Purpose: The purpose of this study was to evaluate, in vitro, the contamination of toothbrushes and pacifiers by Streptococcus mutans, and the efficacy of microwave and chlorhexidine for their disinfection. Methods: Sixty pacifiers and 60 toothbrushes were contaminated with S mutans and then divided into groups according to the disinfection protocol: Group 1-chlorhexidine solution; Group 2-microwave sterilization; and Group 3-sterile tap water. The devices were evaluated microbiologically as to the formation of S mutans colonies/biofilms and were examined by scanning electron microscopy. The results were submitted for statistical analysis by Friedman`s test at a 5% significance level. Results: The results of both types of evaluation showed a large number of S mutans colonies/biofilms after spraying with sterile tap water, and chlorhexidine spraying and microwaving were effective in eliminate colonies/biofilms. Groups 1 and 2 were statistically similar to each other (P>.05) and differed significantly from Group 3 (P<.05). Conclusions: The 0.12% chlorhexidine solution spray and 7 minutes of microwave irradiation were effective for disinfection of pacifiers and toothbrushes. OBJETIVO: Investigar a prevalência de consumo de alimentos complementares e os fatores associados à alimentação complementar oportuna em menores de um ano. MÉTODOS: Participaram do estudo 1 176 crianças, durante a Campanha Nacional de Vacinação de 2003, em São Bernardo do Campo (SP), cujos acompanhantes responderam questionário que incluiu questões sobre a alimentação da criança nas 24 horas precedentes. A estimativa da prevalência de consumo dos alimentos complementares foi realizada por um modelo de regressão logística ajustado por idade; as medianas de introdução de alimentos por análise de sobrevida e os fatores associados à alimentação complementar oportuna por regressão de Poisson com ajuste robusto de variância e seleção hierarquizada de variáveis. RESULTADOS: Observou-se introdução precoce de alimentos complementares: no quarto mês, cerca de um terço das crianças recebiam suco de fruta e um quarto das crianças recebiam mingau, fruta ou sopa, ao passo que a probabilidade de consumir a comida da família aos oito meses foi baixa (48%). A mediana de idade para o consumo de frutas foi de 266 dias (IC95% 256-275), de papa de legumes foi 258 dias (IC95% 250-264) e comida da família, 292 dias (IC 95% 287-303). Os fatores associados ao consumo de alimentos sólidos antes dos seis meses de idade foram: sistema de assistência à saúde; idade materna; trabalho materno e uso de chupeta. CONCLUSÃO: O consumo precoce de alimentos sólidos, um risco potencial para a saúde infantil e para o desenvolvimento de doenças crônicas na idade adulta, evidenciam a necessidade de ações programáticas para reversão deste quadro.

Objetivo: Investigar a avaliação de mães de recém-nascidos pré-termo (RNPT) egressos de unidades de terapia intensiva neonatal (UTIN) quanto à interação mãe-bebê e uso de chupeta nos primeiros dois anos. Método: O planejamento do estudo longitudinal foi baseado na Teoria Bioecológica do Desenvolvimento Humano, com foco nos processos proximais (PP), utilizando entrevistas gravadas com 62 mães de RNPT no contexto da UTIN e 33 aos seis, 12, 18 e 24 meses de idade do bebê, considerando Grupo-A (chupeta) e Grupo-B (não usou chupeta). Resultados: A vivência em UTIN foi considerada evento impactante na vida das mães, mas expectativas futuras para a relação mãe-bebê foram positivas. A tentativa de oferta da chupeta foi 96,2% e seu uso aos seis meses foi 50% (n=52), significativamente associado com prematuridade pela relação peso/idade-gestacional (p-valor=0,044), dificuldades para estabelecer aleitamento materno exclusivo (AME) (p=0,012) e primiparidade (p=0,02). Apresentaram relação com menor frequência de chupeta: AME ≥3 meses (p=0,026) e tempo de aleitamento materno ≥6 meses. A chupeta configurou-se como uma das representações sociais sobre objetos de bebê, elaboradas pelas participantes aos 12 meses de idade do bebê. Características de temperamento calmo/tranquilo da mãe foram mais frequentes no Grupo-A e o temperamento nervoso/agitado/irritado no Grupo-B (p-valor=0,041). No Grupo-A predominou o temperamento do bebê calmo/fácil-de-cuidar/independente, enquanto no Grupo-B as características de temperamento agitado/bagunceiro/teimoso/agressivo (p-valor=0,026), associado também à necessidade de várias tentativas de oferta da chupeta (p-valor=0,006). No Grupo-A, o número de pessoas para apoio social foi uma ou duas (77,8%), enquanto no Grupo-B foram três a sete (66,7%), p-valor=0,001. A contribuição da chupeta como auxiliar nos PP foi indiferente para mães que controlavam o hábito, enquanto o uso irrestrito facilitava a resolução do choro, liberando a mãe para outras tarefas, atuando como limitador dos PP. A análise da evolução e complexidade dos PP demonstrou não haver interferência pelo uso da chupeta, tendo sido mais efetivos quando as mães tinham maior escolaridade e nas classes econômicas A e B. Conclusão: Aspectos culturais influenciaram na oferta da chupeta, mas sua aceitação ocorreu principalmente em RNPT pequeno para idade gestacional, diante das dificuldades para AME, menor extensão do apoio social e temperamento do bebê calmo/fácil-de-cuidar/independente, também associado à aceitação mais fácil da chupeta. O uso irrestrito da chupeta demonstrou atuar como limitador dos processos proximais.

OBJECTIVE To analyze the factors associated with the use of pacifiers and/or bottle feeding in infants aged under one year.METHODS This is a cross-sectional study with 34,366 children and using data from the database of the 2nd Nationwide Survey of Breastfeeding Prevalence performed in the Brazilian capitals and Federal District in 2008. Cluster sampling was used. The questionnaire included questions about the use of artificial nipples in the last 24 hours. The analysis considered three outcomes: exclusive use of pacifier, exclusive use of bottle feeding, and use of artificial nipples (pacifier and bottle feeding). Prevalence ratios were obtained using Poisson regression with robust variance following a hierarchical model.RESULTS The following factors were associated with exclusive use of the pacifier: mother working outside the home, primiparity, child was not breastfed within the first hour, and child had consumed tea on the first day at home. The following factors were associated with exclusive use of bottle feeding: mother working outside the home, primiparity, low birth weight, child not breastfed within the first hour, and child had consumed milk formula and tea on the first day at home. The following factors were associated with use of artificial nipples (pacifier and bottle feeding): mother working outside the home, primiparity, cesarean delivery, the male gender, low birth weight, born in a hospital not accredited as “baby friendly”, required health baby monitoring in the Primary Health Care Unit (PR = 0.91), and child had consumed milk formula, water, or tea on the first day at home.CONCLUSIONS This study identified profiles of exclusive users of pacifiers, bottle feeding, and both. The provided information can guide preventive practices for child health.

ABSTRACT OBJECTIVE To evaluate whether the support offered by maternity hospitals is associated with higher prevalences of exclusive and predominant breastfeeding. METHODS This is a cross-sectional study including a representative sample of 916 infants less than six months who were born in maternity hospitals, in Ribeirao Preto, Sao Paulo, Southeastern Brazil, 2011. The maternity hospitals were evaluated in relation to their fulfillment of the Ten Steps to Successful Breastfeeding. Data were collected regarding breastfeeding patterns, the birth hospital and other characteristics. The individualized effect of the study factor on exclusive and predominant breastfeeding was analyzed using Poisson multiple regression with robust variance. RESULTS Predominant breastfeeding tended to be more prevalent when the number of fulfilled steps was higher (p of linear trend = 0.057). The step related to not offering artificial teats or pacifiers to breastfed infants and that related to encouraging the establishment of breastfeeding support groups were associated, respectively, to a higher prevalence of exclusive (PR = 1.26; 95%CI 1.04;1.54) and predominant breastfeeding (PR = 1.55; 95%CI 1.01;2.39), after an adjustment was performed for confounding variables. CONCLUSIONS We observed a positive association between support offered by maternity hospitals and prevalences of exclusive and predominant breastfeeding. These results can be useful to other locations with similar characteristics (cities with hospitals that fulfill the Ten Steps to Successful Breastfeeding) to provide incentive to breastfeeding, by means of promoting, protecting and supporting breastfeeding in maternity hospitals.

Objective: To study the trends and patterns of exclusive breastfeeding (EBF) for under-6-month-old infants in the city of Bauru, southeastern Brazil.Methods: We compared data from three cross-sectional surveys, using similar methodologies, which were part of a project for monitoring breastfeeding indicators in the state of São Paulo, Brazil. The sample included infants aged 0 to 6 months who attended one of the two rounds of the nationwide infant vaccination campaign in 1999, 2003 and 2006 (respectively: 496, 674 and 509 infants). Descriptive statistics were used to compare the prevalence of EBF according to age (in months) and group of children under 6 months of age. Differences in prevalence were expressed as percentage-points and submitted to statistical analysis (Pearson's chi-square and tendency), and the level of significance was set at p < 0.05. Factors associated with EBF interruption in 2006 were evaluated by univariate and multivariate analyses.Results: An increase in the prevalence of EBF was observed in under-6-month-old infants: 1999-2003, increase of 9.1 percentage-points; 2003-2006, increase of 6.6 percentage-points, resulting in an annual increase rate of 2.3 percentage-points for the first period and 2.2 percentage-points for the second period. Significant inverse association was observed between EBF and the use of pacifiers (prevalence ratio = 2.03; 95% confidence interval 1.44-2.84).Conclusion: EBF prevalence in under-6-month-old infants in the city of Bauru, southeastern Brazil, increased almost threefold over the period studied, from 8.5% in 1999 to 24.2% in 2006, a total increase of 184.7%. The use of pacifiers was the only factor strongly associated with the interruption of EBF.

Objective: To measure and compare the activity of the masseter, temporalis and buccinator muscles in different infant feeding methods. Method: Cross-sectional study of 60 full-term infants with no intercurrent diseases, aged between two and three months, classified into the following groups: 1) exclusive breastfeeding; 2) breastfeeding plus bottle-feeding; and 3) exclusive breastfeeding plus cup feeding. Surface electromyography was performed during infant feeding. The Krushal-Wallis test was used, complemented by multiple paired comparisons of the groups. A 5% significance level was chosen for the tests. Results: Statistically higher results were verified in the breastfeeding group in relation to the bottle-feeding one, both in the range of movement and the mean contraction of the masseter. With regard to the temporalis muscle, statistically higher results were found in the breastfeeding group comparatively to the bottle-feeding one. As to the buccinator muscle, statistically higher results were observed in the breastfeeding group in relation to the bottle-feeding one, although in this case, the difference concerned only the range of contraction. Conclusion: The similarities between the muscle activity in the breastfeeding and in the cup-feeding groups suggests that cup-feeding can be used as an alternative infant feeding method, being better than bottle-feeding, due to the hyperactivity of the buccinator muscle, which could result in changes to the structural growth and development of the stomatognathic system functions. In addition to providing nutrition and immunological protection, breast-feeding has positive effects on the development of the infant's oral cavity. The aim of the present study is to assess breast-feeding patterns and to analyze the influence of breast-feeding practices and maternal sociodemographic variables on the prevalence of non-nutritive sucking habits in a sample of Brazilian infants. This cross-sectional study was carried out in Southern Brazil. A random sample of 100 mothers of infants up to 12 months of age was interviewed during the National Vaccination Campaign Day. The prevalence and median duration of breast-feeding were assessed. Breast-feeding practice, the exposure factor, was categorized as exclusive breast-feeding, predominant breast-feeding, complementary breast-feeding, or weaning. Maternal sociodemographic variables included age, race, marital status, educational level, profession, and family income. The outcome investigated was the prevalence of sucking habits (pacifier use and thumb sucking). We used two-sample tests, the chi-square test and Fisher exact test0 for statistical analyses of the data. The study revealed that 75% of infants were being breast-fed. Pacifier use and thumb sucking were common in 55%. Bottle-feeding was prevalent in 74% of infants. Breast-feeding was negatively correlated with pacifier use and thumb sucking (OR = 0.11; 95% CI: 0.03 to 0.4). Bottle-feeding was strongly associated with weaning (p = 0.0003). Among the sociodemographic variables, only marital status showed a statistical association with sucking habits (p = 0.04). These findings suggest that breast-feeding can prevent the occurrence of sucking habits. Although we could not evaluate causality assessment, malocclusion prevention seems to be yet one more reason for promoting breast-feeding practices.

Este estudo transversal e descritivo objetivou verificar a prevalência de hábitos de sucção em pré-escolares e a percepção dos pais sobre a relação com a ocorrência de maloclusões. A população do estudo constituiu-se por uma amostra representativa de pais de pré-escolares de 4 meses a 6 anos de idade. Utilizou-se um questionário semiestruturado, composto por questões abertas e fechadas, referentes à frequência e conhecimentos dos pais frente aos hábitos de sucção não nutritivos. Dos 356 participantes da pesquisa, 70,8% afirmaram que as crianças apresentavam algum hábito bucal, sendo a sucção de chupeta o mais frequente (45,6%). Apesar da grande maioria dos pesquisados (97,1%) relatarem saber que os hábitos podiam causar prejuízo aos dentes, 70,2% deles já haviam oferecido chupeta à criança, na maioria das vezes para acalmá-la (61,8%). Houve associação estatisticamente significativa entre a oferta da chupeta à criança e o conhecimento sobre a relação da presença de hábitos não nutritivos e a ocorrência de maloclusão (p < 0,0001 e Qui-quadrado = 60,123). A prevalência de hábitos bucais na população estudada é alta e, apesar da maioria dos pais saberem que o hábito de sucção de chupeta pode causar danos à saúde bucal, ofertavam a chupeta a fim de acalmar a criança.

The Rooming favors the permanence of the mother with the newborn, thus establishing a stronger link between the mother and baby, in addition, care assistance and guidance are essential to both through the health care team because they recognize the moments critical that their interventions are necessary to ensure the health of both. The educational lecture was an option to have the opportunity to assist, guide and answer possible questions that may arise for mothers, and also to assess the knowledge of puéperas regarding the care of babies and self care. There was participation from 40 patients. In the study methodology was developed a questionnaire containing two subjects (self-care and care for newborns) was given to patients before and after the lecture held by the author in a rooming Maternity HC de Botucatu. The results were analyzed statistically and studied by the author, allowing note that many significant results were obtained after the lecture, which we realize that the same intervention on self-care was significant in respect of: bleeding, infection in the genital tract and urinary tract, surgical dehiscence , care of the lower limbs and constipation, how to care for the newborn was the significance to the questions about the hygiene of the umbilical stump, cramps, hunger, weeping and pacifiers. In the pre-lecture questions 73.00% of 27.00% were right and wrong. Our conclusion is that the mothers have a high level of prior knowledge and educational lectures enriches their knowledge in a creative and participatory

What is the relationship of a pacifier as for nutrition, speech, dental and psychotherapy? What are its benefits and harm? Which strategies should be developed as a teaching material which can guide employees and teachers of early childhood education regarding the use of pacifier in schools? The survey is a qualitative and an exploratory one. By taking literature as a starting point. We found out that there are many doubts regarding the use of pacifier, even among health professionals. The booklet was prepared containing topics such as: social and historical aspects; psycho-physiological aspects of development, speech therapy and dental aspects; pros and cons of using a pacifier; teaching materials on the pedagogical use of pacifiers. Although sucking activity has been considered an essential behavior of early childhood to satisfy nutritive and non-nutritive needs, digit and pacifier sucking are deleterious oral habits that may interfere with child development. Furthermore, this clinical picture may be compounded by other concerning habits, such as self-mutilating behavior. This article reports 4-year follow-up of a child in whom non-nutritive sucking was associated with an unusual self-mutilating behavior; namely, the child would pull out her own hair after wrapping it around her finger every time she sucked on the pacifier. This occurred specially at bedtime, while she was watching TV, or when she was somewhat anxious, and remitted and recurred throughout the follow-up period. In an attempt to address this behavior, pacifier use was discontinued and the child’s head was shaved. Ultimately, the case was only solved through combined efforts involving the child, her family, and health professionals. Based on the parents’ reports and clinical examination and follow-up findings, we emphasize the importance of investigating the origin of the problem and considering emotional aspects and its association with other habits in such cases.

The objective of this cross-sectional and quantitative study was to identify the prevalence and determinants of exclusive breastfeeding among infants less than six months of age in the city of Serrana, Sao Paulo, Brazil in 2009. A validated semi-structured questionnaire was administered to the guardians of the children less than six months of age who attended the second phase of a Brazilian vaccination campaign against polio. Univariate and multivariate analysis presented in odds ratios and confidence intervals was accomplished. Of the total of 275 infant participants, only 29.8% were exclusively breastfed. Univariate analysis revealed that mothers who work outside the home without maternity leave, mothers who did not work outside the home, adolescent mothers, and the use of pacifiers have a greater chance of interrupting exclusive breastfeeding. In the multivariate analysis, mothers who work outside the home without maternity leave are three times more likely to wean their children early. Results provide suggestions for the redirection and planning of interventions targeting breastfeeding.
