Introduction The purpose of this study was to develop, implement and evaluate the impact of an educational intervention, comprising an innovative model of clinical decisionmaking and educational delivery strategy for facilitating nursing students‘ learning and development of competence in paediatric physical assessment practices. Background of the study Nursing students have an undergraduate education that aims to produce graduates of a generalist nature who demonstrate entry level competence for providing nursing care in a variety of health settings. Consistent with population morbidity and health care roles, paediatric nursing concepts typically form a comparatively small part of undergraduate curricula and students‘ exposure to paediatric physical assessment concepts and principles are brief. However, the nursing shortage has changed traditional nursing employment patterns and new graduates form the majority of the recruitment pool for paediatric nursing speciality staff. Paediatric nursing is a popular career choice for graduates and anecdotal evidence suggests that nursing students who select a clinical placement in their final year intend to seek employment in paediatrics upon graduation. Although concepts of paediatric nursing are included within undergraduate curriculum, students‘ ability to develop the required habits of mind to practice in what is still regarded as a speciality area of practice is somewhat limited. One of the areas of practice where this particularly impacts is in paediatric nursing physical assessment. Physical assessment is a fundamental component of nursing practice and competence in this area of practice is central to nursing students‘ development of clinical capability for practice as a registered nurse. Timely recognition of physiologic deterioration of patients is a key outcome of nurses‘ competent use of physical assessment strategies, regardless of the practice context. In paediatric nursing contexts children‘s physical assessment practices must specifically accommodate the child‘s different physiological composition, function and pattern of clinical deterioration (Hockenberry & Barrera, 2007). Thus, to effectively manage physical assessment of patients within the paediatric practice setting nursing students need to integrate paediatric nursing theory into their practice. This requires significant information processing and it is in this process where students are frequently challenged. The provision of rules or models can guide practice and assist novice-level nurses to develop their capabilities (Benner, 1984; Benner, Hooper-Kyriakidis & Stannard, 1999). Nursing practice models are cognitive tools that represent simplified patterns of expert analysis employing concepts that suit the limited reasoning of the inexperienced, and can represent the =rules‘ referred to by Benner (1984). Without a practice model of physical assessment students are likely to be uncertain about how to proceed with data collection, the interpretation of paediatric clinical findings and the appraisal of findings. These circumstances can result in ad hoc and unreliable nursing physical assessment that forms a poor basis for nursing decisions. The educational intervention developed as part of this study sought to resolve this problem and support nursing students‘ development of competence in paediatric physical assessment. Methods This study utilised the Context Input Process Product (CIPP) Model by Stufflebeam (2004) as the theoretical framework that underpinned the research design and evaluation methodology. Each of the four elements in the CIPP model were utilised to guide discrete stages of this study. The Context element informed design of the clinical decision-making process, the Paediatric Nursing Physical Assessment model. The Input element was utilised in appraising relevant literature, identifying an appropriate instructional methodology to facilitate learning and educational intervention delivery to undergraduate nursing students, and development of program content (the CD-ROM kit). Study One employed the Process element and used expert panel approaches to review and refine instructional methods, identifying potential barriers to obtaining an effective evaluation outcome. The Product element guided design and implementation of Study Two, which was conducted in two phases. Phase One employed a quasiexperimental between-subjects methodology to evaluate the impact of the educational intervention on nursing students‘ clinical performance and selfappraisal of practices in paediatric physical assessment. Phase Two employed a thematic analysis and explored the experiences and perspectives of a sample subgroup of nursing students who used the PNPA CD-ROM kit as preparation for paediatric clinical placement. Results Results from the Process review in Study One indicated that the prototype CDROM kit containing the PNPA model met the predetermined benchmarks for face validity and the impact evaluation instrumentation had adequate content validity in comparison with predetermined benchmarks. In the first phase of Study Two the educational intervention did not result in statistically significant differences in measures of student performance or self-appraisal of practice. However, in Phase Two qualitative commentary from students, and from the expert panel who reviewed the prototype CD-ROM kit (Study One, Phase One), strongly endorsed the quality of the intervention and its potential for supporting learning. This raises questions regarding transfer of learning and it is likely that, within this study, several factors have influenced students‘ transfer of learning from the educational intervention to the clinical practice environment, where outcomes were measured. Conclusion In summary, the educational intervention employed in this study provides insights into the potential e-learning approaches offer for delivering authentic learning experiences to undergraduate nursing students. Findings in this study raise important questions regarding possible pedagogical influences on learning outcomes, issues within the transfer of theory to practice and factors that may have influenced findings within the context of this study. This study makes a unique contribution to nursing education, specifically with respect to progressing an understanding of the challenges faced in employing instructive methods to impact upon nursing students‘ development of competence. The important contribution transfer of learning processes make to students‘ transition into the professional practice context and to their development of competence within the context of speciality practice is also highlighted. This study contributes to a greater awareness of the complexity of translating theoretical learning at undergraduate level into clinical practice, particularly within speciality contexts.

The increase of life expectancy worldwide during the last three decades has increased age-related disability leading to the risk of loss of quality of life. How to improve quality of life including physical health and mental health for older people and optimize their life potential has become an important health issue. This study used the Theory of Planned Behaviour Model to examine factors influencing health behaviours, and the relationship with quality of life. A cross-sectional mailed survey of 1300 Australians over 50 years was conducted at the beginning of 2009, with 730 completed questionnaires returned (response rate 63%). Preliminary analysis reveals that physiological changes of old age, especially increasing waist circumference and co morbidity was closely related to health status, especially worse physical health summary score. Physical activity was the least adherent behaviour among the respondents compared to eating healthy food and taking medication regularly as prescribed. Increasing number of older people living alone with co morbidity of disease may be the barriers that influence their attitude and self control toward physical activity. A multidisciplinary and integrated approach including hospital and non hospital care is required to provide appropriate services and facilities toward older people.

Aim. Our aim in this paper is to explain a methodological/methods package devised to incorporate situational and social world mapping with frame analysis, based on a grounded theory study of Australian rural nurses' experiences of mentoring. Background. Situational analysis, as conceived by Adele Clarke, shifts the research methodology of grounded theory from being located within a postpositivist paradigm to a postmodern paradigm. Clarke uses three types of maps during this process: situational, social world and positional, in combination with discourse analysis. Method. During our grounded theory study, the process of concurrent interview data generation and analysis incorporated situational and social world mapping techniques. An outcome of this was our increased awareness of how outside actors influenced participants in their constructions of mentoring. In our attempts to use Clarke's methodological package, however, it became apparent that our constructivist beliefs about human agency could not be reconciled with the postmodern project of discourse analysis. We then turned to the literature on symbolic interactionism and adopted frame analysis as a method to examine the literature on rural nursing and mentoring as secondary form of data. Findings. While we found situational and social world mapping very useful, we were less successful in using positional maps. In retrospect, we would argue that collective action framing provides an alternative to analysing such positions in the literature. This is particularly so for researchers who locate themselves within a constructivist paradigm, and who are therefore unwilling to reject the notion of human agency and the ability of individuals to shape their world in some way. Conclusion. Our example of using this package of situational and social worlds mapping with frame analysis is intended to assist other researchers to locate participants more transparently in the social worlds that they negotiate in their everyday practice. © 2007 Blackwell Publishing Ltd.

Objective: to analyze what nursing models and nursing assessment structures have been used in the implementation of the nursing process at the public and private centers in the health area Gipuzkoa (Basque Country). Method: a retrospective study was undertaken, based on the analysis of the nursing records used at the 158 centers studied. Results: the Henderson model, Carpenito's bifocal structure, Gordon's assessment structure and the Resident Assessment Instrument Nursing Home 2.0 have been used as nursing models and assessment structures to implement the nursing process. At some centers, the selected model or assessment structure has varied over time. Conclusion: Henderson's model has been the most used to implement the nursing process. Furthermore, the trend is observed to complement or replace Henderson's model by nursing assessment structures.

Notions of the "postmodern" pervade various fields of study, but have rarely been applied to the practice and theory of nursing. This paper uses some conceptions of the "postmodern" to remedy this. Though there are many contested usages of the term, here "postmodern" will be used broadly in a periodical sense to trace changes in society and culture from the "modernism" of the 18th and 19th centuries to current concerns about "postmodernism". How these changes have been reflected in nursing practice and nursing theory will be explored. The changing use of the term "modern" to describe up-to-date practice will be addressed in the course of this. It is suggested that contextualizing nursing as a social/cultural activity in this way offers perspectives which will help us untangle the conflicting agendas and issues which form the fabric of the social world in which current nursing takes place, enabling us to act more effectively in promoting our own professional agendas.

Aim. The aim of this study was to describe, explore and explain the concept of sustainability in nursing. Background. Although researchers in nursing and medicine have emphasised the issue of sustainability and health, the concept of sustainability in nursing is undefined and poorly researched. A need exists for theoretical and empirical studies of sustainability in nursing. Design. Concept analysis as developed by Walker and Avant. Method. Data were derived from dictionaries, international healthcare organisations and literature searches in the CINAHL and MEDLINE databases. Inclusive years for the search ranged from 1990 to 2012. A total of fourteen articles were found that referred to sustainability in nursing. Results. Sustainability in nursing involves six defining attributes: ecology, environment, future, globalism, holism and maintenance. Antecedents of sustainability require climate change, environmental impact and awareness, confidence in the future, responsibility and a willingness to change. Consequences of sustainability in nursing include education in the areas of ecology, environment and sustainable development as well as sustainability as a part of nursing academic programs and in the description of the academic subject of nursing. Sustainability should also be part of national and international healthcare organisations. The concept was clarified herein by giving it a definition. Conclusion. Sustainability in nursing was explored and found to contribute to sustainable development, with the ultimate goal of maintaining an environment that does not harm current and future generations' opportunities for good health. This concept analysis provides recommendations for the healthcare sector to incorporate sustainability and provides recommendations for future research.

The purpose of this study was twofold: (1) to evaluate the effect of a specific instructional Intervention, a Nursing Theory Laboratory, on increasing the retention of high risk students in the associate degree nursing program at Miami-Dade Community College in Miami, Florida; and (2) to identify predictors of success of high risk nursing students in this associate degree nursing program.^ Data were collected from the 195 nursing students enrolled in Nursing Fundamentals during the 1985-1987 academic years, and identified as high risk students. Control and experimental groups were selected based on enrollment in the Nursing Theory Laboratory.^ Results were determined by analyzing several cross-tabulations of selected variables and yielding chi square values, t-tests, and two discriminant analyses. There was no significant relationship between age or ethnic background and enrollment in the Nursing Theory Laboratory. There was no significant relationship between enrollment in the Nursing Theory Laboratory and success in Nursing 1 (Nursing Fundamentals). There was a significant relationship between enrollment in the Nursing Theory Laboratory and success in Nursing 3 (Medical-Surgical Nursing). Writing assessment test scores in two entrance tests and high risk categories, based on the number of enrollments in required science courses, were identified as predictors of success in this program.^ The conclusion was that the Nursing Theory Laboratory does not significantly improve retention of high risk associate degree nursing students if they are enrolled in this intervention at the same time they are enrolled in Nursing Fundamentals. Since those students who were enrolled in the Nursing Theory Laboratory had a significantly higher success rate in Nursing 3, than those students who were not enrolled in the Nursing Theory Laboratory, a recommendation of this study was to offer the Nursing Theory Laboratory to high risk students prior to the beginning of nursing courses. Another recommendation was that students deficient in reading and writing skills should be required to enroll in developmental courses prior to enrollment in the nursing course. ^

Pressupõe-se que o cotidiano do cuidar da integralidade do cliente com câncer de cavidade bucal (CCB) reveste-se de significado especial devido à sensibilidade e responsabilidade dos profissionais de enfermagem; e, do mesmo modo, exige destas pessoas competência e habilidades técnicas e sociais para atuar constantemente com o sofrimento. Portanto, foram elaboradas as questões de pesquisa: quais são os limites e possibilidades da aplicação do processo clínico de cuidar em enfermagem ao cliente com câncer na cavidade bucal? Os objetivos foram: descrever os limites e possibilidades de aplicação do Process Clinical Caritas, formulado por Jean Watson, aos clientes com câncer na cavidade bucal; identificar as características individuais e profissionais dos membros da equipe de enfermagem atuantes na área de oncologia; identificar os aspectos do cuidar componentes do Process Clinical Caritas (PCC) aplicados pela equipe de enfermagem junto aos clientes com CCB, analisando a autopercepção da equipe de enfermagem sobre o seu desenvolvimento de tecnologias de cuidados, comparando-o à aplicação do PCC proposto por Jean Watson. O estudo centra-se na teorização do cuidado transpessoal, visando à compreensão dos aspectos do bem-estar propiciado, inclusive pela autêntica relação interpessoal entre profissional de enfermagem e cliente. Escolheu-se a abordagem de pesquisa quantitativa, aplicando-se o método descritivo e a técnica de autorrelato. A investigação ocorreu no período de agosto a setembro de 2012 em uma Instituição de Saúde Federal do Rio de Janeiro, Brasil, especializada em oncologia. Foram sujeitos do estudo 33 membros da equipe de enfermagem, atuantes em serviço de tratamento de CCB. Para implementar a técnica de autorrelato, utilizou-se um formulário, contendo, na primeira parte, as variáveis sociodemográficas e profissionais, e na segunda, uma adaptação dos 10 aspectos do PCC. Os dados quantitativos foram tratados mediante estatística descritiva simples, e as respostas sobre a aplicação do PCC foram submetidas à análise de conteúdo. Constatou-se que, do total de 33 sujeitos, 88% eram do sexo feminino, 37% na faixa etária de 30 a 39 anos. Predominou a renda individual de 4 a 8 salários mínimos. A maioria da equipe era composta por técnicos de enfermagem. Havia 26 especialistas em oncologia; 78% eram estatutários com carga horária de 40 horas semanais, com exercício predominante de 11 a 15 anos. Referente à aplicação dos 10 aspectos do PCC, foram delimitadas quatro categorias: respeitando as praticas espirituais e a religiosidade; proporcionando ao cliente uma relação de cuidado e conforto; ser presente e tratar o cliente com empatia e respeito; competência e orientação ao autocuidado para o cuidado integral ao cliente. Aplicando o critério de avaliação à prática dos aspectos do PCC, verificou-se que somente três: práticas de amor e gentileza; desenvolvendo relação de ajuda; e ajudar nas necessidades básicas atenderam ao PCC. Conclui-se, que além da realização dos cuidados técnicos ao cliente com CCB, é necessário que a equipe de enfermagem se conscientize da importância do cuidado transpessoal para a reconstituição do equilíbrio corporal físico, mental e espiritual do cliente, visando seu bem-estar, apesar das dificuldades do adoecimento.

A presente dissertação está inserida na linha de pesquisa Fundamentos Filosóficos, Teóricos e Tecnológicos do Cuidar em Saúde e Enfermagem da Faculdade de Enfermagem da UERJ, onde, tem-se como objeto do estudo: a dimensão espiritual do cuidado em Unidade de Terapia Intensiva. A questão norteadora é: considerando o exercício da integralidade como fator essencial para a prática do fazer enfermagem, como é ser enfermeira/o de unidades de terapia intensiva na prática do cuidado ao paciente abordando a religiosidade/espiritualidade? O objetivo é: Investigar o cuidado prestado pelos enfermeiros que trabalham em Unidades de Terapia Intensiva em relação à dimensão espiritual. Quanto à metodologia, trata-se de um estudo descritivo e exploratório, com abordagem qualitativa, de campo, respaldado na Resolução n 466, de 12 de dezembro de 2012 do Conselho Nacional de Saúde (CNS)/Ministério da Saúde (MS), aprovado pelo comitê de ética da UERJ (RJ-Brasil) através do Parecer N 253.923 em 18/04/2013. Os sujeitos do estudo foram 15 enfermeiros que trabalham em Unidade de Terapia Intensiva de instituições hospitalares públicas e privadas do Rio de Janeiro. A técnica para produção dos dados foi o Grupo Focal, realizado no mês de maio de 2013 na Faculdade de Enfermagem da UERJ. Para tratamento dos dados utilizou-se a análise de conteúdo delineado por Bardin. Emergiram três categorias: O espaço do cuidar em UTI; O significado de ser enfermeiro em UTI e A dimensão do cuidar em UTI. Procedeu-se uma segunda análise com categorias pré-estabelecidas apresentadas no Processo Clinical Caritas de Jean Watson. Conclui-se que os enfermeiros de UTI, imersos em um ambiente de tecnologia pesada, onde o foco maior do cuidado é a dimensão corporal, percebem as necessidades da dimensão espiritual dos pacientes, estimulam a esperança e a fé para aliviar o sofrimento causado pelo processo de adoecimento e da internação na UTI e revelam serem capaz de integrar-se a experiência do humano quando cuidam.

O presente estudo tem por objeto a promoção da saúde sexual de adolescentes em situação de acolhimento institucional. A saúde sexual de adolescentes institucionalizadas confere uma temática intrigante. Em um contexto particular, estas jovens recentemente iniciaram suas atividades sexuais e estão cercadas por disparidades sociais que aumentam os riscos de DST/Aids e gravidezes precoces. Esta situação desperta preocupações em relação aos seus comportamentos sexuais e às próprias estratégias desenvolvidas referentes a promoção da saúde sexual. Este trabalho teve como objetivo geral analisar a promoção da saúde sexual de adolescentes em situação de acolhimento institucional considerando a Teoria de Promoção da Saúde de Nola Pender e objetivos específicos, identificar e analisar os comportamentos e aspectos biológicos, psicológicos e socioculturais relacionados à saúde sexual de adolescentes acolhidas; identificar e analisar os sentimentos e fatores influenciadores das adolescentes acolhidas associados à promoção de sua saúde sexual; indicar possibilidades de cuidados auxiliadoras na autopromoção/autocuidado da saúde sexual das adolescentes em acolhimento. A pesquisa foi do tipo descritiva e exploratória, com abordagem qualitativa. O referencial teórico-metodológico utilizado foi A Teoria Modelo de Promoção da Saúde de Nola Pender, o qual, a partir da identificação dos fatores biopsicossociais e comportamentais, busca incentivar atitudes saudáveis como proposta de promoção da saúde. As participantes foram oito adolescentes, institucionalizadas, do sexo feminino, e que vivenciaram vida sexualmente ativa. O cenário do estudo foi uma instituição de acolhimento localizada no município do Rio de Janeiro. Para a produção dos dados, utilizou-se a técnica de entrevista estruturada, utilizando um roteiro pré-elaborado, com base no modelo citado. A análise dos resultados foi baseada em Bardin (análise de conteúdo). A partir da análise dos dados, criou-se duas categorias: a) Contexto da saúde sexual de adolescentes acolhidas: características e experiências; b) Sentimentos e Conhecimentos das adolescentes acolhidas sobre a promoção da saúde sexual. As adolescentes apresentaram comportamentos sexuais lábeis, com uso de métodos contraceptivos descontínuos e uma dualidade nas práticas sexuais protegidas. Foram percebidas mudanças nos comportamentos sexuais através do aumento do uso de preservativos nas relações sexuais, redução do número de gravidez em comparação ao período antes e durante o acolhimento, uma maior percepção das vulnerabilidades sexuais e atitudes de autonomia e empoderamento nas estratégias de promoção da saúde sexual. Percebeu-se influência da família, amigos, mídia e instituição de acolhimento na promoção da saúde sexual das jovens. Observou-se ainda, comportamentos positivos resilientes e mecanismos pessoais de enfrentamento ao histórico de violência sexual. Ações educativas dialógicas, que valorizem a promoção da saúde sexual de adolescentes institucionalizadas, com base no Modelo de Promoção da Saúde de Nola Pender, constitui uma proposta viável e relevante na busca da cidadania dessas adolescentes, principalmente quanto a conquista dos direitos reprodutivos e sexuais sobre sua saúde.

O objetivo geral deste estudo foi: Verificar se a introdução dos princípios da teoria do conforto de Katherine Kolcaba na consulta de enfermagem para pacientes com IC sob cuidados paliativos a partir da sistematização da assistência utilizando as taxonominas NANDA-NOC-NIC promove maior conforto. Trata-se de um ensaio clínico aberto, tipo antes e depois, realizado em hospital federal localizado no Rio de Janeiro com amostra de 20 pacientes. Para avaliação do conforto, utilizou-se o questionário End of life comfort questionnaire patient e o resultado de enfermagem (NOC - Estado de Conforto). As avaliações do questionário ocorreram nas primeira, terceira e sexta consultas de enfermagem e o NOC em todas as consultas. A coleta de dados foi realizada entre os meses de novembro de 2012 e maio de 2013. Utilizou-se o programa SPSS 19.0 para análise dos dados. Foram utilizados os Testes Qui-Quadrado, Exato de Fisher e Mann-Whitney para verificar a associação dos diagnósticos e intervenções de enfermagem com o conforto aferido pelo questionário nos três tempos aplicados. Foi utilizado o Teste ANOVA (análise de variância) para avaliar a melhora na condição de conforto dos sujeitos através do questionário já descrito e também pela NOC Estado de Conforto verificada nas seis consultas. Como resultado encontrou-se três diagnósticos de enfermagem e duas intervenções de enfermagem relacionadas à maior conforto dos pacientes (p <0,05). Já associados ao menor conforto foram encontrados 08 diagnósticos e 12 intervenções. O conforto verificado pelo questionário e o resultado de enfermagem aumentaram significativamente (p <0,001) durante os 06 meses de acompanhamento. A aceitação da hipótese alternativa (a introdução dos princípios da teoria de Katherine Kolcaba na consulta de enfermagem está associada ao maior conforto dos pacientes com insuficiência cardíaca sob cuidados paliativos), se confirmou. O estudo contribuiu para reafirmar que quando se utiliza uma teoria de enfermagem para sustentar a prática/pesquisa da profissão, o desenvolvimento da sistematização da assistência se torna mais claro.
