a median of dropout 26%. Overall, 31% of the participants dropped out of treatment. Few studies distinguish between dropouts at different stages of participation. The evidence on specific variables that predict dropout is limited or inconsistent, and is characterised by a lack of a coherent, gambling-specific model and by methodological problems. Two studies that attempted to apply motivational and compliance-enhancing techniques were found. Both showed promising effects on reduction of dropout and improvement of short-term impact of treatment, but inconsistent results on longer-term outcomes were obtained. The review highlighted a need for more rigorous investigation of the extent of dropout and of variables associated with dropout from pathological gambling treatment programs. Further research on interventions to enhance retention and reduce dropout from psychological treatment is also required.

This paper will examine the literature on ‘anorexia nervosa’, and argue that it is underpinned by three fundamental assumptions. First, ‘anorexia nervosa’ is a reflection of the mismatch between true ‘inner self’ and the external ‘false self’, the latter self being the distorted product of a male dominated society. Second, the explanation for the severe fasting practices constitutive of ‘anorexia nervosa’ (a new social problem) is to be found within the binary opposition of resistance/conformity to contemporary cultural expectations. Finally, ‘anorexia nervosa’ is a problem which exists in nature (i.e., independently of analysis). It was eventually discovered, named and explained. This paper will problematise each of these assumptions in turn, and in doing so, it will propose an alternative way of understanding contemporary fasting practices.
