O diagnóstico precoce e o tratamento são imprescindíveis para o controle da hanseníase, pois visam eliminar o mais precocemente possível as fontes de transmissão. O principal critério de cura da doença é o tempo de tratamento, relacionado às doses fixas da poliquimioterapia. O tempo de tratamento dos pacientes multibacilares foi reduzido de 24 para 12 doses em até 18 meses em 1998. Os objetivos desta tese foram: avaliar a detecção de casos de hanseníase entre os contatos dos pacientes e avaliar fatores de risco associados à piora das incapacidades físicas nos pacientes após a alta do tratamento poliquimioterápico com as 12 doses mensais. Dois artigos foram elaborados. O primeiro artigo avaliou características dos pacientes com hanseníase e de seus contatos associadas ao risco de adoecimento entre os contatos. O segundo artigo avaliou fatores de risco relacionados à piora das incapacidades físicas dos pacientes com hanseníase multibacilar, submetidos ao tratamento com 12 doses fixas da poliquimioterapia. Os resultados mostraram que o risco para hanseníase, relacionado aos contatos foi o tipo de convivência com o caso índice. Entre os fatores dos casos índices, a carga bacilar foi o único associado ao adoecimento dos contatos. Além desses fatores, os contatos que apresentam relação de consangüinidade com o caso índice e os contatos dos pacientes com menor nível de escolaridade mostraram maior chance de se apresentarem doentes no momento do diagnóstico do caso índice. A cicatriz BGC e a vacina recebida após o diagnóstico do caso índice contribuíram independentemente como fatores de proteção. A neuropatia piorou em 40% dos pacientes durante o período de 10 anos após a alta do tratamento. Esta piora foi associada à presença de incapacidades físicas e ao número de lesões cutâneas no momento do diagnóstico assim como à presença de neurite durante o acompanhamento.

Porous manganese oxide (OMS-2) and platinum supported on OMS-2 catalysts have been shown to facilitate the hydrogenation of the nitro group on chloronitrobenzene to give chloroaniline with no dehalogenation. Complete conversion was obtained within 2 h at 25 [degree]C and, although the rate of reaction increased with increasing temperature up to 100 [degree]C, the selectivity to chloroaniline remained at 99.0%. Use of Pd/OMS-2 or Pt/Al2O3 resulted in significant dechlorination even at 25 [degree]C and 2 bar hydrogen pressure giving selectivity to chloroaniline of 34.5% and 77.8%, respectively, at complete conversion. This demonstrates the potential of using platinum group metal free catalysts for the selective hydrogenation of halogenated aromatics. Two pathways were observed for the analogous nitrobenzene hydrogenation depending on the catalyst used. The hydrogenation of nitrobenzene was found to follow a direct pathway to aniline and nitrosobenzene over Pd/OMS-2 in contrast to the OMS and Pt/OMS-2 catalysts which resulted in formation of nitrosobenzene, azoxybenzene and azobenzene/hydrazobenzene intermediates before complete conversion to aniline. These results indicate that for the Pt/OMS-2 the hydrogenation proceeds predominantly over the support with the metal acting to dissociate the hydrogen. In the case of the Pd/OMS-2 both the hydrogenation and the hydrogen adsorption occur on the metal sites.

The World Health Organization (WHO) recommends that prescriptions of medicaments must be one parameter utilized to know how the medication is used by people. To do so, one specific methodology has been indicated, that is, the selected indicators of the use the medicaments. We applied this methodology with the aim of presenting a descriptive study of the physician prescription in Pediatric Clinic of the Health Basics Units. The results obtained are: the average number of medicaments prescribed with prescription was 2.6; the percentage of the medicaments prescribed with the generic name was 32%; the percentage of the prescriptions with antibiotics was 44.6%; the percentage of the prescriptions with injections was 10.4%; the percentage of the medicaments prescribed that are part of the municipal list was 22.8%, 27.5% for the program Dose Certs of the secretary of the State of São Paulo, and 50.4% by adding by adding the two lists. The essentiality of the medicaments prescribed may be considered very low, because only 32.6% appear in the RENAME. The percentage of the medicaments really dispensed was 39.9%. The medicaments more prescribed, according to the classification of the Anatomic-Therapeutic-Chemical (ATC), were the ones which acted in Respiratory System (26.8%), followed by anti-infectives for systemic use (16.4%) and by medicaments with acted in Alimentary tract and metabolism (15.8%). The conclusion is that the prescribers are moved by commercial influence of the drug makers, first because of the low prescription of medicaments by their generic name, second by the low number of medicaments effectively dispensed and then the low concordance between the ones prescribed and the ones included in the lists patterned by Tabatinga and by the State of São Paulo.

WHO has launched the “Safe Surgery Saves Lives” program with the purpose to reduce damage to surgical patients and define safety standards specifically in order to reduce such patients’ morbi-mortality by providing surgery teams and hospital managers with orientation about the standards of safe surgery and a uniform service evaluation instrument for national and international surveillance. Hence, this study aimed at applying the WHO safe-surgery checklist to the surgical specialties of a university hospital and evaluating the team’s opinion about the influence of its application on the safety of surgical process and on the team’s interpersonal communication. It was a descriptive, analytical, qualitative field study conducted in the surgery facilities of a university hospital in a public establishment in São Paulo state. The checklist was applied to eight surgical specialties, resulting in a total number of 30 surgeries. Its application was conducted by the researcher in three phases: Sign in, Time out, Sign Out. Next, one member of the surgery team was invited to voluntarily participate in the study by signing an informed consent form and answering guiding questions. Thirty members of the surgery team participated in the study. Bardin’s Content Analysis Method was used to organize and analyze the data. As regards the safety provided by the checklist, the following thematic categories emerged: “It reduces risk and possible complications”; “It standardizes conducts and reviews safety steps”; “It allows for better understanding of the process”; and “It provides safety to the team as a whole”. The category “It is not included in the institution’s routine” emerged from the subjects’ statements when they understood that, in this form, the checklist does not provide safety to surgical procedures. The World Health Organization (WHO) has created a program “Safe Surgery Saves Lives”, focusing on reduce and minimize life threatening or cause serious risk to health for patients submitted to a surgery. This primary goal for this presented study is to apply the checklist from the WHO´s program for children´s surgery in a public university hospital, located in the countryside of São Paulo State, Brazil. Having the responses from professionals surgery´s team, there is an opportunity for evaluation of the features and easiest parts, which are filled in a quickly and in a easy way; focus, supporting and assistance; issues, such as lack of time for filling in, and viability of application of this protocol considering the improvement of the cross functions relationship, and quality of services, providing better assistance and support to the patient
